BUBONIC PLAGUE U nited S tates of A merica . — On 23 February 1972, a 19-yearold male college student in Flagstaff, Arizona, had onset of fever, headache, and pain in his right arm. He was seen at the Student Health Center on 24 February and started on penicillin and tetra­ cycline. He subsequently experienced right epitrochlear and axillary lymphadenopathy and was hospitalized late the next day with a tentative diagnosis of tularemia. A right epitrochlear lymph node aspirate was obtained on 26 February, and the patient’s therapy was changed to tetracycline and streptomycin. Cultures o f the biopsy material showed growth of gram-negative, bipolar rods which were identified as Yersinia pestis by the Zoonoses Section, Ecological Investigations Program, CDC, Fort Collinss Colorado, on 9 March. The patient made an uneventful recovery.

Après l’avoir examiné au Student Health Center le 24 février, on institua un traitement par la pénicilline et la tétracycline. Dans la soirée du lendemain, souffrant de lymphadénopathie à l’épitrochlée et à l’aisselle droite, l’étudiant fut hospitalisé avec un diagnostic provisoire de tularémie. Après avoir pratiqué par aspiration un prélèvement sur le ganglion épitrochléen droit, le 26 février, on cessa le traitement précédent pour administrer au ma­ lade de la tétracycline et de la streptomycine. Dans les cultures du matériel biopsique, on constata la présence de bâtonnets bipolaires Gram négatifs, qui, le 9 mars, furent identifiés comme étant Yersinia pestis par la section des Zoonoses de Y Ecological Investigations Program, CDC, Fort Collins, Colorado. Le malade a guéri sans complications. Le 21 février, cet étudiant et deux de ses camarades avaient écorché un chat sauvage qu’fis avaient tiré le jour même, à une trentaine de kilomètres au sud de Flagstaff. L ’animal paraissait malade au moment où 1 1avait été abattu. Les deux autres étudiants n ’ont souffert d ’aucun autre trouble et n ’ont reçu aucun traitement. La tête du chat sauvage a été envoyée au laboratoire du State Health Department de l’Arizona pour recherche du virus de la rage; les résultats ont été négatifs. La tête et la carcasse (sans les viscères) furent ensuite expédiées à Fort Collins en vue d ’autre analyses de laboratoire visant à confirmer l’infection par T. pestis. Le chat sauvage avait été tué en un lieu éloigné de toute habitation et, selon les premières enquêtes des autorités sanitaires du Comté de Coconino, l’incident est sans importance sur le plan de la santé publique. Des investigations complémentaires se poursuivent sur le terrain pour déterminer la source de l’infection du chat sauvage.

On 21 February, the patient and two other students had skinned a bobcat which they had shot that day about 18 miles south of Flag­ staff. The animal appeared ill at the time it was shot. The other students have remained well without therapy. The bobcat’s head was sent to the Arizona State Health Depart­ ment laboratory for rabies examination; the results were negative. The head, along with the carcass (minus the viscera), were forwarded to Fort Collins for further laboratory studies to confirm Y. pestis infection. The bobcat was killed in an area remote from residences, and initial investigations by health authorities of Coconino County indicate that there is no significant public health hazard. Additional field studies are being conducted to determine the source of infection to the bobcat.
