‫صد تنفيذ اللوائح الصحية الدولية (‪:)6002‬‬ ‫ور ْ‬ ‫تقييم َ‬ ‫تحقيق الهدف المطلوب في ‪6002‬‬ ‫ِّمة‬ ‫مقد‬ ‫ناقشت مجعية الصحة العاملية الثامنة والستون واعتمدت تقرير جلنة املراجعة املعنية باللوائح الصحية الدولية‬ ‫َ‬ ‫(‪ )5002‬بشأن التمديدات الثانية الالزمة لبناء القدرات الوطنية يف جمال الصحة العمومية‪ ،‬وبشأن تنفيذ اللوائح‬ ‫مرض فريوس اإليبوال‪ ،‬مبا يف ذلك تقرير‬ ‫الصحية الدولية (‪ ،)1‬فضالا عن العديد من التقارير اخلاصة بفاشية َ‬ ‫أثريت كذلك يف مجعية‬ ‫الفريق املعين بالتقييم املبدئي ملرض اإليبوال (‪ .)5‬ومن بني املخاوف اليت أثارها الفريق‪ ،‬و َ‬ ‫الصحة العاملية‪ ،‬مصداقية التقييم الذايت الذي أجرته ِ‬ ‫الدول األعضاء بشأن تنفيذها للمتطلبات اخلاصة بالقدرات‬ ‫َ‬ ‫معرضة خلطر‬ ‫البلدان‬ ‫هذه‬ ‫جيعل‬ ‫"وهذا‬ ‫يق‬ ‫ر‬ ‫الف‬ ‫ير‬ ‫ر‬ ‫تق‬ ‫يف‬ ‫د‬ ‫ور‬ ‫حيث‬ ‫الوطنية الالزمة لتنفيذ اللوائح الصحية الدولية‪،‬‬ ‫َّ‬ ‫َ‬ ‫تعرض اجملتمع الدويل للخطر"‪.‬‬ ‫شديد ويزيد ُّ‬ ‫ضت مجعية الصحة‪ ،‬يف إطار‬ ‫ومن بني األمراض اليت هتد‬ ‫دة األمن الصحي العاملي يف هذا السياق‪َّ ،‬‬ ‫ِّد بش ّ‬ ‫تعر َ‬ ‫مناقشاهتا اخلاصة باللوائح الصحية الدولية‪ ،‬ملتالزمة الشرق األوسط التنفسية النامجة عن الفريوس التاجي كورونا‬ ‫ظهرت أول ما ظهرت يف اململكة العربية السعودية عام ‪ ،5015‬وتنتشر حاليا على نطاق‬ ‫(‪ ،)MERS-CoV‬اليت َ‬ ‫شّت أرجاء ِ‬ ‫استمر‬ ‫واسع يف َّ‬ ‫شبه اجلزيرة العربية‪ .‬كما َ‬ ‫ناقشت اجلمعية حاالت اإلصابة اجلديدة مبرض اإليبوال اليت ّ‬ ‫وقوعها يف غرب أفريقيا يف ذلك الوقت‪ ،‬وفريوس أنفلونزا الطيور (‪ ،)H5N1‬واألنفلونزا اجلائحة (‪،A)H9N2‬‬ ‫ِ‬ ‫التفارز بينها وبني فريوسات أنفلونزا الطيور األخرى املتوطنة يف‬ ‫اليت ما تزال تُواصل انتشارها بني الطيور‪ ،‬وإعادة ُ‬ ‫خمتلف أحناء العامل‪ ،‬وتَسبُّبها يف حاالت العدوى لدى البشر يف مصر‪ .‬ومشلت املناقشات أيضا شلل األطفال‪،‬‬ ‫كذلك التوصيات‬ ‫ِّرة لفريوس شلل األطفال الربي حاليا‪ ،‬و‬ ‫وال سيَّما تنفيذ التوصيات اخلاصة بالبلدان املصد‬ ‫ِّر فريوس شلل األطفال الربي إىل بلدان أخرى‪.‬‬ ‫املتضررة من شلل األطفال اليت ال تُصد‬ ‫اخلاصة بالبلدان ِّ‬ ‫وتفرض اللوائح الصحية الدولية التزامات على الدول األعضاء بشأن تنمية القدرات األساسية يف جمال الصحة‬ ‫الرتصد واالستجابة واحلفاظ عليها‪ ،‬مبا يشمل نقاط الدخول من أجل الكشف ِّ‬ ‫املبكر عن‬ ‫العمومية ألغراض ُّ‬ ‫ثَّ اإلخطار هبا ورفع التقارير عنها إىل‬ ‫األحداث املشمولة يف النصوص اخلاصة هبا وتقييم تلك األحداث‪ ،‬ومن َ‬ ‫املنظمة‪ ،‬وإعداد خطة ملواجهة تلك األحداث تتناسب مع األوضاع احمللية القائمة على أرض الواقع‪ ،‬وذلك‬ ‫أقرت الدول األعضاء خالل اجتماع مجعية‬ ‫بالتشاور مع جلنة الطوارئ املعنية باللوائح الصحية الدولية‪ .‬وقد َّ‬ ‫َّدت على‬ ‫أظهرت مستوى استعداد غري مناسب‪ ،‬وشد َ‬ ‫الصحة أن التهديدات احلالية لألمن الصحي العاملي قد َ‬ ‫قدرات احلالية يف جمال الصحة العمومية‪ .‬كما َّ‬ ‫أكدت‬ ‫ضرورة تسريع وترية تنفيذ اللوائح الصحية الدولية وتعزيز ال ُ‬ ‫الدول األعضاء أيضا على أمهية دور املنظمة يف مواصلة تقدمي الدعم التقين املطلوب للدول األعضاء‪ ،‬إال أهنا‬ ‫ديات املاثلة‪.‬‬ ‫أوضحت أنه ينبغي النظر يف ِّ‬ ‫تبين هنج اسرتاتيجي خمتلف من أجل جماهبة التح ّ‬ ‫‪.1‬‬

‫صد تنفيذ اللوائح الصحية الدولية والثغرات اليت مت‬ ‫ِّ‬ ‫لر ْ‬ ‫وتبني هذه الورقة ُ‬ ‫أوجه القصور املوجودة يف اآللية احلالية َ‬ ‫التعرف عليها يف قدرات الدول األعضاء لتنفيذ تلك اللوائح‪ .‬وتقرتح الورقة هنجا جديدا لتقييم ورصد تنمية‬ ‫ُّ‬ ‫القدرات املطلوبة من أجــل تنفيــذ اللوائـح الصحيـة الدوليـة واحلفاظ عليها‪ ،‬فضالا عن ومن أجل االمتثال لتلك‬ ‫اللوائح‪.‬‬ ‫قررت مجعية الصحة العاملية (القرار ج ص ع ‪ ،)5.21‬أن ترفع الدول األطراف واملديرة العامة‪ ،‬طبقا للفقرة ‪ 1‬من‬ ‫ّ‬ ‫املادة ‪ 24‬من اللوائح الصحية الدولية (‪ ،)5002‬تقارير سنوية إىل مجعية الصحة العاملية عن تنفيذ هذه اللوائح‪،‬‬ ‫وضع‪ ،‬هلذا الغرض‪ ،‬إطار للرصد يف هذا اجملال‪.‬‬ ‫وقد ُ‬ ‫ُّدية ومؤشرات واستبيان للتقييم الذايت‪ ،‬وينطوي على تقييم بشأن تنفيذ‬ ‫ويشتمل إطار الرصد على قائمة تفق‬ ‫القدرات األساسية الثماين وتنمية القدرات يف نقاط الدخول والقدرات اخلاصة مبواجهة األخطار ذات الصلة‬ ‫باللوائح الصحية الدولية‪ ،‬وال سيَّما األمراض املعدية واألمراض احليوانية املصدر والسالمة الغذائية واألخطار‬ ‫الكيميائية واإلشعاعية والنووية‪ .‬وليس املقصود من عملية الرصد استخدامها كأداة لتصنيف أداء البلدان أو‬ ‫احملرز‬ ‫رض منها هو أن تكون أداة ملساعدة فرادى البلدان على رصد التقد‬ ‫ُّم َ‬ ‫ملقارنة األداء بني البلدان‪ ،‬بل إن الغََ‬ ‫التعرف عليها‪.‬‬ ‫وب تلبية متطلبات القدرات األساسية ومعاجلة أي ثغرات قد يتم ُّ‬ ‫َ‬ ‫صْ‬ ‫أشارت النتائج اليت مت احلصول عليها من استبيانات التقييم الذايت اخلاص مبتطلبات اللوائح الصحية الدولية‪،‬‬ ‫و َ‬ ‫دل اإلقليمي لتنفيذ اللوائح نسبة ‪ ،)3( %75‬مع تلبية كثري من البلدان‬ ‫اليت أجريَت يف عام ‪ ،5014‬إىل بلوغ املع ّ‬ ‫ِ‬ ‫دت إىل بلدان اإلقليم يف‬ ‫للعديد من املتطلبات اخلاصة باللوائح الصحية الدولية‪ .‬ومع ذلك‪ ،‬فإن البعثات اليت أوف َ‬ ‫أو ِ‬ ‫التأهب واالستجابة على الصعيد الوطين ألي وفادة حمتملة لفريوس اإليبوال‪،‬‬ ‫اخر عام ‪ ،5014‬لتقييم مستوى ُّ‬ ‫التأهب واالستجابة‪ .‬وقد ِ‬ ‫مشل‬ ‫رت عن وجود العديد من الثغرات اهلامة يف تلك البلدان يف ما يتصل بقدرات ُّ‬ ‫أس َ‬ ‫فَ‬ ‫ذلك بلدان ا من اليت سبق وأعلنت وفاءها بااللتزامات املطلوبة مبقتضى اللوائح الصحية الدولية يف املوعد النهائي‬ ‫األول أو الثاين‪.‬‬ ‫كانت الثغرات الرئيسية اليت مت اكتشافها يف غالبية البلدان هي عدم وجود هيكل تنسيق وطين يعمل بكامل‬ ‫و‬ ‫كز لعمليات الطوارئ داخل وزارة الصحة‪ .‬وبالنسبة لسلسلة القيادة و ُّ‬ ‫التحكم يف احلوادث‪،‬‬ ‫طاقته وعدم وجود مر‬ ‫مهمة‪ ،‬مع عدم وجود إجراءات تشغيل معيارية إلدارة األحداث‬ ‫فقد كانت َّ‬ ‫إما غائبة أو تعرتيها ثغرات بنيوية ّ‬ ‫واإلنذار هبا‪ .‬ومل يكن يُنظر إىل اخلطط الوطنية ملواجهة فريوس اإليبوال باعتبارها جزءا متمما للخطة الوطنية‬ ‫التأهب لألوبئة‪/‬الطوارئ واالستجابة هلا‪ ،‬كما أهنا افتقرت إىل النَهج املتعدد القطاعات‬ ‫للصحة العمومية بشأن ُّ‬ ‫وإلجراءات التشغيل املعيارية‪ .‬أضف إىل ذلك‪ ،‬عدم إجراء التدريبات أو متارين احملاكاة الالزمة الختبار اخلطط‬ ‫القائمة ملواجهة فريوس اإليبوال‪ .‬ويف ما يتعلق بآلية اإلنذار ِّ‬ ‫املبكر لتعزيز اكتشاف أي حالة مشتَبَه فيها‬ ‫واالستجابة هلا‪ ،‬فإما أهنا مل تكن تعمل أو أهنا مل تكن موجودة من األصل‪ .‬هذا إىل جانب إىل عدم إضفاء‬ ‫الرتصد القائم على األحداث‪ .‬ويف السياق ذاته‪ ،‬مل تكن قدرات املوارد البشرية‬ ‫الطابع الرمسي بعد على أنشطة ُّ‬ ‫ِ‬ ‫رق االستجابة السريعة املتعددة‬ ‫مناسبة للقيام بأنشطة ُّ‬ ‫الرتصد وحتليل البيانات‪ .‬وذلك عالوة على عدم توافُر فَ‬ ‫التخصصات الستقصاء الفاشيات واالستجابة هلا على خمتلف املستويات اإلدارية‪ ،‬كما كانت أنشطة‬ ‫االستقصاء امليداين لفاشيات األمراض حمدودة‪.‬‬ ‫‪2‬‬

‫ومل يوجد يف غالبية البلدان برنامج وطين للوقاية من العدوى ومكافحتها ذو بنية وأدوار ومسؤوليات حمددة‪ ،‬وال‬ ‫كن البنود التالية متوافِرة أيضا يف معظم البلدان‪ :‬إجراءات للمعاجلة‬ ‫خطة عمل أو ميزانية خاصة بذلك‪ .‬كما مل ت ُ‬ ‫املأمونة للنفايات الطبية‪ ،‬ومعاجلة انسكابات الدم وسوائل اجلسم؛ برنامج وطين للسالمة املهنية؛ نظام إلدارة‬ ‫اجلودة يف املختربات‪ ،‬مبا يشمل وجود إجراءات تشغيل معيارية خاصة بتداول العينات املختربية‪ ،‬وتعطيلها‬ ‫كوالت أو دالئل إرشادية جلمع العينات ونقلها وشحنها بطريقة آمنة؛ نظام إلدارة املخاطر‬ ‫وتشخيصها؛ بروتو‬ ‫البيولوجية يف املختربات؛ مرفق للسالمة البيولوجية من املستوى الثالث للتعامل مع مسبِّبات األمراض اخلطرة‬ ‫كانت اسرتاتيجية االتصال املتعلقة بإبالغ‬ ‫َّة‪ ،‬وعلب قفازات لتداول وتعطيل العينات املشتبه فيها‪ .‬و‬ ‫املستجد‬ ‫املعلومات عن فريوس اإليبوال‪ ،‬مبا يف ذلك اإلبالغ عن املخاطر والتعبئة اجملتمعية‪ِ ،‬‬ ‫تتسم بالضعف الشديد‪،‬‬ ‫متمم خلطة االتصال الوطنية لإلبالغ‬ ‫باإلضافة إىل غياب خطة شاملة لإلبالغ عن خماطر فريوس اإليبوال كجزء ِّ‬ ‫عن املخاطر‪.‬‬ ‫ويف ما يتعلق بنقاط الدخول‪ ،‬مل تتوافر خطط طوارئ الصحة العمومية وإجراءات التشغيل املعيارية اخلاصة هبا يف‬ ‫معظم البلدان‪ ،‬مع غياب التنسيق بني خمتلف األطراف املعنية يف نقاط الدخول‪ ،‬وبني األجهزة الوطنية املختصة‪.‬‬ ‫هذا إىل جانب غياب إجراءات التشغيل املعيارية اخلاصة بالكشف ِّ‬ ‫املبكر عن احلاالت وتقييمها واالستجابة هلا‪،‬‬ ‫كات الطريان‪ ،‬بشأن احلاالت احملتملة يتّ ِ‬ ‫سم‬ ‫يف حني كان التواصل مع قطاع السفر اجلوي‪ ،‬مبا يف ذلك شر‬ ‫َّدة لعزل‬ ‫بالضعف‪ ،‬مع عدم وجود ترتيبات واضحة لتتبع خمالطي احلاالت‪ .‬وبالنسبة لوجود مساحات حمد‬ ‫املصابني من املسافرين وملقابلة خمالطيهم‪ ،‬فلم تكن متوافرة يف غالب األحيان‪ ،‬بينما كانت القدرات اخلاصة‬ ‫كانت التجهيزات يف املعابر الربية ضعيفة‪ ،‬كما‬ ‫التحري عند الدخول‪ ،‬إن لزم ذلك‪ ،‬حمدودة جدا‪ .‬و‬ ‫بإجراء تدابري ِّ‬ ‫كات األشخاص فيها‪.‬‬ ‫قدرات حمدودة للسيطرة الكفاية على حتر‬ ‫كانت ال ُ‬ ‫سبق وأن حثت مجعية الصحة العاملية الدول األعضاء (القرار ج ص ع ‪ )5.21‬على ضمان استكمال وحتديث‬ ‫اصل ما بني مراكز‬ ‫ني مر‬ ‫بيانات االتصال باملر‬ ‫كز الذي عُ ِّ‬ ‫كزا لالتصال الوطين املعين باللوائح‪ ،‬وذلك لتيسري التو ُ‬ ‫االتصال الوطنية ونقاط االتصال يف املنظمة على مدار اليوم‪ .‬وتقوم الدول األعضاء سنويا بتأكيد تعيني مراكز‬ ‫االتصال الوطنية املعنية باللوائح الصحية الدولية‪ ،‬إال أنه ال يتم احلفاظ على إتاحة التواصل على مدار اليوم‪.‬‬ ‫حة ملراكز االتصال الوطنية‪ ،‬غري أن هذه املهام‬ ‫وتقد‬ ‫املقرت َ‬ ‫ِّم األمانة اإلرشادات والتوجيهات الالزمة حول املهام َ‬ ‫ليست موجودة يف غالبية البلدان‪ ،‬وحّت عندما تكون موجودة‪ ،‬فإهنا ال تكون منظمة أو منشورة على نطاق‬ ‫واسع بني األطراف املعنية لتيسري تنفيذ متطلبات اللوائح الصحية الدولية‪.‬‬ ‫وينبغي للدول األعضاء إخطار املنظمة جبميع أحداث الصحة العمومية اليت قد تُ ِّ‬ ‫قا‬ ‫شكل‪ ،‬حبسب التقديرات‪ ،‬قل ا‬ ‫ِ‬ ‫ا يف ذلك آلية اختاذ القرارات املنصوص عليها يف املرفق ‪5‬‬ ‫مة‬ ‫دوليا‪ ،‬وذلك خالل ‪ 54‬ساعة من تقييمها‪ ،‬مستخد َ‬ ‫مستمر لتقدمي معلومات تفصيلية يف‬ ‫اصل‬ ‫من اللوائح الصحية الدولية‪ .‬والبد أن يلي عملية اإلخطار إجراء تو ُ‬ ‫ّ‬ ‫مرض‪ ،‬يف هذا اإلقليم على وجه العموم‪،‬‬ ‫جمال الصحة العمومية بشأن َ‬ ‫احلدث املعين‪ .‬وال يتم االلتزام على حنو ُ‬ ‫بإخطار املنظمة يف وقت ِّ‬ ‫مبكر‪ ،‬مبثل هذه األحداث ورفع تقارير شاملة عنها‪ .‬ومن املعلوم أن دعم تقييم املخاطر‬ ‫ثَّ االستجابة هلا‪ ،‬أمر يقع ضمن اختصاصات املنظمة مبقتضى‬ ‫اليت تنطوي عليها مثل هذه األحداث‪ ،‬ومن َ‬ ‫اللوائح الصحية الدولية‪ ،‬سواء بالدعم املباشر أو من خالل التنسيق حلشد الدعم املطلوب‪ .‬غري أن غياب أو‬ ‫‪3‬‬

‫وتبادل املعلومات اخلاصة هبا‪ ،‬ميكن أن يعرقل مباشرة‬ ‫تأخري اإلخطار مبعلومات شاملة حول تلك األحداث‪ُ ،‬‬ ‫هذا االختصاص‪.‬‬ ‫تبادل املعلومات ذات الصلة بأحداث الصحة العمومية اليت تقع على أراضيها‪،‬‬ ‫وغالب ا ما تُظهر البلدان ترددا يف ُ‬ ‫واليت قد ِّ‬ ‫تشكل أحداث ا صحية عمومية تثري قلقا دوليا‪ ،‬على اعتبار أنه قد تكون ملثل هذه األحداث تداعيات‬ ‫سياسية واجتماعية واقتصادية‪ .‬ولذلك‪ ،‬فقد َّ‬ ‫أكدت مجعية الصحة العاملية على أمهية وجود مرحلة لإلنذار‬ ‫مبقتضى اللوائح الصحية الدولية‪ .‬وتسبق هذه املرحلة اإلعالن عن األحداث الصحية العمومية اليت قد تثري قلقا‬ ‫دولي ا‪ ،‬والغرض منها تشجيع الدول األعضاء على إخطار املنظمة يف وقت ِّ‬ ‫وتبادل املعلومات حول تلك‬ ‫مبكر ُ‬ ‫َ‬ ‫ِّ‬ ‫تنجم عن مرحلة اإلنذار‪،‬‬ ‫األحداث‪ .‬وقد يساعد االستقصاء املبكر واالستجابة السريعة لتلك األحداث‪ ،‬واليت ُ‬ ‫كف األمانة حالياا على‬ ‫قا دوليا‪ .‬وتع ُ‬ ‫على جتنُّب شروط اإلعالن عن األحداث الصحية العمومية اليت قد تثري قل ا‬ ‫وضع معايري حتديد مرحلة اإلنذار والشروع فيها‪.‬‬ ‫وينبغي للدول األعضاء التعاون مع بعضها البعض يف الكشف عن األحداث وتقييمها واالستجابة هلا‪ ،‬من‬ ‫خالل توفري التعاون التقين املطلوب والدعم اللوجسيت الالزم واملوارد املالية الضرورية لتنفيذ متطلبات اللوائح‬ ‫الصحية الدولية‪ ،‬وفقا للمادة ‪ 44‬من هذه اللوائح‪ .‬وال جيرى يف الوقت الراهن تقدمي هذا الدعم أو احلفاظ عليه‬ ‫ِ‬ ‫ضمن إطار متفق عليه‪.‬‬ ‫املتصور الذي ميكن أن َّ‬ ‫اختذ كثري من الدول األعضاء‬ ‫ويف إطار جماهبة التهديد‬ ‫يتشكل يف سياق مرض اإليبوال‪ّ ،‬‬ ‫َّ‬ ‫صدرت عن املديرة العامة‬ ‫تدابري صحية عمومية‪ ،‬مثل حظر السفر‪ ،‬وهو ما جتاوز التوصيات املؤقتة (‪ )4‬اليت َ‬ ‫للمنظمة بشأن طوارئ الصحة العمومية اليت تثري قلقا دوليا‪ ،‬ومل تكن متَّسقة مع الشروط اليت حددهتا‬ ‫املتضررة‬ ‫املادة ‪ 43‬من اللوائح‪ .‬و‬ ‫كان هلذه التدابري اإلضافية آثارا سلبية على السفر وعلى التجارة بالنسبة للبلدان ِّ‬ ‫من مرض فريوس اإليبوال‪.‬‬ ‫إن قرار منح متديد آخر لبناء القدرات الوطنية يف جمال الصحة العمومية املطلوبة مبقتضى اللوائح الصحية العمومية‬ ‫هو قرار يصدر عن املديرة العامة ملنظمة الصحة العاملية‪ ،‬مع األخذ يف االعتبار املشورة التقنية للجنة املراجعة املعنية‬ ‫انعقدت يف املدة ‪ 14–13‬تشرين الثاين‪/‬نوفمرب ‪ ،5014‬لتقدمي املشورة‬ ‫باللوائح الصحية الدولية‪ .‬و‬ ‫كانت اللجنة قد َ‬ ‫َّمة من الدول األطراف ملنحها متديدا ثاني ا (‪)5012 – 5014‬؛ و‪ )5‬كيفية‬ ‫للمديرة العامة حول‪ )1 :‬الطلبات املقد‬ ‫األمديْن القريب والبعيد‪.‬‬ ‫تعزيز وتقييم القدرات األساسية الالزمة لتنفيذ اللوائح الصحية الدولية بشكل أفضل على َ‬ ‫أوصت جلنة املراجعة املعنية باللوائح الصحية الدولية‪ ،‬يف تقريرها (‪( )1‬التوصية ‪ ،)7‬األمانة بأن "تلجأ إىل آليات‬ ‫و َ‬ ‫جتمع ما بني التقييم الذايت‬ ‫التشاور اإلقليمية لوضع خيارات لالنتقال من التقييم الذايت احلصري إىل هنوج َ‬ ‫كة جمموعة خمتلطة من اخلرباء احملليني واملستقلني"‪ .‬وقد اعتمد‬ ‫ومراجعة األقران والتقييم اخلارجي الطوعي مبشار‬ ‫اجمللس التنفيذي يف دورة انعقاده السادسة والثالثني بعد املائة ومجعية الصحة العاملية الثامنة والستون تنفيذ هذه‬ ‫التوصية‪.‬‬ ‫وذكر الفريق املعين بالتقييم املبدئي ملرض اإليبوال‪ ،‬يف تقريره (‪ ،)5‬أنه "ينبغي للمنظمة أن تقرتح خطة شاملة‪،‬‬ ‫مبوجب‬ ‫متنح األولوية ُ‬ ‫ُ‬ ‫وحتسب تكلفتها‪ ،‬لتطوير القدرات األساسية جلميع البلدان يف جمال الصحة العمومية‪َ ،‬‬ ‫‪4‬‬ ‫‪.14‬‬

‫كاالت املاحنة والدول األعضاء وسائر اجلهات املعنية‬ ‫ح هذه اخلطة على الو‬ ‫اللوائح الصحية (‪ .)5002‬وينبغي طر‬ ‫للحصول على التمويل‪ .‬وستحتاج املنظمة إىل معلومات موثوقة عن الوضع احلايل يف كل بلد من أجل اقرتاح‬ ‫ستخدم‬ ‫هذه اخلطة‪ ،‬وهذا أمر يستلزم شكالا من أشكال مراجعة األقران أو وسيلة أخرى للتقييم اخلارجي‪ .‬وال يُ َ‬ ‫سوى التقييم الذايت يف الوقت احلاضر‪ .‬ومن مصلحة البلدان أن حتصل على حتليل شامل وموضوعي؛ ويوصي‬ ‫السبُل لالضطالع بذلك"‪.‬‬ ‫الفريق باستكشاف ُ‬ ‫ح على هذا األساس تشكيل جلنة التقييم اإلقليمية املعنية باللوائح الصحية الدولية‪ ،‬كهيئة مستقلة‪،‬‬ ‫ر‬ ‫ويُقتَـَ‬ ‫عهد إليها باالختصاصات التالية‪:‬‬ ‫لإلشراف على تنفيذ اللوائح الصحية الدولية يف هذا اإلقليم ويُ َ‬ ‫‪.50‬‬

Results obtained d from the 2014 IHR R self-assessment ques stionnaires i indicated a regional implement tation level of 72% (3), with many y countries having met many of th he IHR requ uirements. However, during the missions car rried out to countries of the Region n in late 20 014 to assess s national preparedne ess and resp ponse to a potential p imp portation of Ebola, many y critical ga aps were ide entified in countries r related to th he preparedness and resp ponse capaci ities. This fin nding includ ded countries s that had previously y reported ha aving met the e obligations under the IH HR by the fir rst or second d deadline. 8. In th he majority of countries, the main gaps identif fied were the absence o of a fully op perational national co oordination structure s and d also of an e emergency operating o cen ntre within th he Ministry of o Health. The incid dent comman nd and cont trol chain w was either ab bsent or had d major stru uctural gaps, , with no standard o operating pr rocedures for r event man nagement an nd alerts. Na ational plans s for Ebola were not perceived as an integ gral part of the nationa al public he ealth plan fo or preparedn ness and res sponse to /emergencies and lacked d a multisec ctoral approa ach and standard operati ing procedur res. Drills epidemics/ and simulation exercises to test th he existing E Ebola plans had not bee en conducted d. An early y warning m for enhanced detection and respo onse to any suspected ca ase was eith her not funct tioning or mechanism absent, an nd event-bas sed surveillance was n not yet form malized. The e human re esources cap pacity for surveillanc ce and ana alysis of da ata was in nadequate. Multidisciplin M nary rapid response te eams for investigati ion and outb break respon nse at the dif fferent admi inistrative levels were no ot available and field investigati ion of disease outbreaks was w limited. 9. In the e majority of countries, a national in nfection prev vention and control c progr ramme with a defined structure, r sponsibilities s, plan of act tion and bud dget was not in place. Th he following were also roles and res not in plac ce in most countries: c pro ocedures for r safe medica al waste man nagement an nd the manag gement of blood and d body fluid spills; a national pr rogramme of o occupational safety; a laborator ry quality manageme ent system, including i sta andard opera ating proced dures for spe ecimen hand dling, inactiv vation and diagnosis; protocols or r guidelines on safe colle ection, transp portation and d shipment of f specimens; ; a biorisk laboratory y manageme ent system; and a biosa afety level 3 facility fo or handling emerging dangerous d pathogens and glove boxes b for ha andling and inactivation of suspected samples. E Ebola comm munication ncluding risk communic cation and s ocial mobili ization, was very weak and a comp prehensive strategy, in Ebola risk k communica ation plan as an integral p part of a natio onal risk com mmunication n plan was lac cking. 10. With regard to po oints of entry y, in most co ountries, pub blic health co ontingency p plans with fu unctioning standard o operating pro ocedures wer re not in plac ce, and coord dination betw ween the diff ferent stakeholders for points of entry and with w the nat tional system m was lacking. Standard d operating procedures for early onse to cases s were also not n in place, while comm munication with the air detection, assessment of and respo tor, includin ng airline co ompanies, re egarding pot tential cases was weak with a lack k of clear travel sect arrangeme ents for tracin ng contacts. Designated spaces to iso olate sick pas ssengers and d to interview w contacts were often n not availab ble, while ca apacity to un ndertake ent try screening g measures, if required, was very limited. G Ground cross sings were poorly p equip pped and had limited ca apacities to control mov vement of people ade equately.

Monitoring the co ompliance with IHR 11. The W World Health h Assembly has previous sly urged Me ember States s (resolution n WHA61.2) to ensure that the co ontact details s of the centr re designated d as the Nati ional IHR Fo ocal Point ar re complete and up to date to fac cilitate the co ommunicatio on between n national focal points and WHO contac ct points on a 24 hour basis. Mem mber States are a annually confirming the designation of IHR national n foca al points. How wever, 24 hour acces ssibility is no ot being main ntained.

12. The S Secretariat has h provided guidance on n proposed fu unctions for national n foca al points. Ho owever, in most coun ntries the fun nctions have not been pu ut in place. Where W availab ble, they are e neither regu ulated nor widely dis sseminated am mong the rel levant stakeh holders to fac cilitate imple ementation o of the IHR. 13. Mem mber States ar re required to notify WH HO of all pub blic health ev vents that are e assessed as s possibly constitutin ng internatio onal concern n within 24 hours of assessment by b using the e decision in nstrument provided i in Annex 2 of the IHR R. The notifi ication must t be followed by ongoin ng communi ication of detailed pu ublic health information on the even nt. Generally in the Regio on, the early y notification n to WHO and compr rehensive re eporting of such events i is not well maintained. m WHO W is man andated to su upport the risk assess sment of suc ch events and d to respond d to them wit thin the fram mework of th he IHR, eithe er directly or through h coordinatin ng mobilizati ion of the re quired suppo ort. However r, implement tation of this s mandate could be im mpeded by absence a of or r delay in no otification and sharing of comprehens sive informat tion about such event ts. 14. Coun ntries have often shown n reluctance e to share information i related to public health events occurring in their terri itory that ma ay constitute a public hea alth event of internationa al concern (P PHEIC) as such event ts may have political and d socioecono omic implica ations. The World Healt th Assembly therefore identified the need fo or an “alert” phase unde er the IHR. This T phase would w preced aring of a de the decla nd is intended d to encourage Member States to not tify WHO ea arly and to sh hare informa ation. The PHEIC an early investigation and d rapid respo onse to such events that would w result from an aler rt phase mig ght help to conditions fo or declaring a PHEIC. Th he Secretaria at is currently y developing g criteria to define d and avoid the c trigger the e alert phase. 15. Mem mber States are a required to collabora ate with eac ch other in the detection n, assessmen nt of and response t to events by y providing the necessar ry technical cooperation n, logistical s support and financial resources for the imp plementation of the IHR R in accorda ance with Article A 44. Th The provision n of such support is not currently y being prov vided in the c context of an agreed syste em or mainta ained. 16. Évaluation et suivi de l’application du Règlement sanitaire international 2005 : respect de la date butoir de 2016 Introduction 1. La Soixante-Huitième Assemblée mondiale de la Santé a examiné et approuvé le rapport du Comité d’examen du Règlement sanitaire international – RSI (2005) portant sur un deuxième délai supplémentaire pour la mise en place de capacités nationales de santé publique et sur l’application du RSI (1), ainsi que plusieurs rapports sur la flambée de maladie à virus Ebola, notamment celui du Groupe d’experts chargé de l’évaluation intérimaire de la riposte à Ebola (2). Parmi les préoccupations soulevées par le Groupe d’experts ainsi que par l’Assemblée mondiale de la Santé figurait la fiabilité des autoévaluations réalisées par les États Membres au sujet de l’obtention des capacités nationales requises au titre du RSI. Le Groupe d’experts a noté que « les pays concernés sont exposés à un risque important, ce qui majore celui qui pèse sur la communauté internationale ». 2. Concernant les maladies qui constituent une source de préoccupation immédiate en termes de sécurité sanitaire mondiale dans ce contexte, l’Assemblée mondiale de la Santé, dans le cadre de la discussion sur le RSI, s’est penchée sur les points suivants : le coronavirus du syndrome respiratoire du Moyen-Orient (MERS-CoV), identifié pour la première fois en Arabie saoudite en 2012 et qui circule aujourd’hui largement dans toute la péninsule arabique ; les nouveaux cas de maladie à virus Ebola qui continuaient de se déclarer en Afrique de l’Ouest au moment de la discussion ; les virus de la grippe aviaire A(H5N1) et A(H9N2), qui se propagent toujours chez les oiseaux, en se réassortissant avec d’autres virus de la grippe aviaire endémiques dans diverses parties du monde, et provoquent des infections chez l’homme en Égypte ; et la poliomyélite, en particulier la mise en œuvre de recommandations pour les pays qui exportent actuellement des poliovirus sauvages et les pays qui sont touchés par la poliomyélite mais n’exportent pas de poliovirus sauvage. 3. Le RSI fait obligation aux États Membres d’acquérir et de maintenir les principales capacités de santé publique requises pour la surveillance et la riposte, en particulier aux points d’entrée, afin de détecter rapidement, d’évaluer, de notifier et de déclarer à l’OMS les événements couverts par les dispositions du RSI, et de mettre en œuvre une riposte adaptée aux situations locales sur le terrain, sur les conseils du Comité d’urgence du RSI. Durant l’Assemblée mondiale de la Santé, les États Membres ont reconnu que les menaces actuelles pour la sécurité sanitaire mondiale révèlent un niveau insuffisant de préparation, et ont mis en exergue le besoin d’accélérer l’application du RSI et de renforcer les capacités de santé publique existantes. En outre, les États Membres ont souligné le rôle que l’OMS devait jouer en continuant de leur fournir l’appui technique requis, tout en notant qu’une approche stratégique différente devait être envisagée pour relever les défis actuels. 4. Le présent document décrit les limites du mécanisme actuel de suivi de l’application du RSI et les insuffisances constatées chez les États Membres en termes de capacités d’application. Il propose une nouvelle approche pour l’évaluation et le suivi de l’acquisition et du maintien des capacités requises pour le RSI d’une part, et du respect des dispositions contenues dans le Règlement d’autre part.

Suivi de la mise en œuvre des capacités du RSI pour la surveillance et la riposte 5. L’Assemblée mondiale de la Santé a décidé (dans sa résolution WHA61.2), conformément au paragraphe 1 de l’article 54 du Règlement sanitaire international, que les États Membres et le Directeur général devaient rendre compte chaque année de l’application du Règlement à l’Assemblée mondiale de la Santé. Un cadre de suivi a été élaboré à cet effet.

6. Ce cadre de suivi comprend une liste de contrôle, des indicateurs et un questionnaire d’autoévaluation. Il consiste à évaluer la mise en place de huit capacités principales, et à développer des capacités aux points d’entrée et pour les risques liés au RSI, en particulier les risques concernant les maladies infectieuses, zoonotiques, la sécurité sanitaire des aliments ainsi que les risques d’origine chimique, radiologique et nucléaire. Ce processus de suivi n’est pas destiné à être utilisé pour classer la performance des pays ni comparer leurs résultats. Il doit plutôt constituer un moyen d’aider les différents pays à suivre les progrès accomplis en vue de se doter des principales capacités requises en vertu du RSI et de remédier aux insuffisances constatées. 7. Les résultats découlant des questionnaires d’autoévaluation du RSI de 2014 faisaient ressortir un niveau d’application régional de 72 % (3), et de nombreux pays s’étaient dotés des principales capacités requises en vertu du RSI. Néanmoins, lors des missions effectuées dans les pays de la Région fin 2014 pour évaluer la préparation au niveau national et la riposte à une importation potentielle d’Ebola, de nombreuses insuffisances critiques ont été identifiées dans certains pays en termes de préparation et de capacités d’action. Ces constatations concernaient également des pays qui avaient précédemment déclaré s’être acquittés des obligations énoncées dans le RSI avant la fin du premier ou du deuxième délai. 8. Dans la majorité des pays, les insuffisances constatées portaient principalement sur l'absence de structure de coordination nationale pleinement opérationnelle et de centre opérationnel d’urgence au sein du ministère de la Santé. La chaîne de commandement et de contrôle des interventions était inexistante ou présentait des lacunes structurelles majeures, et ces pays ne disposaient d’aucun mode opératoire normalisé pour la gestion des événements et les alertes. Les plans nationaux de lutte contre la maladie à virus Ebola n’étaient pas considérés comme une composante essentielle du plan national de santé publique pour la préparation et la riposte aux épidémies et aux situations d’urgence, et étaient dépourvus d’approche multisectorielle et de modes opératoires normalisés. Ces pays n’avaient effectué aucun exercice ni aucune simulation pour évaluer les plans actuels de lutte contre Ebola. Les mécanismes d’alerte précoce permettant des améliorations en termes de détection de cas suspects et de riposte ne fonctionnaient pas ou étaient inexistants, et la surveillance des événements n’était pas encore systématisée. Les capacités en ressources humaines pour la surveillance et l’analyse des données étaient insuffisantes. Les pays ne disposaient pas d’équipes d’intervention rapide multidisciplinaires pour mener des enquêtes et intervenir en cas de flambées aux différents niveaux administratifs, et les enquêtes de terrain portant sur les flambées épidémiques étaient limitées. 9. La majorité des pays n’avaient pas adopté de programme national de lutte contre les infections présentant une structure, des rôles et des responsabilités, un plan d’action et un budget bien définis. La plupart d’entre eux n’avaient pas non plus mis en œuvre les éléments suivants : des procédures de gestion sans risque des déchets médicaux et de gestion des déversements de sang et de liquides biologiques ; un programme national de médecine du travail ; un système de gestion de la qualité au laboratoire, incluant des modes opératoires normalisés pour la manipulation, l’inactivation et le diagnostic des échantillons ; des protocoles ou des lignes directrices portant sur le prélèvement, le transport et l’expédition sans risque des échantillons ; un système de gestion des risques biologiques en laboratoire ; et un laboratoire de sécurité biologique de niveau 3 pour la manipulation d’agents pathogènes émergents et dangereux, ainsi que des boîtes de gants pour la manipulation et l’inactivation d’échantillons suspects. La stratégie de communication concernant Ebola, notamment la communication sur les risques et la mobilisation sociale, était particulièrement déficiente, et les pays ne disposaient d’aucun plan exhaustif de communication sur les risques pour Ebola susceptible de s’intégrer dans un plan national de communication sur les risques. 10. En ce qui concerne les points d’entrée, la plupart des pays n’avaient pas établi de plan d’intervention pour les urgences de santé publique assorti de modes opératoires normalisés fonctionnels, et il n’existait aucune coordination entre les différentes parties prenantes pour les points d’entrée et avec le système national. En outre, aucun mode opératoire normalisé pour la détection précoce et l’évaluation des cas et la riposte n’était en place. La communication avec le secteur du transport aérien – en 2

particulier les compagnies aériennes – au sujet des cas potentiels était très limitée et dépourvue de dispositions claires pour le suivi des contacts. Les espaces spécialement désignés pour isoler les voyageurs malades et interroger les personnes ayant été en contact avec ces derniers étaient rares, et les possibilités de prendre des mesures pour le dépistage aux points d’entrée, le cas échéant, étaient fort restreintes. Les postes-frontières souffraient d’un manque d’équipements et présentaient des capacités limitées en termes de contrôle adéquat des déplacements des voyageurs.

Suivi du respect des dispositions du RSI 11. L’Assemblée mondiale de la Santé avait précédemment invité instamment les États Membres (dans sa résolution WHA61.2) à veiller à ce que les coordonnées du centre désigné comme point focal national RSI soient complètes et à jour afin de faciliter la communication entre les points focaux nationaux et les points de contact de l’OMS 24 heures sur 24. Les États Membres continuent de confirmer chaque année les coordonnées de leurs points focaux nationaux RSI. En revanche, la possibilité de communiquer avec ces points focaux 24 heures sur 24 n’est pas maintenue. 12. Le Secrétariat a fourni des orientations sur les fonctions proposées pour les points focaux nationaux. Néanmoins, dans la majorité des pays, ces fonctions n’ont pas été mises en place. Là où elles ont été établies, elles ne sont ni réglementées ni largement diffusées auprès des parties prenantes concernées afin de faciliter l’application du RSI. 13. Les États Membres sont tenus de notifier à l’OMS les événements pouvant constituer une urgence de santé publique de portée internationale dans les 24 heures suivant l’évaluation au moyen de l’instrument de décision présenté à l’annexe 2 du RSI. Après une notification, l’État Partie continue de communiquer des informations de santé publique détaillées sur l’événement. D’une manière générale dans la Région, la notification rapide à l’OMS et le compte-rendu exhaustif de ces événements laissent à désirer. L’OMS a pour mission de soutenir l’évaluation des risques pour de tels événements et d’y faire face dans le cadre du RSI, soit directement, soit en coordonnant la mobilisation de l’appui requis. Or, l’exécution de cette mission pourrait être entravée par l’absence ou le retard de notification et de partage d’informations exhaustives sur ces événements. 14. Les pays se montrent souvent peu enclins à communiquer des informations relatives aux événements de santé publique qui se produisent sur leur territoire et sont susceptibles de constituer une urgence de santé publique de portée internationale, étant donné que de tels événements peuvent avoir des répercussions politiques et socioéconomiques. Aussi l’Assemblée mondiale de la Santé a-t-elle estimé qu’il était nécessaire de prévoir une phase « d’alerte » dans le cadre du RSI. Cette phase précèderait la déclaration d’une urgence de santé publique de portée internationale, et vise à inciter les États Membres à notifier rapidement ces événements à l’OMS et à communiquer des informations à ce sujet. Une enquête à un stade précoce et une réaction rapide à ce genre d’événements après une phase d’alerte pourraient permettre d’éviter la déclaration d’une urgence de santé publique de portée internationale. Le Secrétariat élabore actuellement des critères de définition et de déclenchement de cette phase d’alerte. 15. Les États Membres sont tenus de collaborer pour détecter et évaluer les événements, et y faire face, en fournissant la coopération technique, le soutien logistique et les ressources financières nécessaires à l’application du RSI conformément à l’article 44. Actuellement, ce soutien n’est ni fourni ni maintenu dans le cadre d’un système approuvé. 16. Pour faire face à la menace perçue dans le contexte d’Ebola, nombre d’États Membres ont mis en œuvre des mesures de santé publique, telles qu’une interdiction de voyager, qui allaient au-delà des recommandations temporaires (4) faites par le Directeur général de l’OMS dans le cas d’urgence de santé publique de portée internationale, et ne concordaient pas avec les conditions prévues à l’article 43 du Règlement. Ces mesures supplémentaires ont eu un impact négatif sur les voyages et le commerce pour les pays touchés par la maladie à virus Ebola. 3

Nouvelle approche d’évaluation et de suivi du RSI 17. La décision d’accorder un nouveau délai supplémentaire pour la mise en place des capacités nationales de santé publique requises dans le cadre du RSI est prise par le Directeur général, en tenant compte de l’avis technique du Comité d’examen du RSI. Le Comité s’est réuni les 13 et 14 novembre 2014 afin de conseiller le Directeur général sur : 1) les demandes reçues des États Membres relatives à l’obtention d’un deuxième délai supplémentaire (de 2014 à 2016) ; et 2) la meilleure manière de renforcer et d’évaluer les principales capacités du RSI à court et à long terme. 18. Dans son rapport (1) (recommandation 7), le Comité d’examen du RSI a recommandé au Secrétariat de « mettre au point, par le biais de mécanismes de consultation régionale, des options permettant de passer d’une autoévaluation exclusive à des formules associant l’autoévaluation, l’examen par les pairs et l’évaluation extérieure volontaire faisant intervenir un groupe mixte d’experts intérieurs et indépendants ». Le Conseil exécutif à sa cent trente-sixième session et la Soixante-Huitième Assemblée mondiale de la Santé ont approuvé la mise en œuvre de cette recommandation. 19. Dans son rapport (2), le groupe d’experts chargé de l’évaluation intérimaire de la riposte à Ebola a déclaré que « l’OMS devrait proposer un plan solide, hiérarchisé et chiffré visant à renforcer les principales capacités de santé publique de l’ensemble des pays au regard du Règlement sanitaire international (2005). Ce plan devrait être présenté aux organismes donateurs, aux États Membres et aux autres parties prenantes en vue d’un financement. Pour proposer un tel document, l’OMS devra disposer d’informations fiables concernant la situation actuelle dans chaque pays. Une forme d’examen collégial ou un autre type de validation externe est nécessaire pour cela. Seule l’autoévaluation est utilisée aujourd’hui. Il est dans l’intérêt des pays de faire une analyse approfondie et objective ; le groupe d’experts recommande d’étudier les moyens d’y procéder. » 20. Sur cette base, il est proposé d’établir une Commission d’évaluation régionale du RSI en tant qu’organe indépendant chargé de superviser l’application du RSI dans la Région, dont le mandat sera le suivant : a) examiner l’application du RSI dans la Région et vérifier les données recueillies par le biais de l’outil d’autoévaluation du RSI ; b) identifier les activités prioritaires stratégiques au niveau des États Membres pour l’application du RSI ; c) identifier les principaux aspects et problèmes devant être traités par l’OMS pour aider les États Membres dans la Région à s’acquitter de leurs obligations ; et d) rendre compte chaque année au Comité régional des progrès réalisés dans l’application du RSI dans la Région. 21. Il est également proposé d’établir un Groupe spécial régional pour le RSI, constitué d’experts de l’OMS et d’autres organismes partenaires, destiné à veiller à la mise en œuvre harmonisée des politiques, des stratégies et des plans établis conformément aux recommandations de la Commission d’évaluation régionale du RSI, dont le mandat sera le suivant : a) rassembler les éléments de preuve requis pour les décisions stratégiques de la Commission d’évaluation régionale du RSI ; b) appuyer la mise en œuvre des plans nationaux et régionaux pour le RSI ; c) conseiller le Directeur régional et la Commission d’évaluation régionale du RSI sur la marche à suivre ; et d) préconiser l’application du RSI et collaborer avec des partenaires susceptibles de contribuer à assurer la sécurité sanitaire mondiale.

22. L’OMS remplira les fonctions de secrétariat pour la Commission d’évaluation régionale du RSI et continuera à : a) évaluer l’application du RSI à la demande des États Membres et de la Commission d’évaluation régionale du RSI ; b) communiquer les informations pertinentes à la Commission et aux États Membres ; et c) orienter les États Membres et fournir l’appui nécessaire à l’application du RSI. 23. Les États Membres : a) continueront de compléter le questionnaire d’autoévaluation annuel relatif au suivi de l’application du RSI ; b) rendront compte à la Commission d’évaluation régionale des progrès réalisés dans l’application du RSI sur leur territoire ; et c) mettront en œuvre les recommandations de la Commission afin de s’acquitter des obligations énoncées par le RSI.

Méthode de travail dans le cadre de la nouvelle approche d’évaluation et de suivi du RSI 24. Le Secrétariat montrera la voie en appuyant les États Membres dans l’évaluation, le suivi et l’application du RSI. À la demande volontaire des États Membres et à la requête de la Commission d’évaluation régionale du RSI, le Secrétariat effectuera des missions auprès des États Membres pour évaluer l’application du RSI. 25. Un rapport de situation sur l’application du RSI dans chaque État Membre sera élaboré sur la base de chacune des visites d’évaluation et transmis à la Commission d’évaluation régionale du RSI. 26. Les États Membres sont incités à évaluer et documenter leur riposte aux événements de santé publique qui se sont produits dans leurs pays. Des exercices de simulation devront être réalisés en vue de poursuivre l’examen et l’amélioration des capacités nationales en termes de surveillance et de riposte. 27. La Commission d’évaluation régionale du RSI se réunira une fois par an lors de la rencontre annuelle des parties prenantes au RSI au niveau régional, et les États Membres présenteront les progrès accomplis dans l’application du RSI. La Commission examinera tous les documents et rapports mis à sa disposition par les États Membres et identifiera les activités prioritaires devant être menées par chaque État Membre pour renforcer et continuer à maintenir ses capacités nationales de santé publique sur la base des rapports disponibles. 28. Sur les conseils du Groupe spécial régional pour le RSI, le Secrétariat élaborera un plan d’action régional, incluant des activités prioritaires à court et à long terme, sur la base des rapports de situation et des documents disponibles et conformément aux recommandations du Comité régional, du Conseil exécutif et de l’Assemblée mondiale de la Santé pour appuyer l’application du RSI dans la Région. 29. Le plan d’action régional sera communiqué aux partenaires techniques et aux donateurs aux niveaux régional et mondial pour les inciter à le mettre en œuvre. 30. La Commission d’évaluation régionale du RSI pourra se réunir ponctuellement pour discuter de questions urgentes liées au RSI, à la demande du Directeur régional. 31. Chaque année, la Commission d’évaluation régionale du RSI, avec le soutien du Secrétariat, élaborera et présentera au Comité régional un rapport de situation sur l’application du RSI dans la Région, à partir des résultats obtenus par le biais de l’outil d’autoévaluation du RSI, des rapports de situation par pays sur l’application du RSI et des rapports établis par chaque pays sur des questions liées 5

au RSI. Les observations fournies en retour par le Comité régional seront communiquées au Conseil exécutif et à l’Assemblée mondiale de la Santé.

Outils destinés à appuyer l’évaluation et le suivi de l’application du RSI 32. Les outils existants décrits ci-dessous seront utilisés pour l’évaluation et le suivi de l’application du RSI. a) Une liste d’experts pour le RSI issus de la Région de la Méditerranée orientale et d’autres régions de l’OMS sera formée dans les spécialités suivantes : législation, suivi, laboratoire, points d’entrée, sécurité sanitaire des aliments et sécurité chimique, sécurité radiologique et communication sur les risques. Ces experts aideront l’OMS à évaluer l’application du RSI. b) Le protocole d’évaluation du RSI sera utilisé pour évaluer l’application du RSI au sein des États Membres. Les documents d’orientation de l’OMS viendront également étayer cette évaluation. c) Le questionnaire d’autoévaluation du RSI continuera d’être utilisé chaque année. Les résultats que livreront ces autoévaluations feront l’objet de vérifications durant les visites d’évaluation. 33. Par ailleurs, les outils ci-dessous seront mis au point pour appuyer l’évaluation et le suivi du RSI : a) Une matrice destinée à orienter la présentation des progrès réalisés dans l’application du RSI ; b) Une liste d’indicateurs de performance essentiels pour évaluer l’application du RSI ; c) Des modes opératoires normalisés et des scénarios pour mener à bien des exercices et des simulations visant à évaluer l’application du RSI. 34. Le Secrétariat continuera de mener des activités stratégiques s’appliquant à tous les États Membres afin de les aider à s’acquitter de leurs obligations.

Action du Comité régional 35. Le Comité régional est invité à étudier la proposition d’établissement de la Commission d’évaluation régionale du RSI et du Groupe spécial régional pour le RSI, ainsi que leurs fonctions et leur méthode de travail.
