CHOLERA SURVEILLANCE Multiply Antibiotic-Resistant 0-Group 1 Vibrio cholerete B a n g l a d e sh . — Ten multiply antibiotic-resistant strains of Vibrio choleroe biotype El Tor isolated in an outbreak in Bangladesh1 have been examined at the WHO Collaborating Centre for Phage Typing and Resistance of Enterobacteria. Three strains of the Inaba serovar and seven strains of the Ogawa serovar were included. The patterns of resistance and the levels of resistance to the various drugs in the resistance spectrum (R-type) are shown in Table 1. The complete resistance spectrum of each strain was encoded by a plasmid of the C compatibility group. Three R-types were iden­ tified, the most comprehensive of which was that of ampicillin, kanamycin, streptomycin-spectinomycin, tetracyclines, sulfon­ amides and trimethoprim (AKSSpTSuTm). The other R-types were those of AKTSuTm and ATSuTm. It was demonstrated that resistance to ampicillin, kanamycin, streptomycin-spectino­ mycin and tetracyclines could be lost independently from the AKSSpTSuTm group C plasmid, spontaneously either from a nutrient broth grown culture in 18 hours or on storage on Dorset’s egg medium for four weeks, o r on transfer to standard recipients. Segregants and exconjugants coding for AKTSuTm and ATSuTm were readily obtained and these results suggest that the strain of R-type AKTSuTm and ATSuTm may have been spontaneously derived from a strain of R-type AKSSpTSuTm.

SURVEILLANCE DU CHOLÉRA Apparition d’une antibio-résistance multiple chez des souches de Vibrio choleroe du groupe 0-1 B a n g l a d e sh . — Dix souches multi-résistantes aux antibiotiques de Vibrio choleroe biotype El Tor isolées à l’occasion d ’une épi­ démie survenue au Bangladesh1 ont été examinées au Centre colla­ borateur OMS de lysotypie et d ’étude de la résistance des entéro­ bactéries. Il s’agissait de trois souches du sérovar Inaba et de sept souches du sérovar Ogawa. Les spectres et les niveaux de résistance (type R) sont indiqués au Tableau J. Le spectre complet d ’antibio-résistance de chaque souche était codé par un plasmide appartenant au groupe de compatibilité C. Trois types R ont été identifiés, le plus complet étant un spectre de résistance à l’ampicilline, à la kanamyrine, à la streptomycinespectinomycine, aux tétracyclines, aux sulfamides et au triméthoprime (AKSSpTSuTm), les autres types étant AKTSuTm et ATSuTm. Il a été démontré que la résistance à l’ampicilline, à la kanamycine, à la streptomyrine-spectinomycine et aux tétracy­ clines pouvait être perdue spontanément indépendamment du plas­ mide AKSSpTSuTm du groupe C, à partir d ’une culture de 18 heures, en bouillon nutritif ou après stockage de quatre semaines sur un milieu à l’œuf de Dorset ou encore lors d ’un transfert dans des réci­ pients standards. Il a été facile d ’obtenir des ségrégants et des exconjugants codant les résistances AKTSuTm et ATSuTm et ces résultats donnent à penser que la souche de type R AKTSuTm et ATSuTm peut avoir dérivé spontanément d ’une souche de type R AKSSpTSuTm. » Voir N» 22. pp .161-162.

The transfer o f the group C plasmids from the wild V. choleras host strains was enhanced by incubating the mating mixtures at 28° C rather than a t 37° C, but in subsequent crosses from a K12 host, the effect of temperature was reversed and the plasmids trans­ ferred at higher frequency at 37° C than at 28° C. It seems likely that the thermosensitive plasmid transfer demonstrated here is a property of the bacterial vector and not of the group C plasmid, and this phenomenon is under further investigation. The Ogawa and Inaba serovars of V. choleras biotype Ei Tor are interconvertible and it has been demonstrated that serovar changes in V. cholerae El T or are phage-induced. The presence of indis­ tinguishable group C plasmids in these serovars in an outbreak in a small area of Bangladesh could be explained by the dissemination of a single, multiply-resistant strain of V. cholerae biotype El Tor which has undergone serovar conversion in the course of the outbreak. The first widespread and protracted outbreak o f a multiply anti­ biotic-resistant strain o f V. cholerae El Tor occurred in the United Republic of Tanzania in 1977.1 This strain was of R-type ACKSSuT (C = chloramphenicol) and like the strain from the outbreak in Bangladesh, these resistances were encoded by a group C plasmid. Group C plasmids have been identified in sporadic isolations of drug-resistant V. cholerae, and it has been suggested that plasmids of this compatibility group have a special affinity for V. cholerae. The results with the strains from Bangladesh support this hypothesis. The WHO Collaborating Centre for Phage Typing and Resistance of Enterobacteria would be pleased to receive further isolations of drug-resistant strains of V. cholerae for genetical and molecular examination. * See N o. 43, 1979, pp. 333-334.

Le transfert de plasmides du groupe C à partir de souches hôtes sauvages de V. cholerae a été facilité par incubation des mélanges à 28“ C plutôt qu’à 37“ C mais lors de transmissions ultérieures à partir d’un hôte K 12, l’effet de température a été inversé et la fré­ quence de transfert des plasmides a été plus élevée à 37° C qu’à 28° C. La présence de plasmides indifférendables du groupe C dans ces sérovars à l’occa­ sion d ’une poussée survenue dans une petite région du Bangladesh pourrait s’expliquer par la diffusion d ’une souche unique, multi­ résistante de V. cholerae biotype El T or ayant subi une conversation de sérovar au cours de l’épidémie. La première épidémie étendue et prolongée d ’une souche multi­ résistante de V. cholerae El T or a été observée en République-Unie de Tanzanie en 1977.1 Cette souche était du type R ACKSSuT (C = chloramphénicol) et de même que pour la souche isolée au Bangladesh, ce spectre de résistance était codé par un plasmide du groupe C. Des plasmides du groupe C ont été identifiés lors d ’isole­ ments de souches pharmacorésistantes de V. cholerae et il a été suggéré que les plasmides appartenant à ce groupe de compatibilité ont une affinité spéciale pour V. cholerae. Les résultats obtenus sur les souches isolées au Bangaladesh vont dans le sens de cette hypothèse. Le Centre collaborateur OMS de lysotypie et d ’étude de la résis­ tance des entérobactéries serait heureux de recevoir de nouveaux échantillons de souches pharmacorésis tantes de K. cholerae afin de les soumettre à des examens génétiques et moléculaires. 1 Voir N “ 43,1979. pp. 333-334.
