Il y a donc eu augmentation par rapport aux deux années précé­ dentes (Tableau 1).

In addition, three chronic carriers, all o f whom were middle-aged or elderly females, were discovered during the year to be excreting S. typhi (1 case) and S. paratyphi B (2 cases); S. paratyphi B was also isolated from a bone abscess in the heel of a 62-year-old man who had a history of enteric fever as a child. Sixteen of the 28 cases during 1977 occurred as single sporadic infections, four patients were associated with two family outbreaks, and the other eight were involved in more extensive outbreaks, in a hospital, on a cruise ship, and in the general community. Two persons acquired their infection from other patients and two were infected by chronic carriers. The source of infection of the other 24 patients was never established. Seventeen patients were male and 11 were female. The age distribution of cases was widely scattered between four months and S9 years. It is interesting to note that 11 were children under 15 years of age, 14 were aged between 15 and 44 years and three were 45 years or older. Eleven cases were indigenous infections acquired within Scotland, and 17 were imported from overseas. Among the nine persons who developed typhoid, seven acquired infection abroad; o f these, four had been m countries in Southern Europe and three were from India o r Pakistan. Both paratyphoid A infections were imported from southern Asia where this serotype is known to be prevalent. Eight of the 17 paratyphoid B cases had been on holiday or business travel overseas—Middle East (3 cases), N orth Africa (2), Pakistan 0), F ar East (1). The eighth patient had been a passenger on a cruise ship from Casablanca which was associated with an extensive outbreak of various enteropathogenic infections during the autumn. La distribution par âge montrait une large dispersion des cas entre quatre mois et 59 ans. 0 est intéressant de noter que 11 sujets avaient moins de 15 ans, 14 étaient âgés de 15 à 44 ans, et trois de 45 ans et plus. L ’infection a été contractée en Ecosse même dans 11 cas et importée d ’outre-mer dans 17 autres. Parmi les neuf personnes qui ont eu la typhoïde, sept l’ont contractée à l’étranger: quatre dans des pays situés en Europe méridionale et trois en Inde ou au Pakistan. Les deux cas de paratyphoïde A étaient importés d ’Asie du Sud où l’on sait que ce sérotype est fréquent. Huit des 17 personnes ayant contracté la fièvre paratyphoïde B étaient allées en vacances ou en voyage d’affaires à l’étranger — Moyen-Orient (3 cas), Afrique du N ord (2), Pakistan (1), Extrême-Orient (1). La huitième avait voyagé à bord d’un bateau de croisière parti de Casablanca, qui a été associé à une vaste poussée épidémique d ’infections intestinales au cours de l’automne. In Scotland, the proportion of imported infections of typhoid and paratyphoid fevers in relation to those acquired at home is less than that experienced elsewhere in the United Kingdom. Such differences probably reflect the larger immigrant population, with its related travel to and from overseas, who have settled in England and Wales. Among the home-acquired cases of paratyphoid were five children and one adult from the West of Scotland who stayed at a summer holiday camp on the East coast during the same fortnight, One of the two home-acquired cases of typhoid was a four-year-old Glasgow child, whose father had recently returned from a holiday in India and was found to be excreting S . typhi. There was also one episode of secondary spread of paratyphoid B infection when a business tra­ veller from Glasgow, who had been in the Middle East, infected his wife after his return home. Towards the end o f the year, a nurse in a psycho-geriatric hospital in the North of Scotland, developed typhoid fever from an elderly patient who was a hitherto unknown chronic excreter ; two further cases o f typhoid were confirmed among other patients in the same hospital in early 1978.

Il y eut aussi un épisode de transmission secondaire de paratyphoïde B, un homme d’affaires de Glasgow ayant infecté sa femme au retour d ’un voyage au Moyen-Orient. Vers la fin de l’année, une infirmière d’un hôpital psycho-gériatrique du nord de l'Ecosse a contracté la fièvre typhoïde auprès d ’un malade âgé, excréteur chronique jusque-là passé inaper­ çu; deux autres cas de typhoïde confirmés sont survenus dans ce même hôpital au début de 1978.
