As a result of high coverage with childhood vac­ cines, the reported incidence of EPI diseases declined con­ siderably in the 1980s.1 In recent years, however, the rate of decrease of measles and diphtheria has slowed down, and outbreaks o f poliomyelitis were reported in 1989 and 1990 (.Fig. 1). T etanus toxoid has not been routinely adm inistered and control o f neonatal tetanus (N T) in China is based on the “3Cs”: d ean hands during delivery; d ean cutting o f die umbilical cord; and d ean perineum . A nationwide pro­ gramme to prom ote m odem delivery practices has been im plem ented since the 1950s w ith die following main com­ ponents: training the traditional birth attendants (TBAs) w ith emphasis on the “3Cs”, equipping the TBAs w ith a simple kit, educating the public and increasing die commu­ nity’s perception o f the N T problem .1 In the early 1980s, it was believed th at the N T m ortality rate did no t exceed 1 per 1 000 live births, and die total num ber o f neonates dying each year from N T was between 10 000 and 20 000. Recently, however, several studies showed that the inddence o f N T cases and deaths due to N T were higher in China than previously thought. Retrospective sample surveys were undertaken in 9 provinces in 1986. T he average neonatal and infant mor­ tality rates observed were 22 and 34 per 1 000 live births, respectively (Table 1). T he average N T mortality found in urban areas was 1.7 per 1 000 live births and in rural areas 4.8 per 1 000 live births. T he weighted average for urban and rural areas was 4.2 per 1 000 live births. Among neo­ nates in both urban and rural areas n e onatal tetanus was the second m ost frequent cause o f death (19%) after pre­ m aturity.

U n programme national privilégiant les pratiques d’accouchement modernes est mis en œuvre depuis les années 50; ses principales composantes sont les suivantes: form er les accoucheuses traditionnelles en insistant sur le triple principe de propreté, équiper les accoucheuses traditionnelles de trousses simples, éduquer le public et sensibiliser la communauté au problème du tétanos néo­ natal.1 Au début des années 80, on pensait que le taux de m ortalité par tétanos néonatal ne dépassait pas 1 pour 1 000 naissances vivantes et que le nom bre total des nouveau-nés qui m ouraient chaque année des suites du tétanos néonatal se situait entre 10 000 et 20 000. O r plusieurs études récentes ont m ontré que l’inadence des cas de tétanos néonatal et des décès dus au tétanos néonatal était plus élevée en Chine qu’on ne le pensait auparavant. D es enquêtes rétrospectives par sondage ont été effectuées dans 9 provinces en 1986. Elles ont fait apparaître des taux moyens de m ortalité néonatale et in fan tile de 22 et 34 pour 1 000 naissances vivantes respectivem ent (Tableau ï). Le taux moyen de m ortalité par tétanos néonatal était de 1,7 pour 1 000 naissances vivantes en zone urbaine et de 4,8 pour 1 000 naissances vivantes en zone rurale. La moyenne pondérée pour les zones urbaines et les zones rurales était de 4,2 pour 1 000 naissances vivantes. Le tétanos néonatal était la deuxième cause de décès (19% ), après la prém aturité, chez les nouveau-nés en zone urbaine et en zone rurale.

A project initiated in 1990 by the M aternal and Child H ealth D epartm ent of the M inistry o f Public H ealth and supported by U N IC EF and the U nited N ations Fund for Population Activities (UNFPA) seeks to strengthen the quality of health care at the grassroots level and to reduce infant and child mortality. T he project covers a population of 120 million people in 300 selected counties (10% of all counties). T he selection criteria included high infant mor­ tality and birth rates and low per capita income. T he num ­ ber of counties selected for each province ranged from 3 to 20. A baseline survey for N T mortality was conducted in each selected county. Townships were the unit of sampling and typically 5 or 6 townships out o f 20 or so in a county were chosen. W ithin each township selected, in each village the village leader and health workers listed all births, includ­ ing unregistered ones, that had occurred in the previous year (1989), and separately registered all infants and 1-4-year-old child deaths. T he causes of infant deaths recorded included: asphyxia/birth traum a, neonatal tetanus, pneum onia, diar­ rhoea, prem aturity, injury/accident, and others. T he provincial figures from the baseline survey are not necessarily indicative of the whole province, and are expected to be higher than the average for the whole province because o f selection o f the areas at high risk. U n projet mis sur pied en 1990 par le Département de la Santé maternelle et infantile du M inistère de la Santé publique avec l’appui de FUNICEF et du Fonds des N ations Unies pour la Population (FNUAP) vise à renforcer la qualité des soins de santé au niveau de la base et à réduire la mortalité infanto-juvénile. Le projet couvre 120 millions d ’habitants dans 300 districts (10% de l’ensemble des districts). Les critères de sélection étaient les suivants: taux élevé de mortalité infantile et de natalité et faible revenu par habitant. Le nombre des districts choisis dans chaque province allait de 3 à 20. Une enquête de base sur la mortalité a été réalisée dans chacun des districts choisis. Les communes consum aient l’unité d’échan­ tillonnage et 5 ou 6 communes sur une vingtaine ont été choisies par district. Dans chaque commune choisie, le chef du village et les agents de santé de chaque village ont dressé la liste de toutes les naissances, y compris les naissances non enregistrées, survenues au cours de l’année précédente (1989) et ils ont enregistré séparément tous les décès de nourrissons et d’enfants de 1 à 4 ans. Les causes de décès des nouveau-nés signalées étaient les suivantes: asphyxie/tiaumatisme au cours de l’accouchement, tétanos néonatal, pneumonie, diarrhée, prém aturité, traum atism e/accident, et autres. Les chiffres provenant de l'enquête de base concernant les pro­ vinces ne sont pas nécessairement représentatifs de toute la province et il est prévisible qu’ils dépassent la moyenne de l’ensemble de la province en raison de la sélection de zones à haut risque. ‘ Voir N ” 2, 1985, pp 5-6

Table 2 and Fig. 2 show the estim ated neonatal and N T m ortality rates by province. Between individual provinces, neonatal m ortality rates range from 28.8 to 71.5 per 1000 live births. As regards N T m ortality rates, the range was m uch wider, from less than 1 per 1 000 live births in Liaoning, Jiangsu and Xizang Provinces to about 18 per 1 000 live births in H ainan and Gizhou Provinces. In the latter provinces, N T represented 27% -46% o f all neonatal mortality. T he average N T m ortality rate for 300 counties was 6.1 per 1 000 live births.

M ap 1 shows the location of provinces with an estimated N T m ortality rates of 4 or m ore per 1 000 live births. AU these provinces are grouped in the central southern part of the country. W ith the assum ption th a t the national N T mortality rate is 4.2 per 1 000 Uve births (1.7 in urban areas and 4.8 per 1 000 Uve births in rural areas), one may estim ate that about 90 000 deaths due to N T occur each year in China (10 000 in urban areas and 80 000 in rural areas). In 1992, the Chinese health authorities introduced teta­ nus immunization of women o f child-bearing age in the selected 300 counties. T he imm unization schedule includes 3 doses of tetanus toxoid. T he first 2 doses are given at intervals of 4 weeks and the third dose at least 6 m onths after the second dose. Further expansion of the tetanus toxoid im m unization is planned. Potential years of life lost before age 65, 1990 and 1991 U n ited S ta te s o f A m erica. Potential years o f life lost (PYLL) are a public health measure th at reflects the im pact of deaths occurring in years preceding a conventional cut-off year o f age, usually 65 years. PYLL are calculated using final mortality data from the N ational C enter for H ealth Statis­ tics of the Centers for Disease Control and Prevention (CDC) for the m ost recent year available, provisional mor­ tality data (i.e., a 10% sample o f deaths) for the following year, and population estimates from the U nited States Cen­ sus. T his report summarizes final PYLL data for 1990 and provisional data for 1991. D uring 1990, potential years o f life lost before 65 years (PYLL-65) totalled 12 237 379 in the U nited States (Table 1). U nintentional injuries accounted for die largest proportion of PYLL-65 from all causes (17.5% ), followed by malignant neoplasms (15.1% ), suiride/homicide (12.2% ), diseases o f the heart (11.2%)', congenital anomalies (5.4% ), and human im m n n o defirie n ry virus infection including ac­ quired immunodeficiency syndrome (HTV7AIDS) (5.4%). Les années potentielles de vie perdues (APVP) sont une mesure utilisée en santé publique pour déterminer l’im portance des décès survenant au cours des années précédant un âge lim ite conventionnel, habituellem ent 65 ans. HTV/AIDS, which accounted for 6.3% o f all PYLL-65, replaced congenital anomalies as the fifth leading cause of PYLL-65. M M W R E d ito ria l N ote: Leading causes of death in the U nited States are ranked by using absolute counts of death for selected causes, thus giving each death a weight of 1.0. In comparison, PYLL gives a weight to each death propor­ tionate to its distance from the arbitrarily designated age of 65 years. PYLL-65 emphasizes deaths at early ages in 2 ways: 1) by not including deaths occurring at ages beyond the cut-off, and 2) by giving greater com putational weight to deaths among younger persons. Provisional m ortality estimates for selected conditions are based on a 10% sample of death certificates and are adjusted for reporting biases (e.g., provisional reporting of cause o f death). Because 1991 data are provisional, PYLL estimates based on 1990 final mortality data are not com­ pared with 1991 data. T he 12.7% increase in PYLL-65 for HTV/AIDS corres­ ponds to increases in the annual num ber' of deaths from AIDS. Prevention programmes for communities and indi­ viduals are crucial for reducing behaviours that lead to transm ission of HIV. T he 6.5% increase in PYLLr65 for suicide/homicide reflects, 1.8% and 10.8% increases in PYLL-65 for suicide and hom icide, respectively. Several factors may have contributed to these changes, including increases in substance abuse, access to handguns, poverty, urbanization and crowding, and family disruption and disor­ ganization. For prevention of suicide/homicide, CD C has recom m ended interventions that can be incorporated in community programmes. These include use of school cur­ ricula based on nonviolent conflict-resolution skills; peer­ counselling programmes; enforcem ent or enactm ent o f local drinking and firearm s-control regulations; crisis-interven­ tion services; and improved recognition and comprehensive treatm ent o f persons with m ental disorders. T he 9.2% decrease in PYLL-65 for prem aturity from 1989 to 1990 reflects a 1.2% increase in PYLL-65 for disorders related to short gestation and unspecified low birfhweight and a 21.5% decrease in PYLL-65 for respira­ tory distress syndrome. Recent improvements in medical management o f respiratory distress syndrome may have contributed to this trend. Plusieurs facteurs peuvent avoir contribué à cette évolu­ tion, dont la progression des toxicomanies, l’accès aux armes à feu, la pauvreté, l’urbanisation et le surpeuplem ent, ainsi que la dés­ union et la désintégration des familles. Pour prévenir les suicides/ homicides, les CD C ont recommandé des interventions pouvant être intégrées dans des programmes com m unautaires, tels que: pro­ grammes scolaires enseignant à régler les conflits par des moyens non violents; programmes de conseil par des pairs; application ou prom ulgation des réglementations locales concernant, la boisson et les armes à feu; services d’intervention en cas de crise; et m eilleur dépistage et traitem ent plus exhaustif des personnes présentant des troubles mentaux. La dim inution de 9,2% des APVP pour la prém aturité entre 1989 et 1990 se décompose en une augm entation de 1,2% pour les troubles liés à une période de gestation écourtée et à un faible poids de naissance (sans précision) et une dim inution de 21,5% pour le syndrome de détresse respiratoire. Les récentes améliorations du traitem ent médical du syndrome de détresse respiratoire peuvent avoir contribué à cette évolution.
