ONCHOCERCIASIS CONTROL PROGRAMME IN WEST AFRICA 01 P.O. BOX 549 - Tel.(226) 30.23.12 - Telex 52418F OUAGADOUGOU 01 - BURKINA FASO The National Onchocerciasis Team Ministry of Public Health and Social Services Makeni Sierra Irone Registry File: 08/181.1 lll Date: 31./10/1996 Allotment: OU/ICP I CTD / 426 / oN / 96 / 834 I 04 TECHNICAL SERVICES - LETTER OF AGREEMENT For the purpose of carrying out the training of nurses of the District and Peripheral Health Teams for the installation of community-based ivermectin treatment in the villages in four districts in Sierra Leone: The World Health Organisation, Onchocerciasis Control Programme, hereinafter referred to as "WHO/ONCHO" on the one hand, and The National Onchocerciaisis Team Ministry of Public Health and Social Services Makeni Seirra leone hereinafter referred to as the "INSTITUTION", on the other hand; HAVE AGREED AS FOLLOWS: 1. Duties to be performed Missions: Training of 35 nurses of the Tonkolili district in the Rokel II river basin from 18-22 November 1996. Training of 50 nurses of the Port Loko district in the Rokel I, Kaba I and Mabole I river basins from 25-29 November 1996. Training of 35 nurses of the Koinadugu district in the Kaba, Mongo II, Bagbe and Seli river basins from 25-29 November, 1996. Training of 55 nurses of the Bomboli district in the Kaba II, Mabole II and Mongo I river basins from 2-6 December 1996. A B C D I\ 2The working programme for this agreement could be modified in the course of its execution by WHO/ONCHO after discussions with the National team. 2. Methodolosv The national team (the National Coordinator and his deputy and 2 national technicians), the District Health Officer and a member of the District Health Team in collaboration with a member from the PET Unit and a temporary advisor will carry out this training as for missions A,, B, C, D above. The training will consist of theoretical training in the diagnosis and the treatment of onchocerciasis and the information, education and communication (IEC) messages to be conveyed to the village communities a micro-planning by each of the nurses for their health areas of operation for the training of the village distributors and their supervision practical training in the field in 3 or four villages The following will be need to be established with the microplanning for each of the health areas: number of villages and the respective populations to be treated in the area a training schedule (calendar) for the community distributors which should be intergrated in the health activities in the area After the training and microplanning, each nurse will be provided with the required number of tablets of ivermectin to be determined by the number of villages to be treated. On their return to their respective health areas the nurses, using the available means at their disposal, will in turn sensitize the communities to choose their community/village distributors to be trained to undertake ivermectin treatment 3. Obligations of WHO/ONCHO 3.1. Travel oerdiem See annex 1 A total amount of Le 7,023,250 [r (seven million, twenty three thousand two hundred and fifty Leones) representing the per diem will be paid to the participants. The per diem will be paid only for the number of days spent by the participants for the training. The number of days spent could be less than that assigned in the letter of agreement. N.B.: Resrdents from the trarning srte (town) will receive half of the per diem. 3.2. Training C: Training D: Total = 1,000 l. Other operating costs An amount of 400,000 (four hundred thousand leones) shall be provided to cover stationary, secretariat backup tea breaks and unexpected expenses per each training session i.e Le 1,600,000 one million six hundred thousand leones for the 4 training sessions. WHO/ONCHO shall provide the cost of transportation for the nurses to and from the training place. 175 nurses x 6,000 lE = 1,050,000t€. 3.3. Means of travel WHO/ONCHO shall provide: MissionA:5vehicles Mission B and C : 5 vehicles each MissionD:5vehicles 3.4. Office Supplies and others. WHO/ONCHO shall provide the necessary office supplies, for the training. 4. Contribution of the Institution The Institution, in agreement with his government will provide the requisite personnel for the training for the installation of community-based ivermectin treatment . This personnel will consist of : A. Tonkolili District B. Port Loko District Total 48 Total 60 4D. Tonkolili District B. Port Loko From 25-29 November 1996 C. Koinadugu district From 25-29 November 1996 D. Bombali district From 2-6 Devember 1996 6. Financial arrangements C. Koinadugu District - District Medical Officer 1 - District Health Team 1 - Nurses 35 - National Technicians 2 - National or Deputy coordinator L - Drivers 5 45 ditto ditto ditto Total From 18-22 November 1996, inclusive for a period of 5 days. 6.1. The total amount of expenditure estimated by this agreement is Ir 9,966,950 (nine million nine hundred and sixty six thousand nine hundred and fifty Leones) 6.2. This amount will be paid in one instalment as soon as this agreement is duly signed by the two parties concerned and the document returned to the WHO/ONCHO in Ouagadougou. 6.3 At the end of each mission all accounting documents (bank statement, receipts, travel claims etc.) shall be forwarded to the following address The Programme Director Attention: Budget and Finance Officer wHo/oNCHO 01 P.O. Any unliquidated balance at the end of the period covered by this current agreement shall be paid back to WHO/ONCHO by the Institution or deducted later on from the amount of the sum WHO/ONCHO will have to pay in the framework of any possible subsequent agreement. 7. lrsal arransements It is understood that the execution of this work does not constitute any employer/employee relationship between WHO/ONCHO and the Institution. The Institution shall therefore be solely responsible for the modalities for the execution of the work. WHO/ONCHO shall not be responsible for any loss, accident, damage or injury suffered by the Institution or any person claiming under it arising in and out of the execution of this work or in any manner whatsoever. Any dispute relating to the interpretation or execution of this agreement shall, unless amicably settled, be subject to conciliation. In the event of failure of the latter, the dispute shall be settled by arbitration. The arbitration shall be conducted in accordance with the modalities to be agreed upon by the parties or, in the absence of any agreement, with the rules of arbitration of the International Chamber of Commerce. The parties shall accept the arbitral award as final. lfindly comply with section 6.4 above for smooth communication. 6If your Institution accepts this proposal, we should be grateful if you would see to the signing of all the 4 copies of this agreement by two authorities responsible for the execution of the work and return three copies to us.
