SALMONELLA SURVEILLANCE, 1965-1972 A lgeria . — A total of 7 923 strains of salmonella were isolated in Algeria between 1965 and 1972. O f these isolations, S. typhi and S. paratyphi accounted for 2 062 from human sources and six from non-human sources, while 5 585 from human sources and 270 from non-human sources belonged to other salmonella serotypes. The total of 7 647 human isolations belonged to 42 serotypes while 276 non-human isolations belonged to 83 serotypes.

A lgérie . — On a isolé en Algérie entre 1965 et 1972 un total de 7 923 souches de salmonella. Parmi ces isolements, 2 062 souches de S. typhi et 5. paratyphi étaient d’origine humaine et six d'originenon humaine, contre 5 585 souches d ’origine humaine et 270 d'ori­ gine non humaine pour les autres sérotypes de salmonella. Les 7 647 isolements humains appartenaient à 42 sérotypes et les 276 isolements non humains à 83 sérotypes.

Human Isolations In the region of Algiers, 1 421 strains of S. typhi and S. paratyphi and 5 204 strains of other salmonella serotypes were isolated from man; in the rest of the country, these figures were 641 and 381 respectively. Of the 2 062 isolations of S. typhi and S. paratyphi, the predomin­ ant serotype was S. typhi with 1 898 isolations (92.0 %) ; * 1* S . paratyphi A and B were relatively infrequent with 85 isolations (4.1%) and 78 isolations (3.8 %) respectively, while S. paratyphi C was isolated only once. The majority of these isolations was obtained from haemocultures (1 503) and coprocultures (424). During the period 1965-1966 only 15 isolations from coprocultures were obtained, as against 350 in 1971-1972. The reason for this increased frequency as from 1971 is undoubtedly linked with the intensification of surveillance of typhoid and paratyphoid fevers in Algeria as well as an improvement of methods of stool examination. Of the 5 585 isolations of salmonella serotypes other than S. typhi and S, paratyphi, S. wien was predominant with 3 333 isolations, followed by S. typhimurium (1 034 isolations), S. derby (476), S. seriftenberg (454), S. manhattan (164) and various other serotypes (124). Most of these isolations originated from coprocultures (5 074) and haemocultures (228). The septicaemic syndrome was observed mostly in children under three years of age. O f the 3 333 isolations of S. wien, 81 were found in blood (2.4%) against 116 out of 1 034 S. typhimurium (11.2%), which suggests that S. typhimurium causes septicaemia more easily than S. men. Menin­ gitis among children under three years of age was relatively significant since 40 strains of salmonella serotypes other than .S. typhi and S. paratyphi were isolated from cerebrospinal fluid, including 17 S. wien and 14 5. typhimurium. The incidence of S. wien represented 59.7%; this serotype was first isolated in Algeria in November 1969, and has since been responsible for epidemic outbreaks of severe gastro-enteritis in paediatric depart­ ments of the country.4 In contrast to reports of numerous other national centres, S. typhimurium represented slightly less than 20% of the strains of salmonella other than S. typhi and S. paratyphi identified; moreover, until 1967, this serotype was very rarely observed in Algeria. S. senftenberg appeared in Algiers m 1971 and, between May 1971 and December 1972,454 strains were isolated (8.1%). S. derby and S. manhattan were the most frequently isolated serotypes during the period 1965-1968, with 389 and 81 strains respectively, while the other serotypes were rare during this period. Between 1969 and 1971, these two serotypes were re­ placed by S. wien and S. typhimurium, to which S. senftenberg was added in 1972 when most of the 454 strains of this serotype were isolated. S. isangi, isolated for the first time in Algeria in 1970 from two coprocultures, has been responsible for a severe epidemic out­ break of salmonellosis in paediatric departments of Algiers since 1973. It is also reported that investigations earned out among the medical and para-medical staff of two paediatric departments in Algiers revealed that out of 96 staff members from whom copro­ cultures were taken 26 were salmonella carriers; S. wien was isolated 18 times and S. typhimurium eight times (on two occasions both serotypes were present).

Isolements d ’origine humaine Dans la région d ’Alger, 1421 souches de S. typhi e t S . paratyphi et 5 204 souches d ’autres sérotypes de salmonella ont été isolées chez l’homme contre, respectivement, 641 et 381 dans le reste du pays. Parmi les 2 062 isolements de S. typhi et S . paratyphi, S . typhi prédomine avec 1 898 isolements (92,0% );1 S . paratyphi A et S. paratyphi B furent relativement peu fréquentes avec 85 isolements (4,1 %) et 78 isolements (3,8%) respectivement, alors que S. para­ typhi C ne fut isolée qu’une seule fois. La plupart de ces isolements furent obtenus par hémoculture (1 503) et par coproculture (424). Pour la période 1965-1966, on n ’a obtenu que 15 isolements par coproculture contre 350 pour la période 1971-1972. Cette augmenta­ tion à partir de l ’année 1971 est liée, certes, à un renforcement de la surveillance des fièvres typhoïde et paratyphoïdes en Algérie, mais elle est également liée à une amélioration des méthodes de recherche des germes dans les selles. Sur les 5 585 isolements de salmonella autres que S . typhi et S. paratyphi, S . wien fut prédominante avec 3 333 isolements, et elle est suivie par S. typhimurium (1 034 isolements), S . derby (476), 5. senftenberg (454), 5. manhattan (164) et divers autres sérotypes (124). La plupart de ces souches furent isolées de coprocultures (5 074) et d ’hémocultures (228). Les syndromes septicémiques furent observés, dans la plupart des cas, chez des enfants de moins de trois ans. 81 isolements de S. wien sur 3 333 (2,4%) provenaient de pré­ lèvements de sang, contre 116 isolements de 5 . typhimurium sur 1 034 (11,2%), ce qui laisse à penser que S. typhimurium provoque des manifestations septicémiques plus facilement que S . wien. Les manifestations méningées observées chez les enfants de moins de trois ans sont relativement importantes puisque 40 souches de Salmonella autres que S. typhi et S. paratyphi furent isolées dans du liquide céphalo-rachidien, dont 17 S . wien et 14 S. typhimurium. L ’incidence de S . wien s’élève à 59,7%; ce sérotype fut isolé pour la première fois en Algérie en novembre 1969, et fut ensuite responsable de poussées épidémiques de gastroentérite grave dans des services de pédiatrie du pays.4 Contrairement à ce qui est signalé par de nombreux centres nationaux, S. typhimurium ne représente qu’un peu moins de 20% des souches de salmonella autres que S . typhi et S. paratyphi identifiées; en outre, avant 1967, ce sérotype n ’était qu’exceptionnellement signalé en Algérie. S. senftenberg a fait son apparition à Alger en 1971 et, entre mai 1971 et décembre 1972,454 isolements de ce sérotype ont été enregistrés (8,1 %). S . derby et S. manhattan ont été les sérotypes le plus couramment isolés au cours de la période 1965-1968, avec, respectivement, 389 et 81 souches, alors que les autres sérotypes étaient rares au cours de la même période. Entre 1969 et 1971, ces deux sérotypes ont fait place à S. wien et S. typhimurium, auxquelles est venue s’ajouter S . senften­ berg en 1972, année où la plupart des 454 souches die ce sérotype ont été isolées. S. isangi, isolées pour la première fois en Algérie en 1970 dans deux coprocultures, est, depuis 1973, responsable d ’une grave poussée épidémique de salmonellose dans les services de pédiatrie d’Alger. Signalons d ’autre part que des enquêtes effectuées parmi le personnel médical et paramédical de deux de ces services ont révélé que, sur 96 membres du personnel pour lesquels o n pratiqua des coprocultures, 26 étaient porteurs de salmonella; S . wien fut isolée 18 fois et S. typhimurium huit fois (S. wien et S. typhimurium furent trouvées deux fois en association). 1 Une souche de S. lypki envoyée pour examen en 1973 au Centre international de Référence de Lysotypte entérique, Londres, s’est montrée résistante au chloramphéntcol. 1 Le génie épidémique de la souche de S. wien en cause dans ces épidémies semble lié à un facteur transférable de résistance aux antibiotiques. Les souches en cause étaient toutes résistantes a !'ampicilline, a la streptomycine, au chlorampbénlcol et aux sulfamides, et, pour plus de 90% d'entre elles, à la kanamycine et à la étracyclmc; dies étaient cependant toutes sensibles i la furazolidone et à la gentamyciac, et la plupart d’entre elles étaient sensibles i la coltsune.

The 0-3 years age group accounted for only 4.5 % of the cases of typhoid fever, against 82% of the cases associated with other salmonella serotypes. In the 4-49 years age group, salmonella other than S, typki and S. paratyphi were observed mostly among patients contaminated during prolonged hospitalization. Isolations from Non-Human Sources From non-human material, 276 strains belonging to 83 serotypes were isolated. The principal sources o f isolation as well as the principal serotypes isolated from each of these sources are listed below. Human foods and animal feeds: 11 isolations were obtained from sausages (merguez)—S. muenster, S. bovismorbificans, S. typhi­ murium; 14 isolations from dehydrated food—14 S. oranienburg; 33 isolations from meat meal and bone meal for animals—S. eastbourne, S. cerro, S. typhimurium; 4 isolations from fish meal for animals—5. Havana.

Isolements d’origine non humaine On a isolé dans du matériel non humain 276 souches appartenant à 83 sérotypes. Les principales sources d ’isolement et les principaux sérotypes isolés dans chacune d ’elles sont indiqués ci-après.

Parmi les aliments pour l'homme et les animaux, on signala surtout les saucisses (merguez) avec 11 isolements (S. muenster, S. bovismorbificans, S. typhimurium), les aliments déshydratés avec 14 isolements (14 S . oranienburg), les farines de viande ou d ’os pour la nourriture des animaux avec 33 isolements (S. eastbourne, S. cerro, S. typhimurium) et les farines de poisson pour la nourriture des animaux avec 4 isolements (S. havana).
