RECOMMENDED COMPOSITION OF INFLUENZA VIRUS VACCINES FOR USE IN TH E 1988-1989 SEASON Influenza activity, October 1987-February 19881 During the 1987-1988 season, influenza A(H3N2) viruses have been associated with outbreaks in China, the United States of America, and the USSR and have been isolated from sporadic cases in a few other countries. Influenza B viruses have been isolated from outbreaks in Japan and the USSR, as well as from sporadic cases in several other countries. Influenza A(H1N1) has been rare. Influenza A(H 3N2) Sporadic cases of influenza A(H3N2) were reported in Japan, Singapore and the United States during October and November 1987. In the United States, the virus spread during December 1987 and January 1988 and hasnow been detected in 38states. An increasing number of outbreaks were reported, several of which were in homes for the elderly. Virus isolations in Japan were mainly from children, and outbreaks were recorded only rarely. Influenza A(H3N2) also spread during December 1987 and January 1988 in the northern part of China and in the USSR. Children were reported to be most affected and in China the illness was described as severe. The overall incidence of influenza-like illness has been very low for the season in Europe, and Hungary and Norway are the only countries reporting influenza A(H3N2) this season: 2 spo­ radic cases were confirmed in Norway in December 1987 and January 1988 and 1 in Hungary in February 1988. Influenza B Influenza B was reported in Trinidad and Tobago in October and November, and outbreaks occurred in the USSR at the end of December 1987 and early in January 1988. During January, influenza B viruses were also isolated in Japan. Sporadic cases of influenza B have also been reported in Canada, in 12 states in the United States and in several European countries (Finland, France, Federal Republic of Germany, Greece, Sweden, United Kingdom). Influenza A(H1N1) Influenza A(H1N1) viruses have been very infrequently detected this season. A few sporadic cases were reported in Ma­ laysia and Singapore in October 1987 and in the United States in early February 1988. 1 See No 5, 1988, pp. 25-27 and 30-31 Epidemiological notes contained m this issue:

La grippe A(H3N2) s’est également propagée en décemhre 1987 et janvier 1988 dans la partie nord de la Chine et en URSS. Les enfants ont semble-t-il été les plus touchés et en Chine, la maladie a été décrite comme grave. L’incidence globale des syndromes d’allure grippale et restée très faible pour la saison en Europe, et la Hongrie et la Norvège sont les seuls pays ayant enregistré des cas de grippe A(H3N2) cene saison: 2 cas sporadiques ont été confirmés en Norvège en décembre 1987 et janvier 1988, et 1 cas en Hongrie en février 1988. Grippe B La grippe B a été signalée à la Trinité-et-Tobago en octobre et novembre et des flambées se sont produites en URSS à la fin de décembre 1987 et au début de janvier 1988. En janvier, des virus grip­ paux B ont également été isolés au Japon. Des cas sporadiques de grippe B ont également été isolés au Canada, dans 12 Etats des Etats-Unis d’Amérique et dans plusieurs pays d’Europe (République fédérale d’Allemagne, Finlande, France, Grèce, Suède et Royaume-Uni). Grippe A(H 1N1) Le virus grippal A(H1N1) n ’a été que rarement observé cette saison. Antigenic analysis of recent isolates Influenza A(H 1N1) virus , Influenza A(H1N1) viruses isolated in 1987 and 1988 were amigenically similar to A/Singapore/6/86. Influenza A(H 3N2) virus Influenza A(H3N2) viruses isolated in Asia, Oceania and most recently in the United States were found to be antigenically dif­ ferent from previously characterized strains, such as A/Mississippi/1/85 and A/Leningrad/360/86, Isolates ware observed to be antigenically heterogeneous in reciprocal haemagglutination inhibition (HI) tests with post-infection ferret sera, as shown by the examples given in Table I.

Influenza B virus Influenza B virus isolates from Asia, Oceania, the Americas and Europe (including those from recently reported outbreaks in the USSR) have been analysed in HI tests with post-infection ferret sera (Table 2). Despite the relatively low homologous anti­ body titres, the results show that the recent strains differred from B/USSR/100/83 and B/Ann Arbor/1/86. Reference strains B/Beijing/1/87 and B/Victoria/2/87 are similar to each other and representative of most recent isolates of influenza B.

Virus grippal B Des isolements de virus grippal B obtenus en Asie, en Océanie, aux Amériques et en Europe (notamment à l’occasion des flambées récem­ ment notifiées en URSS) ont été soumis à des épreuves d’inhibition de l'hémagglutination à l’aide de sérums de furet après infection (T ib lea u 2 ). Malgré de faibles titres d’anticorps homologues, les résultats montrent que les souches récemment isolées diffèrent de B/USSR/ 100/83 et B/Ann Arbor/1/86. Les souches de référence B/Beijing/1/87 et B/Victoria/2/87 se ressemblent et sont représentatives de la plupart des isolements récents de virus grippaux B.

Serological surveys T h e prevalence of antibodies to the haemagglutmins of influenza A and B viruses in sera collected in 1987 from persons of different ages, was reported from Norway, the United Kingdom and the United States. T he levels of antibodies to A/Sichuan/2/87 varied with age and country. T he prevalence of antibodies at titre > 1 : 4 0 varied between 0 and 50%. In the same sera the prevalence of antibodies to A/Leningrad/360/86 with titres > 1 : 4 0 ranged from 20 to 70%.

Enquêtes sérologiques L a prévalence des anticorps dirigés contre les hémagglutinines des virus A et B dans des sérums prélevés en 1987 sur des personnes d’âges divers a été étudiée en Norvège, aux Etats-Unis d’Amérique et au Royaume-Uni. For B/Ann Arbor/1/86 neutralizing antibodies at titres > 1:100 were detected in 13-42% of sera from the United States, whilst for B/Victoria/2/87, a virus similar to Beijing/1/87, the frequency of, antibody was lower, ranging from 0 to 18% with titres > 1:100. For A/Singapore/6/86(HlNl), antibodies were detected at titres > 1:40 in 3-30% of persons of all ages.

Studies with inactivated influenza virus vaccines Haemagglutination inhibiting antibodies were measured in the sera of recipients of single doses of trivalent, inactivated influenza vaccines containing 10-15 jig of the haemagglutinins o f A/Leningrad/360/86(H3N2), A/Singapore/6/86(HlNl) and B/Ann Arbor/1/86. Results from several studies in the United Kingdom and the United States indicated that antibody to A/Leningrad/360/86 was present at titres > 1:40 in about40-50% of healthy adults prior to vaccination and in about 60-90% after vaccination, depending on age and prior vaccination history. In the same post-vaccine sera, the presence o f antibody at times o f > 1 :4 0 to the representative recent viruses A/Sichuan/2/87 and A/Shanghai/11/87 ranged from 14 to 80%, with times about onehalf of those against A/Leningrad/360/86. Antibody to A/Singapore/6/86-like virus was present at times > 1 :40 in about 10-30% of healthy adults prior to vaccination and in more than 80% after vaccination. Antibody to B/Ann Arbor/1/86 was present at tim e s> l:4 0 in about 10-30% prior to vaccination and in about 75-85% after vaccination. In the same post-vaccine sera, the prevalence of antibody at times > 1:40 to B/Victoria/2/87 ranged from 30 to 50%. Neutralization antibody tests confirmed lower times to B/Victoria/2/87 than to B/Ann Arbor/1/86,

Recommendations for the composition of influenza virus vaccines Influenza A(H3N2), influenza A(H1N1) and influenza B viruses have all continued to circulate in the world during 1987 and early 1988. The recent influenza A(H3N2) viruses and influenza B viruses have both shown antigenic variation from previous reference viruses. Representative of these recent viruses are A/Sichuan/2/87(H3N2) and B/Beijing/1/87. T he influenza A(H1N1) viruses isolated during 1987 on the other hand con­ tinued to be antigenically similar to A/Singapore/6/86. Consequently, it is recommended that vaccines for use in the 1988-1989 season be trivalent and contain the following anti­ gens: • an A/Singapore/6/86(HlNl)-like antigen • an A/Sichuan/2/87(H3N2)-like antigen • a B/Beijing/l/87-like antigen.1 As in previous years the specific viruses used in each country should be approved by the national control authorities. Most of the population is likely to have been infected with influenza A(H3N2), influenza A(H1N1) and influenza B viruses in recent years. As a consequence, 1 dose of inactivated vaccine should be immunogenic for individuals of all ages except young children. Previously unvaccinated children should receive 2 doses of the vaccine, with an interval between doses of at least 4 weeks. Reagents for use in laboratory standardization of inactivated vaccine may be obtained from the Division of Viral Products, National Institute for Biological Standards and Control, Blanche Lane, South Mimms, Potters Bar, Herts EN6 3QG, United Kingdom, or from the Division of Virology, Center for Biologies Evaluation and Research, Food and Drug Administration, Building 29A, 8800 Rockville Pike, Bethesda, Maryland 20892, United States of America.

Recommandations pour la composition des vaccins antigrippaux Les virus grippaux A(H3N2), A(H1N1) et B ont continué à circuler dans le monde en 1987 et au début de 1988. Les virus A(H3N2) et B récemment isolés se sont révélés différents, du point de vue antigénique, des précédents virus de référence. Les souches A/Sichuan/2/87(H3N2) et B/Beijing/1/87 sont représentatives de ces isolements récents. En revanche, les virus grippaux A(H1N1) isolés en 1987 étaient toujours antigéniquement analogues à A/Singapore/6/86. En conséquence, U est recommandé d’utiliser pour 1a saison 19881989 des vaccins trivalents contenant les antigènes suivants: • un antigène analogue à A/Singapore/6/86(HlNl) • un antigène analogue à A/Sichuan/2/87(H3N2) • un antigène analogue à B/Beijing/1/87. Comme les années précédentes, les virus utilisés dans chaque pays devront être approuvés par les autorités nationales de contrôle. La plus grande partie de la population a probablement été infectée au cours de ces dernières années par les virus A(H3N2), A(H1N1) et B. Il s’ensuit qu’une dose de vaccin inactivé devrait être immunogène pour les sujets de tous âges, à l’exception dés jeunes enfants. Les enfants non encore vaccinés auront besoin de 2 doses de vaccin, l’intervalle entre les doses étant d’au moins 4 semaines. Reference strains for antigenic analysis may be obtained from the WHO Collaborating Centres for Reference and Research on Influenza, Atlanta and London. , | A review of the prevalence of influenza viruses in the world is' scheduled for publication in the WER on the last Friday of October 1988 for consideration by those making recommenda­ tions for the composition of inactivated influenza virus vaccines for use in the southern hemisphere.
