BACTERIAL M ENINGITIS U nited K ingdom . — The total number o f patients in England, Wales and Northern Ireland with bacterial meningitis (isolate from cerebrospinal fluid or cerebrospinal fluid and blood) reported for the Communicable Disease Report varies little from year to year (Fig, 1). During the 9-year period 1975-1983, 13 131 patients with bacterial meningitis were reported, i.e. an average of 1459 cases each year (range 1 294—1 596) ; 1 800 o f these patients are known to have died (13.7%) and this proportion too has hardly changed (range 12.6% in 1981—15.6% in 1978). In 1983, 216 of 1 596 (13.5%) patients were reported to have died. Data from laboratory reports on deaths from bacterial meningitis are likely to be an underestimate o f the number o f deaths as not all infections will have been reported and some patients die before reaching hospital and being investigated.. Statutory notifications o f meningitis (all forms) show peaks and troughs (Fig. I) and the pattern of these fluctuations is similar to that in the number of patients with proven virus meningitis. Cumulative totals of patients reported by laboratories for the Communicable Disease Report with meningitis due to echoviruses, coxsackie viruses and mumps are shown in the figure. It is apparent that there is a considerable degree o f under-notification o f meningitis, particularly of bacterial infections,

Pendant la période de 9 ans allant de 1975 à 1983,13 131 casde méningite bactérienne, soit une moyenne de 1459 cas par an (minimum 1294 — maximum 1 596 cas), ont été signalés, dont 1 800 (13,7%) ont été mortels. La proportion des cas mortels n’a pas beaucoup varié non plus (entre 12,6%en 1981 et 15,6%en 1978). En 1983, sur 1 596 cas, 216 décès (13,5%) ont été signalés. Les notifications de décès par méningite bactérienne fondées sur les résultats de laboratoire repré­ sentent vraisemblablement une sous-estimation, car tous les cas ne sont pas notifiés et certains patients meurent avant d’être hospitalisés et de subir des analyses. La courbe des notifications obligatoires de cas de méningite (toutes formes) présente des pics et des creux (Fig. I) et son allure est analogue à celle du nombre de cas de méningite virale confirmés. La Fig. I donne les totaux cumulatifs de cas de méningite due aux écbovirus, aux virus cox­ sackie et aux virus des oreillons notifiés par les laboratoires à l’intention du Communicable Disease Report. Il est manifeste que le degré de sousnotification des cas de méningite, notamment de méningite bactérienne, est considérable.

Three-quarters of the patients with bacterial meningitis reported by laboratories are infected with N. meningitidis, H. influenzae or S, pneumoniae. Fig. 2 shows the number of reports received each year during the period 1975-1983 and fig. 3 shows the ages of these patients.

Les trois quarts des cas de méningite bactérienne signalés par les labo­ ratoires sont imputables à N. meningitidis, H. influenzae ou S. pneumo­ niae. La Fig. 2 donne le nombre de notifications reçues chaque année au cours de la période 1975-1983 et la Fig. 3, l’âge des patients.

O f the 3 984 patients with N. meningitidis meningitis, 7.0% (280 patients) died (range 5.1% in 1981 to 8.5% in 1978). During the same period 20% o f 820 patients reported with meningococcal septicaemia died. O f the 3 242 patients reported with H. influenzae meningitis 4.4% (142 patients) died (range 2.9% in 1978 to 6.0% in 1977). During the same period 186 of 2019 patients (9.2%) with H. influenzae bacteraemia died.' During the last 4 years the total number of serious H. influenzae infections (meningitis and septi­ caemia) has exceeded the number o f serious meningococcal infec­ tions reported. Very little evidence on the occurrence of associated cases or any change in ampicillin-sensitivity in the United Kingdom has reached the Communicable Disease Surveillance Centre. A short prospective study o f these features will be earned out on cases reported by laboratories.

Sur les 3 984 cas de méningite à N. meningitidis, 7,0% (280 cas) ont été mortels (5,1% en 1981 — 8,5% en 1978). Pendant la même période, 20% des 820 cas de septicémie méningococcique notifiés ont eu une issue fatale. Sur les 3 242 cas de méningite à H. influenzae notifiés, 4,4% (142 patients) ont été mortels (2,9% en 1978 — 6,0% en 1977). Pendant la même période, 186 des 2 019 patients (9,2%) atteints de bactériémie à H. influenzae sont décédés. Au cours des 4 dernières années le nombre total d’infections graves à H. influenzae notifiés (méningite et septicémie) a dépassé celui des infections graves à méningocoques. Parmi les données parvenues au Centre de surveillance des maladies transmissibles, il y a peu d’indications de l’existence de cas associés ou d’un changement quelconque de la sensibilité à l’ampicilline au Royaume-Uni. Une brève étude prospective de ces aspects sera effectuée sur les cas rapportés par les laboratoires. Fig. 3

Pneumococcal meningitis was reported in 2 565 patients during the period 1975-1983; 20.7% (532 patients) died (range 16.3% in 1981 to24.5%in 1983). The annual number of reports has changed little during this period. In contrast, the number of patients reported with pneumococcal bacteraemia increased annually from 378 in 1975 to 1 635 in 1983 and 19.9% of the 8 707 patients died.
