aware of the reported cases and has closely monitored their investigation by national authorities in the respective countries. WHO endorses the call for a renewed discussion of the safety of the yellow fever vaccine and the indications for its use. In particular, WHO recommends that travellers should be carefully assessed regarding their need for the vaccine and their personal level of risk. Although the exact frequency of serious adverse events following yellow fever vaccination is yet to be determined, information currently available suggests that they are extremely rare. An estimated 150 million doses of vaccine were administered worldwide over the past 4 years, of which 54 million in Brazil, where only 2 cases of serious adverse events were reported. Yellow fever remains a serious public health problem in Africa and South America and is a re-emerging disease which kills an estimated 30 000 people every year. It is also a major risk for travellers to endemic countries. The risk to unimmunized individuals either living in or travelling to areas where there is known yellow fever transmission is far greater than the risk of having a vaccine-related adverse event. Therefore the current WHO policy2 on yellow fever vaccination remains unchanged. However, there is a need to improve monitoring to detect potential severe adverse events and to quantify the actual rate of such events. Ⅲ 2

Notamment, lOMS recommande que soient soigneusement évalués le besoin de vaccination et le niveau personnel de risque des voyageurs. Bien que la fréquence exacte de manifestations postvaccinales graves suite à la vaccination antiamarile reste à déterminer, linformation actuellement disponible donnerait à penser quelles seraient extrêmement rares. On estime que 150 millions de doses de vaccin ont été administrées dans le monde ces 4 dernières années, dont 54 millions au Brésil, où seuls 2 cas de manifestations postvaccinales indésirables graves ont été signalés. La fièvre jaune reste un problème de santé publique sérieux en Afrique et en Amérique du Sud, étant une maladie réémergente qui tue daprès les estimations 30 000 personnes par an. Cest aussi un risque majeur pour les voyageurs en pays endémiques. Le risque pour les personnes non immunes qui vivent ou voyagent dans des zones où la transmission de la fièvre jaune est démontrée est beaucoup plus important que le risque de souffrir dune manifestation postvaccinale indésirable grave. De ce fait, la politique actuelle de lOMS concernant la vaccination antiamarile reste inchangée. Il est cependant nécessaire daméliorer la surveillance afin de dépister de potentielles manifestations postvaccinales indésirables graves et de quantifier le taux réel de telles manifestations. Ⅲ 2
