PLAN OF ACTION VECTOR COMROL The vector control prograune is based upon the aerial application of insecticide to blackfly breeding sites. An entomological evaluation nethrork has been established to provide weekly information concerning the fly population, river flow rates and the status of breeding sites. Weekly flight plans are formulated on the basis of information provided through this network. The Progranrne area remains divided into trro zones - east and Idest - with headquarters respectively in Tamale, Ghana and Bobo Dioulasso, Upper Vo1ta. During 1981, Ereatments of Phases I, II, III and IV will continue, Protecting a total ground area of 764 OOO square kilometres and 18 034 kllometres of river per treatment cycle during the wet season, and 8OL4 kilometres of river per treatment cycle during the dry season. Five sectors and 25 sub- sectors fielding a total of 93 vector collecting teams are planned for 1981. A total of 375 catching points are identified and categorized as requiring visits once per week (109 points); once every two weeks (228 points); or occasionally (38 points). During 1981, a reassessment of the location of catching points as well as the current organization of sectors and subsectors will be made to adaPt the net$lork of catching sites to the evolving entomological situation. The cdllection of baseline data on Phases I, II, III, IV and in the extension study zone wa8 complqted by the end of 1980, following the examination of some I-30 OOO persons La 4L2 vi1lages. Xwo types of examinations were performed: basic and detailed. The basic examination consislts of taking two skin snips to determine the prevalence of the disease by age and sex, and EhE intensity of infection; visual acuity is measured by a simple screening procedure. The detailed examination performed on about 20% of the persons surveyed, consists of a more speciff.c clinical exaninati-on, the recording of the number and distribution of nodules, theidentiEication and classlfication of skin lesions and a detailed ophthalmological examinatiorrfor a frecise diagnosis of ocular lesions and the ocular parasite load. _ The following table shows the number of villages visited for the first time by phase and yedr: Phase I Phase II Phase III Phase IV Extension studies Totals NO. For the global evaluati.on, 14O villages have been randomly selected from this group and it is these viLlages which have been retained for long-term fol1ow-up. Ihe epidemiological situation in L98O, excluding the reinvasion zones, is as folLows: The prevalence of onchocerciasis has decreased by LO-ZO% in all villages. The disease has become rare in persons under 9-1O years of age. The incidence is zero since 1977 -t978 . Ocular parasi.tism is decreasing. Existing lesions have stabiLized. The natural evolution towards the formation of irreversible lesions seems to have been halted and the incidence of blindness due to onchocerciasis is minimal. The adult worm populations are aging because they are not being renewed. In January 198O, it was observed that 387. of the rrrorms in subjects examined had died or were degenerating (Phase I, Upper VoLta). This situation has direct repercussions on the evaluation itself and on research. The traditional evaluation (skin snips) will continue in the 14O villages selected in 1978 which represent an adequate group for surveillance. The anti.cipated acceleration of the evolution of the medical situation strongly argues for a return to triennial visits. The risk of blindness due to onchocerciasis will also be studied in zones of reslflual transmission. Two supplementary components will be added to the surveillance activity: (1) The parasitological diagnosis lacks sensitivity when onchocerciasis infection is in its early stages or minimal, which is the most coutron situation with residual transmi.s- sion. Ihe Unit wil1, in accordance with therecomrendations of STAC,1 develoP amethod of iuurunological investigation to measure the presence of infection in the young age' groups: O-1O years at Present gradually rising to O-15 years. (2) Ttre analysis of the viability of adult r^rorms is becoming an indispensable comPlEment to the assessment of microfilarial densiti.es. Measurement of the rate at which adult worms are dying, and the speed at which the proportion of dead worms inside nodules ls increasing, will be a further means of evaluating the effectiveness of the Prograrmo. The operational phase of this activity will begin in 1981 lrith the identification of action zones, the number of annual campaigns possible, the togistic support to local health services and the definition of the role of the latter in the nodulectomy exerGlse. As regards diseases other than onchocerciasis, the Epidemiological Unit l^rill continuO locp1 studies such as the two polyparasltic surveys done io L979, and studies of the pregCnce of other filarial diseases in certain zones. ECOt{O}fi C DEI/EIOPMEI{T The Economic Development Unit will concentrate its activities in 1981 on the evaluatlon of lthe extent to which the control of onchocerciasis transmission has resulted in the respttlement of previously deserted lands, and on the assessment of the resultant econoulc andl social benefits. This evaluation will evolve in three stages: (i) a study of the effect of the vector control operations on the resettlemenE of people in potentially exploitable agricultural land; (ii) a study of the principal economic gains from the occupation of these lands; (iii) a study of the public health and socioeconomic effects of the population movemsnts within the OCP zone. During 1981 stage one wilL be implemented in aLl- OCP countries. The basic methodology willl consist of the interpretation of aerial and satellite photographs. Moreover, the high rate of spontaneous mortal-ity of adult worms would introduce such a bias into an evaluation of the effects of drugs that Ehe Programte area is no longer appropriate for chemotherapeutic trials. Therefore such trials are not among the main research themes which now include: - analysis of conputer stored data; fromnowonthis material is the only available utrich will perrrit the description of di-sease evolution under rrnon-contro1lr circtrmstances; - continuation of demographic observations already begun: the relationship between mortality and blindness; differential birth rates in endemic areas and possibLy in rel-ation to the effects of disease controL, etc.; - differentiation of parasite strai.ns (forest/savanna) by analysis of the iso-enz5mes of adult worms; - assessment of the danger from untreated areas in of the parasite strains, Vector ecology by infected migrants entering the Prograrme area countries (extent of reintroduction, pathogenicity represented neighbouring transmission by residual Simulturn) . tiorphological characters have been found for separating the six comrcn species of the S. damnosum complex into four groups. Investigations are continuing to deEermine whether these characters are consistent throughout the Prograrrme area. Having found a strain of S. soubrense nhich m4tes in captivity, efforts to establish colony of bLackflies wil-L be intensified. The need for an alternative to human bait for catching flies and assessing their density in a locality is as great as ever. Fleld studies have not achieved satisfactory results and it is intended to pursue laboratory tests to obtain a cLearer understanding of the reaction of the Slmulitrm darnnosum s.1. to different stimuli and the most suitable type of trapping device for different physiological stages. Studies wilL continue, particularly at the periphery of the Progranme area, to improve our knowledge of the behaviour of the flies and enable greater efficiency in control of the vector. In Parts of the Progranrme area about 357" of larvae morphologically indistinguishable from O. volvulus have been shown by histochemical staining to belong to another species. Efforts wi1l be made to develop a field technique for differentiating these larvae. Vector control ," "*-ance with the recormerrdatio.rl of the Scientific Advisory Panel Working Groupon Insecticides Ehich met in Geneva from 3 to 7 July 1978, increased emphasis is being placed on insecticide evaluation. Following a work plan developed in col-laboration with headquarters units, OCP has asstlned responsibility for short-tem oPerational research on insecticide evaluation, and large scale field trials. Ttre former involves the planning, suPervision, and evaluation of studles designed to determine minimum d.osages, dispersion and carrying characteri- stics, as well as iuurediate effects of new for:nuLations on non-target organisms. Latge scale fietd testing is to determine the effectiveness of candldate i.nsecticides on Simulium damnosum under operational conditions, as well as to measure the medium-term impact on target organisms. To carry out this work, the Progranme wiI1, in addition to maintaining contractual association with competent institutions, establish its orm unit with a fu11-time scientist, a hydrobiologist and supporting staff. For large scale testing, project aircraft will be used" L oefsracftS.l Annex Tr (ocpfswcf78.25). IocP/80.L page 1q r ' Should alternate fornulations require methods of application other than those currently used, diffeient spraying equipment will be developed. lnsecticide susceptibility testing will be undertaken on a regular basis by the Programle. Envir(nmenta 1 monlt or:lgg+ ln 1981, national teams in Ivory Coast, Benin, Togo and Upper Volta will al-so be involved in entironmental monitoring. Each team will be composed of an invertebrate specialist and an ichthfologist, four technicians, and supporting personnel. Eight specialists have received trainlng from the ORSTOI{ Hydrobiol-ogy Unit and from the Institute of Aquatic Biology, Achi-mta, Ghanal The standatdized application of the surveillance protocol wilL be coordinated by ttre Progrfuune hydrobiologist referred to under Vector Control research (above). The Prograrme will Assist each natj.onal unit, during the initial stages, in particular through the provision of babic equipment and maLerial. Health econouics I ilittle is known of the socioeconomic effects of onchocerciasis control. In 1980 the progrlame signed four contracts for research in this f ield . IWo contracts in the amount of US$ 97OO each were concluded with the University Assooiation of Economic Research Clermont (AUREC), University of Clermont-Ferrand, France, lnd rwo Others amounting to US$ 33 572 and US$ 44 4O5 respecEively, with the Economic Developmelt Rese4rch Centre, Uni.versity of I'lichigan, United States of America. These contracts cover the folldwing four research Projects: (a) a pilot study on the material facilities made available to migrants in AW settlement villages, particularly agricul'tural credits ; (b) a pilot study on analysis of the economic role and social status of persons affected by onchocereiasis; (c) two studies on health problems and policies in the OCP countries ' one on the present situation and Ehe other on comparison of the policies of sources of financing for heaLth PrograElmes ; (d) preliminary phase of a study on the impact of onchocerciasis on Labour producEivity. These contracts will be continued and expanded to define the requirements for an econpmically sound and socially acceptable basic health service couPonent for rural develop- mentl prograrules . TRAiNING The programre will continue to promote the three levels of technical training related to the medical, entomological, or hydrobiological asPecEs of onchocerciasis control, as in thc past. Also, the Prograsme will continue Eo secure financing for graduate fellowships of two yeats'duration for studies in public health economics; for fellowships of one year's dur4tion for university workers and senior officials who wish to improve their skills in t onclro-related analytical and methodological research; and for one seminar in public health economy and prograuming for senior public health officials. INDEPENDENT COMMISSION The IndependenE Cormrisslon on the long-term ProsPecEs of the Programte will terminate its work and submit a final report to WIIO in nid-1981. Insecticide: one litre of insecticide is estimated to cost $ 8.79 including sea freight charges; Research- Training, Rent, Supplv, Maintenance, Operational travel, I"leetings: costs are based on 198O revised estimates of expenditures plus 8% rnajoration for infl-ation for 1980 to 1981; Consultants: one consultant man-month is estimated at $52OO including fees, travel and per diem; Vehicles: average cost per vehicle is estimated at $ 12 84O. Eight per cent. annual cost increase is applied for the period 1982-1985 for all cooponents except Personal services for r^rtrich I1.s% annual increase is used. Exchange rates per US dollar adopted for calculations in this docurnent are as follows: Frenchfranc- 4.46; GEA franc - 223; Sterling pound 0.460; Gerrnan rnark - 1.94; Swiss franc - 1.85. No provision has been rnade for future fluctuations in these rates; it is expected, however, that the Progranune will be able to absorb within the proposed budget reasonable amounts of appreciation to these currencies should that occur. The cost of extensi.on activities in the Ivory Coast was estimated at $ 2 345 fOO. net amount earmarked for "Extension Studiesrris Eherefore $ 1 1S2 600 ($ 3 527 7OO -$ Z lt+S 1oo = $ 1 182 600). Ihe At the sixth session of the JCC in December 1979, the decision was taken to extend these studies by two years up ro December 1981. Expenditures during the first two years were less than originally estimated because of delays in establishing the subsectors for entomological surveillance and a reduction in requirements for flight hours and outside consultants. Savings thus accrued were used to finance the third year (1980) of these studies. Although funds were initially allocated for Mali the kogranme is no longer directly concerned since this extension area forms part of the Feasibility Study of an Onchocerciasi.s Control Campaign in the Senegal River Basin area. Additional financing in the amount of $ 437 200 is required to extend these studies in Benin, Ghana and Togo through 1981 and, as was suggested during the 1979 meeting of the JCC(see paragraph 9.4, report on the sixth session of the JCCI), these funds are now solicited.
