New technologies for tuberculosis control: the Stop TB Partnership’s Task Force on Retooling Tuberculosis (TB) is a global problem that killed 1.6million people in 2005. The mounting problem of drug resistance, including the emergence of multidrugresistant TB (MDR-TB) and extensively drug-resistant TB (known as XDR-TB), coupled with the growing number of TB patients who are also infected with HIV are combining to make the pandemic more deadly. These problems risk compromising the progress made in TB control during the past decade. Beyond the current efforts to prevent, detect and cure TB, new tools are needed to radically transform the fight against TB, to contain the threat of deadly drug-resistant strains and to reach the elimination target by 2050. New technologies to control TB are urgently needed for several reasons.

Le problème croissant de la résistance aux médicaments, et notamment l’émergence de la tuberculose à bacilles multirésistants (tuberculose MR) et à bacilles ultrarésistants (tuberculose UR), associé au nombre croissant de malades de la tuberculose qui sont également infectés par le VIH rend la pandémie d’autant plus mortelle. Ces problèmes risquent de remettre en cause les progrès de la dernière décennie en matière de lutte antituberculeuse. Au-delà des efforts consentis pour prévenir, détecter et soigner la tuberculose, de nouveaux outils sont nécessaires pour transformer radicalement la lutte, endiguer la menace des souches mortelles résistantes aux médicaments et atteindre le but de l’élimination de la maladie d’ici 2050. De nouvelles technologies s’imposent d’urgence contre la tuberculose pour plusieurs raisons.

against disseminated forms of disease in infants and little, if any, protection beyond childhood. Progress is being made in the development of new medicines, diagnostics and vaccines to combat the TB pandemic and eventually eliminate this disease. The Global Plan to Stop TB 2006–2015, which is underpinned by WHO’s new Stop TB Strategy, estimated that in 2006 there were 27 medicines, 15 diagnostics and 8 vaccines in the production pipeline at various stages, ranging from product development to field trials. Since publication of the Global Plan in 2006, the number of candidate technologies has increased. With the anticipated launch of the first of the new diagnostic tools occurring within the next 2 years, the time is right to start preparing to use such tools in order to minimize the delay between licensure, availability and adoption. The goal is to spread rapidly and widely the use of new tools to care for the people that will benefit from them most. This process is termed “retooling”.

Le Plan mondial Halte à la tuberculose 20062015, qui repose sur la nouvelle stratégie Halte à la tuberculose de l’OMS, estimait qu’on disposait en 2006 de 27 médicaments, 15 outils diagnostiques et 8 vaccins à divers stades de la mise au point, allant du développement du produit aux essais de terrain. Depuis la publication du Plan mondial en 2006, le nombre des produits candidats s’est accru. Avec le lancement des premiers nouveaux outils diagnostiques prévu ces 2 prochaines années, il est temps de commencer à préparer leur introduction et de réduire autant que possible le décalage entre l’homologation, la mise à disposition et l’adoption. Il s’agit de généraliser rapidement l’utilisation des nouveaux outils pour soigner ceux qui sont susceptibles d’en bénéficier le plus. C’est cela qu’on entend par «renouvellement des outils».

The promise of new tools The introduction of new tools for TB control and prevention should be regarded as a means of improving the quality of care by making available a wider choice of technologies to address unmet needs; it is also an opportunity to align the new tools with the capacity of health systems to deliver care, to address the changing nature of the epidemic and to meet the needs of people with or at risk of contracting TB. The plans and investments made thus far by the global partnership to Stop TB to accelerate the development of new tools have led to high expectations that these technologies will provide national TB control programmes with better options to prevent, detect and treat TB. At the same time, making new tools available and accessible will require global-level and country-level systems and procedures that can rapidly and effectively assess these products and incorporate them into TB control strategies and programmes where appropriate.

Perspectives offertes par les nouveaux outils L’introduction de nouveaux outils de prévention et de lutte doit être considérée comme un moyen d’améliorer la qualité des soins en offrant un éventail de choix technologiques plus large pour répondre aux besoins insatisfaits; elle donne aussi l’occasion d’aligner les nouveaux outils sur la capacité des systèmes de santé à apporter les soins, de suivre l’évolution de la nature de l’épidémie et de répondre aux besoins des gens qui sont atteints par la tuberculose ou qui risquent de la contracter. Les plans établis et les investissements consentis jusqu’ici par le Partenariat mondial Halte à la tuberculose pour accélérer la mise au point de nouveaux outils ont donné lieu à de fortes attentes quant aux possibilités de déboucher grâce à ces technologies sur des programmes nationaux de lutte antituberculeuse offrant de meilleures options en matière de prévention, de détection et de traitement de la tuberculose. Parallèlement, pour que de nouveaux outils deviennent disponibles et accessibles, il faudra des systèmes et des procédures aux niveaux mondial et national permettant d’évaluer ces produits rapidement et de façon satisfaisante et de les incorporer le cas échéant aux stratégies et aux programmes de lutte.

The Task Force on Retooling Previous experiences with the introduction of new tools to prevent and control other communicable diseases have shown that there is often a significant delay between the availability of new tools at the global level and their eventual adoption and implementation at country level. Recognition of the critical delay that often occurs between the time the evidence for policy-making becomes available and the implementation of policy led the Stop TB Partnership Coordinating Board to establish a Task Force on Retooling. This task force is charged with developing a framework that will encourage policy-makers and practitioners at global and national levels to accelerate the introduction of new tools into national TB control and immunization programmes. One of the aims of the task force is to stimulate discussion of, and planning for, optimal, timely and appropriate introduction, adoption and implementation of new tools as they become available. The main activities of the task force are:

Le Groupe spécial sur le renouvellement des outils L’expérience de l’introduction de nouveaux outils destinés à prévenir et combattre d’autres maladies transmissibles a montré qu’il y a souvent un décalage important entre le moment où les nouveaux outils deviennent disponibles au niveau mondial et le moment où ils sont adoptés et mis en œuvre au niveau des pays. C’est l’existence d’un décalage critique qui sépare souvent l’obtention des éléments nécessaires à l’élaboration d’une politique et sa mise en place qui a amené le Conseil de coordination du Partenariat Halte à la tuberculose à constituer un groupe spécial sur le renouvellement des outils chargé d’élaborer un cadre pour encourager les responsables politiques et les praticiens aux niveaux mondial et national à accélérer l’introduction de nouveaux outils dans les programmes nationaux de lutte antituberculeuse et de vaccination. Il a entre autres pour objectif de stimuler le débat et la planification afin de parvenir à une intégration, à une adoption et à une mise en œuvre optimales, rapides et appropriées des nouveaux outils à mesure qu’ils deviennent disponibles. Les principales activités du Groupe spécial sont les suivantes:

on the expansion of the DOTS strategy, MDR-TB and TB/HIV to facilitate the adoption and implementation of new TB tools in the field; stakeholders from countries with a high burden of TB, including ministries of health, nongovernmental organizations, affected communities, etc; facilitating the mobilization of financial and human resources for country-level introduction and deployment; consolidating relevant lessons learnt from other disease areas to inform TB-specific processes for adoption, introduction and implementation; facilitating operational research on the introduction of new tools; generating evidence to support the adoption of new tools; fast-tracking the incorporation of effective new tools into WHO’s policies and national policies and guidelines; enhancing communication among all working groups about retooling.

Framework for the adoption, introduction and implementation of new tools One of the first jobs of the task force was to develop a framework for the adoption, introduction and implementation of new tools. This framework provides guidance on what actions need to be taken when improvements in existing medicines, diagnostics and vaccines become available or when new tools become available. The framework is primarily intended to support national TB and immunization programmes, and clinical laboratory diagnostic services. It is also meant to inform the activities of the constituencies of the Stop TB Partnership, including advocacy and community-based organizations, donors, intergovernmental agencies, new product developers, national policy-makers and decision-makers, and academic and technical partners. The framework identifies the challenges inherent in retooling and proposes key steps to be taken in order to facilitate appropriate and timely adoption and implementation. It also provides an overview of the technical and operational considerations associated with retooling at the global and national levels. Additionally, the annexes summarize technologies in the development pipeline. They also provide an illustrative list of the key actions that need to be taken so that new tools in each technology category can be adopted and their use implemented effectively. There is also an illustrative generic timeline – or sequencing – of key tasks needed for adoption, introduction and implementation. A list of suggested further reading is also provided.

Cadre pour l’adoption, l’intégration et la mise en œuvre des nouveaux outils L’une des premières tâches du Groupe spécial a consisté à élaborer un cadre pour l’adoption, l’intégration et la mise en œuvre des nouveaux outils. Ce cadre recommande les mesures à prendre lorsque des améliorations sont apportées aux médicaments, produits diagnostiques et vaccins qui existent déjà ou que de nouveaux outils deviennent disponibles. Le cadre vise avant tout à appuyer les programmes de lutte antituberculeuse et de vaccination nationaux et les services de diagnostic clinique au laboratoire. Il doit aussi servir aux activités des parties prenantes du Partenariat Halte à la tuberculose, notamment les organisations de sensibilisation et les organisations communautaires, les donateurs, les organismes intergouvernementaux, ceux qui mettent au point de nouveaux produits, les responsables politiques et décideurs nationaux ainsi que les partenaires scientifiques et techniques. Le cadre précise les enjeux du renouvellement et propose des démarches essentielles pour faciliter une adoption et une mise en œuvre appropriées et rapides. Il donne également un aperçu des considérations techniques et opérationnelles du renouvellement aux niveaux mondial et national. Par ailleurs, les annexes résument les technologies en cours de mise au point, et donnent à titre d’exemple une liste des principales mesures qui doivent être prises pour que les nouveaux outils dans chaque catégorie technologique puissent être adoptés et utilisés de manière efficace. Elles définissent aussi un calendrier – ou une séquence – type des tâches à accomplir pour l’adoption, l’intégration et la mise en œuvre. Future work of the task force In addition to the framework described above, the task force is engaging national and global policy-makers and stakeholders to ensure that they support retooling efforts. Documents that will be produced later in 2007 include a stakeholder engagement plan; detailed illustrative timelines for adoption and implementation of new diagnostics, and monitoring and evaluation indicators; and pipeline updates for medicines, diagnostics and vaccines.
