BOTULISM SURVEILLANCE — In March 1983,2 cases of botulism were reported after consumption of potted sandwich paste. The first case, a man aged 33 years, showed signs of indigestion 48 hours after ingesting the sand­ wich paste ; 4 days later he developed diplopia and difficulty in swal­ lowing supervened. His 7-year-old daughter also developed diarrhoea and diplopia after 5 days. Those cases were mild and no specific treatment was required. This paste, prepared from various vegetable substances, had been autoclaved under conditions such that proper sterilization could not be guaranteed. The autoclaves were not equipped with any recording B e l g iu m .

SURVEILLANCE DU BOTULISME — En mars 1983, 2 cas de botulisme ont été signalés après la consommation de pâte à tartiner en conserve. Le premier cas, un homme de 33 ans, présentait des signes d’indigestion 48 h. après ingestion de la pâte à tartiner; 4 jours plus tard venaient s’ajouter une diplopie et des difficultés de déglutition. Sa fille de 7 ans a également été atteinte de diarrhée et de diplopie au bout de 5 jours. Les 2 cas étant bénins, ils n’ont nécessité aucun traitement spécifique. Cette pâte, préparée à partir de différentes substances végétales, avait été traitée à l’autoclave dans des conditions qui ne permettaient pas de garantir une stérilisation adéquate. device. The temperature of 115 "C, which was to be maintained for 1 hour, was measured by a thermometer inserted into the wall of the autoclave. Inside the jars, therefore, the required temperature was not reached until much later, which could have led to under-stenlization. Furthermore, the cooling was done with water from a shallow well of doubtful potability and this could have caused subsequent contam­ ination of some improperly closed jars. Serological examination o f the patients and analysis of the incrimi­ nated jar showed that type B botulism was mvolved. Other samples were analyzed and the same toxin detected in a jar fom a different batch. The firm has recalled its entire output from the market

n’étaient pourvus d’aucun appareillage d'enregistrement. La tempé­ rature de 115 ‘C, qui devait être maintenue pendant 1 h., était mesurée au moyen d’un thermomètre, inséré dans la paroi de l’autoclave. A l'in teneur des bocaux la température requise n’était donc atteinte que bien plus tard, ce qui aurait pu entraîner une sous-stérilisation. Par ailleurs, le refroidissement était réalisé à l’aide d’eau d'un puits peu profond et de potabihté douteuse ce qui aurait pu causer une post­ contamination de certains bocaux mal fermés. L’examen sérologique des patients et l’analyse du bocal incriminé ont démontré qu’il s’agissait de botulisme du type B. L’analyse d’autres échantillons a permis de découvrir la même toxine dans un bocal d’un lot différent. La firme a retiré toute la production du commerce.
