Revision of the International Health Regulations Progress report, January 1999 The International Health Regulations (IHR) are being revised in accordance with a resolution adopted by the World Health Assembly in 1995 (WHA48.7). As described in previous progress reports,1 the purpose of the revision is to develop IHR which will be applicable to the epidemiology of communicable diseases and to international traffic in the 21st century. It is proposed that all disease outbreaks of urgent international importance will be notifiable. Regularly occurring endemic diseases will not be notifiable unless an outbreak has special features and poses an international threat. Many of the other public health provisions in the current IHR remain valid and will be maintained, after any necessary modification. The IHR will delineate basic provisions and principles, with specific technical details provided in a series of annexes. The annexes will be kept under review and updated as necessary. The following is a summary of the main activities undertaken since the last progress report in July 1998.
Révision du Règlement sanitaire international Rapport de situation, janvier 1999 Le Règlement sanitaire international (RSI) est révisé conformément à une résolution adoptée par l’Assemblée mondiale de la Santé en 1995 (WHA48.7). Ainsi que l’ont déjà indiqué les précédents rapports de situation,1 le but de cette révision est de mettre au point un RSI qui sera applicable à l’épidémiologie des maladies transmissibles et aux voyages internationaux au 21e siècle. Il est proposé que toutes les flambées de maladies impliquant une urgence en santé publique internationale soient notifiables. Les maladies endémiques survenant régulièrement ne le seront pas sauf si une flambée donnée présente des caractéristiques particulières et constitue une menace sur le plan international. Maintes autres dispositions du RSI actuel en matière de santé publique demeurent valables et seront maintenues, après avoir été modifiées si nécessaire. Le RSI définira des dispositions et principes de base, certains détails techniques étant fournis dans une série d’annexes. Ces annexes seront passées en revue et actualisées selon que de besoin. On trouvera ci-après un résumé des principales activités entreprises depuis le dernier rapport de situation en juillet 1998.
Provisional draft of the revised IHR Comments from Member States on the provisional draft of the revised IHR distributed in early 1998 were taken into account in the preparation of an amended draft in August 1998. This amended draft was discussed by the Committee on International Surveillance of Communicable Diseases (CISCD) which met in November 1998. The Committee’s comments and recommendations are being used in preparing the next version of the provisional draft IHR, which will again be circulated widely for review and comment by Member States. 1

Meeting of the Committee on International Surveillance of Communicable Diseases (CISCD), 16-20 November 1998 The 7 members of the Committee were drawn from all WHO regions and represent different areas of specialized expertise in public health. In addition, the World Trade Organization (WTO) was represented and the meeting was also attended by several scientific and legal advisers. The Committee considered several technical and legal issues in the provisional draft IHR which required resolution. It also reviewed the work done to date on revising the IHR and the plans for the future. In its conclusions and recommendations, it noted with satisfaction the progress made in revising the IHR, although acknowledging that much work remained to be done on a number of provisions in the Regulations and the preparation of the annexes to the IHR, on the pilot study to evaluate the use of syndrome notification and on resolving potential differences with WTO. In view of the limited amount of information received so far from countries participating in the pilot study, it recommended that the study be extended through March 1999. It further recommended that WHO should work more closely with a limited number of the participating countries for the remainder of the study. Although acknowledging that reports of disease syndromes, outbreaks and risks were often widely known, the Committee indicated that notification to WHO should continue to be made via the national authorities. It reaffirmed the desirability of syndrome notification and the principle that measures specified in the IHR and its annexes should be the maximum that States can apply to international travel. The Committee also endorsed the concept of keeping the core content of the IHR broad and stable over the long-term, using annexes for precision and technical detail and for more frequent updating.
Réunion du Comité de la surveillance internationale des maladies transmissibles, 16-20 novembre 1998 Les 7 membres du Comité viennent de toutes les régions de l’OMS et offrent différentes spécialisations en santé publique. Par ailleurs, l’Organisation mondiale du Commerce (OMC) était représentée à la réunion à laquelle participaient également plusieurs conseillers scientifiques et juridiques. Le Comité a examiné plusieurs questions techniques et juridiques figurant dans le projet provisoire de RSI et demandant à être résolues. Il a également passé en revue les travaux de révision du RSI effectués à ce jour ainsi que les plans pour l’avenir. Dans ses conclusions et recommandations, il a pris note avec satisfaction des progrès réalisés touchant la révision du RSI, tout en admettant qu’il y avait encore beaucoup à faire au sujet de plusieurs provisions du Règlement et de la préparation des annexes au RSI, de l’étude pilote destinée à évaluer l’utilisation de la notification des syndromes et de la résolution de divergences éventuelles avec l’OMC. Eu égard à la quantité limitée d’informations envoyées jusqu’ici par les pays participant à l’étude pilote, le Comité a recommandé que l’étude soit prolongée jusqu’en mars 1999. Il a en outre recommandé que l’OMS travaille plus étroitement avec un nombre limité de pays participants pour le restant de l’étude. Tout en reconnaissant que les syndromes de maladie, flambées et risques étaient souvent largement signalés, le Comité a précisé que la notification des maladies à l’OMS devait continuer de se faire par l’intermédiaire des autorités nationales. Il a réitéré que la notification des syndromes était souhaitable et réaffirmé le principe selon lequel les mesures précisées dans le RSI et ses annexes constituent le maximum de ce que les Etats peuvent exiger à l’égard des voyages internationaux. Le Comité a également approuvé l’idée de conserver un caractère général, stable et durable au noyau du RSI, et de se servir des annexes pour y faire figurer les précisions et détails techniques nécessaires et pour l’actualiser plus fréquemment.

Member State notification needs to be evaluated emphasizing the criteria for “events” of urgent international public health importance. The application of health measures based on the known or suspected mode of transmission and disease source may be appropriate for the IHR, and should be evaluated, as well as the scope for application of such measures. 26

Member States have requested that the IHR submission to the World Health Assembly include not only a final draft IHR core text, but also completed technical and other annexes. The public health requirements in the annexes were originally envisaged for review over a period of 3-5 years. Considerable effort and international consultation will be required to achieve scientific validation for many items in the annexes and in the core articles. For this reason the target submission date will need to be extended.

Il faudra déployer des efforts considérables et procéder à des consultations internationales pour parvenir à valider sur le plan scientifique de nombreux points figurant dans les annexes et les articles essentiels. Destruction of the smallpox virus The World Health Organization’s ad hoc Committee on orthopox virus infections, meeting in Geneva on 14 and 15 January 1999, voted in favour of the destruction of the last stocks of smallpox virus still kept in the WHO collaborating centres for smallpox and other poxvirus infections in the Russian Federation and the United States. Seven of the 9 Committee members voted for destruction, 5 of them favouring immediate destruction, whilst 2 were in favour of eventual destruction following a review in 5 years’ time. There was a dissenting view, with 2 Committee members supporting indefinite retention of the stocks. Since the global eradication of smallpox was confirmed in 1980, the Committee has met a number of times to oversee post-eradication policies and programmes. From 1986 onwards, it foresaw the possible destruction of smallpox virus stocks and recommended a programme of work involving preservation of the basic genetic information of the virus, wide consultation with the scientific community and development of specific procedures to be followed when the virus is destroyed. In 1990 after the World Health Assembly adopted a resolution recommending destruction of smallpox viruses, WHO recommended the destruction by 31 December 1993 of existing stocks in the 2 WHO collaborating centres which were depositaries of these viruses, located in the Russian Federation and the United States. Destruction was postponed at the end of 1993, to allow further studies of the variola virus genome. The WHO ad hoc Committee, convened in September 1994, proposed destruction by 30 June 1995. The World Health Assembly decided in 1996 (resolution WHA49.10) that destruction should take place by 30 June 1999. It was envisaged that the period between the date initially chosen (31 December 1993) and June 1999 would be used to achieve a broader consensus on the issue. Prior to the meeting of the Committee, and in order to determine if a broader consensus had been reached on the issue of the destruction of smallpox stocks, WHO undertook an inventory of the positions of its 191 Member States on the proposed destruction in accordance with the 1996 resolution. The survey showed that among the 79 countries which responded, situated in all WHO regions, a large majority (94%) was in favour of its complete implementation. 27
Destruction du virus de la variole Le Comité ad hoc de l’Organisation mondiale de la Santé sur les orthopoxviroses, réuni à Genève les 14 et 15 janvier 1999, a voté la destruction des derniers stocks de virus de la variole conservés dans les centres collaborateurs de l’OMS pour la variole et les orthopoxviroses aux Etats-Unis d’Amérique et en Fédération de Russie. Sept des 9 membres du Comité ont voté pour, 5 se prononçant en faveur de la destruction immédiate, les 2 autres pour une destruction après un nouvel examen dans 5 ans. Il y avait une opinion contraire avec 2 membres du Comité en faveur d’une conservation indéfinie des stocks.
Depuis la confirmation de l’éradication mondiale de la variole en 1980, le Comité s’est réuni un certain nombre de fois pour surveiller les programmes et les politiques dans la période suivant l’éradication. A partir de 1986, il a prévu la destruction possible des stocks de virus et a recommandé un programme de travail impliquant la conservation des informations génétiques fondamentales du virus, de larges consultations avec la communauté scientifique et l’élaboration de procédures spécifiques à suivre lors de la destruction du virus. En 1990, après que l’Assemblée mondiale de la Santé ait adopté une résolution recommandant la destruction des stocks de virus de la variole, l’OMS a recommandé la destruction avant le 31 décembre 1993 des stocks existants dans les 2 centres collaborateurs OMS dépositaires de ces virus, situés aux Etats-Unis d’Amérique et en Fédération de Russie. Fin 1993, la destruction a été remise pour permettre des études supplémentaires du genome du virus de la variole. Le Comité ad hoc de l’OMS, réuni en septembre 1994, a proposé la destruction avant le 30 juin 1995. L’Assemblée mondiale de la Santé a décidé en 1996 (résolution WHA49.10) que la destruction aurait lieu avant le 30 juin 1999. Il était envisagé que la période entre la date choisie initialement (31 décembre 1993) et juin 1999 serait utilisée pour obtenir un consensus plus large en la matière. En préalable à la réunion du Comité, et afin de déterminer si un consensus plus large s’était établi sur la question de la destruction des stocks de virus conformément à la résolution de 1996, l’OMS avait entrepris de recenser les positions des 191 Etats Membres à ce propos. L’enquête a montré que, parmi les 79 pays ayant répondu, situés dans toutes les régions de l’OMS, une grande majorité (94%) se prononçaient en faveur d’une application pleine et entière de cette mesure.

The members of the Committee recommended that if the World Health Assembly decides to delay the destruction, WHO should ensure regular inspections of the 2 repositories of smallpox virus. They also agreed that WHO should advise its Member States to retain their existing stocks of smallpox vaccine, and that the deliberate release of smallpox virus into the environment should be considered a crime against humanity. The Committee also recommended that WHO Member States should be asked to reconfirm that they do not have stocks of smallpox virus.

Ils se sont également mis d’accord pour que l’OMS conseille à ses Etats Membres de garder leurs stocks de vaccin antivariolique et pour que le rejet délibéré de ce virus dans l’environnement soit considéré comme un crime contre l’humanité. Le Comité a également recommandé de demander aux Etats Membres de l’OMS qu’ils confirment à nouveau qu’ils ne détiennent pas de stocks de virus de la variole.
Influenza Finland (21 January 1999). The first influenza A viruses isolated for this season were from a 1-year-old boy and a baby in Oulu. Sporadic cases of influenza A and B were reported until the first week of January. There were local outbreaks of influenza A among conscripts in the Finnish defence forces in the south, and of both influenza A and B in the north. The majority of isolates and antigenically detected viruses this season are influenza A, of which some have been identified as subtype A(H3N2). Germany (21 January 1999). In mid-November, 7 cases of influenza A and 2 of influenza B were confirmed by virus isolation in Hanover. The influenza A strains are antigenically similar to A/Sydney/5/97(H3N2) and the influenza B isolate to B/Harbin/7/94. Marburg reported an increasing number of influenza isolates with sporadic and local activity mainly in the south-western and western regions since the first week of January. Influenza A(H3N2) is still the dominant strain, but some isolates were influenza B viruses. Italy (15 January 1999). The first isolates to be reported by collaborating laboratories for the season were influenza A(H3N2), of which 1 was antigenically similar to influenza A/Sydney/5/97. There was 1 confirmed case of B/Beijinglike influenza. A notable increase of influenza cases was reported in Genoa in the last 2 weeks of December. During the first 2 weeks of January, additional cases of influenza A, A(H3N2) and B were confirmed in Florence, Genoa, Milan, Parma and Perugia. All influenza B viruses were B/Beijing-like strains. Poland (5 January 1999). Influenza activity was low until the last week of November 1998. One sporadic case of influenza A(H3N2) was detected in a 56-year-old woman living in Warsaw. Influenza and influenza-like illness increased significantly during the first 2 weeks of December and doubled during the last 2 weeks of that month. The total number of cases was 15 591, with an incidence of 40.3 per 100 000. No isolates were reported. Sweden (17 January 1999).1 The season started with a few imported cases of influenza B at the beginning of November 1998. Since mid-November there have been 118 laboratory-confirmed cases of influenza A and 9 of influenza B. The influenza A strains have all been A(H3N2). The number of laboratory-diagnosed cases is increasing and local outbreaks have been reported since late December, but no epidemic activity. Most cases diagnosed are from the southern part of the country. 1

Des cas sporadiques de grippe A et B ont été signalés jusqu’à la première semaine de janvier. Il y a eu des flambées locales de grippe A parmi des conscrits dans les forces armées finlandaises au sud du pays, et de grippe A et B dans le nord. Allemagne (21 janvier 1999). A la mi-novembre, 7 cas de grippe A et 2 de grippe B ont été confirmés par isolement du virus à Hanovre. Un cas sporadique de grippe A(H3N2) a été dépisté chez une femme de 56 ans vivant à Varsovie. La grippe et les syndromes grippaux ont connu une importante augmentation pendant les 2 premières semaines de décembre, et ont doublé pendant les 2 dernières semaines de ce même mois. Il y a eu au total 15 591 cas, avec une incidence de 40,3 pour 100 000 habitants. Aucun isolement de souche n’a été notifié. International travel and health Vaccination requirements and health advice, situation as on 1 January 1999 1 The 1999 edition of International travel and health has just been published. This booklet is addressed to national health administrations and to practising physicians, tourist agencies, shipping companies, airline operators and other bodies who are called upon to give health advice to travellers. A convenient country-by-country list of required vaccinations is provided, together with pertinent information on the malaria situation, for every country in the world. For malaria, epidemiological details are given for all countries with endemic areas, including notes on geographical and seasonal distribution, altitude, predominant species and status of resistance. The recommended chemoprophylactic regimen is also given for each country with malarious areas. In view of the deteriorating malaria situation, information on prophylaxis and treatment is especially detailed. Further assistance is provided through a fold-out map summarizing recommended prophylactic regimens by area and a table indicating doses of antimalarial drugs for children. A section on multidrug-resistant malaria is also included. Other chapters cover certain health hazards to which the traveller may be exposed and indicate the areas in which these hazards are most likely to occur. The booklet also recommends a number of precautions that the wise traveller should take when visiting unfamiliar places. 1
Voyages internationaux et santé Vaccinations exigées et conseils d’hygiène, situation au 1 er janvier 1999 1 L’édition 1999 de Voyages internationaux et santé vient de paraître Cette brochure s’adresse aux administrations de la santé, au corps médical, aux agences de tourisme, aux compagnies maritimes et aériennes et aux autres organismes qui sont amenés à donner des conseils d’hygiène aux voyageurs. La publication fournit une liste commode des vaccinations exigées pays par pays ainsi que des informations pertinentes sur la situation du paludisme pour chacun des pays. En ce qui concerne cette maladie, elle donne des détails épidémiologiques sur tous les pays comportant des zones d’endémie, et notamment des notes sur la répartition géographique et saisonnière, l’altitude, l’espèce prédominante et l’état de résistance. Elle indique aussi le schéma chimioprophylactique recommandé pour chaque pays comptant des régions impaludées. Etant donné l’aggravation de la situation du paludisme, l’information sur la prophylaxie et le traitement est particulièrement détaillée. D’autres renseignements sont donnés dans une carte-dépliant qui résume les schémas prophylactiques recommandés par zone et dans un tableau indiquant les doses d’antipaludiques à administrer aux enfants. Il existe également une section sur le paludisme polypharmacorésistant. D’autres chapitres décrivent certains risques pour la santé des voyageurs et indiquent les régions où ces risques se rencontrent le plus souvent. La publication recommande aussi certaines précautions que le voyageur serait avisé de prendre lorsqu’il se rend dans des régions peu connues. 1

Informal information meeting on antimicrobial resistance surveillance in foodborne pathogens WHO Geneva, 31 March – 1 April 1999 Since the WHO meeting on the medical impact of the use of antimicrobials in food animals held in 1997, a considerable number of international groups, including WHO, have begun projects to strengthen antimicrobial resistance surveillance in foodborne pathogens - e.g. Confédération mondiale de l’industrie de la santé animale (COMISA), Brussels; Office international des épizooties (OIE), Paris; European Commission (FLAIR-Project); International surveillance network for the enteric infections: Salmonella and EHEC 0157 (ENTERNET). Whilst having distinctive approaches and methods, the general objectives of the various surveillance projects are often similar. WHO is organizing this informal meeting in order to facilitate the exchange of information between the various ongoing programmes with the aim of encouraging international collaboration and the convergence of surveillance activities at an early stage of programme development and implementation. It is proposed that coordinators of ongoing/planned surveillance programmes/projects present their activities and discuss areas of collaboration. In addition, possibilities of, and needs for sharing and distribution of data could be identified. Réunion informelle d’information sur la surveillance de la résistance antimicrobienne des agents pathogènes d’origine alimentaire OMS Genève, 31 mars – 1er avril 1999 Depuis la réunion sur l’impact médical de l’utilisation d’antimicrobiens chez les animaux destinés à l’alimentation, tenue en 1997, un grand nombre de groupes internationaux, dont l’OMS, ont lancé des projets pour renforcer la surveillance de la résistance antimicrobienne des agents pathogènes d’origine alimentaire – par exemple la Confédération mondiale de l’industrie de la santé animale (COMESA), Bruxelles; l’Office international des épizooties (OIE), Paris; la Commission européenne (Projet FLAIR); l’International surveillance network for the enteric infections: Salmonella and EHEC 0157 (ENTERNET). Tout en ayant des approches et des méthodes distinctes, les objectifs généraux des divers projets de surveillance sont souvent similaires. L’OMS organise cette réunion informelle afin de faciliter l’échange d’informations entre les divers programmes en cours, dans le but d’encourager la collaboration internationale et la convergence des activités de surveillance au stade initial du développement des programmes et de leur mise en œuvre. Il est proposé que les coordonnateurs des programmes et projets courants ou prévus de surveillance présentent leurs activités et discutent des domaines de collaboration. En outre, les possibilités et les besoins de partage et de distribution de données pourraient être identifiés. Outbreak news Meningococcal meningitis, Sudan (update). Meningococcal meningitis has been reported from the following communities in the Northern Darfur region: El Fascher (population 657 852) – 21 cases, 4 deaths; Kutum (population 348 000) – 135 cases, 11 deaths; Kebkabiya (population 240 017) – 43 cases, 15 deaths. Cases have continued to be reported at an increasing rate, reaching a total of 199 cases and 30 deaths by 20 January. So far, 91 000 people have been vaccinated by 40 vaccination teams. While it appears that the number of new cases has diminished over the last few days, plans are being drawn up to strengthen surveillance and control measures throughout the country in preparation for any further outbreaks of the disease. 1

Méningite à méningocoques, Soudan (mise à jour).1 La méningite à méningocoques a été signalée dans les communautés suivantes de la région du Darfur septentrional: El Fascher (population 657 852) – 21 cas, 4 décès; Kutum (population 348 000) – 135 cas, 11 décès; Kebkabiya (population 240 017) – 43 cas, 15 décès. L’on continue de signaler un nombre croissant de cas, atteignant un total de 199 cas dont 30 décès au 20 janvier. A ce jour, 91 000 personnes ont été vaccinées par 40 équipes de vaccination. Tandis que le nombre de nouveaux cas semble avoir diminué ces derniers jours, des plans sont en préparation pour renforcer les mesures de surveillance et de lutte dans tout le pays afin de pouvoir faire face à toute autre flambée de la maladie.
