Introduction In recognition of the fact that field research studies are urgently needed for implementation of the anti- malaria action programme in Africa south of the Sahara, the UNDP/World Bank/WHO Special Pro- gramme for Research and Training in Tropical Diseases, through its Scientific Working Group on Applied Field Research in Malaria, convened an Ad hoc Consultation to review the situation and select research subjects considered to be of primary impor- tance. The Consultation took place in the Kenyatta Conference Centre, Nairobi, Kenya, from 24 to 29 October 1983.° Its main objectives were: (1) the identification of priority areas for applied field research in relation to malaria control in tropical Africa, implemented through primary health care, with emphasis on the maximum involvement of the community; (2) the selection of research subjects within each identified priority area relevant to future malaria control activities, with outlines of study designs; and (3) preliminary identification and delineation of situations where research on the selected subjects could be carried out with, if possible, the selection of institutions and investigators who would be willing to conduct and could undertake the studies. To assist the participants in their deliberations and in the formulation of research proposals, 13 working papers were presented and these are reproduced in this Supplement to the Bulletin of the World Health Organization. In preparation for the meeting, a number of experts on malaria had been invited to submit lists of research questions, the answers to which they considered to be important for the future control of malaria in tropi- cal Africa. Preconditions for the selection of the questions were that they should be technically and ethically amenable to field research and that the necessary resources for studying them could probably be mobilized. On the basis of the lists submitted, four major areas of research were identified: (1) development of an operational strategy for the control of malaria through primary health care based mainly on the provision of adequate treatment; (2) problems of drug action and of Plasmodium fakciparum drug resistance; (3) epidemiology of malaria, studies on human ecology, and sociological aspects; and (4) vector biology and control. Within each of these research areas, the Con- sultation formulated proposals for specific studies, details of which are given in the report of the meet- ing.b A summary of these proposals, with brief state- ments of the objectives of each study, will be found in Annex 1. The Consultation also approved a number of conclusions and recommendations relating to the implementation of the proposed studies. A summary of these is contained in Annex 2. a A list of participants will be found in Annex 3. b The report is issued as unpublished document TDR/FIELD- MAL/NAIROBI/83.3, copies of which are available, on request, from: Epidemiological Methodology and Evaluation, Malaria Action Programme, World Health Organization, 1211 Geneva 27, Switzerland. - 9 - Introduction Conscient de l'urgente necessite de faire des re- cherches de terrain pour mettre en ceuvre le Pro- gramme d'action antipaludique en Afrique au sud du Sahara, le Programme special PNUD/Banque mon- diale /OMS de recherche et de formation concernant les maladies tropicales, agissant par l'intermediaire de son Groupe de travail scientifique Recherche ap- pliquee de terrain sur le paludisme, a convoque une consultation speciale chargee de faire le point de la situation et de choisir des sujets de recherche re- vetant, selon elle, une importance primordiale. La consultation, qui a eu lieu au Centre de Conference Kenyatta a Nairobi (Kenya) du 24 au 29 octobre 1983,a avait trois objectifs principaux: 1) degager les secteurs prioritaires dans lesquels devraient etre faites des recherches appliquees de terrain dans l'optique de la lutte antipaludique en Afrique tropicale A travers les soins de sante pri- maires, l'accent etant mis sur une participation maximale de la collectivite; 2) choisir des sujets de recherche dans chaque sec- teur prioritaire ainsi degage, interessant les activites futures de lutte antipaludique, et esquisser des plans d'etude; 3) reperer et circonscrire dans un premier temps les situations ofu des recherches pourraient etre faites sur les sujets retenus et, si possible, choisir les insti- tutions et chercheurs qui seraient desireux et a meme de faire les etudes. En vue d'aider les participants dans leurs delibe- rations et dans l'qetablissement des propositions de recherche, 13 documents de travail ont ete presente's; ils sont reproduits dans ce supplement au Bulletin de l'Organisation mondiale de la Sante. En prevision de la reunion, plusieurs experts du paludisme avaient ete invites a soumettre des listes de questions de recherche dont les reponses leur sem- blaient importantes pour I'avenir de la lutte antipalu- dique en Afrique tropicale. La condition prealable a la selection des questions etait qu'elles devaient se preter techniquement et ethiquement a des recherches de terrain et qu'il etait possible de mobiliser les res- sources necessaires aux etudes. Sur la base des listes soumises, quatre grands secteurs de recherche ont ete degages: 1) mise au point d'une strategie operationnelle pour la lutte antipaludique a travers les soins de sante primaires, fondee essentiellement sur un traitement adequat; 2) problemes lies A l'action des medicaments et a la pharmacoresistance de Plasmodium falciparum; 3) epidemiologie du paludisme, etudes sur l'eco- logie humaine, et facteurs sociologiques; 4) biologie des vecteurs et lutte antivectorielle. Pour chacun de ces secteurs de recherche, les parti- cipants a la consultation ont formule des propositions d'etude dont les details figurent dans le rapport de la reunion.b L'annexe 1 donne un resume de ces propo- sitions ainsi qu'un bref expose des objectifs de chaque etude. Les participants A la consultation ont egale- ment approuve un certain nombre de conclusions et recommandations pour la mise en oeuvre des etudes proposees.
