DIPHTÉRIE Une épidémie de diphtérie a été signalée dans une institution hébergeant 700 grands débiles mentaux. Le premier cas suspect a été celui d’un adulte jeune qui décéda après une maladie fulgurante d ’une seule journée. A l’autopsie, le décès sembla imputable à une bronchopneumonie, mais une importante formation de membranes fut constatée. Il a été impossible de cultiver Corynebacterium diphtheriae à partir d’écouvillonnages des bronches et du pharynx, peut-être parce que le malade avait été soumis à un traitement antibiotique intensif. Dix-huit jours plus tard, un sujet atteint de pneumonie, dans une autre salle, mourait subitement. Trois et quatre jours après, deux autres malades de la même salle étaient atteints d ’une infection de la gorge et de pyrexie; sur l’un d’eux, on isolait C. diphtheriae. A peu près au même moment, un qua­ trième malade de cette salle présentait de graves troubles pulmo­ naires et mourait deux jours plus tard. L’autopsie révéla la forma­ tion d’une membrane très importante et l’on put isoler C. diphthe­ riae mitis. Au cours des dix jours suivants, C. diphtheriae mitis a été isolé à partir de frottis du pharynx chez 12 autres malades et chez un membre du personnel, ainsi qu’à partir d’un échantillon provenant d’un malade décédé. Tous les sujets chez lesquels le bacille a été détecté — à l’exception de l’employé de l’institution — présentaient des symptômes, généralement bénins. Quelques jours plus tard, on découvrait C. diphtheriae mitis dans la gorge d’un infirmier travaillant dans un pavillon distinct. 1 1 fut mis à l’isolement et pré­ senta, par la suite, une diphtérie clinique bénigne. Tous les individus atteints au cours de cette épidémie, y compris ceux qui sont décédés, avaient été vaccinés ou revaccinés 3 à 12 ans plus tôt. Onze des souches de C. diphtheriae isolées se sont révélées toxigènes; la onzième culture était mixte.

An outbreak of diphtheria has been reported in an institution that accomodates 700 severely mentally subnormal patients. The first suspected case was in a young adult who died after a fulminating illness lasting one day. At autopsy death appeared to have been due to bronchopneumonia, but extensive membrane formation was noted. Cultures of swabs from the bronchi and pharynx failed to grow Corynebacterium diphtheriae, perhaps because the patient had received intensive antibiotic treatment. Eighteen days later another patient with pneumonia in a different ward died suddenly. Three days and four days after this two more patients in the same ward developed sore throat and pyrexia, and C. diphthe­ riae was isolated from one of them. At about the same time a fourth in this ward contracted a severe chest illness and died two days later; at autopsy an extensive membrane was found and C. diphtheriae mit is was isolated. During the next ten days C. diphtheriae mitis was isolated from the throats of 12 more patients and one member of staff and from a post-mortem specimen from another patient who died. All those from whom the organism was isolated, except the member of staff, had symptoms, which were usually mild. A few days later a male nurse working in a different villa was found to be harbouring C. diphtheriae mitis in his throat. He was isolated and later developed mild clinical diphtheria. All those affected in this episode, including the patients who died, had been immunized or re-immunized during the last 3 to 12 years. Eleven of the strains of C. diphtheriae isolated were shown to be toxigenic and the eleventh was a mixed culture.
