HUMAN ANTHRAX U n it ed Sta tes o f A m erica . — On 28 December 1973, a 22-yearold woman, a Navy journalist-photographer assigned to a hospital ship, noted irritation in her left eye. She attributed this to her contact lens. Upon waking the next morning, she had painless oedema o f the upper left eyelid. During the next 24 hours, the swelling increased, and the lid became slightly inflamed. By 30 December, her eye was completely closed due to the swelling, which now extended laterally to the side o f her face and to her forehead. She went to a naval station dispensary where she was diagnosed as having severe conjunctivitis and treated with hot and cold packs as well as a topical antibiotic preparation. By that evening, the swelling had increased considerably, how­ ever, and she began experiencing pain around her eye, which prompted her to return to the dispensary for further evaluation. An aspirant of the upper eyelid was gram-stained and cultured, and she was given penicillin as therapy. The gram-stained smear contained encapsulated gram-positive bacilli. Bacillus anthracis was isolated from the culture.

CHARBON HUMAIN E tats-U nis d ’Amérique. — Le 28 décembre 1973, une journa­ liste-photographe de la marine, âgée de 22 ans, qui travaillait à bord d ’un navire-hôpital, constata qu’elle avait l’œil gauche irrité, ce qu’elle attribua à ses verres de contact. Le lendemain matin, à son réveil, elle avait un œdème indolore à la paupière supérieure gauche. Au cours des 24 heures suivantes, le gonflement s’accentua et une légère inflammation gagna la paupière. Le 30 décembre, l’œil était entièrement fermé par la tuméfaction qui s’étendait latérale­ ment jusqu’au côté de la face et au front. La malade se rendit alors au dispensaire d ’une station navale où l’on diagnostiqua une conjonctivite grave qui fut traitée avec des compresses chaudes et froides et des application topiques d’antibiotique. Le soir, le gonfle­ ment avait beaucoup augmenté et la malade commença à éprouver une douleur autour de l’œil qui l’incita à retourner au dispensaire pour un examen plus approfondi. Un prélèvement effectué par aspiration au niveau de la paupière supérieure fut coloré au gram et mis en culture tandis que la malade était traitée à la pénicilline. Le frottis coloré au gram contenait des bacilles gram-positifs encap­ sulés que l’on identifia comme étant Bacillus anthracis. Le 31 décembre, une aggravation de l’œdème facial et de la douleur motivèrent l’hospitalisation dans un centre médical régio­ nal de la marine. La paupière supérieure avait alors pris une teinte bleu-noir. A son arrivée à l’hôpital, la malade avait une tempéra­ ture de 38,3° C et une numération leucocytaire de 7 500 avec une formule normale. Malgré l ’administration de fortes doses de pénicil­ line, le gonflement augmenta, causant une gêne considérable, et la malade suivit alors un traitement de courte durée aux corticostéroïdes. Elle est maintenant guérie de son infection mais incapable de fermer complètement l’œil gauche en raison d ’une cicatrice résiduelle. L ’enquête a permis d ’établir que la malade avait pris place le 8 novembre à bord du navire-hôpital qui se trouvait alors sur le canal de Panama et faisait route vers Haïti. Entre le 13 novembre et le 14 décembre, alors que le navire était à quai à Port-au-Prince, elle s’acquitta de ses fonctions à bord, se promenant à terre pendant ses heures de repos. Elle acheta dans la ville sept tambours de bois (six bongos et un congo) dont les fonds en peaux de chèvre conser­ vaient une frange de poils intacts. Pendant cette période, le per­ sonnel du bord traita environ 40 cas de charbon chez des habitants d ’Haïti avec lesquels la malade n ’eut aucun contact.

On 31 December, the woman was hospitalized at a naval regional medical center because of increasing facial oedema and pain. By this time, the upper eyelid had a bluish-black hue. On admis­ sion to the hospital, the patient had a temperature of 38.3° C and had a white blood cell count of 7 500 with a normal differential. Although she was treated with large doses of penicillin, the swelling continued, causing considerable discomfort, and she was given a short course of corticosteroid therapy. She has since recovered from her infection but is unable to completely close her left eye­ lids due to residual scarring. Investigation revealed that on 8 November, the patient boarded the hospital ship, which was enroute to Haiti, at the Panama Canal. Between 13 November and 14 December, while the ship was docked a t Port-au-Prince, she carried out her responsibilities on the ship, sightseeing in her free time. In Port-au-Prince, she bought seven wooden drums which had goat hide drumheads with a fringe of hair intact. Six of the drums were bongo drums, and the seventh was a larger congo drum. During this time period, personnel on the hospital ship treated about 40 cases of anthrax in residents o f Haiti. The patient had no contact with these cases.

The hospital ship arrived at a Florida naval station on IS De­ cember. During the last week in December, the patient giftwrapped and mailed three drums to her parents in Louisiana and two drums to other friends in Michigan. These five drums as well as two drums kept by the patient were sent to the Center for Disease Control (CDC) for culture. B. anthracis was isolated from one of the two drums sent to Michigan, one of the three drums sent to Louisiana, and one of the two drums still remaining in Florida. One hundred and thirty items recently imported from Haiti made in whole or part of goat skin have had a preliminary screening at CDC for contamination with B. anthracis. Six of 124 drums, two of two rugs, zero of two purses, and zero o f two mosaic pic­ tures were contaminated. Because examination of these items is still incomplete, the true contamination rate may be higher. Two rugs examined were contaminated with B. anthracis, even though they appeared to have undergone some type of tanning process. All necessary control measures have been taken, and recom­ mendations for appropriate disposal of items suspected to be con­ taminated with B. anthracis have been sent to state health depart­ ments and are also available from CDC.

Cent trente articles récemment importés d ’Haïti et faits entière­ ment ou en partie de peau de chèvre ont été soumis au C N C à des examens préliminaires visant à établir la présence de B. anthra­ cis. Six tambours sur 124, deux carpettes sur deux, zéro portemonnaie sur deux et zéro ouvrage de mosaïque sur deux étaient contaminés. Les examens n ’étant pas encore achevés, il se pourrait que le taux réel de contamination soit plus élevé. Deux des carpettes examinées étaient contaminées par B. anthra­ cis, alors même qu’elles semblaient avoir subi une sorte de tannage. Toutes les précautions nécessaires ont été prises et des recommanda­ tions sur la manière de se débarrasser d ’articles qui seraient suspects de contamination par B. anthracis ont été adressées à tous les Départements de la Santé des Etats et peuvent être obtenues auprès du CJ9C. N ote de la Rédaction : Le charbon est endémique en Haïti mais on en ignore l’incidence. L’Organisation panaméricaine de la Santé a reçu des informations sur 387 cas de charbon humain enregistrés, en 1973, dans la région de Les Ceyes. Comme indiqué plus haut, le personnel du bord avait traité environ 40 cas de charbon humain pendant le séjour du navire à Port-au-Prince. E ditorial N ote : Although the incidence is unknown, anthrax is endemic in Haiti. The Pan American Health Organization received data on 387 human anthrax cases in the area of Les Ceyes in 1973. As noted, personnel of the hospital ship had treated about 40 cases of human anthrax while the ship was berthed in Port-au-Prince. These data suggest that both human and animal anthrax infections are not uncommon in Haiti.
