RUBELLA SURVEILLANCE - An epidemic of skin rash accompanied by mild fever, pain in the joints and swelling of lymph nodes occurred in an infant school from mid-February through mid-April 1983. The disease was first noted in one class in the week ending 19 February when 10 children were found to be ill. O f these, 6 had become ill during the preceding week. The number of cases increased rapidly to a peak, with 49 cases occurring in the first week of March, before dwindling to zero 6 weeks later. A total of 143 cases were reported during this outbreak compared with a total of 11 cases for 1982. O f the 143 cases reported during the 1983 outbreak, 133 were children aged 5-12 years, 43% of a school population of 312. Four cases occurred among the teaching staff of 12, 5 among mothers of schoolchildren and 1 case in the investi­ gating public health nurse. The attack rate was highest in the youngest children (76%) and remained high among those aged 6 and 7 years (52% and 62% respectively)—see also Table 1. SURVEILLANCE DE LA RUBÉOLE - Une épidémie de fièvre éruptive avec douleurs articulaires et adénopathie s’est produite de mi-fëvner à mi-avril 1983 dans des classes enfantines. La maladie a fait son appari­ tion dans une classe au cours de la semaine s’achevant le 19 février, date à laquelle on comptait 10 enfants malades. Sur ces 10 enfants, 6 étaient tombés malades la semaine précédente. La morbidité a atteint rapide­ ment un maximum de 49 cas au cours de la première semaine de mars, avant de retomber progressivement à zéro en 6 semaines. Cent quarantetrois cas au total ont été signalés au cours de cette flambée, chiffre à rapprocher du total de 11 cas survenus en 1982. Sur ces 143 cas, 133 étaient des enfants âgés de 5 à 12 ans, soit 43% d'un effectif scolaire de 312 Quatre cas se sont produits parmi les 12 enseignants, S parmi les mères des enfants scolarisés et 1 cas parmi le personnel médical (l’in­ firmière de santé publique chargée de l’enquête). C est chez les plusjeunes enfants que le taux d’atteinte était le plus fort (7 6%) et il était encore élevé chez ceux âgés de 6 et 7 ans (52% et 62% respectivement) — voir aussi le Tableau 1. No virus was isolated, but serological examination o f 9 paired serum specimens obtained 7 weeks apart showed a significant rise in rubella antibody titre in 3 and evidence of rubella infection in 5 others, 3 o f which had signs o f recent infection. Following the outbreak, a special study was carried out on pregnant women. Three women who conceived before the out­ break (in November-December 1982) delivered normal full-term infants. Another 3 women who became pregnant during the out­ break, but did not present clinical symptoms of rubella, are being followed up. Rubella immunization has not yet been introduced in the country. Congenital Rubella Syndrome In early 1982, Grenada experienced an epidemic of rubella, the first in at least 7 years. This was followed some months later by an epidemic o f congenital rubella syndrome which affected 20 infants (Fig 1). Most o f the infants were brought to the clinic or hospital when they were about a month old and were found to have heart defects together with cataracts, glaucoma hepatosplenomegaly, microcephaly, and purpura. Syndrome rubéolique congénital - Au début de 1982, la Grenade a connu une épidémie de rubéole, la première depuis au moins 7 ans. Ellea été suivie quelques mots plus tard d’une épidémie de syndrome rubéolique congénital atteignant 20 noumssons (Fig. I). La plupart des nourrissons ont été conduits au dispensaire ou à l’hôpital lorsqu’ils étaient âgés d'environ 1 mois et l’on a diagnostiqué des troubles cardiaques avec cataractes, glaucome, hépato­ splénomégalie, microcéphalie et purpura. Sept des 20 nourrissons sont morts, dont 1 pendant la période néonatale. G renade.

A review of the General Hospital records showed an increase in the number of stillbirths in 1982 compared to 1981. Grenada had no rubella immunization programme in 1982.

L’examen des dossiers de l’Hôpital général a révélé une augmentation du nombre des mortinaissances en 1982 par rapport à 1981. Il n’existait aucun programme de vaccination contre la rubéole à la Grenade en 1982.
