Impact of large-scale rectal artesunate deployment in the emergency management of malaria Invitation for statements of interest to participate in a collaborative multicountry study WHO is inviting letters of interest from scientists interested in participating in a collaborative study to assess the impact of large-scale rectal artesunate deployment in the emergency management of non per os (treatment “not through the mouth”) malaria.

Impact du déploiement de l’artésunate rectal à grande échelle dans la prise en charge d’urgence du paludisme Invitation à la présentation de lettres déclarant un intérêt à participer à une étude collective multipays L’OMS invite les scientifiques qui le souhaitent à présenter une lettre déclarant leur intérêt à participer à une étude collective pour évaluer l’impact du déploiement à grande échelle de l’artésunate par voie rectale dans la prise en charge d’urgence du paludisme (traitement par des voies autres que buccales).

Background WHO has directed the development of a new drug likely to offer considerable survival benefit for malaria patients who currently die as a result of lack of immediate access to appropriate treatment. The antimalarial in question is a rectal formulation of artesunate, which achieves rapid and substantial (99%-100%) reduction of parasitaemia within 24 hours of administration. It permits emergency treatment of patients who cannot take drugs orally and who do not have immediate access to injectable treatment (normally only available in a hospital or district clinic). WHO is now preparing for large-scale implementation programmes to take the drug into use and to gain further understanding of how best it should be used to achieve its potential impact. The focus of the proposed programme will be at community and district levels, and the informed and willing participation of communities and health professionals will be essential for its success. Problématique L’OMS a dirigé la mise au point d’un nouveau médicament devant offrir des avantages considérables du point de vue de la survie de malades du paludisme qui meurent actuellement faute d’un accès à un traitement immédiat approprié. L’antipaludéen en question est une forme galénique de l’artésunate à usage rectal qui permet une réduction rapide et considérable (99%-100%) de la parasitémie dans les 24 heures suivant l’administration. Ce médicament permet le traitement d’urgence de malades chez qui la prise par voie orale n’est pas possible et qui n’ont pas immédiatement accès à un traitement injectable (normalement disponible seulement à l’hôpital ou dans un dispensaire de district). L’OMS prépare des programmes d’application à grande échelle pour utiliser le médicament et déterminer le meilleur moyen d’en tirer parti. Le programme envisagé privilégiera le niveau communautaire et de district, le consentement éclairé et la volonté de participer des communautés et des professionnels de santé étant une condition indispensable du succès. 93

Country selection/participation WHO is inviting indication from countries interested in participating in a collaborative programme to take this drug into use and monitoring its effect. Collaboration in the development of relevant health education materials and an information system that will support policy and quantify impact will be required. Around 5 countries will be selected on the basis of: – endemic Plasmodium falciparum malaria; – interest and potential to develop education materials and data collection at district level; – expected commitment to the programme at all levels of the health service; – willingness to collaborate with other participating countries, and to share results.

Sélection/participation des pays L’OMS invite les pays souhaitant participer à un programme de collaboration à l’utilisation de ce médicament et à l’évaluation de ses effets à se manifester. Il s’agira de collaborer à la mise au point de matériels pertinents d’éducation pour la santé et à un système d’information qui appuiera la politique suivie et quantifiera l’impact. Cinq pays environ seront choisis sur les bases suivantes: – endémie palustre à Plasmodium falciparum; – intérêt et possibilité de la mise au point de matériels d’éducation et de la collecte de données au niveau du district; – engagement escompté en faveur du programme à tous les niveaux du service de santé; – désir de collaborer avec d’autres pays participants et d’échanger des résultats.

How to apply Applications, which should be brief, need to include reference to the following: – professional details of the principal investigator(s) and their institution(s); – full contact details (mail, telephone and fax), and email addresses; – confirmation of expected participation of the ministry of health; – identification and a brief description of the malaria-endemic districts/regions to be studied; – confirmation of willingness to collaborate in introduction of appropriate information systems, data collection and health education programmes. Applicants from countries requiring national endorsement should submit their applications through proper government channels. A copy of all applications should be sent to the WHO Representative’s office in the home country for information purposes. Ⅲ
