LEPTO SPIR O SIS — In 1989, there was a considerable fall in the num­ ber of cases of leptospirosis recorded in metropolitan France. The total of 179 cases represents a reduction of 55% over the figure in 1988, whereas the total number of human serum specimens delivered to the laboratory (4 352) was only 8% Less.1 On the other hand, there was a very significant increase in the number of cases diagnosed in overseas departments and territories(381 cases versus 270in 1988), owing mainly to the assump­ tion of responsibility for diagnosis by the centres in Tahiti and New Caledonia, which has entailed an increase in requests for tests. In metropolitan France, the decrease in the number of cases is evenly spread over the year, with a curve showing the usual summer/autumn peak (maximum number of cases in Septem­ ber). Among the serogroups incriminated, Icterokaemarrhagiae was responsible for nearly 50% of cases and, despite a fall in the absolute number of cases, regained a preponderant position owing mainly to the very sharp reduction in the number of cases of.,leptospirosis due to Grippotyphosa (27 cases, or 15% of the total), The relative proportions of the various other serogroups remained much the same as in previous years. LEPTOSPIROSE — L’année 1989 a été marquée par une diminution consi­ dérable du nombre de cas de leptospirose enregistrés en France métro­ politaine: 179 cas correspondent à une chute de 55% par rapport à 1988, alors que le nombre global de sérums humains parvenus au laboratoire (4 352) n’ a diminué que de 8%.* A l’opposé, on observe une augmentation très significative du nombre de cas diagnostiqués dans les départements et territoires d’outre-mer (381 cas contre 270 en 1988) — augmentation due principalement à la prise en charge du diagnostic par les centres de Tahiti et de NouvelleCalédonie, laquelle a entraîné une augmentation des demandes d’exa­ men. . En métropole, la diminution du nombre de cas est régulièrement répartie sur toute l’année, avec une courbe dessinant le pic estivoautomnal habituel (maximum de cas en septembre). Les sérogroupes responsables sont Icterohaemorrhagiae>à l’origine de près de 50% des cas et qui, malgré une chute du nombre absolu de cas, retrouve une position prépondérante du fait essentiellement de la très nette diminution du nombre de cas de leptospirose à Grippotyphosa (27 cas, son 15% du total). La proportion relative des différents autres sérogroupes reste sensible­ ment identique à cqlle des années précédentes. F rance.

The distribution of cases in metropolitan France has not changed in relation to previous years : the regions most affected remain the southern, western central and eastern parts of the country. The northern, central and south-eastern regions have very low endemic levels In overseas France, the use of local diagnostic facilities resulted in the identification of 76 cases in Tahiti and 144 In New Cal­ edonia (compared with 19 and 88 respectively in 1988). There, too, the serogroup Icterohaemarrhagiae is by far the most fre­ quent ; Gnppoiyphosa is practically absent. The patients’ occupations are known in 160 cases. Persons not at occupational risk accounted for 62% of cases of leptospirosis, schoolchildren, students and retired persons still being the most affected. While benefiting from the general fall in the rate of infection, agricultural workers still constitute the occupation most at risk The incidence of the disease among sewage workers and refuse collectors is very low (5 cases, all due to Icterohaemorrhagiae), reflecting the high level of vaccination for this occupa­ tional category. The 2 salient facts for this year are the general decrease of leptospirosis in metropolitan France and the falling-off of the upward trend of the serogroup Grippotyphosa which had been observed over the past 3 years. Climatic conditions, notably the year-long drought and its impact on wild rodents, may partly explain the trend in the endemic situation observed in 1989 for metropolitan France.

La répartition des cas en métropole n’est pas modifiée par rapport aux années précédentes; les régions les plus touchées demeurent le Sud et le Centre-Ouest, ainsi que l’Est du pays. Le Nord, le Centre et le Sud-Est ont des niveaux d’endémie très bas. En France d’outre-mer, la possibilité d’effectuer les analyses locale­ ment a permis d'identifier 76 cas à Tahiti et 144 en Nouvelle-Calédonie (çontre 19 et 88 respectivement en 1988). Là encore,lcterohaemorrhagiae est largement prédominant; le sérogroupe Grippotyphosa est quasiment absent. L ’activité professionnelle était connue pour 160 patients; 62% des cas de leptospirose sont survenus chez des personnes non exposées par leur profession, écoliers, étudiants et retraités étant toujours les plus touchés. Bien que bénéficiant de la baisse générale du taux d’infection, les agri­ culteurs représentent toujours la profession la plus exposée. L’incidence de la maladie chez les égoutiers-éboueurs est très faible (5 cas, tous dus à lcterohaemorrhagiae), soulignant l’importance de la vaccination dans cette catégorie de personnels. Les 2 faits marquants cette annéesont la régression globale de la lepto­ spirose en métropole et l’infléchissement de la courbe de progression du sérogroupe Grippotyphosa qui se poursuivait depuis 3 ans. Les condi­ tions climatiques, avec la sécheresse qui a sévi tout au long de l’année et son impact sur les rongeurs sauvages, peuvent expliquer en partie l’évo­ lution de la situation endémique observée en 1989 en France métropo­ litaine.
