Box 316 Monrovra, Libena Fax: (231) 22 62 08 t- ,Ltv+ OF 2005 END LETTER N'08/181/634 TREA TIN PROJECT THE SOUTH REGION OF LIBERI IIBSWlA) For the purpose of developing and establishing sustainable Community Directed Treatment with Ivermectin (CDTI) for the control of onchocerciasis in Liberia, The World Health Organization's African Programme for Onchocerciasis Control, hereinafter referred to as "WHO/APOC" on the one hand, and The National Onchocerciasis Task Force, Ministry of Health and Social Welfare, Liberia, hereinafter referred to as "the Institution", on the other hand, HAVE AGREED AS FOLLOWS 1. Obli of WHO/APOC 2.1 inancial O The total financial obligation of WHO/APOC, under this Agreement is US$ 206,g25 (two hundred and six thousand, nine hundred and twenty five dollars US). An initial amount of US$ 127 ,584 was disbursedin 2002 and 2003 and an amount of US$17,200 in 2004 for the purchase of equipment. The maximum obligation of APOC for any budget line item in any project is the amount indicated in the approved budget Iine, and the total obligation may not exceed the sum of these budget lines for any single project or groupof Projects. 2The amended and approved first year budget (summary and details) is attached herewith as Annex 1 2.2. Capital Equipment Unchanged (See Letter of Agreement N"08/181 16341(1)l0Z). 2.3. Audit of accounts Unchanged (See Letter of Agreement N"08/18116341(r)102) 3. Contribution of the Institution Unchanged (See Letter of Agreement N"08/18116341(i)102) 4. Period covered by this Agreement This Agreement covers a period of 12 months from 1" November 2005 to 3L't October 2006. 5. Financial arrangements 5. I . The total amount of expenditure estimated by this Agreement is US$ 206,925 (two hundred and six thousand, nine hundred and twenty five dollars US). Of the above, US$ 17,200 (sevenreen thousand, two hundred dollars US) has already been retained and used by WHO/APOC for the purchase of motorbikes and other equipment needed for the performance of the Project activities. Moreover, an amounr of US$ lT'l ,584 was also disbursed for equipment purchase in2002 and 2003. 5.2. The remaining amount of US$ 62,141will be used for field activrtres. From the above amount, a first tnstallment of US$ 40,000 (forty thousand US dollars) will be paid to the Institutron as soon as possrble after the signature of the Agreement by both parties, provided that all the other conditions precedent to dtsbursement have been met. A new sticker number will be given for the field activities. 5'3. The subsequent installment(s) will be paid on request from the Institution accompanied by a statement reconciling expenditure already made to the various budget lines. A request for further funding for a Project should be made when 80% of the previous advances have been spent. The payment of these replenishments will be subject to: (a) The reception of the statement mentioned above; (b) The regular transmission at the end of each month of a copy of the bank statement, reconciled with a list of all cheques drawn on the account. One or both of these reports wrll be formally reviewed as a basis for extension of this Agreement for a further period or the signing of a subsequent Agreement for further funding.
