O f these the most remarkable has been the further dramatic decrease in numbers of isolates of Shigella sonnet with only 78 reported so far this year in contrast to 533 last year. Throughout the past decade there has been a steady decline in the incidence of Sonne dysentery throughout the United Kingdom, during which time annual totals in Scotland fell from 3 199 confirmed cases in 1971 to 742 by 1980. A similar fall was observed also during the early 1970’s m the numbers of isolates of Sh. jlexneri from 1 166 in 1971 to 61 by 1980, with the greatest reduction occurring in the city of Glasgow from 1972-1973 onwards. In recent years imported infections have ac­ counted for an increasing proportion of cases of bacillary dysentery in Scotland. There has been a marked decline also in indigenous enteric fever, with 72% of cases of typhoid or paratyphoid reported in Scotland during 1975-1980 occurring in persons recently returned from over­ seas travel, usually the Mediterranean area or the Indian sub-conti­ nent. This compares over the previous six years from 1969-1974 with a level of 35% of enteric infections being imported. In contrast, the incidence of salmonellosis caused by S. typhimurium and other serotypes continues to show an upward trend. In 1980 the highest ever number of human cases (1 577) was reported in Scotland, while for the first six months of this year 640 cases have been recorded already, compared with 442 for the same period last year (Fig. I), an increase of 198 cases or 45%, with the peak seasonal incidence over the summer and early autumn months still to come. The serotypes most commonly identified so far in 1981 have been S. typhimurium (201), S enteritis (126), and S.ytrchow (116). Even more noticeable, however, has been the further increase in the diagnosis o f Campylobacter gastro-ententis with 918 cases reported so far this year, compared with 499 during the first half of last year— an increase of 419 cases or 84%. Although most infections have occurred either in family groups or sporadically throughout Scotland, without any apparent source or food vehicle identified, poultry has featured frequently in the food histories of many of those affected. One apparently common-source outbreak occurred in Fife towards the end of May when cases associated with the consumption of untreated milk were reported in and around the town of Dunfermline. It is uncertain as yet whether or not the increase in recorded incidence of Campylobacter infections is real, o ris there both awareness of the problem among medical practitioners, and an increase in the number of laboratories undertaking cultural examination for Campylobac­ ter.

Ces dix dernières années, l’incidence de la dysen­ terie à Sh sonnet a constamment baissé dans tout le Royaume-Uni, et pendant cette période le total annuel de cas confirmés en Ecosse est passé de 3 199 en 1971 à 742 en 1980. Une baisse analogue a aussi été observée pendant les premières années 70 dans le nombre de cas à Sh Jlexneri (1 166 en 71 contre 61 en 1980), la baisse la plus forte se produisant à Glasgow à partir de 1972-1973. Ces dernières années, les infections importées représentaient une proportion croissante des cas de dysenterie bacillaire en Ecosse. Il y a également eu une baisse marquée de la fièvre enténque indi­ gène, avec 72% des cas de typhoïde ou de paratyphoïdes signalés en Ecosse en 1975-1980 se produisant chez des personnes revenant de voyages outre-mer, dans la région méditerranéenne ou le sous-continent mdien surtout. Pour les six années précédentes (1969-1974) la proportion d’infections importées était de 35%. Par contre, l’incidence des salmonelloses causées par S. typhimurium et d’autres sérotypes continue à augmenter. En 1980, un nombre de cas humains record (1 577) a été signalé en Ecosse, et pour les six premiers mois de cette année, il y a déjà eu 640 cas d’enregistrés, contre442 pour la même période l’année précédente (Fig. 1), donc 198 cas (soit 45%) de plus, alors que la pointe saisonnière de l’été et du début de l’automne est encore à venir. Les sérotypes les plus commu­ nément identifiés à ce jour en 1981 sont S. typhimurium (201), S enteritidis (126) et S. virchow (116). Plus remarquable encore est la nouvelle augmentation des cas de gastro-entérites à Campylobacter: 918 à ce jour cette année, contre 499 pour le premier semestre de l’année passée, donc 419 cas (soit 84%) de plus. Bien que la plupart des infections se produisent soit dans des groupes familiaux soit sporadiquement dans l’ensemble de l’Ecos­ se sans que l’on puisse identifier une source apparente ou un vecteur alimentaire, on retrouve fréquemment de la volaille dans le régime alimentaire des malades. Une poussée apparemment de la même source s’est produite dans le comté de Fife vers la fin mai : des cas liés à l’absorption de lait non traité ont été signalés dans la ville de Dun­ fermline et aux environs. On ne saurait encore dire si l’augmentation de l'incidence des infections à Campylobacter est réelle ou si elle tient à la fois à une meilleure connaissance du problème par les médecins et à une augmentation du nombre de laboratoires fàisant des cultures pour rechercher la présence de Campylobacter.
