E. Smirnov & T. A. Makinskij Institute of Neurology Moscow, USSR The Stroke Register Programme has been in progress since 1972. Record forms (initial, follow-up and death records) are being sent to WHO for persons who had a stroke after 1 January 1973. The present report includes the basic data on 493 initial record forms for persons who had a stroke in 1974, comprising 211 men (42.8%) and 282 women (57.2%). Most cases of stroke occurred in the age group over 65 years (293 persons). There are 1.5 times more women (178) than men (115) in this group (in the Tusinskij rayon, women greatly outnumber men in the over 65 age group). In the other age groups the distribution of cases was as follows: 83 persons under 54 (34 men, 49 women) and 117 persons aged from 55 to 64 (62 men, 55 women). During the acute phase of stroke (first 21 days) there was a fatal outcome in 163 cases (33.06%). Fifty-nine men died (27.96% of all male cases) and 104 women (36.88% of all female cases). The age distribution among fata'l cases was as follows: 23 persons under 54 years (8 men, 15 women); 21 persons aged 55 to 64 years (11 men, 10 women) and 119 persons over 65 years (40 men, 79 women). Twenty-six persons (15.95%) died during the first 24 hours, 48 persons (29.45%) between the second and third days and 89 persons (54.6%) between the fourth and 2lst days. Thirteen patients died after the 2lst day and before the end of the first three months. During the days immediately following the stroke, 236 patients (47.9%) were admitted to hospital and 257 (52.1%) remained at home. Three hundred and thirty patients remained under observation after the 2lst day, and 317 after three months. As before, the principal sources of information on cases of stroke were the polyclinics of the rayon, the Moscow City Emergency Medical Care Centre and the hospitals to which the patients were admitted. The Registry staff keep a constant watch on the completeness of information on cases of stroke by checking death certificates and the records of the Moscow City Emergency Medical Care Centre and the hospitals. A number of measures are now being carried out to improve the medical care given to stroke patients and include the introduction of "stroke teams" at emergency medical care sub- centres serving the Tusinskij rayon, further training courses on cardiovascular diseases The issue of this document does not constitute formal publication.
