SM A LLPO X SURVEILLANCE Except for two cases of smallpox imported into Somalia in January and two in February, all cases of smallpox reported during the past three and a half months have occurred in Ethiopia (Table 1). In Asia, the last known case of smallpox occurred on 16 October in Bangladesh. Subsequently, thorough searches throughout Bangladesh as well as in India have failed to detect additional cases. In Ethiopia, during the past three months, all provinces except Begemdir have recorded a decrease in the number of cases detected and in the number of infected villages (Fig. 1). In Begemdir, recent search operations in previously inaccessible areas of Debre Tabor have detected 13 infected villages and 169 cases, 60% of all cases reported this year in Ethiopia. The outbreaks are all located in the Blue Nile River gorge adjacent to endemic areas in Bahar Dar, Gojam (Fig. 2). Although originally thought to have

been related to the Gojam outbreaks, epidemiological studies reveal that they date back to known outbreaks in Begemdir which oc­ curred in August o f last year. Vaccination and containment measures are well accepted by villagers in this area and search operations are being extended throughout the gorge and to adjacent areas on the plateau. The outbreaks in Gojam are under good control with more than 400 Ethiopian staff now engaged in system­ atic search and vaccination throughout the province.

In the Ogaden desert, only three cases have been detected during the past month. Intensive vaccination programmes being con­ ducted in and around feeding shelters are believed now to have reached more than 60% of the nomadic population. However, there are now three known widely scattered foci in Hararghe and Bale, each of which has resulted in an exportation to Somalia. Except for single infected villages in Arusi and Hararghe (Habro Awiaja) reported only this week, the only other known focus in Ethiopia is in Garamuleta Awraja, Hararghe. This has proved more difficult to eliminate than had been expected because of the practice of variolation. The population in this area is receptive to vaccination and ceases variolation when vaccination is offered. However, because o f the rugged, difficult terrain, systematic search and vaccination throughout the endemic area has progressed more slowly than the wave of variolation preceding it. Teams from the Habro-Arusi area are now proceeding across country toward Garamuleta in a search-vaccination campaign and will enventually join the Garamuleta teams. All possible efforts are being made by the Government and by WHO to eliminate the last remaining foci in Ethiopia at the earliest possible time. While progress is good, the unexpected discovery of long persistent outbreaks in the Blue Nile gorge of Begemdir and the persistence of infection among Ogaden desert nomads, emphasizes the need for caution in appraisal of the present situa­ tion and the critical importance o f longer-term intensive active surveillance throughout the country.

Dans le désert d ’Ogaden, trois cas seulement ont été découverts au cours du mois dernier. Les programmes intensifs de vaccination qui sont en cours dans les abris où les nomades nourrissent leurs troupeaux et au voisinage de ces abris semblent avoir atteint main­ tenant plus de 60 % de la population nomade. Cependant, on connaît maintenant trois foyers dispersés dans les provinces d ’Hararghe et Baie qui sont chacun à l’origine d ’un cas importé en Somalie. A l’exception de quelques villages infectés à Arusi et Hararghe (Habro Awraja), signalés seulement au cours de la semaine écoulée, le seul autre foyer connu en Ethiopie, celui de l’Awraja de Gara­ muleta (Hararghe), s’est révélé plus difficile à éliminer qu’on ne le pensait, car la variolisation s’y pratiquait encore. Mais la population de cette région accepte la vaccination et renonce à la variolisation quand le vaccin lui est offert. Cependant, le terrain étant accidenté et difficile, les recherches systématiques et les opérations de vaccina­ tion dans tout le secteur d ’endémicité ont progressé plus lentement que la vague de variolisation qui les ont précédées. Des équipes venues de la région d ’Habro-Arusi, et qui finiront par se joindre aux équipes de Garamuleta, avancent maintenant à travers le pays en direction de Garamuleta, recherchant au passage les cas qui seraient passés inaperçus et vaccinant les gens. Le Gouvernement et l’OMS n ’épargnent aucun effort pour arriver à éliminer dans les plus brefs délais les derniers foyers qui subsistent en Ethiopie. Les progrès sont satisfaisants mais la découverte inat­ tendue de poussées déjà anciennes dans la gorge du Nil Bleu (pro­ vince de Begemdir) et la persistance de l’infection parmi les nomades du désert d’Ogaden, montrent bien que la plus grande prudence s’impose dans l’évaluation de la situation actuelle et qu’il est capital d’exercer à long terme dans tout le pays une surveillance active rigoureuse.

INFLUENZA WHO C o l l a b o r a t in g C e n t r e f o r R e fe r e n c e a n d R e sea rc h o n I n f l u e n z a , A t l a n t a . — In an outbreak of influenza at an Armed Forces recruit camp in New Jersey, 11 strains of influenza A were isolated. Seven were similar to A/Victoria/3/75 influenza virus and four (from four separate cases) were similar to the swine influenza A virus. One case due to this virus was reported in the USA in 1974 and one in 1975. A similar strain was isolated from a case in Czechoslovakia in 1960. Serological evidence of infec­ tion with strains of virus similar to this one is common in persons po u r la

An increase in the incidence of influenza cases has been reported in DpiMARK (14 February: small epidemic in Copenhagen area (nine isolations and 12 serological confirmations of virus A during the week), sporadic cases elsewhere (two serological confirmations of virus B)); the N etherlands (7 February: mild cases all over the country; virus close to A/Victoria/3/75); R omania (12 February: localized outbreaks since end-January in Moldavia, especially in schools; virus A); Spain (7 February: sporadic cases in Barcelona; isolation of one strain of virus B) and Switzerland (7 February: slight increase in most regions; mostly virus A, but virus B in a few cases). H ungary (6 February 1976). — 4 The small outbreak which was reported in different parts o f the country was over by the end of January. The strains isolated were close to A/Victoria/3/75 (information from the WHO Collaborating Centre for Reference and Research on Influenza, London). S weden (8 February 1976). — Sporadic cases and localized out­ breaks of influenza-like illness have been reported during the last week from different places in central and southern Sweden, including Stockholm, Gothenburg and Malmo. Twelve strains of virus A, preliminarily identified as similar to A/Victoria/3/75, have been isolated. Serological evidence of infection with virus A (a few cases) and virus B (two cases in Malmo) was also obtained. Besides sporadic cases, many outbreaks have occurred in schools, institutions, military camps and in hospital wards (particularly noticeable in geriatric wards).

So far nearly 400 strains o f influenza viruses have been examined. O f these 200 have been found to be influenza B viruses mostly similar to B/Hong Kong/5/72 virus. O f the 200 strains of influenza A virus 50 were similar to A/England/864/75 and 150 to A/Victoria/ 3/75. No strains similar to A/Tokyo/1/75 have been found. The early predominance of influenza B and the A/England/864/75 variant has now decreased and the A/Victoria/3/75 variant is un­ doubtedly the most widely encountered. However outbreaks are still being associated with both influenza B and the A/England/864/ 75 variant. In a few outbreaks both variants of influenza A have been found at the same time and in one school the A/Victoria/3/75 virus as well as virus B have been isolated. U nited States o f A merica (week ended 7 February 1976). — 4 Pneumonia and influenza mortality from 121 cities in the United States has remained at or near expected levels, except for the New England Region which shows excess deaths significantly above the epidemic threshold.

A côté de cas sporadiques, de nombreuses poussées se sont produites dans des écoles, des institutions, des camps militaires et des salles d ’hôpitaux (particulièrement dans des départements gériatriques). Etats-U nis d ’A mérique (semaine terminée le 7 février 1976). — 1 La mortalité par pneumonie et grippe notifiée par 121 villes des Etats-Unis est restée aux niveaux prévus (ou bien s’en est peu éloi­ gnée) — excepté dans la région de la Nouvelle-Angleterre ou l’on observe un excédent de mortalité qui dépasse le seuil épidémique dans des proportions significatives. Des souches de virus B ont été isolées en Oregon (cas sporadiques), et des souches de virus A au Texas (cas sporadiques) ainsi que dans les Etats suivants qui ont signalé des poussées localisées: Nevada, Michigan, Colorado, Nebraska, Tennessee et Géorgie. LASSA FEVER SURVEILLANCE M ozambique. — A virus closely related immunologically to Lassa virus was recovered from the tissues and organs of wild Mastomys natalensis captured in 1972 at Mopeia Velha, Mozam­ bique, during the course of ecological studies of arboviruses in mammals. Lyophilized brain material from the fifth mouse passage produced typical Lassa—like cytopathic effect in Vero cell cultures and fluorescent staining of infected cells was observed with anti— Lassa but not anti-lymphocytic choriomeningitis serum.
