COMMUNICABLE DISEASE SURVEILLANCE U nited K ingdom . — Since their inception in 1840 public health services in the United Kingdom have always been locally organized and continue to be the responsibility of medical officers of environ mental health. With the ever-present risk of imported infections and the increasing emphasis which should be given to commonsource infections affecting many parts of the country, there has developed a need for a wider coordinating activity. To assist in this coordination of the investigation and control of infectious disease in England and Wales a Communicable Disease Surveillance Centre (CDSC) has been established.1 The Centre with its staff o f com munity physicians with varying backgrounds in epidemiology, clinical medicine, infectious diseases and microbiology has three areas of activity;

Le risque constant d ’infections importées et la nécessité d ’accorder plus d’attention aux infections ayant une source com mune et sévissant dans de nombreuses régions du pays ont imposé une plus large coordination des activités. U n Centre pour la Sur veillance des Maladies transmissibles (Communicable Disease Surveillance Centre — CDSC) a donc été créé1 pour aider à coor donner les activités de dépistage et d ’endiguement des maladies infectieuses en Angleterre et au Pays de Galles. Ce centre, dont le personnel est composé de médecins des collectivités spécialisés selon le cas en épidémiologie, médecine clinique, maladies infectieuses ou microbiologie, exerce ses activités dans trois domaines:
• Surveillance of Communicable Disease (1) To provide through the Communicable Disease Report(CDR) and by other means up-to-date information about communicable disease in England and Wales and relevant information from other countries. (2) To have available information about the previous incidence of communicable disease in England and Wales. (3) To monitor the information obtained to detect significant changes in the patterns of communicable disease. These activities depend upon the collection of data about com municable diseases, their analysis, and the presentation of informa tion derived from these data in a readily assimilable form. Although initially concerned in particular with microbiological data, the Centre intends to develop its interests in other sources of data and hopes to undertake a review of the need for and uses of communicable disease information and o f the existing systems for providing it. The purpose of such a review would be to try to integrate and rationalize these systems to provide more effectively for the needs of micro biologists, infectious disease physicians, community physicians, environmental health officers and others concerned in the control of communicable disease. 1 The Communicable Disease Surveillance Centre; Public Health Laboratory Service, 61 Colindale Avenue, Loudon NW9 $EQ. • Surveillance des maladies transmissibles Les objectifs du centre sont les suivants: 1) Communiquer, grâce au Relevé des cas de maladies trans missibles (Communicable Disease Report — CDR) et par d’autres moyens, des renseignements à jour sur les maladies transmissibles en Angleterre et au Pays de Galles, ainsi que toutes données pertinentes concernant d’autres pays. 2) Réunir les données disponibles sur l’incidence antérieure des maladies transmissibles en Angleterre et au Pays de Galles. 3) Analyser les renseignements obtenus afin de déceler tout change ment significatif du tableau des maladies transmissibles. Ces activités supposent la collecte de données sur les maladies transmissibles, leur analyse et la présentation des renseignements ainsi obtenus sous une forme aisément assimilable. Bien qu’essentiellement axé à l’origine sur les données d ’ordre raicrobiologique, le Centre à l’intention de s’intéresser à d ’autres sources de données et de recenser les besoins en informations sur les maladies trans missibles, les utilisations qui sont faites de ces données et les systèmes actuels de collecte. • Advice and assistance in the investigation and control of com municable disease The Centre will be particularly concerned in providing assistance and coordination in incidents of communicable disease of national importance and those that involve several local authority areas but will be willing to help community physicians in the control of com municable disease in other ways. In carrying out this function of advice and assistance in the control of communicable disease the staff of CDSC will work closely with public health laboratories, thereby ensuring that full use is made of existing local facilities. • Assistance with the teaching of epidemiology of communicable disease The CDSC hopes to contribute to teaching of community phy sicians on the epidemiology of communicable disease in three main ways. Firstly, by providing information and material for teaching; secondly by taking part in teaching programmes and seminars; and thirdly by developing senior registrar training in this sub-specialty of community medicine. The Centre is being organized around its information function. An information officer has been appointed recently to manage the day-to-day provision of the information service, advised by the medical epidemiologists of the Centre and with expert computing and statistical support. The epidemiologists will have particular fields of interest, within which they can make specialist contributions to the information services and carry out field investigations, in cooperation with public health and hospital laboratories and the community physicians locally responsible. In the future it is intended to provide more immediate local support for community physicians and the Public Health Laboratory Service proposes to establish epidemiology posts based in several regional public health laboratories. The posts will be closely linked with the CDSC so that their holders will have national as well as regional responsibilities. The Centre intends to develop an interest in the epidemiology of hospital-acquired infection in association with the Cross Infection Reference Laboratory and hopes to appoint to its staff an infection control nurse for this purpose. Another possible development is a joint appointment of a veterinary epi demiologist with the Ministry o f Agriculture Fisheries and Food Epidemiology Unit a t Weybridge, to facilitate medical/veterinary cooperation in the investigation o f zoonoses, especially foodbome disease. Such an appointment would be closely linked with the Salmonella and Shigella Reference Laboratory and the Food Hygiene Laboratory a t Colindale. Later, it is hoped to develop the environmental health aspects of the Centre’s work and possibly to include on the staff an environmental health officer, as has been done in the corresponding unit in Scotland, the Communicable Disease (Scotland) Unit.

Dans l’exercice de ces fonctions consultatives et d ’assistance, le personnel du CDSC collaborera étroitement avec les laboratoires de santé publique, assurant ainsi une utilisation optimale des installa tions locales existantes. • Assistance pour des cours de formation sur l ’épidémiologie des maladies transmissibles Le Centre espère contribuer à familiariser les médecins des collec tivités avec l’épidémiologie dès maladies transmissibles par trois moyens principaux: premièrement, en fournissant des informations et du matériel pédagogique; deuxièmement, en prenant part à des programmes et séminaires de formation ; troisièmement, en organisant des programmes de formation supérieure dans cette branche de la médecine des collectivités. La structure du Centre s ’articule autour de ses activités d ’informa tion. Un responsable de l’information a été nommé récemment pour fournir chaque jour des services d ’information, avec les conseils des médecins épidémiologistes du Centre et l’aide de services spécialisés d ’informatique et de statistique. Les épidémiologistes s’intéresseront chacun à des domaines spéciaux au sujet desquels ils peuvent fournir au service d’information des avis autorisés et effectuer des études sur le terrain, en collaboration avec des laboratoires de santé publique et d’hôpitaux et avec les médecins responsables à l’échelon local des services de santé des collectivités. Il est prévu de fournir à l’avenir un soutien local plus immédiat aux médecins des collectivités, et le Service des Laboratoires de Santé publique se propose de créer des postes d ’épidémiologistes dans plusieurs laboratoires régionaux de santé publique. Ces postes seront étroitement liés au CDSC, de sorte que leurs titulaires exerce ront leurs responsabilités à l’échelon national aussi bien que régional. Le Centre a l ’intention d ’étudier l’épidémiologie des infections nosocomiales en association avec le Laboratoire de Référence pour les Infections croisées, et il espère pouvoir nommer à cette fin une infirmière spécialisée dans la lutte contre les infections. Il est égale ment question de nommer à Weybridge, de concert avec le Service d’Epidémiologie du Ministère de l’Agriculture, des Pêches et du Ravitaillement, un épidémiologiste vétérinaire en vue de faciliter la collaboration entre services médicaux et vétérinaires pour l’étude des zoonoses, et notamment des maladies transmises par les aliments. Cette nomination se ferait en étroite association avec le Laboratoire de Référence des Salmonella et des Shigella et le Laboratoire d ’Hygiène des Denrées alimentaires à Colindale. Enfin, on espère développer plus tard les activités du Centre relatives à la salubrité de l’environnement et adjoindre éventuellement à son personnel un spécialiste de l’hygiène du milieu, comme cela s’est déjà fait au Service écossais de Surveillance des Maladies transmissibles. (Com municable Disease (Scotland) Unit). Le CDSC envisage de nouer des liens étroits avec d’autres services nationaux d ’épidémiologie, ainsi qu’avec le service de surveillance épidémiologique de l’Organisation mondiale de la Santé, afin d ’encourager l’échange d’informations. A plus long terme, le Centre pourrait étendre son champ d ’activité à d ’autres aspects de la sur veillance et de l’endiguement des maladies pour venir en aide aux médecins des collectivités.
The CDSC proposes to form close links with other national epidemiology units and with the World Health Organization Sur veillance Unit to encourage the exchange of information. In the longer term the Centre could widen its scope into other aspects of disease surveillance and control in support of community physicians. RABIES U nited State of A merica. — In 1976 a total of 3146 laboratoryconfirmed cases of animal rabies were reported in the United States and outlying areas of United States juridiction. There were 471 more cases than for 1975,1 but 14% less than the average for the preceding five years. Wild animals (primarily skunks, raccoons and bats) accounted for the increase over the previous year’s totaL Forty-eight states plus Puerto Rico reported infected animals; only the District of Columbia, Hawaii, Vermont, Guam and the Virgin Islands reported no rabies cases. States reporting 100 or more cases were California (357),, Texas (347), Georgia (225), >See No. S0, 1976, p. 381.

Les animaux sauvages (.essentiellement mouffettes, ratons laveurs et chauves-souris) sont à l’origine de l'augmentation par rapport au total de l’année précédente. Quarante-huit Etats et Porto Rico ont signalé des animaux infectés; seuls le District de Columbia, Hawaii, le Vermont, Guam et les Iles Vierges n ’ont pas déclaré de cas de rage. Minnesota (193), Oklahoma (181), Arkansas (155), North Dakota (137), Iowa (125), South Dakota (114), and Florida (100). Thirtyfour states and Puerto Rico reported more cases of rabies in 1976 than in 1975 and 13 reported less. Infected animals most frequently reported, by percentage of the total cases, were the following: skunks, 47% (for the 16th con secutive year, infected skunks were the animals most frequently reported); bats, 23% (highest total of bat cases reported for any year since rabies was first diagnosed in bats in 1953); raccoons, 9 % (highest number of cases ever reported for a year); foxes, 6% (lowest total of fox cases reported in any year on record and 45 % below the annual average reported for the preceding five years); cattle, 5% ; dogs, 4% ; cats, 3% ; and horses and mules, 1%. In addition, 40 cases of mongoose rabies were reported in Puerto Rico, where rabies is enzootic in this species. Other cases reported were in coatimudis (2), an opossum, a badger, and a white mouse in a laboratory manufacturing rabies vaccine (it is assumed that this anim al had escaped from a cage after receiving an injection for titrating the rabies vaccine virus).
Two human deaths caused by rabies were reported; a boy from Texas died after he was bitten by a dog while visiting Mexico, and a woman from Maryland died after she was bitten by a bat.
