CHOLERA M alaysia: Sarawak. — 1 A total of 26 cases due to V. cholerae biotype eltor Ogawa, and 5 suspected cases have been reported since the beginning of March in the Districts of Limbang and Lawas (Fifth Division). There were three deaths. In the District of Limbang, no cases have been observed since 28 March. In Lawas District, all cases (seven confirmed and five suspected cases) have occurred in a village which had been submitted to mass vaccination 4-5 weeks ago. Arrangements are being made to improve the quality of water and a wide scale vaccination campaign has been undertaken. Practically all inhabitants in the Fifth Division have already been vaccinated. 1 See No. 11, p. 118.

Il y a eu trois décès. Dans le District de Limbang, aucun nouveau cas n ’a été signalé depuis le 28 mars. Dans celui de Lawas, tous les cas, soit sept cas confirmés et cinq suspects, se sont produits dans un village qui avait été atteint par la campagne de vaccination il y a 4 à 5 semaines. Des dispositions sont prises afin d ’améliorer la qualité de l’eau et la campagne de vaccination de masse se poursuit. Czechoslovakia (situation on 21 March 1970). — 1 The incidence of notified influenza cases is continuing to decline in all regions of the Czech Socialist Republic. During the past week, two strains of virus A2/Hong Kong/68 (in Kosice) and three strains of virus B (in Prague and Olomouc) have been isolated from persons who were ill between 20 February and 8 March. South A frica (information dated 14 March 1970). — 2 In Witwatersrand mining population, an influenza outbreak started on 11 February. Five to ten new cases are occurring each week.

LA GRIPPE DANS LE MONDE: PÉRIODE OCTOBRE 1968 — SEPTEMBRE 1969 (suite) The A2 viruses associated with influenza during the period October 1968 - September 1969 were identical or similar to the variant A2/Hong Kong/68, and the B viruses to the variant pre valent in recent years (B/Switzerland/265/67, B/Rome/1/67, B/Massachusetts/3/66). As shown in the Table, most outbreaks were due to virus A2j Hong Kong/68 though a few were due to virus B. From July to September 1968, the first epidemics associated with virus A2/Hong Kong/681 behaved similarly to those which occurred at the begin ning of the 1957 pandemic. However, certain differences became soon apparent, for example in Japan, where the disease failed to spread in August and September in spite of numerous introduc tions of the virus by sea travellers. Later in the year, many separate foci developed in Japan, often in schools, but coalescence into a general epidemic (which was only of moderate extent) was delayed until mid-January. Other differences from the 1957 experience were observed. Between the end of September and the end of the year there was little evidence of spread in areas not yet affected in the world, despite the occurrence in these areas of small foci associated with importation of infection from countries in which epidemics were present. In the United States o f America, the experience was exceptional. Here the first apparently indigenous outbreak occurred in October in California. Infection spread rapidly in November in a roughly west-east direction and by Christmas outbreaks had been reported from nearly all the states. The epidemic was extensive and the peak occurred at about the end of the year. It was associated with a great increase in deaths from pneumonia-influenza in each of the country’s geographical divisions. On the basis of excess deaths (i.e., the number of deaths above the average for the same weeks in previous years), the epidemic was as severe as that of 1957-58 when the original A2 virus was prevalent. This experience of widespread infection associated with a high level of excess mortality was unique. It did not occur in other temperate-climate countries in the Americas. Canada, for example, experienced a relatively slight increase in incidence of disease and practically no excess deaths. Aux Etats-Unis d’Amérique, par contre, le déroulement présenta un caractère exceptionnel. Dans ce pays, la première poussée n ’ayant apparemment pas son origine à l’étranger se produisit en Californie au mois d ’octobre. L ’infection se propagea rapidement pendant le mois de novembre, en général d ’ouest en est, et vers Noël des poussées avaient déjà été rapportées dans presque tous les états. L’épidémie fut très importante et son sommet se produisit vers la fin de l’année. Elle fut associée à une augmentation consi dérable des décès par broncho-pneumonie et grippe dans chacune des neuf divisions géographiques du pays. Si l’on se base sur l’excès de mortalité (c’est-à-dire sur le nombre de décès dépassant la moyenne de la mortalité notifiée pour les semaines correspondantes des années précédentes), il apparaît que l’épidémie fut aussi grave que celle qui fut causée en 1957-58 par le virus A2 initial. On n ’observa pas ailleurs une aussi vaste extension de l’infection associée à un excès aussi important de la mortalité. En particulier, rien de semblable ne se produisit dans les autres pays tempérés des Amériques. Au Canada, par exemple, l’augmentation de l’incidence de la maladie fut relativement faible et l’on n ’enregistra pratiquement pas de mortalité excessive. En Europe, l’incidence ne s’éleva généralement qu’au début de 1969, c’est-à-dire plus tard qu’aux Etats-Unis d'Amérique, mais l’affection fut prévalente jusqu’en avril. Presque tous les pays d’Europe rapportèrent des poussées, mais leur extension fut variable. Les plus importantes se produisirent en Pologne, où le nombre de cas d ’affections d ’allure grippale fut estimé à plus de quatre millions, et aux Pays-Bas. Des épidémies généralisées furent également notifiées en Bulgarie, en Tchécoslovaquie, dans certaines parties de la République fédérale d ’Allemagne, en Finlande, en Hongrie, en Islande, en Suède et dans certaines parties de l’URSS. Cependant, dans tous ces pays, l’affection fut bénigne et l’on ne constata pas un excès considérable de mortalité. Dans les autres pays européens, on observa parfois une grande diffusion de l’infection, mais son influence sur l’absentéisme fut plus faible que d ’habitude et elle ne causa pas d’augmentation importante de la mortalité. Ceci fut particulièrement évident au Royaume-Uni, où le Service des Labo ratoires de Santé publique offre des facilités de diagnostic virolo gique pour l’ensemble du pays, ce qui permit de constater que l’infection fut fréquente et largement répandue, mais qu’elle ne fut pas grave cliniquement. Dans d’autres pays européens, on rapporta que la maladie causa surtout des poussées localisées ou bien que sa diffusion fut relativement faible et non explosive comme d ’habitude. In Europe, the rise in incidence started generally at the beginning of 1969, i.e. later than in the United States o f America, and a high morbidity continued into April. Nearly all the European countries reported outbreaks but these were of variable extent. The largest were in Poland, where it was estimated that more than four million cases of influenza-like disease occurred, and in the Netherlands. Generalized epidemics were also reported from Bulgaria, Czechoslo vakia, parts of the Federal Republic o f Germany, Finland, Hungary, Iceland, Sweden and parts of the USSR. In all these countries, however, the disease was reported to be mild and no great excess of deaths was observed. In the other European countries, the infec tion was sometimes apparently widespread, but its influence on sickness-absence was slighter than usual and it did not cause a large rise in mortality. This was shown in the United Kingdom, where the Public Health Laboratory Service provides readily available virus diagnostic facilities over the whole country, and where there was evidence that infection was frequent and wide spread but not clinically severe. Other European countries also reported that the disease caused predominantly focal outbreaks or spread relatively slowly instead of in the usual clearly recognizable explosive pattern. In the temperate countries of the northern hemisphere, outbreaks ceased by the end of April.

Some tropical countries not affected during the first wave of July, August and September 1968 became involved later on. Outbreaks were recorded in Kenya and epidemics in Brazil, Ceylon, Fiji, Mexico, Puerto Rico and Senegal. In the southern hemisphere, no large epidemics were reported in 1968. In 1969, outbreaks began in March in South Africa and, as from May, outbreaks or epidemics were reported from Argentina, Australia, Chile, New Zealand and Uruguay.

In 1969, the Mexican Social Security Institute carried out a sample survey of the families of its beneficiaries in the Federal District and Valley of Mexico, to obtain information on naturally acquired immunity against rubella and from this to be able to calculate the risk of birth defects due to congenital rubella. Venous blood specimens were obtained from 2 291 persons and tested for hemag glutination inhibition to establish the antibody levels. Levels of 1:8 or higher, considered to indicate immunity, were found in 71.8% of the specimens (most had titers higher than 1: 238). The percentage of immunity was 31.4 for children 1-4 years of age, 75.1 for those 5-9 years, and 88 for the group 10-14 years of age. For women over 15 years of age, the percentages increased with age from 93.5 to 98.6. The indices of immunity varied with the socio-economic condition in the early ages but were not signifi cantly different for groups of adult women. The investigators concluded that the protection rates of Mexican women are high, in comparison with those found in similar surveys in other countries, and that mass vaccination against measles probably should be given higher priority than that against rubella in view of the impor tance of measles as a cause of death in Mexico.

CRITERIA U S E D IN CO M PILING TH E INFECTED AREA LIST CRITÈRES APPLIQ U ÉS P O U R LA CO M PILATIO N D E LA LISTE D ES TERRITOIRES INFECTÉS Based on the International Sanitary Regulations (as amended by Additional Regulations of 1955, 1956 and 1963) the following criteria are used in compiling and maintaining the infected area list (only official governmental information is used): I. An area is entered in the list on receipt o f information of: (i) a declaration of infection under Article 3; (ii) the first case of plague, cholera, yellow fever or smallpox that is neither an imported case nor a transferred case; (iii) plague infection among rodents on land or on craft which are part of the equipment of a port; (iv) activity o f yellow-fever virus in vertebrates other than man using one of the following criteria: (a) the discovery of the specific lesions of yellow fever in the liver of vertebrates indigenous to the area; or (b) the isolation of yellow-fever virus from any indigenous vertebrates; (v) an epidemic of typhus or relapsing fever [an epidemic is interpreted to mean the occurrence of two or more cases which are neither imported nor transferred cases in a local area during a four-week (typhus) or a three-week (relapsing fever) period]. n. An area is deleted from the list on receipt of information as follows: (i) if the area was declared infected (Article 3), it is deleted from the list on receipt of a declaration under Article 6 that the area is free from infection.
