La dcurir\nrc réuni«rn lcclrni<;uc (05/12l01) SUMMARY In the framework of the implementation of the onchocerciasis vector eliminationproject in the Tukuyu focus, a mission was carried out from 15 November 2001 to 03Irebruary 2002. After various courtesy visits to the WHO Representation in Tanzania and to the national structures involved in onchocerciasis control, we began the preparatory activities forthe ground larviciding campaign by participating in the national ,*aàn"ss seminar whichtook place in Mbeya, the capital town of the Tukuyu focus region, from 26 to zg Novenrber 200 t. The presentation of Dr Y. Bissan, OCP/APOC experÇ on the topic "ConhoVecological monitoring in West African OCP Countries" on 27 Novèmber 2001 rvas a determining factor in the understanding of the activities which were to be carried out in the focusarea, mainly theimpact of larviciding on the enüronment which seemed to be the majàr àr,""* of theparticipants. The ground larviciding experiment made on 28 November 2001 finally convinced all those who were still worrying about the destruction of the non target aquaticfauna due to the direct spraÿng of the temephos in the rivers. This seminar at the national level was followed few days later by another one at thedistrict level which brought together the main personalities and notables of the three (3)districts of the focus area. Finally, the main riverside villages of the targeted basins were ser-rsitized by the NIMR technicians. Afler various teclmical rneetings, 04 teams were established (03 teams for grou,d spraying, 01 for supervision ar-rd larval prospection) and 03 treatment rounds were established. The ground larviciding campaign in the focus effectively began on 13 December 2001. The Lhree basins of the focus (Kiwira, Mbaka, Lufilyo) went tluough a weekly spraying of ternephos at a dosing of 1.33 mÿ\i.e.0.4 I per rn3/s during 06 weeks. O, the whole, 881 liters of temephos were dropped in 53 treatment points. Conceming the immediate evaluation of treatments (larval prospection), the results are excellent. Dala on adult fly catching before and during sprayings are not reliable. In our opi1iol, they do ttot reflect tlie reality of the dlmarnics of tlie sirnulium populatiop of the focus. Suggestions 1.1 . Technical A- During the dry season, the carry of the temephos is considerably reduced rnaki,g necessary sometimes to do the spraying site by site something which is practically impossibË because of the topography of the area. Furtherïnore, the vector eliminatiop requiies tàatrne.t oIall thc vector breeding sites. Therefore, we suggest the search for access: a) At tl-re level KI 600 above Kapeta old bridge to reduce the distance betwee, treatment points on the confluence of Mwalisi/kisyesye-Kapeta old bridge.b) At the level of confluences of all the tributaries with a pernanent flow in the distribution area of the onchocerciasis vector. c) At the level of the right bank tributaries of River Songwe in the focus (one of the,r was found positive). d) The renewal of treatments before the dry season, mainly on tributaries. B- The results of the six weeks spraying of the temephos are excellent with regard to the pre- irnaginal vector population. The renewal of treatments will be done after .J,o" months of suspension. We suggest a general evaluation of the density of the aquatic stages of the vector for the entire hydrographic nefwork of the focus mainly at the upstream of the first treatr,ent points, with a couservation of the specimen collected in camoy's and 70 o% alcoSol. 1.2. Adrninistrative Considering the likely increase of the number of the treatment cycles, the poor conditio^ oftwo vehicles of the current fleet, we suggest the acquisition of 03 additiopàl vehicles (to be negotiated with the other national onchocerciasis projects and the national coordinator). 2. Recommendations 1- Considering that: - The treatment of the river Lurnbira is indispensable for the eliminatiop of the rrector in the Tukuyu focus, - Access to its unique treatment point is virtually impossible by foot (,rorc than 06 hours of walking). Après les differentes visites de courtoisies à la représentation de I'OMS en Tanzanie et aux structures nationales impliquées dans la lutte contre l'onchocercose ; les activitéspréparatoires de la campagne des traitements au sol ont débuté par notre participatio, au séminaire national de sensibilisation tenue du 26 au ZSllllzOOL à Mbeya "n"f tl"u de larégion du foyer de Tukuyu. Ce séminaire de niveau national fut suivi quelques jours après par un autre de niveauDistrict qui a regroupé les principales personnalités et notables des trois districts de I'aire dufoyer. Enfin les principaux villages riverains des bassins visés furent sensibilisés à leur tourpar les techniciens de laNIMR. A f issue de différentes réunions techniques 04 équipes ont été constituées( 03 equipes pour les épandages, 01 pour la supervision et le contrôle larvaire ) et 03 circuits de - traitement furent établis. La campagne des traitements au sol du foyer a effectivement débuté le L3/I2/2001. Les trois bassins du foyer ( Kiwira" Mbaka,Lufilyo ) ont été soumis aux épandages hebdornadaires de téméphos à la dose de 1.33 mg/l soit 0.4 I par m3/s pendant 06 semaines. Au total 881 litres de téméphos ont été injectés sur 53 points. Sur le plan de l'évaluation directe des traitements (contrôle larvaire) les résuttats solt excellents. Les données des captures d'adultes avant et pendant les épandages ne nous inspirent pas confiance. A notre avis elles ne reflètent pas la réalité de la dynamique de la population simulidienne du foyer. L'analyse des résultats en notre possession ne nous permet pas de déceler l,impact des traitements effectués. L'isolement du foyer a quelques failles. Au Sud Est, non loin de la R Lumbira se trouvent des affluents du lac Nyassa (deux petits cours d'eau et ta R Kilondo ). A l,Ouest, laR Songrve et ses affluents. A l'intérieur même du foyer, la couverture totale des gîtes sernble difficile à réaliser(l'accès à nombre d'affluents à écoulement pemranent est pratiquement inrpossible du fait de la topographie du terrain). Réaiustement des points d'application Compte tenu du niveau d'eau extrèmement bas cette année, la portée du larvicidepourrait être considérablement réduit, aussi un réajustement de points de dosage s'impose.(voir carte appendice I) 5.2. Les écheWes de crue et ruesure de débtt : Sur les quatre échelles existantes, seule celle de la prison civile sur la Kwi 100 estfonctionnelle. Il serait souhaitable que les trois autres soient réparées. De nou'elles échelles sont nécessaires notamment sur le moyen Kiwira sur la haute Mbaka le moyen Luf,rlyo. Ces retarages et les nouvelles installations allégeraient Ie travail des équipes ,rais sutlout rendraieut les débits plus précis et les doses d'insecticide à épaldre plui aàéquates.Selon un des participants, des contacts intéressants avaient été pris avec le responsable régional de l'hydrologie qui s'était proposé de satisfaire nos requêtès moyennant un perdie,rpour son personnel. Un rapport dans ce sens a été adressé au responsable du projet. Nous souhaitons que ces travaux soient réatisés dans les plus bref délais. Divers En attendant I'arrivée des techniciens OCP, I'accent sera mis sur les préparatifs de ta démonstration du traitement au sol, servant de lancement ofliciel de la campagne d'élimination du vecteur de I'onchocercose dans le foyer de Tuku5ru, dans le caaie-au séminaire national de sensibilisation et l'acquisition du matériet complémentaire indispensable. 6.Le séminaire national de sensibilisation <<26 au28llll200l » La participation de I'OCP a porté sur les sujets ci après Control / ecological monitoring in West African OCP Countries (présenté par Dr Bissan) puis la démonstration du traitement au sol (M BARRO TELE et I'équipé de IaNMR). La préoccupation majeure des participants à ce séminaire était le yolet environnemental. Ils furent apaisés par la communication du Dr Bissan. Les travaux en commission adoptèrent I'utilisation du téméphos comme larvicide du foyer. La démonstration de I'épandage au sol du téméphos a été faite le 2811L12002 en présence de tous les participar-rts au sérninaire et Ie District Commissiorurer de Tukuyu. Une introduction a été îaite par le coordonnateur du projet le Dr Maegga, les explications tecturiques par l'expert OCP en la matière puis sous [a direction de I'expeft, la pratique de l'épandage par les techniciens nationaux. Nous réferant aux commentaires de quelques specialistes en pcsticide participant au séminaire, cette démonstration les a convaincus de la faisabilité du projet mais aussi de l'impact mineure du téméphos sur la faune aquatique non cible eu égard à la quantité du produit par rapport au débit, disent-ils. 7. L'équipe A qui assurera les épandages au niveau des affluents rive droite, du cours aval et supérieur de la rivière Kiwira, et compte tenu de la nature du terrain utilisera Ia plus neuve des deux Toyota Hilux. L'équipe B se servira de la voiture Vitara pour couvrir leur circuit (moyen kiwira et le bassin de la R. Mbaka). Les techniciens en charge du circuit C seront basés à ttete. Ils y seront transpoftés a'ecdeux motos qui leur serviront de moyen de déplacement pour couvrir tout le circuit. Les contrôles larvaires Méthodologie Nous avons opté tant que faire se peut, pour une inspection des gîtes avant et aprèséparrdage du larvicide au niveau de chaque point d'application. De" plus, une équipeindépendante specialement affectée était chargée de l'évaluation générale dls haitements sur une base hebdomadaire. a) Sur les rivières principales Résultats excellents aucun gîte visité n'a été trouvé positif après la première semaine detraiternent. b) Sur les affluents Résultats également satisfaisants, nous avons noté une absence totale de nymphe de S.damnosum sl au niveau de tous les gîtes inspectés avant et après chaque traiteme,t. Cependant quelques points ont été trouvés positifs sur le Lu 300 (kasyaboïa affluent duLufilyo) à hedikota à la semaine 04 et sur Mb 300 (affluent du Mbaka) a kitoUa de la semaiue2 àla semaine 4. L'absence totale de nymphe indique une excellente efficacité du téméphos. Les lan esde simulie (notamment des autres espèces) occasionnellement trouvées pounaient provenir des gîtes en amont des premiers points d'application ou des affluents inaccessibles décroche,t arrivent par dérive sur les biefs traités. Ces dérives sont certainement provoquées par de forts courant suite aux pluies par moment importantes en cette saison- L2.3.2. Les caotures dlarfurltes Méthodologie Les captures de S. damnosum pour évaluer la densité de la population simulidienne dufoyer sont effectuées depuis plusieurs aunées.
