YELLOW FEVER SURVEILLANCE : Epidemic in Ghana and Upper Yolta = SURVEILLANCE DE LA FIÈVRE JAUNE : Epidémie au Ghana et en Haute-Volta

YELLOW FEVER SURVEILLANCE Epidemic in Ghana and Upper Yolta The first report of yellow fever cases in Ghana was received in mid-October. Retrospective inquiries suggested that the outbreak began in July 1983. Up to 1 December, 232 cases including 158 deaths were notified from the Bole and West Gonja districts of the Northern Region, which borders on Upper Volta. The diagnosis was confirmed by liver histopathology. Immunization was carried out. In U pper Volta, the first clinically suspect cases were reported to the authorities on 4 October 1983.1Retrospective inquiries suggested that the earliest cases had occurred on 18 September 1983 at Fada N’Gourma. Vaccination was begun on 6 October, only 2 days after the first report. The diagnosis was confirmed histologically on 19 October, WHO and the 6 neighbouring countries were officially noti­ fied on 24 October, and 2 days later the public in Upper Volta was advised by radio and television. As of 29 November, 356 cases, including 286 deaths, had been notified from 4 departments to the east and south-east of Ouaga­ dougou, mainly among children under 15 years of age belonging to the nomadic Peulh peoples. The entomological findings implicate the jungle mosquitos Aedes furafer and Ae. taylori Although cases occurred in the area of Koupela, only about 120 km by road from Ouagadougou, a city of 172 661 people (1978 census), not a single urban case was reported. Very few domestic Aedes aegypti were found in the towns, or in Ouagadougou. The dry season began at the end of October and it was expected that this would interrupt transmission by its effect in reducing mosquito breeding. In fict, the outbreak appeared to have ended by mid-November. The following control measures were taken: — formation of a high-level national committee to coordinate activ­ ities against yellow fever; — immunization in affected localities and large-scale immunization in Ouagadougou; over 1 million people were immunized in 3 weeks with vaccine supplied by the WHO Collaborating Centre at the Pasteur Institute (Dakar) through the Organization, with the participation of other agencies; — establishment of health controls at the borders with Niger, Togo and Ghana; vaccination was carried out in Togo; — ground spraying against domestic mosquitos in Ouagadougou; this began on 11 November. In addition, WHO provided 3 consultants (a virologist, an ento­ mologist and an epidemiologist) as well as various equipment and supplies. 1See No 44, 1983, p. 342.

SURVEILLANCE DE LA FIÈVRE JAUNE Epidémie au Ghana et en Haute-Volta Le premier rapport signalant des cas de fièvre jaune au Ghana a été reçu à la mi-octobre. D’après les enquêtes rétrospectives, la flambée aurait débuté en juillet 1983. Jusqu’au 1erdécembre, 232 cas, dont 158 mortels, ont été notifiés dans les districts de Bole et de West Gonja de la Région Nord, qui jouxte la Haute-Volta. Le diagnostic a été con­ firmé par examen histopathologique du foie. Des vaccinations ont été effectuées. En Haute-Volta, les premiers cas cliniquement suspects ont été signalés aux autorités le 4 octobre 1983.' D’après les enquêtes rétro­ spectives, les premiers cas s’étaient produits le 18 septembre 1983 à Fada N’Gourma. La vaccination a débuté le 6 octobre, 2 jours seule­ ment après la première notification. Le diagnostic a été confirmé par examen histologique le 19 octobre, TOMS et les 6 pays voisms ont été officiellement notifiés le 24 octobre et, 2 jours plus tard, le public en Haute-Volta a été avisé par la radio et la télévision. Au 29 novembre, 356 cas, dont 286 mortels, avaient été notifiés dans 4 départements à l’est et au sud-est de Ouagadougou, principa­ lement chez des enfants de moins de 15 ans appartenant à l'ethnie nomade des Peuls. Les résultats des études entomologiques incrimi­ nent les moustiques selvatiques Aedes fitrcifer et Ae. taylon. Bien que des cas se soient produits dans la région de Koupela, à 120 km par la route seulement de Ouagadougou, ville de 172 661 habitants (recen­ sement de 1978), pas un seul cas urbain n’a été signalé. Très peu de moustiques Aedes aegypti domestiques ont été découverts dans les villes ou à Ouagadougoa La saison sèche a débuté à la fin d’octobre et l’on a pensé qu'elle interromprait la transmission du fait qu’elle a pour effet de réduire la multiplication des moustiques. D’ailleurs, il semble que la flambée se soit éteinte dès la mi-novembre. Les mesures de lutte suivantes ont été prises: — constitution d’un comité national de haut niveau pour coordonner les acuvités contre la fièvre jaune; — vaccination dans les localités atteintes et vaccination sur une grande échelle à Ouagadougou; des vaccins ont été administrés à plus d’un million de personnes en 3 semaines ; ces vaccins ont été fournis par le Centre collaborateur de l’OMS à l’Institut Pasteur (Dakar) par l’intermédiaire de l’Organisation, avec la participation d’autres organismes; — institution d’un contrôle sanitaire aux frontières avec le Niger, le Togo et le Ghana; la vaccination a été entreprise au Togo; — à partir du 11 novembre, pulvérisations au sol contre les mousti­ ques domestiques à Ouagadougou. En outre, l’OMS a fourni 3 consultants (un virologiste, un entomo­ logiste et un épidémiologiste) ainsi que divers matériels et fourni­ tures. 'V o irN ° 4 4 , 1983, p, 342.

Except for persons actually entenng or leaving the area concerned, the outbreak was not considered to be of direct significance to inter­ national travel. The WHO Collaborating Centre at the Pasteur Institute in Dakar, the Centre Muraz run by the OCCGE1 m Bobo-Dioulasso, ORSTOM2 *and OCP! played a major part in the epidemiological 3 surveys and in the arrangements for controlling the disease. A number of agencies from various countries also provided assistance. Abstraction faite des personnes qui pénétraient effectivement dans le pénmètre atteint ou qui le quittaient, la flambée n'a pas été jugée comme ayant une incidence directe sur les voyages internationaux. Le Centre collaborateur de l’OMS à l’Institut Pasteur de Dakar, le Centre Muraz géré par l’OCCGE1à Bobo-Dioulasso, l’ORSTOM2et l’OCP5ont joué un grand rôle dans les enquêtes épidémiologiques et les dispositions prises pour maîtriser la maladie. Un certain nombre d ’institutions de divers pays ont également fourni une aide.
