First National Programme Managers1 Meeting T he national managers or other representatives o f dracun culiasis eradication programmes of every African endemic country, except Mali, met at the WHO Regional Office for Africa in Brazzaville, Congo on 25-28 March 1991. Other sponsoring agencies’ representatives (Global 2000, Health and Development International, U N D P , U NICEF, the United States Peace Corps) were present. Having reviewed the individual national dracunculiasis eradication pro grammes and noting that although remarkable progress had been made since the Third Regional Conference on Dracunculiasis in Africa,1the participants emphasized that a lot still remained to be done before the target eradication date. The 11 following recommendations were adopted: 1. The individual action plans for dracunculiasis eradication activities in 1991-1992 developed at this meeting should be endorsed for implementation. 2. Each endemic country should prepare a brief quar terly summary of its dracunculiasis eradication activities and forward it to the WHO Regional Office for Africa by 31 March, 30 June, 30 September and 31 December for dissemination to all countries concerned. 3. A greater effort should be made to improve the coor dination o f information and related dracunculiasis eradica tion activities between the endemic countries, WHO and the participating collaborating agencies. In view of resolu tion W H A 39.21 adopted by the Thirty-ninth World Health Assembly and resolution AFR/RC38/R13, adopted by the WHO Regional Committee for Africa in 1988, it is impor tant for W HO to strengthen its key role o f coordination and direction o f technical activities related to dracunculiasis eradication. 4. The Forty-fourth World Health Assembly should adopt a resolution that endorses unequivocally the goal of * See No. 32, 1990, pp. 245-247.
Première réunion des administrateurs de programmes nationaux Les administrateurs nationaux et d ’autres représentants des pro grammes d’éradication de la dracunculose dans tous les pays d ’Afrique où la maladie est endémique, à l’exception du Mali, se sont réunis au Bureau régional de l’OMS pour l’Afrique à Brazzaville, Congo, du 25 au 28 mars 1991. Les autres organismes parrainant cette manifestation (Global 2000 , Health and Develop ment International, P N U D , UNICEF, Peace Corps des Etats-Unis d ’Amérique) étaient également représentés. Après avoir passé en revue les programmes d’éradication de la dracunculose au niveau national et pris acte des remarquables progrès accomplis depuis la Troisième conférence régionale sur la dracunculose en Afrique,1 les participants ont souligné qu’il restait beaucoup à faire avant la date fixée pour l’éradication. Les 11 recommandations suivantes ont été adoptées: 1. Les différents plans d’action pour les activités d ’éradication de la dracunculose en 1991-1992 élaborés à cette réunion devraient être approuvés pour exécution. 2. Chaque pays d’endémicité devrait établir tous les trimestres un bref résumé de ses activités d’éradication de la dracunculose et le faire parvenir au Bureau régional de l'OMS pour l’Afrique le 31 mars, le 30 juin, le 30 septembre et le 31 décembre au plus tard afin qu’il puisse être diffusé à tous les pays intéressés. 3. H faudrait s’attacher davantage à améliorer la coordination entre les pays d’endémicité, l’OM S et les organismes collaborateurs en ce qui concerne l’information et les activités connexes d ’éradi cation de la dracunculose. Compte tenu de la résolution WHA39.21 adoptée par la Trente-Neuvième Assemblée mondiale de la Santé et de la résolution AFR/RC38/R13 adoptée par le Comité régional de l'OMS pour l’Afrique en 1988, il importe que l’OMS renforce son rôle d é de coordination et de direction à l’égard des activités techniques d ’éradication de la dracunculose. 4. dracunculiasis eradication by 1995. This would be in keep ing with resolution AFR/RC38/R13. T he new resolution should also mandate W HO to begin the formal process o f certifying elim ination o f the disease from formerly endemic countries. In order to im plem ent these activities, an appro priate budget should be allocated. 1 5. T hose endem ic countries that have not yet done so need to com plete nationwide searches to determ ine the full extent o f dracunculiasis, and develop national plans o f action as soon as possible. T he assistance o f U NIC EF and o f other organizations in carrying out these searches and follow up interventions will be needed. 6. Endem ic countries should extend village-based sur veillance and appropriate intervention m ethods such as health education, the use o f filters, and other m ethods of ensuring safe drinking-water to all the known endemic vil lages as quickly as possible. 7. Endemic countries should establish national policies giving appropriate priority for the provision o f safe sources o f drinking-water to villages where dracunculiasis is en dem ic. The docum ented presence o f dracunculiasis should be used to help solicit additional external assistance for the rural water supply sector. Appropriate bilateral, multi lateral, and nongovernmental assistance agencies need to quickly help bring the resources o f the rural water supply sector to bear effectively on the problem o f dracunculiasis in endem ic countries. 8. N ational dracunculiasis eradication programmes should establish an intersectoral steering com m ittee, a national task force, or some similar mechanism to help dis sem inate information and coordinate relevant actions of all agencies and institutions involved in the eradication effort. 9. W HO and other international agencies should help mobilize the remaining resources needed for this pro gramme, so that availability o f appropriate resources is n o t. a constraint for populations that are working to rid them selves o f this disease. 10. Appropriate operational research should be sup ported to facilitate this eradication effort including such areas as: low -cost comm unity surveillance, improved health education, comm unity m obilization m ethods, improved case containm ent strategies, continued research on vector control, worm extraction, reporting incentives, user effec tiveness o f nylon filters and problem s presented by popu lation m ovem ents. 11. Endem ic countries, for whom the major focus o f dracunculiasis is in a border area, should seek to establish forums for discussion and action w ith neighbouring coun tries. N ecessary facilitation and assistance should be pro vided by the appropriate U nited N ations and other col laborating agencies. 1 Resolution WHA44.5 “Eradication of dracunculiasis” was adopted by die Forty-fourth World Health Assembly in Geneva on 13 May 1991.

U n appui devrait être apporté à la recherche opérationnelle pour faciliter cet effort d’éradication, dans des dom aines tels que la surveillance des comm unautés à un faible coût, l’amélioration de l’éducation pour la santé, la m obilisation communautaire, l’am élioration des stratégies d’isolem ent des cas, la poursuite des travaux sur la lutte antivectorielle, l’extraction des vers, les incitations à la notification des cas, l'utilisation rationnelle des filtres de nylon et les problèm es fixés par les m ouvem ents de populations. 11. Les pays où le principal foyer de dracunculose se situe dans une zone frontalière devraient chercher à établir des m écanism es de dialogue et d'action avec les pays voisins. Since 1982, new cases o f m easles have been registered by a network o f general practitioners.1T his mor bidity surveillance system has been in operation in Belgium since 1979. Materials and methods A large number o f general practitioners (±1 500) in all parts o f d ie country were contacted so that this network could be set up. T he objective is to involve around 150 physicians on a voluntary basis in order to cover an adequate popula tion o f patients (±150 000) for statistical analysis o f data. Geographical representativeness is controlled by using a hierarchical m ethod o f classifying administrative districts which allows administrative entities to be grouped together on an epidem iological basis. T he population covered is estim ated from the number o f consultations and hom e visits (contact betw een patients and sentinel physicians) recorded in the participating phy sicians' practices. T he coverage in a given district is worked out by dividing the annual number o f contacts made by sen tinel physicians in this district by the average number o f contacts made per inhabitant and per year in the district. T he latter parameter is worked out from data collected by the N ational Institute o f Sickness and D isability Insurance. T he sentinel population is estim ated at 1-2% o f the Belgian population. T he registration form is filled out weekly. It is systematically returned by the physician, even if no cases have been recorded. M easles diagnosis is based on clinical symptoms: characteristic rash and fever exceeding 38.5 °C. T he age, sex, presence or absence o f com plications, any hospitaliza tion (since 1985), and im m unization status (excluding years 1984, 1985 and 1986) are recorded for each case. Since m easles is rare in subjects over the age o f 20, incidence figures have only been calculated for the under20 age group. Results
La représentativité géographique est contrôlée en utilisant une m éthode hiérarchique de classification des arrondissements admi nistratifs qui permet de regrouper des entités administratives sur des bases épidém iologiques. Le dénominateur de population est estim é à partir du nombre de consultations et de visites à dom icile (contacts patients-m édecins vigies) relevé dans la pratique des m édecins participants. La cou verture dans un arrondissement donné est obtenue en divisant le nombre annuel des contacts des m édecins vigies de cet arrondisse m ent par le nombre moyen de contacts par habitant et par an de l'arrondissement. Ce dernier paramètre est obtenu à partir des données rassemblées par l'Institut national d'Assurance M aladieInvalidité. La population vigie est estim ée à 1-2% de la population belge. Le formulaire d'enregistrement est hebdomadaire. Il est renvoyé par le m édecin de façon systématique m êm e si aucun cas n'a été relevé. Le diagnostic de rougeole est basé sur les symptômes cliniques: érythème typique et fièvre dépassant 38,5 °C. L'âge, le sexe, la pré sence (ou l'absence) de com plications, l'hospitalisation éventuelle (depuis 1985) et l'état vaccinal (à l'exception des années 1984, 1985 et 1986) sont enregistrés pour chaque cas. Incidence T he incidence rate in 1982-1983 was estim ated at 292 per 10 000 subjects under the age o f 20. In 1990, it fell to 16 per 10 000 {Fig, 1).

Seasonal variations Fig, 2 shows the typical seasonal variations in m easles for all reporting periods.

Table 3 shows that im m unization coverage increased very significantly in the Flem ish and W alloon com m unities during the period 1987-1990.

Discussion T he annual number o f m easles cases among young people under the age o f 20 was estimated in 1982 - when immu nization was practically nonexistent - at 81 480, which gives an annual incidence rate o f292 cases per 10 000 inhabitants under the age o f 20. Since 1982, these figures have steadily decreased: in 1989 only 22 595 cases under the age o f 20 were recorded, but this level is still markedly higher than for those countries where there has been routine im munization for many years. T his significant reduction since 1982 has been observed in both com m unities.

T he downward trend in incidence is greater in the Flem ish region than in the W alloon region, but the disparity between the 2 com m unities gradually decreased during the observation period and reached its low est level in 1990. T his trend can be compared with the trend in im munization coverage. In 1983, the Conseil supérieur d'Hygiène recom m ended im munization at 13 m onths. T he com bined vac cine (m easles, mumps, rubella) has been administrered without charge since the beginning o f 1985 in the Flem ish region and since Septem ber 1985 in the W alloon region. Free distribution o f the vaccine has led to a m uch wider use. Compared w ith the period 1982-1983, im munization coverage during the period 1987-1990 has increased con siderably m the country as a whole. It is estim ated at 81% for the 1-4-year age group in the Flem ish region and at 63% in the W alloon region. T his marked im provement com pared with 1982-1983 is a result o f an active im munization policy in the 2 com m unities. T his reporting programme also makes it possible to evaluate the severity o f m easles by looking at the rate o f com plications and the rate o f hospitalizations. Com pli cations are observed in approximately 15% o f the cases. T he m ost frequent com plication is otitis media (7.6% ), follow ed by pneum onia (4.1% ) and by “other com plica tions” (3.4% ) (for example: bronchitis and sore throat); 1.6% o f cases are hospitalized. La tendance dégressive de l'incidence est plus forte en région flamande qu'en région wallone. Par rapport à la période 1982-1983, la couverture vaccinale au cours de la période 1987-1990 a fortem ent augm enté dans l'en sem ble du pays. Elle est estim ée en région flamande à 81% pour le groupe d'âge de 1 à 4 ans et à 63% en région wallone. C ette nette amélioration par rapport à 1982-1983 est le résultat d'une politique active de vaccination dans les 2 com m unautés. H epatitis A U n ited S ta tes o f A m erica . D uring 1988, the N ational M orbidity Reporting System received 56 773 reports o f viral hepatitis, a 1% reduction from the total reported m 1987. T he overall incidence rate was 23.10 per 100 000 population, a reduction o f alm ost 2% from 1987 (Fig. /) . Fifty per cent o f the cases were reported as hepatitis A, 41% as hepatitis B, 5% as non-A, non-B hepatitis and 4% 317

Historically the incidence o f hepatitis A in the U nited States has had recurring peaks, w ith nationwide epidem ics spaced roughly 7 to 10 years apart. After 1971, the reported incidence declined steadily until 1984, w hen cases began again to increase (Fig. 1). By the end o f 1988, a 10% increase in the incidence rate over that reported in 1987 was evident, with an overall increase o f 32% since 1983. W hile the nationwide incidence has not reached the level o f 1971, epidem ic activity has been increasing, particularly in western states. Comparison o f state incidence rates for hepatitis A between 1987 and 1988 shows an increase in high-incidence states from w est to east. Incidence rates for hepatitis A have slowly increased since the m id-1980s in m ost regions; the m ost dramatic increases occurred in the M ountain and Pacific regions, follow ed by slight declines in 1987-1988. T he highest rates o f hepatitis A were observed in persons in the 20-29-year age groups. Children younger than 15 years o f age had rates as high as a decade earlier; those younger than 10 years o f age had rates higher than in 1980. From 1975 to 1985, the incidence for the 20-24-year age group dropped, then increased to current levels. In addition, increases in incidence were reported across all other age groups. L'incidence de l'hépatite A aux Etats-U nis a connu des pointes périodiques puisque des épidém ies se produisent dans tout le pays à peu près tous les 7 à 10 ans. A partir de 1971, l'incidence signalée a diminué régulièrement jusqu'en 1984, année où il y a eu recru descence des cas (Fig. 1). A la fin de 1988, on a noté une augmen tation de 10% du taux d'incidence par rapport à celui qui avait été signalé en 1987, l'augmentation globale étant de 32% depuis 1983. Alors que l'incidence sur le plan national n'a pas atteint le niveau de 1971, l'activité épidém ique a augm enté, en particulier dans l'ouest du pays. La comparaison des taux d'incidence de l'hépatite A dans les différents Etats entre 1987 et 1988 m ontre qu'il y a eu augmentation dans les Etats à forte incidence dans le sens ouestest. Les taux d'incidence de l'hépatite A ont lentem ent augmenté depuis 1985 dans la plupart des régions; les augmentations les plus spectaculaires concernaient la région des m ontagnes R ocheuses et la région du Pacifique, où il y a eu par la suite une légère diminurion en 1987-1988. C'est dans les groupes d'âge des 20 à 29 ans que l'on a relevé le plus grand nombre de cas d'hépatite A. Chez les enfants de m oins de 15 ans, les taux étaient aussi élevés que 10 ans plus tôt; chez les enfants de m oins de 10 ans, ces taux étaient plus élevés qu'en 1980. D e 1975 à 1985, l'incidence pour le groupe d'âge 20-24 ans a chuté, puis est rem ontée au niveau actuel. D e plus, on a signalé une aug m entation de l'incidence pour tous les autres groupes d'âge.
