FOODBORNE DISEASE OUTBREAKS WHO is interested in receiving and disseminating information obtained by national programmes o f surveillance o f foodborne infec­ tions and intoxications. I t is considered that such programmes are useful in evaluating the impact on human health o f various contamin­ ants identified in foods, and in detecting the sources and means o f dissemination o f the infection, including food mishandling. Such information should assist health administrations o f all countries by making them more aware o f problems in this field and thereby enabling them to plan national measures fo r prevention and control.

POUSSÉES D’AFFECTIONS D’ORIGINE ALIMENTAIRE L ’OM S souhaite recevoir et diffuser les renseignements recueillis dans le cadre des programmes nationaux de surveillance des infections et intoxications d'origine alimentaire. Ces programmes permettent en effet d'évaluer les répercussions sur la santé de l'homme de différents contaminants identifiés dans les aliments et de détecter les sources et les modalités de propagation des infections, y compris les fautes commises lors de la manipulation des aliments. Ces renseignements devraient être utiles aux administrations sanitaires de tous les pays, du fa it qu’elles seront ainsi mieux renseignées sur les problèmes à résoudre, ce qui facilitera la planification des mesures de prévention et de lutte à l’échelon national. F in l a n d e . — En 1975,40 poussées d ’affections d ’origine alimen­ taire (390 cas connus) ont été enregistrées. Staphylococcus aureus a été responsable de 19 de ces poussées; les autres agents étiologiques étaient Clostridium perfringens (dix poussées), Bacillus cereus (sept), Streptococcus faecalis (deux), Escherichia coli (une) et Salmonella typhimurium (une). Près d’un tiers des 390 cas se sont produits au cours d ’une seule épidémie à C. perfringens, provoquée par la con­ sommation de langue de bœuf bouillie servie dans un restaurant. La deuxième poussée importante (40 personnes atteintes) a été due à S. aureus, également transmise par de la langue de bœuf bouillie servie dans un restaurant d’hôpital. Pour toutes les poussées notifiées, la responsabilité des aliments incriminés a été confirmée par les examens de laboratoire, le poisson étant l’aliment le plus fréquemment responsable (dix poussées), suivi par le poulet (six poussées), les saucisses (cinq), le jambon (quatre), de la langue de bœuf bouillie (trois), divers potages (trois), du pâté au riz (deux), du veau, du foie de bœuf cuit, de la viande hachée (une), un plat de viande (une), un plat de macaronis (une), un plat de riz (une) et de la sauce (une). La plupart des intoxications staphy­ lococciques ont été dues au poisson (grillé, fumé ou en conserve: neuf poussées) et à diverses viandes (saucisses surtout). D u bœuf, du veau, du jambon et du poulet ont été responsables de la majorité des poussées à C. perfringens et B. cereus. La poussée associée à S . typhimurium a été causée par du maquereau en conserve. Des sources végétales ont également été notifiées, par exemple le riz, les macaronis et un potage aux asperges dans le cas de B. cereus, et de la soupe de pois dans le cas de C. perfringens. Vingt-deux des poussées ont eu pour origine un aliment consommé au domicile des malades et les 16 autres des plats consommés au restaurant, notamment dans le restaurant d ’un hôpitaL Le lieu où les aliments ont été contaminés a été déterminé pour cinq poussées:

F in l a n d . — In 1975,40 outbreaks of foodborne illness with 390 known cases were reported. Nineteen o f these outbreaks were caused by Staphylococcus aureus-, the other causative agents were Clostridium perfringens (ten outbreaks), Bacillus cereus (seven), Strep­ tococcus faecalis (two), Escherichia coli (one) and Salmonella typhimurium (one). Almost one-third of the 390 cases belonged to a single outbreak with C. perfringens where the food vehicle was boiled beef tongue served in a restaurant. The second largest outbreak, in which 40 persons were infected with S. aureus in a hospital res­ taurant, was also due to boiled beef tongue.

The responsible vehicle was confirmed by laboratory in all . reported outbreaks, fish being the most commonly incriminated (ten ' outbreaks). Other vehicles included chicken (six outbreaks), sausage (five), ham (four), boiled beef tongue (three), various soups (three), rice pasties (two), veal, cooked beef liver, minced meat, meat dish, maccarom dish, rice dish, and gravy (one each). Most staphylococcal intoxications were associated with fish (charred, smoked or canned: nine outbreaks) and various meats (mostly sausages). Beef, veal, ham and chicken were responsible for the majority o f C. perfringens and B. cereus outbreaks. The S . typhimurium outbreak was associated with canned mackerel. Vegetable causes were also reported, such as rice, maccaroni and asparagus soup for B . cereus and pea soup for C. perfringens.

The food was eaten in the home in 22 o f the outbreaks and in restaurants in 16, one being a hospital restaurant. The place of contamination was known for five outbreaks: four o f them, in four different towns, were associated with the'same sausage factory, and EpidemiologieaJ notes contained in this number:

the fifth one, due to a rice dish, occurred in a restaurant where raw rice was found to be contaminated with B. cereus. Factors contributing to the cause of infection were storage and reheating o f soup in one instance, recycling o f a ham-curing solution in another, and the use o f automatic food-serving equipment in a third. The above results show the presence of antibody in a high propor­ tion (about 50 %) o f persons aged over 40 years. It is also interesting to note that those under 20 years of age showed no H I reaction to this strain. Recent Information (10 April 1977). —1 The influenza epidemic is reported to be declining (mostly A/Victoria/3/75; a few infections with virus B).
