TRAINING After the completion of the training of 200 nurses in January 1997 in 5 Districts, the training of Community lvermectin Distributors was supposed to have taken place in May 1997 as planned. Due to the Military take-over and the subsequent insecurity with the displacement of the population, no training of Community Volunteers took place in this period. However, the Non Governmental Orgarization - Lunsar Eye Hospital - our collaborators managed to secure funds and trained 94 Community Volunteers along the Rokel I River Basin from December 16-21,1991 and a refresher training for 50 Volunteers from July 17-18,1998. 2. INFORMATTON, EDUCATION AND COMMUNICATION (IEC) Public Awareness Raising and Sensitization has always been carried out at National, District and Community Levels through the mass media, radio and television news. 3 SUPERVISION The supervision of Community-Directed Treatment at Community Level will be done by the Village Health Committee and Nurses of the Peripheral Health Units. The nurses in turn will be supervised by the District Health Team At District Level, the supervision will be carried out by a supervisory team at the national Coordination level. This supervisory team will compose of : - the National Coordinator - the Medical Officer - Three from the Epidemiology team The three EPI Technicians will be posted in each of the trhee Provinces - North, South and East. The Medical Coordinator will play a more active role in supervision by making on-the- spot checks on the alledged treated villages as reported by the District Health team or the EPI Supervisors. Now that the Ministry of Health has a Manager for Disease Prevention and Control, he will act as a "watch person" for the Director-General of Medical Services as the need arises or as reported by the National Coordinator. 4 MANAGEMENT OF DATA AND INFORMATION FLOW Data collected at the Peripheral Level is being transmitted to the Central Level via the District for analysis. I JPC19.B(j ) Page 4 Efforts are being made in the country to train district health staff for analysing data. However, districts are yet to be equipped with data processing equipments and accessories. 5. CONSTRAINTS, SOLUTIONS AND SUGGESTIONS Constraints (i) (ii) (iii) (i") (v) Security situation in the country Inadequate logistics such as transport and stationeries Inadequate training of Volunteers The inadequate availability of Ivermectin and Monitoring Drugs for adverse reactions the insufficient mobilization and participation of the community for Community-Based Ivermectin Distribution activities Solutions and Suggestions I (i) (ii) Improved security situation Vehicles should be at the disposal of the District Health Team members to effect good supervision Zonal Supervisors (three Technicians) should be provided with motorbikes for proper monitoring of activities in the affected villages. Stationeries will be solicited from Non GovernmentalOrgaruzations, Ministry of Health and Sanitation and Oncho Control Programme Ivermectin tablets will be requested for, on time and adequate storage facilities provided For active participation of the communities, the District and Community Administration will be adequately sensitised to mobilise the community for their effective participation in the control activities. (iii) (i") (") (vi) 6. PLAN OF ACTION T999-2002 (See attached) CONCLUSION Community-Directed treatment with ivermectin is supposed to be the mainstay of Oncho treatment in Sierra Leone. Due to the unfortunate events in 1997, this mode of distribution could not be carried out. The training of nurses was completed in January 1997. Budgetory approval for the training of Community Volunteers was received. This activity could not be carried out due to the dictates of the security situation in the country. Meanwhile, passive Ivermectin distribution was carried out in 51 health Institutions. I A total of 21,891 patients were treated with 29,728 tablets JPC19.
