Haemochromatosis United States of America. The Centers for Disease Control and Prevention (CDC), during a meeting held in Atlanta in February 1996, reached a consensus that universal screening for iron levels should be carried out starting in young adults as early as age 18. Several studies have demonstrated the cost-effectiveness of screening of the population. Such a measure will save a large number of human lives. It will also save financial resources which are misspent owing to wrong or late diagnosis, by preventing medical problems and disabilities caused by the disease. An additional benefit will be that cases of iron deficiency will be identified, giving an early warning of chronic internal bleeding or tumours. Editorial Note: Haemochromatosis is a genetic disease, fatal in untreated cases.1 It is characterized by an iron excess in the blood and most organs, especially the liver. It is largely associated with the gene HLA-H which was recently identified and was found in 87% to 93% of the patients tested. This overload gives only late clinical symptoms, and the usual manifestations (hepatomegaly – evolving later towards cirrhosis and, often, hepatocellular carcinoma – “bronzed” skin, cardiomyopathy, diabetes, hypogonadism, demineralization, arthropathies and weakness) will generally begin with mild clinical signs after the age of 30 years for males and later for females, who are protected by menstruation. Treatment by weekly phlebotomies will usually result in significant improvement, which varies according to symptoms, but cirrhosis, diabetes and advanced cardiomyopathy will not disappear. Studies carried out in the United States have confirmed the cost-effectiveness of universal screening for iron levels in the body. These studies are of interest for health administrations in other countries where haemochromatosis appears to be, as in the United States, the most common among fatal genetic diseases, with prevalences ranging from 2 to 4.5 per 1 000 reported during limited surveys in Australia, France, Italy and the United States. Screening for haemochromatosis in the general population will essentially mean the detection of excess iron before body damage. Transferrin saturation (fasting, with a cut-off set at 50%, low enough to be sensitive for women as well) is the screening test recommended by experts during the Atlanta meeting. The mere addition of this test to routine serological examinations requested by health centres or private practitioners will enable the detection of cases of excess iron, which will then be submitted for complementary serological, clinical and genetic investigations. The recent discovery of gene HLA-H has been a crucial development for the diagnosis of haemochromatosis. It was identified in 1996 by a Californian society of biotechnology and, in the United States, it was found in 156 of 178 patients (87%) with a diagnosis of haemochromatosis. A confirmation was obtained in Rennes, France where it was found in 61 of 65 patients (93%). This discovery will greatly simplify the investigation process necessary for confirming suspected haemochromatosis in cases of excess iron. c In 1993, WHO held an international meeting in Israel, where experts from several countries made a synthesis of recent knowledge on haemochromatosis. The report of this meeting has been published in document WHO/HDP/ HF/FHA/93.2, which is available (in English only) on request from the Chief, Human Genetics unit. 1

Ces études présentent un intérêt pour les administrations sanitaires des autres pays où l’hémochromatose paraît être, comme aux EtatsUnis, la plus répandue des maladies génétiques mortelles, des prévalences de l’ordre de 2 à 4,5 pour 1 000 ayant été rapportées lors d’enquêtes limitées effectuées en Australie, aux Etats-Unis d’Amérique, en France et en Italie. Le dépistage de l’hémochromatose dans la population générale consiste avant tout à dépister l’excès de fer avant que ne se produisent les lésions organiques. La saturation de la transferrine (pratiquée à jeun, en retenant un seuil de 50%, assez bas pour dépister aussi les femmes) est le test de dépistage recommandé par des experts lors de la réunion d’Atlanta. La simple adjonction de ce test aux examens sérologiques de routine demandés par les centres de santé ou les médecins privés permettra de déceler les cas présentant un excès de fer qui seront alors soumis à des investigations sérologiques, cliniques et génétiques complémentaires. La découverte récente du gène HLA-H est un développement capital pour la procédure de diagnostic de l’hémochromatose. Il a été mis en évidence en 1996 par une société californienne de biotechnologie et, aux Etats-Unis, il a été trouvé chez 156 patients sur 178 (87%) qui avaient fait l’objet d’un diagnostic d’hémochromatose. Une confirmation a été obtenue à Rennes, France, où il a été trouvé chez 61 patients sur 65 (93%). Cette découverte va simplifier considérablement la procédure d’investigation nécessaire pour confirmer une hémochromatose suspectée en raison d’un excès de fer. c L’OMS a tenu en 1993, en Israël, une réunion internationale lors de laquelle des experts de plusieurs pays intéressés ont fait une synthèse des connaissances récentes sur l’hémochromatose.
