Between October 1994 and February 1995, influenza A(H3N2), influenza A(H1N1) and influenza B viruses continued tp circulate in many countries in Asia, the Americas and Europe. There were few reports of influenza in October-December 1994, but in January and February 1995 influenza activity increased. Influenza was confirmed for the first time in the 19941995 season in an outbreak of influenza B in northern Portugal in October 1994. Elsewhere in Europe, a few cases of influenza A and influenza B were reported during October, November and December 1994. Outbreaks of influenza A(H3N2) were reported in the United States of America and in several regions of China during December 1994 and January 1995. By mid-February 1995, 33 coun­ tries had reported influenza A and/or influenza B, includ­ ing 8 countnes in Asia, 4 in the Americas and 20 in Europe. Influenza A(H3N2)

D ’octobre 1994 à février 1995, les virus grippaux A(H3N2), A(H1N1) et B ont continué à circuler dans nombre de pays d'Asie, des Amériques et d’Europe. La grippe n ’a guère été signalée d'octobre à décembre 1994, mais l'activité grippale s’est accrue en janvier et février 1995. La présence de la gnppe a été confirmée pour la première fois durant la saison 1994-1995 lors d’une flambée de grippe B q r n a éclaté dans le nord du Portugal en octobre 1994. Ailleurs en Europe, on a signalé quelques cas de grippe A et de gnppe B en octobre, novembre et décembre 1994. Des flambées de grippe A(H3N2) ont été signalées aux Etats-Unis d’Amérique et dans plusieurs régions de Chine en décembre 1994 et en janvier 1995. Grippe A(H3N2)

Very few isolates of influenza A(H3N2) virus were re­ ported during October and November. By mid-February 1995, influenza A(H3N2) viruses had been isolated in Asia (China, Guam, Israel, Japan, and the Republic of Korea), Europe (Finland, France, Iceland, Italy, the Netherlands, Norway, the Russian Federation, Spain, Sweden, Switzer­ land, the United Kingdom and the former Yugoslavia), N orth America (Canada and the United States) and South America (Argentina). In Spain and most regions of the United States, influenza A(H3N2) viruses were the predominant cause of influenza. 1 See No. 50, 1994, pp 373-377

In haemagglutination inhibition ( H I ) tests with post-infec­ tion ferret sera, many of the recent influenza A(H3N2) isolates were antigenically distinguishable from A/Shangdong/9/93, the vaccine strain recommended in 1994.' The antigenic characteristics of a number of them are shown in Table 1 which includes A/Tohannesburg/33/94 as a refer­ ence virus.

Dans les épreuves d ’inhibition de l’hémagglutination (IH) effec­ tuées à l’aide de sérums de furet post-infection, nombre d ’isole­ ments grippaux A(H3N2) récents se sont révélés antigéniquement distincts de la souche A/Shangdong/9/93 qui avait été recomman­ dée comme souche vaccinale en 1994.' Post-immunization H I antibodies at titres of & 40 against the H3N2 vaccine virus were detected in the sera of 48% of children, 52%-93% of adults, and 52%-87% of the elderly. However, post-immunization H I antibody litres at & 40 to representative recent isolates such as A/Johannesburg/33/94, were found in a lower proportion of vaccinées: 39% of children, 30%-90% of adults, and 38%-77% of the elderly. The post-immunization geometric mean utres in sera of adults and the elderly were approximately 50% lower to A/Iohannesburg/33/94 than to the vaccme virus. Post-immunization H I antibodies at titres of =* 40 against the influenza A(H1N1) vaccine virus were detected in the sera of 67% of children, 85%-100% of adults, and 50%-100% of elderly vaccinées. For representative recent isolates of A(H1N1) virus, 74% of children, 80%-100% of adults and 53%-100% of elderly vaccinées had H I titres 3= 40. Geometric mean titres for the recent isolates were generally similar to those for the vaccine viruses. For the influenza B vaccme virus, post-immunization H I antibodies at litres of 3= 40, or equivalent titres ob­ tained by single radial haemolysis assays, were detected in the sera of 65% of children, 75%-100% of adults, and 55%-100% of the elderly. Antibodies at similar frequen­ cies were detected against representanve recent influenza B virus isolates such as B/Shanghai/4/94 (a B/Beijing/184/ 93-like virus): 67% of children, 72%-97% of adults, and 33%-100% of the elderly. However, in some studies, the post-immunizanon geometric mean titres of H I antibodies to B/Shanghai/4/94 were 30%-50% lower in adults and the elderly than those to the vaccine virus. Recommendations fo r the composition of influenza virus vaccines

During the 1994-1995 season, influenza A(H3N2), A(H1N1) and influenza B viruses continued to circulate. In many countries, influenza A(H3N2) viruses were isol­ ated from outbreaks and sporadic cases. Many recent iso­ lates were antigenically distinguishable from the current vaccine strain A/Shangdong/9/93 and were similar to the recent reference strain A/Johannesburg/33/94. Vaccines containing A/Shangdong/9/93-like viruses induced serum H I antibody responses to A/Johannesburg/33/94-like strains at a lower frequency and titre than to the vaccine strain. Sporadic isolates of influenza A(H1N1) virus from China and Singapore were closely related antigenically to the current vaccine viruses. Influenza B viruses circulated widely and were the predominant type in several countries early in the season. The majority of characterized isolates were antigenically distinguishable from the vaccine strain B/Panama/45/90 and were similar to the recent reference strain B/Beijing/ 184/93. 55

Au cours de la saison 1994-1995, les virus grippaux A(H3N2), A(H1N1) et B ont continué à circuler. Dans de nombreux pays, on a isolé des virus A(H3N2), soit à l’occasion de flambées, soit sur des cas sporadiques. Les virus B ont très largement circulé et, en début de saison, ils constituaient le type prédominant dans plusieurs pays. La caracté­ risation des isolements a montré que la plupart d’entre eux, anti­ géniquement distincts de la souche vaccinale B/Panama/45/90, étaient analogues à la souche de référence récente B/Beijing/184/ 93.

As m previous years, the specific viruses used in each country should be approved by the national control authorities. Most of the population is likely to have been infected with influenza A(H3N2), influenza A(H1N1) and influ­ enza B viruses in recent years. As a consequence, 1 dose of inactivated vaccine should be immunogenic for individuals of all ages except young children. Previously ummmunized children should receive 2 doses of vaccines, with an inter­ val between doses of at least 4 weeks. Reagents for use in laboratory standardization of inactivated vaccine may be obtained from the Division of Virology, National Institute for Biological Standards and Control, Blanche Lane, South Mimms, Potters Bar, Hertfordshire, EN6 3QG, United Kingdom (fax: +44(707)-64 68 54), or from the Division of Viral Products, Center for Biologies Evaluation and Research, Food and Drug Administration, 1401 Rockville Pike, Rockville, M D 20852, United States of America (fax: +l(301)-496 18 10). Reference strains for antigenic analysis may be ob­ tained from the W HO Collaborating Centers for Refer­ ence and Research on Influenza, at Influenza Branch, Division of Viral and Rickettsial Diseases, National Center for Infectious Diseases, Centers for Disease Control and Prevention, Atlanta, GA 30333, United States of America (fax: +1(404)-639 23 34) and at National Institute for Medical Research, The Ridgeway, Mill Hill, London NW7 1AA, United Kingdom (fex: +44(81)-906 44 77). • A review of the prevalence of influenza viruses in the world is scheduled for publication in th e WER on the last Friday of September 1995 for consideration by those mak­ ing recommendations for the composition of influenza virus vaccines for use in the southern hemisphere.

Expanded Programme on Immunization immunization of adults against diphtheria P o lan d . A recent study has assessed the reactogenicity and immunogenicity of diphtheria vaccines containing a reduced amount of diphtheria toxoid intended for the immunization of adults. Locally produced tetanusdiphtheria (Td) vaccine, licensed in Poland in 1993, was adsorbed on aluminium hydroxide (0.4 mg per dose) and contained 2 L f‘ of diphtheria toxoid (10 IU ” . of potency) and 10 B U “‘ o f tetanus toxoid (40 IU o f potency) per 0.5 ml dose. Toxoid purity was high: 2 000 BU of tetanus toxoid and 2 100 L f of diphtheria toxoid per mg of nitro­ gen protein. Two groups o f adult volunteers each received 1 dose of this T d vaccine: 51 students of medical schools, aged 19 to 24 years, and 40 workers of the National Institute of Hygiene and their families, aged 25 to 65 years. All stated that they had been previously vaccinated against diphtheria and tetanus. Reactions after immunization were assessed during 3 post-vaccinal days by means of a special ques­ tionnaire filled by vaccinées. Blood samples were taken just before and 4 weeks after immunization, and die level of Floculation units, International units of potency Rinding units.

Des volontaires adultes répartis en 2 groupes ont reçu chacun une dose de ce vaccin Td: 51 étudiants en médecine, âgés de 19 à 24 ans, et 40 agents de l'Institut national d'Hygiène et leur femille, âgés de 25 à 65 ans. Tous ont dit avoir déjà été vaccinés contre la diphtérie et le tétanos. Les réactions à la vaccination o nt été évaluées lois de 3 journées postvaccinales à l’aide d ’un question­ naire spécial rempli par les sujets vaccinés. Des échantillons de sang ont été prélevés juste avant la vaccination et 4 semaines après, et l’on a déterminé par hémagglutination passive les taux d’antiUnités floculantes Umtes internationales d ’acuvitc. diphtheria and tetanus antibodies was determined by passive haemagglunnauon. A level o f 0.1 IU/ml was considered as a minimum protective level and 1 IU/ml as the evidence of a strong immunity. T he vaccine was well tolerated. Slight and temporary reacnons were reported 1-2 days after immunization and disappeared after 2-3 days. Forty per cent of vaccinées reported some pain, 19% redness and/or induration at the injection site, 13% malaise and 34% a slight increase of body temperature up to 37 °C. Before immunization, 73% of vaccinées (68/91) had diphtheria annbody levels of 0.1 IU/ml or more and 44% (40) had at least 1 IU/ml (Table 1). T he group aged 19 to 24 years showed a higher level of immunity than the oldest group aged 50 to 65 years (22% [11/51] and 36% [5/14] of persons with < 0 . 1 IU/ml and 51% [26/51] and 29% [4/14] with at least 1 IU/ml respectively). T he geometric mean of diphtheria antibodies was 0.6 IU/ml in persons aged 19 to 49 years and 0.2 IU/ml in persons aged 50 years or more.

Avant la vaccination, 75% des sujets (68/91) présentaient des taux d ’anticorps antidiphténques de 0,1 Ul/ml ou plus, et 44% (40) des taux au moins égaux à 1 Ul/ml (.Tableau 1). One dose of T d vaccine stimulated a good production of diphtheria antibodies (Table 2). All but 1 person showed a diphtheria annbody level of 0.1 IU/ml or more and 90% (82/91) showed a level of 1 IU/ml or higher. The geometric mean of diphtheria antibody levels increased 11,13 and 21 times in the age groups 19-24, 25-49 and 50-65 years, respecnvely. The exception, a person of 44 years of age, had no measurable diphtheria antibodies before immuni­ zation (<0.03 IU/ml). His diphtheria immunization history is unknown. He responded well to the tetanus component of T d vaccine with an antibody level of more than 1 IU/ml. Six persons from the group of 40 workers of the National Institute of Health who responded to T d vaccine with moderate levels of antibodies (0.1-0.5 IU/ml) were reimmunized with monovalent diphtheria toxoid (“d ”) containing 2 Lf per dose. All these persons had responded to T d vaccination with high levels of tetanus antibody (Table 3) and monovalent diphtheria toxoid was therefore used for their subsequent diphtheria vaccination. Editorial Note: The outbreak of diphtheria in Belarus, the Russian Federation and Ukraine poses a threat to neighbouring countries.1 ,2,3 An increased incidence of diphtheria was reported in Poland in 1992 and 1993 (1 case in 1992, 10 in 1993), and 2 cases had been reported by November 1994. To prevent the spread of diphtheria in Poland, several measures were undertaken, including the provision of an additional routine booster dose of T d vaccine for all adolescents at the age of 19, and selecnve T d immunization in 10 eastern provinces for customs and passport control officers at the border checkpoints and for those who have frequent contact with visitors from eastern neighbouring countries.4 T he potential for spreading diph­ theria infection has been determined by waning immunity among adults, and the increased flow of visitors from countries suffering severe outbreaks of diphtheria. Primary immunizations are performed in Poland with diphtheria-pertussis-tetanus (DPT) vaccine at 3, 4‘ A and 6 months, followed by an additional dose of D P T vaccine at 19-24 months of age, and 2 booster doses of low volume of diphtheria-tetanus (DT) vaccine at 6 and 14 years of age. Diphtheria cases occurred in persons aged 20-29 years who had been immunized at pre-school and school age but in whom the last dose of vaccine containing diphtheria toxoid had been given 8-12 years earlier, with subsequent waning of acquired diphtheria immunity. The routine ad­ ministration of T d vaccine in adolescents and the selective immunization of adults aimed at increasing diphtheria immunity in these age groups.

Note de la Rédaction: La flambée de diphtérie au Bélarus, dans la Fédération de Russie et en Ukraine fait peser une menace pour les pays voisins.1,2 ,3 Une augmentation de l’incidence de la diphté­ rie a été signalée en Pologne en 1992 et 1993 (1 cas en 1992, 10 cas en 1993) et, fin novembre 1994, 2 cas avaient été signalés. Pour éviter la propagation de la maladie en Pologne, plusieurs mesures ont été prises, dont l'administration systématique d'une dose sup­ plémentaire de rappel de vaccin T d à tous les adolescents âgés de 19 ans, et une vaccination sélective dans 10 provinces de l’est du ; pays parmi les agents des douanes et de la police des frontières et j les personnes qui sont fréquemment en contact avec des visiteurs j en provenance de pays de l’Est voisins.4 Le recul de l’immunité parmi les adultes et l’accroissement du nombre de visiteurs en provenance de pays touchés par de graves flambées de diphtérie ont permis de déterminer qu’il y avait un potentiel de propagation de la diphtérie. En Pologne, les primovaccinations sont pratiquées avec du vaccin antidiphtérique-antitétanique-anticoquelucheux (DTC) à 3, 4‘ A et 6 mois et sont suivies par l’administration d ’une dose supplémentaire de vaccin D TC à 19-24 mois et de 2 doses de rappel d’un faible volume de vaccin antidiphtérique-antitétanique (DT) à 6 et 14 ans. Des cas de diphtérie se sont produits chez des sujets âgés de 20 à 29 ans qui avaient été vaccinés à l’âge pré­ scolaire et scolaire mais chez lesquels la dernière dose de vaccin contenant de l’anatoxine diphtérique avait été administrée 8 à 12 ans auparavant et l’immunité acquise contre la diphtérie avait diminué par la suite. This small study shows that 1 dose of T d vaccine containing a reduced amount of diphtheria toxoid is well tolerated and exerts a reasonable booster action in persons previously immunized against diphtheria. However, in about 10% of the persons studied 1 dose of this vaccine did not trigger a sufficient response. Seven of these persons (who had responded well to the tetanus component of T d immunization) responded to an additional booster dose of monovalent vaccine containing a reduced amount of diph­ theria toxoid (diphtheria antibody levels of 1 IU/ml or more). Further data are needed to develop a safe and effective schedule for the immunization of adults against diphtheria.

Influenza B elgium (12 February 1995).J A few cases of influenza B and 1 of influenza A were confirmed by virus isolation during January. T he incidence of acute respiratory illness has remained below the epidemic threshold throughout the season and the number of cases of influenza-like illness has also been low. F in lan d (14 February 1995) .2 Outbreaks of influenza-like illness have been reported in a few military units in the southern part of the country. Several cases of influenza A were diagnosed in the past week; in 3 cases influenza A(H3N2) virus was isolated. Influenza B was also diag­ nosed in a few cases this season. F ra n c e (15 February 1995).3 The number of laboratoryconfirmed cases of influenza increased in the past 3 weeks, but cases have remained sporadic. About two-thirds of the isolates have been influenza A and one-third influenza B. All influenza A viruses further identified have been of the H3N2 subtype. Italy (14 February 1995).4 Several influenza A(H3N2) and influenza B viruses were isolated from cases during local outbreaks in Milan, Rome and Siena in the last week of January and first week of February.

Influenza B virus was isolated from 3 cases at the beginning of January and influenza A(H1N1) virus from 1 case at the end of the month.

Slovakia (16 February 1995).6 The incidence of acute respiratory diseases was moderate and mainly associated with non-influenza viruses in January. Influenza A was, however, diagnosed by anngen detection in 2 cases, influ­ enza B in 1 and both types in 1 case. Three of the cases were in the elderly and 1 case of influenza A was in a schoolboy. United Kingdom (21 February 1995).4 Clinical reports on influenza and influenza-like illness in one surveillance scheme in England have increased and reached 157.9 per 100 000 population in the week ending 12 February. This is higher than in the 1990-1991 season, the last time influenza B was predominant. Other indices suggest that influenza infections are continuing to rise. U n ited S tates o f A m erica (17 February 1995).1Influen­ za continues to gradually increase and in the second week of February 18 states reported regional or widespread activity compared with 14 states in the first week. Seventytwo per cent of the influenza viruses isolated this season have been influenza A and 28% influenza B; influenza A has been the predominant type in m ost regions except the South-Atlantic Region where influenza B accounted for 56% of the reported isolates. Influenza B is, however, increasing in the Mid-Atlantic Region. Except for 1 influ­ enza A(H1N1) virus isolate reported in January, all influ­ enza A viruses further typed have been of the H3N2 subtype. T he propomon of positive isolates from specimens tested increased from 2% in early December to 21% in early February. Pneumonia and influenza mortality has remained at, or near, expected levels so far this season. F o rm e r Y ugoslavia (14 February 1995). The incidence of influenza-like illness has been below epidemic levels but showed some increase, in January to between 5.2% and 7.5% in the period 8 January to 11 February.
