SMALLPOX SURVEILLANCE As of 19 December, 59 485 cases of smallpox have been reported to the Organization (Table 1). A total of about 64000 cases is expected to be reported for the year, an increase of 23% over the total reported during 1971. Primarily, this is attributed to more intensive surveillance activities in the remaining endemic areas and, in consequence, more complete notification of cases. To date 19 countries have reported one or more cases of small­ pox, 11 of which experienced importations. The most recent country to experience imported smallpox was Somalia which, in November, reported five cases in a border area (Hargeisa) among nomads from known infected areas of Ethiopia. These are the first cases to be detected in Somalia since 1966. Intensive con­ tainment measures both in Ethiopia and Somalia have been in­ stituted and in Somalia no further spread of the disease has occurred. Sudan and Ethiopia In Sudan and Ethiopia more than three months have now elapsed since the beginning of the expected seasonal increase in smallpox cases but, in both countries, the incidence has remained com­ paratively low (Fig. 2). Since 1 September, Sudan has detected only 39 cases, all located in two southern provinces, compared to 438 cases during the same period last year; Ethiopia has reported

Cette augmentation parait due essentiellement à l’inten­ sification de la surveillance dans les zones d’endémicité qui sub­ sistent et, par suite, à une notification plus complète des cas. Jusqu’ici, 19 pays ont signalé un ou plusieurs cas de variole; dans 11 d’entre eux, la maladie avait été importée. C’est la Somalie qui a connu le plus récemment des importations de variole: au mois de novembre, cinq cas ont été signalés dans une région frontière (Hargeisa) parmi des nomades venant de zones d’Ethiopie infectées. Ce sont les premiers cas enregistrés en Somalie depuis 1966. Des mesures d’endiguement intensives ont été prises dans les deux pays et la maladie ne s’est pas propagée davantage en Somalie. Soudan et Ethiopie Plus de trois mois se sont maintenant écoulés depuis le moment où devait s’amorcer au Soudan et en Ethiopie l’augmentation saison­ nière habituelle, mais l’incidence de la maladie est restée relative­ ment faible dans les deux pays (Fig. 2 ). SMALLPOX SURVEILLANCE (continued) 1670 cases in this period compared to over 9 000 cases last year. The known infected areas are receding (Fig. 2 ). Surveillance pro­ grammes are present throughout the endemic areas in both coun­ tries and are steadily being intensified. In Botswana, the only other known endemic country in Africa, only two cases have been detected during the past eight weeks. The epidemic, which began when smallpox was reintroduced into the country early in 1971, reached its peak in July 1972, when 416 cases were recorded (Fig. 1). With an active surveillance pro­ gramme and intensive vaccination activities, the outbreak receded rapidly thereafter. Four national surveillance teams are presently engaged in an active search to detect any residual foci With the advent of the smallpox season, all countries throughout Africa are urged to be especially alert for possible importations and to maintain the highest possible level of vaccination immunity. Although importations are less likely than they were only a year ago, a definite risk is still present.

Un programme de surveillance efficace et des opérations de vaccination intensives l’on fait rapidement régres­ ser. Quatre équipes nationales de surveillance s’emploient activement à dépister les foyers résiduels qui pourraient subsister. L’époque de la recrudescence saisonnière arrivant, tous les pays d’Afrique sont vivement engagés à rester en alerte pour faire face à d’éventuelles importations et à maintenir par la vaccination le plus haut niveau d'immunité possible dans la population. Le risque d’im­ portation est moindre qu’il n’était il y a un an encore, mais il persiste.
