VIRAL HEPATITIS SURVEILLANCE M alaysia. — Viral hepatitis which includes hepatitis A and hepatitis B is endemic and notifiable in Malaysia. Viral hepatitis as commonly notified in Malaysia includes a proportion of hepatitis B; however, there is no laboratory confirma­ tion in over 90% of cases. The incidence of viral hepatitis ranges from 16 to 29 per 100 000 population. A rising trend similar to that for typhoid has been noted since 1974. Common source epidemics of hepatitis A have occurred sporadically, but the occurrence of epidemics is uncommon. Hepatitis A tends to follow outbreaks of typhoid suggesting that similar epidemiological factors are involved. The case-fatality rate of hepatitis A is approximately 2 %.

SURVEILLANCE DE L’HÉPATITE VIRALE M alaisie. — L’hépatite virale, de type A ou de type B, est endé­ mique en Malaisie où sa déclaration est obligatoire. Sur le total des notifications, une certaine proportion concerne actuellement l’hépatite B; toutefois, pour plus de 90% des cas, il n’y a pas de confirmation en laboratoire. L’incidence de l’hépatite virale va de 16 à 29 pour 100 000 habitants. On note depuis 1974 une tendance à l ’augmentation semblable à celle de la fièvre typhoïde. Des poussées d’hépatite A du type « à une source unique » s’obser­ vent sporadiquement, mais les épidémies sont rares. L ’hépatite A tend à sévir lors de poussées de fièvre typhoïde, ce qui laisse penser que des facteurs épidémiologiques similaires interviennent; son taux de létalité est d ’environ 2%. Si l’hépatite virale atteint tous les groupes d ’âge, plus de 70% des malades ont de 10 à 39 ans. Le rapport de masculinité est de 2; 1. Les Malais sont les plus touchés, suivis par les Indiens et les Chinois. Aucune mesure spécifique de prévention ou de lutte n ’a été appli­ quée, mais celles prises contre la fièvre typhoïde peuvent avoir contribué à endiguer l’hépatite virale. Parmi 299 malades présentant une hépatite clinique aiguë, l’électro-synérèse a révélé 10% de sujets HBsAg-positifs.

Viral hepatitis affects all age groups; however, over 70% of cases are between 10-39 years o f age. T hem ale/fem aleratiois2:1. The Malays are most commonly affected, followed by Indians and Chinese. No specific control and preventive measures for viral hepatitis have been implemented in Malaysia, although anti-typhoid measures may have contributed to the control of the disease. In a series of 299 cases of acute clinical hepatitis, 10% were positive for HBsAg by counter-immunoelectrophoresis (CEEP) technique. The HBsAg rate in male blood donors varies from 1.7% by CEEP to 9.4% by reverse passive haemagglutination (RPHA). The antiHBs rate was about 30 % by radio immunoassay (RIA) technique. A voluntary donor system is utilized in Malaysia. Blood is screened for HBsAg by CIEP. N o post-transfusion hepatitis has been reported.

Chez les donneurs de sang de sexe masculin, les taux d’HBsAg obtenus varient de 1,7% (électro-synérèse) à 9,4% (hémagglutina­ tion passive inversée). Le dosage radio-immunologique a fait appa­ raître un taux d’anti-HBs d ’environ 30%. En Malaisie, le don de sang est bénévole. On recherche l’HBsAg par électro-synérèse. Aucune hépatite post-transfusionnelle n ’a été signalée.
