JUNGLE YELLOW FEVER

FIÈVRE JAUNE DE BROUSSE E q u a t e u r . — Le 23 juillet 1976, un jeune homme de 24 ans a été atteint soudainement d ’une maladie infectieuse aiguë à Shushufindi dans le Canton de Putumayo (Province de Napo). H a été hospitalisé deux jours plus tard et est mort le 26 juillet après une grave bématémèse, le diagnostic porté étant celui d ’hépatite hémorragique. Un échantillon du foie a été prélevé par viscérotomie et l’examen microscopique a montré qu’il s’agissait de la fièvre jaune. Le malade, qui n ’avait pas été vacciné contre la fièvre jaune, était arrivé dans la jungle orientale 15 jours avant les premiers symptômes. Il avait travaillé 12 jours dans la région comme bûche­ ron et, au cours de cette période, avait passé les nuits dans un hamac suspendu entre les arbres. Trois autres bûcherons, qui vivaient dans les mêmes conditions mais avaient été vaccinés contre la fièvre jaune l'année précédente, n ’ont présenté aucun symptôme. Les habitants des alentours n ’avaient observé ni maladie affectant les singes, ni signe d ’épidémie locale. Les enquêtes épidémiologiques nécessaires ont été faites et une campagne intensive de vaccination a été lancée. La population de la zone se compose en grande partie de gens qui s’y sont établis cette année. Le dernier cas notifié dans la Province de Napo l’avait été en avril 1975.

Ecuador. — On 23 July 1976, in the locality of Shushufindi, Putumayo Canton in Napo Province, a 24-year-old male experienced the sudden onset of an acute infectious disease. Two days later he was admitted to hospital and, after suffering a severe haematemesis, died on 26 July with a diagnosis of haemorrhagic hepatitis. A liver specimen was taken by viscerotome and the diagnosis of yellow fever was confirmed microscopically. The patient, who had not been vaccinated against yellow fever, had arrived in the eastern jungle 15 days before the symptoms appeared. He worked in the area for 12 days as a wood-cutter and, during this period, slept on a hammock which was hung from trees. Three fellow workers who lived under the same conditions, but had been vaccinated against yellow fever the previous year, showed no symptoms. Residents in the neighbouring areas had not observed any illness affecting monkeys or any evidence of a local epidemic. The necessary epidemiological studies were carried out and an intensive vaccination campaign undertaken. Most of the population in the area were settlers who had arrived there during the current year. The last case notified in the Napo Province was in April 1975.
