Leptospirosis in Scotland, 1 9 6 7 -1 9 9 0 U n ited K ingdom (S co tla n d ). Over the 24-year period, a total o f 179 cases were reported to the Communicable D iseases (Scotland) U nit, with a mean o f 7.4 and a range o f 1-15 infections per year. There were 2 reported fatalities: 1 due to infection with the serogroup Icterokaemorrhagiae and the other w ith Leptospira spp. Three o f the cases were acquired abroad. A cyclical pattern was evident, the infection rising steadily to reach a peak in the early 1970s, followed by a fall in incidence over the next decade, and then another rise to 1990 (Fig. 1). In addition, a seasonal pattern was noted, infections rising in incidence from June to reach a maxi­ mum in Decem ber. T he majority o f cases were in the age group 15-65 years, with an overall m ale-to-fem ale ratio o f 6.4 to 1.

L’infection a manifesté une tendance cyclique évidente, pro­ gressant régulièrement pour atteindre un pic au début des années 70; l’incidence a ensuite reculé au cours de la décennie suivante avec une nouvelle progression jusqu’en 1990 (Fig. I). En outre, on a noté des variations saisonnières, l’incidence des infections aug­ mentant à partir de juin pour atteindre un maximum en décembre. La majorité des malades appartenait au groupe d’âge 15-65 ans avec une proportion de 6,4 homm es pour 1 fem me.

Only 58% o f isolates (104) were serogrouped. O f these Sejroe serovar hardjo was the m ost frequently reported (45% ). Camcola accounted for 27% , Icterokaemorrhagiae for 23% and other identified serogroups for 5% o f the isolates (Table 1).

The epidemiological pattern of leptospirosis has changed. Measures such as modem pest control, the wearing of protective clothing and the presence of deter­ gents in waste waters have contributed to a marked decline in acquired occupational infections in sewermen, miners and water workers. Today in Scotland the main occupational groups at risk from leptospirosis are farmers and agricultural workers. There has not yet been a rise in infections associated with recreational activities, which to date account for only 2% of cases (Table 2). L’épidémiologie de la leptospirose a évolué. Grâce à des mesures telles que la destruction des nuisibles par des moyens modernes, le port de vêtements protecteurs, et aussi en raison de la présence de détergents dans les eaux usées, les infections d’origine profession­ nelle qui frappaient les égoutiers, les mineurs et les employés des services des eaux ont notablement régressé. En Ecosse, les principaux groupes professionnels exposés au risque sont aujourd’hui les cultivateurs et les ouvriers agricoles. On n’a pas encore enregistré d’augmentation dans les infections qui soit attribuable aux activités de loisirs: celles-ci ne sont actuellement res­ ponsables que de 2% des cas (Tableau 1).
