RESPIRATORY DISEASES SURVEILLANCE Atypical Pneumonia S p a in . — From the beginning of May, a number of cases o f atypi­ cal pneumonia were notified in Madrid and subsequently to a lesser degree m other provinces. The majority of the 800 cases so far reported have been mild and pneumonia was diagnosed only on the basis of chest radiography. The response to several antibiotics has been favourable and generally leads to full recovery. There have however been eight fatal cases. Although not yet definitive, labora­ tory information suggests Mycoplasma pneumoniae may be the cau­ sative agent. E d it o r ia l N o t e : If the aetiology of the present outbreak is con­ firmed, the situation m Spain is consistent with that observed in other outbreaks. Although previously considered as a mild respira­ tory disease, which is still the picture for the majority of cases, severe lower respiratory tract infections with interstitial pneumonia, hypox­ emia and pleural efiusion have recently been more frequently reported. Mycoplasma infections are endemic in most countries, although an increase in the number o f cases to an epidemic level with a periodicity of four to six years has been described. Once apparent, such epidemics have been observed to last for many months. As in the outbreak in Spain, antibiotics, particularly erythromycin and tetracycline, have been shown to be effecuve. The present situation would not appear to warrant any special measures with respect to international travel.

SURVEILLANCE DES MALADIES RESPIRATOIRES Pneumonie atypique E sp a g n e . — Depuis le début de mai, des cas de pneumonie atypi­ que ont été notifiés à Madrid, puis, en moins grand nombre, dans d’autres provinces. La majorité des 800 cas signalés jusqu’ici ont été bénins, et la pneumonie n'a été diagnostiquée que sur radiographie thoracique. Les malades ont bien réagi à divers antibiouques et, en général, se sont complètement rétablis. Cependant, il y a eu huit décès. D’après les résultats, non encore défimufs, des travaux de laboratoire, l’agent causal pourrait être Mycoplasma pneumoniae, N o t e d e la R é d a c t io n : Si l’éuologie de la poussée est confirmée, ce qui se passe en Espagne correspond à ce qu’on a observé ailleurs. Bien que considérées précédemment comme provoquant une mala­ die respiratoire bénigne, ce qui demeure vrai dans la majorité des cas, de fortes infections des voies respiratoires inférieures, avec pneumo­ nie interstitielle, hypoxémie et pleurésie, ont été plus fréquemment signalées ces derniers temps. Les infections à mycoplasmes sont endémiques dans la plupart des pays, mais des poussées épidémiques à intervalles de quatre à six ans ont été décrites. Une fois apparues, elles ont duré plusieurs mois. Comme en Espagne, les antibiotiques, en particulier l'érythromycine et la tétracycline, se sont montrés effi­ caces. La situation actuelle ne paraît justifier aucune mesure spéciale en ce qui concerne les voyages internationaux.
