Zoonotic influenza viruses: antigenic and genetic characteristics and development of candidate vaccine viruses for pandemic preparedness March 2017 The development of candidate influenza vaccine viruses (CVVs), coordinated by WHO, remains an essential component of the overall global strategy for influenza pandemic preparedness. Selection and development of CVVs are the first steps towards timely vaccine production and do not imply a recommendation for initiating manufacture. National authorities may consider the use of one or more of the CVVs for pilot lot vaccine production, clinical trials and other pandemic preparedness purposes based on their assessment of public health risk and need.

La sélection et la mise au point de virus vaccinaux candidats représentent les premières étapes vers une production en temps utile des vaccins, mais n’impliquent pas qu’il soit recommandé d’en démarrer la fabrication. Les autorités nationales peuvent envisager d’utiliser un ou plusieurs de ces virus vaccinaux candidats pour la production de lots pilotes de vaccins, la réalisation d’essais cliniques et d’autres activités de préparation aux pandémies, selon leur évaluation des risques et des besoins de santé publique. Des virus grippaux zoonotiques présentant une évolution à la fois génétique et antigénique continuent d’être identifiés, d’où la nécessité de mettre au point des virus vaccinaux candidats supplémentaires aux fins de la préparation aux pandémies. Zoonotic influenza viruses continue to be identified and evolve both genetically and antigenically, leading to the need for additional CVVs for pandemic preparedness purposes. Changes in the genetic and antigenic characteristics of these viruses relative to existing CVVs, and their potential risks to public health, necessitate the need to select and develop new CVVs.

This report summarizes the genetic and antigenic characteristics of recent zoonotic influenza viruses and related viruses circulating in animals1 that are relevant to CVV updates. Institutions interested in receiving these CVVs should contact WHO

(1)  Influenza A(H5) Since their emergence in 1997, highly pathogenic avian influenza (HPAI) A(H5) viruses of the A/goose/Guangdong/1/96 haemagglutinin (HA) lineage have become enzootic in some countries, have infected wild birds, and continue to cause outbreaks in poultry and sporadic human infections. These viruses have diversified genetically and antigenically, including the emergence of viruses with replacement of the N1 gene segment by N2, N3, N5, N6, N8 or N9 gene segments, leading to the need for multiple CVVs. This summary provides updates on the characterization of A/goose/Guangdong/1/96-lineage A(H5) viruses and the current status of the development of influenza A(H5) CVVs.

1)  Grippe A(H5) Depuis leur émergence en 1997, des virus de la grippe aviaire A(H5) hautement pathogènes contenant le gène de l’hémagglutinine (HA) de la lignée A/goose/Guangdong/1/96 sont devenus enzootiques dans certains pays, ont infecté des oiseaux sauvages et continuent de provoquer des flambées chez les volailles, ainsi que des infections sporadiques chez l’homme. Ces virus se sont diversifiés sur le plan génétique et antigénique, notamment par l’apparition de virus porteurs d’une substitution génétique du segment N1 par des segments N2, N3, N5, N6, N8 ou N9, ce qui impose de mettre au point plusieurs virus vaccinaux candidats. Le présent résumé fait le point sur la caractérisation des virus A(H5) de la lignée A/goose/ Guangdong/1/96 et sur l’état d’avancement des activités de préparation de virus candidats devant entrer dans la composition d’un vaccin contre la grippe A(H5). Activité de la grippe A(H5) du 27 septembre 2016 au 27 février 2017 Des infections humaines par les virus A(H5) ont été notifiées à l’OMS par la République populaire de Chine (2 cas) et l’Égypte (2 cas), où des infections par les virus A(H5) ont également été détectées chez des oiseaux. Les cas d’infection humaine en Égypte, dont l’un a été mortel, étaient dus aux virus A(H5N1), tandis que ceux de la Chine étaient imputables aux virus A(H5N6). Des virus A(H5) de la lignée A/goose/Guangdong/1/96 ont été identifiés chez des volailles et des oiseaux sauvages dans de nombreux pays: 1072 et 644 détections dans ces populations respectives ont été notifiées à l’Organisation mondiale de la santé animale (OIE) par les autorités nationales (Tableau  1). Caractéristiques antigéniques et génétiques des virus grippaux A(H5) La nomenclature des liens de parenté phylogénétiques existant entre les gènes de l’hémagglutinine des virus grippaux A(H5) de la lignée A/goose/Guangdong/1/96 est définie en consultation avec des représentants de l’OMS, de l’Organisation des Nations Unies pour l’alimentation et l’agriculture (FAO), de l’OIE et d’établissements universitaires.3 Les virus circulants qui ont été caractérisés entre le 27 septembre 2016 et le 27 février 2017 appartenaient aux clades suivants: Clade 2.2.1.2: Des virus de ce clade ont été identifiés chez des volailles en Égypte. Bien que les virus identifiés en 2016 chez les oiseaux présentent plusieurs substitutions d’acides aminés sur le gène de l’hémagglutinine par rapport à la souche A/Egypt/N04915/2014 ayant servi à la préparation d’un virus vaccinal candidat, ils demeurent analogues à ce virus vaccinal candidat sur le plan antigénique. On ne dispose d’aucune donnée antigénique ou génétique sur les virus détectés chez l’homme en Égypte.

Influenza A(H5) activity from 27 September 2016 to 27 February 2017 A(H5) human infections have been reported to WHO by the People’s Republic of China (China) (2 cases) and Egypt (2 cases) where A(H5) infections have also been detected in birds. The human infections in Egypt, of which one was fatal, were caused by A(H5N1) viruses, while the human infections in China were caused by A(H5N6) viruses. A/goose/Guangdong/1/96-lineage A(H5) viruses were detected in poultry and wild birds in many countries with 1072 and 644 events, respectively, being reported to the World Organisation for Animal Health (OIE) and by national authorities (Table  1). Antigenic and genetic characteristics of influenza A(H5) viruses The nomenclature for phylogenetic relationships among the HA genes of A/goose/Guangdong/1/96-lineage A(H5) viruses is defined in consultation with representatives of WHO, the Food and Agriculture Organization of the United Nations (FAO), OIE and academic institutions.3 Viruses circulating and characterized from 27 September 2016 to 27 February 2017 belong to the following clades: Clade 2.2.1.2 viruses were detected in poultry in Egypt. Although the HAs of the 2016 viruses from birds had accumulated a number of amino acid substitutions relative to A/Egypt/N04915/2014, from which a CVV has been developed, they remained antigenically similar to the CVV. No antigenic or genetic data are available for the human viruses from Egypt.

Clade 2.3.2.1a viruses were detected in birds in Bangladesh, Bhutan and India. The HA genes of these viruses were similar to viruses detected in the region in previous periods. Viruses from Bangladesh were available for antigenic testing and reacted well with post-infection ferret antiserum raised against the A/duck/Bangladesh/19097/2013 CVV. Clade 2.3.2.1c viruses were detected in birds in China, Côte d’Ivoire, Ghana, Lao People’s Democratic Republic, Niger, Nigeria, Togo and Viet Nam. Viruses from Africa were genetically and antigenically distinct from those in Asia. Viruses from Africa were genetically and antigenically similar to those detected previously, including A/chicken/Ghana/20/2015 from which a CVV is under development. Viruses from Asia were also similar to previously detected viruses and CVVs.

Clade 2.3.2.1a: Des virus de ce clade ont été détectés chez des oiseaux au Bangladesh, au Bhoutan et en Inde. Les gènes de l’hémagglutinine de ces virus sont analogues à ceux des virus identifiés dans la région au cours des périodes précédentes. Clade 2.3.4.4: Des virus de ce clade ont été détectés chez des oiseaux dans de nombreux pays d’Afrique, d’Asie et d’Europe, ainsi qu’aux États Unis d’Amérique. Ils ont également été identifiés chez 2 cas humains en Chine (Tableau 1). Les virus du clade 2.3.4.4 isolés en Afrique et en Europe appartenaient principalement au sous type A(H5N8); en Asie, il s’agissait essentiellement de virus A(H5N6) et aux États Unis d’Amérique de virus A(H5N2). Les gènes de l’hémagglutinine des virus humains détectés en Chine appartenaient au même groupe phylogénétique que ceux de la souche A/Hubei/29578/2016, à partir de laquelle un virus vaccinal candidat a été mis au point (Figure 1); aucune information antigénique n’est disponible à ce jour. Les virus du clade 2.3.4.4 détectés en Afrique et en Europe étaient génétiquement analogues aux virus identifiés lors des périodes précédentes (Figure 1). Les virus d’Europe réagissaient bien avec un antisérum de furet postinfection dirigé contre le virus vaccinal candidat élaboré à partir de la souche A/chicken/Viet Nam/NCVD 15A59/2015 (Tableau 2). Les virus A(H5N6) isolés au Japon et en République de Corée étaient comparables sur le plan génétique et présentaient une réaction médiocre avec un antisérum de furet postinfection dirigé contre les virus vaccinaux candidats disponibles (Tableau 2). Un nouveau virus vaccinal candidat de type A/duck/Hyogo/1/2016 est proposé. Virus candidats destinés à la préparation d’un vaccin contre la grippe A(H5) Les virus vaccinaux candidats A(H5) disponibles et en attente sont recensés dans le Tableau 3. Ces virus continuant d’évoluer, de nouveaux virus vaccinaux candidats A(H5) pourraient être mis au point.

Clade 2.3.4.4 viruses were detected in birds in many countries in Africa, Asia, Europe and the United States of America (USA), and in 2 humans in China (Table 1). Clade 2.3.4.4 viruses from Africa and Europe were primarily of the A(H5N8) subtype; those in Asia were primarily A(H5N6) and that in the USA was A(H5N2). The HA genes of the human viruses from China fell within the same phylogenetic cluster as A/Hubei/29578/2016, from which a CVV has been developed (Figure 1); antigenic information is not yet available. Clade 2.3.4.4 viruses from birds in Africa and Europe were genetically similar to viruses detected in previous periods (Figure 1). Viruses from Europe reacted well with post-infection ferret antiserum raised against the A/chicken/Viet Nam/NCVD15A59/2015 CVV (Table 2). The A(H5N6) viruses detected in Japan and the Republic of Korea were genetically similar and did not react well with post-infection ferret antiserum raised against available CVVs (Table 2). A  new A/duck/Hyogo/1/2016-like CVV is proposed.

Influenza A(H5) candidate vaccine viruses The available and pending A(H5) CVVs are listed in Table 3. As the viruses continue to evolve, new A(H5) CVVs may be developed.

(2)  Influenza A(H7) Influenza A(H7N9) Human infections with avian influenza A(H7N9) viruses were first reported to WHO on 31 March 2013. A(H7N9) viruses are enzootic in poultry in China and reassortment with A(H9N2) viruses has continued to generate multiple genotypes.

2)  Grippe A(H7) Grippe A(H7N9) Les premiers cas d’infection humaine par des virus grippaux aviaires A(H7N9) ont été notifiés à l’OMS le 31 mars 2013. Les virus A(H7N9) sont enzootiques parmi les volailles en Chine et leur réassortiment avec des virus A(H9N2) continue de produire de nombreux génotypes.

Influenza A(H7N9) activity from 27 September 2016 to 27 February 2017 During this period, 460 cases of A(H7N9) virus infection with 78 deaths were reported in China, Hong Kong Special Administrative Region of China and Macao Special Administrative Region of China, bringing the total number of cases since 2013 to 1258 with 328  deaths. Despite control measures being implemented, the number of human cases was higher in this fifth wave compared with prior waves. Recent A(H7N9) viruses belong to the Yangtze River Delta (YRD) or Pearl River Delta (PRD) HA lineages (Figure 2). Recent viruses of the YRD lineage reacted less well with postinfection ferret antiserum raised against the available A/Anhui/1/2013 and A/Shanghai/2/2013-derived CVVs (Table 4). A new A/Hunan/2650/2016-like CVV is proposed (A/Hong Kong/125/2017 is an A/Hunan/2650/2016like virus). Additionally, A(H7N9) viruses of the YRD lineage with multiple basic amino acids at the cleavage site have been detected in humans, poultry and environmental samples from live poultry markets. These viruses fulfil the requirements for classification as HPAI viruses. The highly pathogenic avian influenza A(H7N9) viruses were genetically and antigenically distinct from other A(H7N9) viruses including A/Hunan/2650/2016 and the current CVVs (Figure 2, Tables 4 and 5). Therefore, a new CVV derived from an A/Guangdong/17SF003/2016-like virus (HPAI) is proposed. Influenza A(H7N9) candidate vaccine viruses The available and pending A(H7N9) CVVs are listed in Table 6. Activité de la grippe A(H7N9) du 27 septembre 2016 au 27 février 2017 Pendant cette période, 460 cas d’infection par le virus A(H7N9), dont 78 mortels, ont été notifiés à l’OMS par la Chine, la Région administrative spéciale de Hong Kong et la Région administrative spéciale de Macao, ce qui porte à 1258 le nombre total de cas enregistrés depuis 2013, dont 328 mortels. Malgré la mise en œuvre de mesures de lutte contre l’infection, cette cinquième vague s’est caractérisée par un plus grand nombre de cas humains que les vagues précédentes. Les virus A(H7N9) récemment détectés appartenaient aux lignées Yangtze River Delta (YRD) ou Pearl River Delta (PRD) de l’hémagglutinine (Figure 2). Les virus récents de la lignée YRD réagissaient moins bien avec un antisérum de furet postinfection dirigé contre les virus vaccinaux candidats disponibles dérivés des souches A/Anhui/1/2013 et A/Shanghai/2/2013 (Tableau 4). Un nouveau virus vaccinal candidat de type A/Hunan/2650/2016 est proposé (A/Hong Kong/125/2017 est un virus de type A/Hunan/2650/2016). En outre, des virus A(H7N9) de la lignée YRD présentant plusieurs acides aminés basiques sur le site de clivage ont été détectés chez l’homme, parmi les volailles et dans des échantillons environnementaux provenant de marchés de volailles vivantes. Ces virus répondent aux conditions requises pour être classés parmi les virus hautement pathogènes de la grippe aviaire. Les virus A(H7N9) hautement pathogènes étaient génétiquement et antigéniquement distincts des autres virus A(H7N9), dont A/Hunan/2650/2016 et les virus vaccinaux candidats actuels (Figure 2, Tableaux 4 et 5). Un nouveau virus vaccinal candidat dérivé d’un virus de type A/Guangdong/17SF003/2016 (virus aviaire hautement pathogène) est donc proposé. Virus candidats destinés à la préparation d’un vaccin contre la grippe A(H7N9) Les virus vaccinaux candidats A(H7N9) disponibles et en attente sont recensés dans le Tableau 6. Influenza A(H7N2) Influenza A(H7N2) viruses have been detected in poultry populations worldwide with rare human infections. Influenza A(H7N2) activity from 27 September 2016 to 27 February 2017 A(H7N2) viruses were detected in cats in animal shelters in New York, USA. A veterinarian with prolonged exposure to infected cats was confirmed infected with a virus very similar to those in the cats. The individual presented with mild illness, was placed on oseltamivir treatment and recovered. The virus was genetically similar to previously identified A(H7N2) viruses in live poultry markets, commercial flocks and a human case in 2003 in the USA, and reacted well with post-infection ferret antiserum raised against the A/turkey/ Virginia/4529/2002 CVV.

Grippe A(H7N2) Des virus grippaux A(H7N2) ont été identifiés chez les volailles partout dans le monde, avec quelques rares infections humaines. Activité de la grippe A(H7N2) du 27 septembre 2016 au 27 février 2017 Des virus A(H7N2) ont été détectés parmi des chats hébergés dans des refuges pour animaux à New York (États-Unis d’Amérique). Une infection humaine par un virus fortement apparenté au virus identifié chez les chats a été confirmée chez un vétérinaire qui avait été exposé de manière prolongée aux chats infectés. Présentant des signes bénins de maladie, il a été traité par l’oseltamivir et s’est rétabli. Le virus était génétiquement analogue aux virus A(H7N2) précédemment détectés dans les marchés de volailles, les élevages commerciaux et un cas humain identifié en 2003 aux États Unis d’Amérique. Il a bien réagi avec un antisérum de furet postinfection dirigé contre le virus vaccinal candidat dérivé de la souche A/turkey/ Virginia/4529/2002.

(3)  Influenza A(H9N2) Influenza A(H9N2) viruses are enzootic in poultry populations in parts of Africa, Asia and the Middle East. The majority of viruses that have been sequenced belong to the A/quail/Hong Kong/G1/97 (G1) and A/chicken/ Beijing/1/94 (Y280/G9) lineages. Since 1998, when the first human infection was identified, the detection of A(H9N2) viruses from humans and swine has been reported infrequently. In most human cases the associated influenza-like symptoms have been mild and there has been no evidence of human-to-human transmission. Influenza A(H9N2) activity from 27 September 2016 to 27 February 2017 Three human cases of A(H9N2) infections have been identified in China in this period. Sequence information could be generated from samples for only one of these cases. The HA gene of this virus was similar to Y280lineage A(H9N2) viruses known to circulate in birds in China. Antigenic information for this virus is pending. A(H9N2) viruses from birds were characterized from a small number of other countries, with most being similar to those detected in previous periods. Influenza A(H9N2) candidate vaccine viruses Based on the current antigenic, genetic and epidemiologic data, no new CVVs are proposed. The available A(H9N2) CVVs are listed in Table 8. 3)  Grippe A(H9N2) Les virus grippaux A(H9N2) sont enzootiques dans les populations de volailles de certaines parties d’Afrique, d’Asie et du Moyen-Orient. La majorité des virus qui ont été séquencés appartiennent aux lignées A/quail/Hong Kong/G1/97 (G1) et A/chicken/Beijing/1/94 (Y280/G9). (4)  Influenza A(H1) variants (v)4 Influenza A(H1) viruses circulate in swine populations in many regions of the world. Depending on geographic location, the genetic characteristics of these viruses differ. Human infections with swine A(H1) viruses have been documented for many years. Influenza A(H1)v activity from 27 September 2016 to 27 February 2017 Four non-fatal A(H1)v human cases were detected in this reporting period. One case of A(H1N2)v infection was identified in the USA in November 2016 in an individual with reported exposure to swine. The HA from this virus, A/Iowa/32/2016, belonged to the delta 1  lineage of swine influenza viruses (Figure 3). The Netherlands detected an influenza A(H1N1)v case in October 2016. The individual developed a severe infection following exposure to infected pigs but recovered. Another case of influenza A(H1N1)v virus was detected in Italy in October 2016. The individual developed acute respiratory distress syndrome following exposure to swine and was hospitalized with pneumonia, but recovered following oseltamivir treatment. In December 2016, an A(H1N1)v case was detected in Switzerland following swine exposure. The 3 A(H1N1)v viruses detected in Europe were Eurasian avian-like A(H1N1)v influenza viruses related to viruses known to circulate in swine (Figure 3). Antigenic testing demonstrated that ferret antisera raised against current CVVs reacted poorly with the A(H1)v viruses available for testing (Table 9). New CVVs generated from A/Iowa/32/2016 and A/Netherlands/3315/2016-like viruses are proposed. Influenza A(H1)v candidate vaccine viruses The available A(H1)v CVVs are listed in Table 10. As the viruses continue to evolve, new A(H1)v CVVs may be developed.

4)  Variants des virus grippaux A(H1) (v)4 Des virus grippaux A(H1) circulent dans les populations porcines de nombreuses régions du monde. Les caractéristiques génétiques de ces virus diffèrent selon le lieu géographique. Des infections humaines par les virus porcins A(H1) sont observées depuis de nombreuses années. Activité de la grippe A(H1)v du 27 septembre 2016 au 27 février 2017 Quatre cas humains non mortels de grippe A(H1)v ont été détectés au cours de cette période. Un cas d’infection par le virus A(H1N2)v a été identifié aux États Unis d’Amérique, en novembre 2016, chez un patient qui avait été exposé à des porcs. Le gène de l’hémagglutinine de ce virus, A/Iowa/32/2016, appartenait à la lignée delta 1 des virus grippaux porcins (Figure 3). Les Pays Bas ont détecté un cas de grippe A(H1N1)v, en octobre 2016. La personne concernée a contracté une infection sévère après avoir été exposée à des porcs infectés, mais s’est rétablie. Un autre cas d’infection par le virus grippal A(H1N1)v a été identifié en Italie, en octobre 2016. Présentant un syndrome de détresse respiratoire aiguë après avoir été exposé à des porcs, le patient a été hospitalisé pour une pneumonie, mais s’est rétabli suite à un traitement par l’oseltamivir. En décembre 2016, un cas de grippe A(H1N1)v a été détecté en Suisse chez un sujet ayant été exposé à des porcs. Les 3 virus grippaux A(H1N1)v isolés en Europe étaient de type aviaire eurasien, apparentés à des virus dont la circulation dans les populations porcines est connue (Figure 3). Les tests antigéniques ont montré que les antisérums de furet dirigés contre les virus vaccinaux candidats actuels présentaient une réaction médiocre avec les virus A(H1)v disponibles aux fins de l’analyse (Tableau 9). De nouveaux virus vaccinaux candidats, dérivés des souches A/Iowa/32/2016 et A/Netherlands/3315/2016, sont proposés. (5)  Influenza A(H3N2)v Influenza A(H3N2) viruses are enzootic in swine populations in most regions of the world. Depending on geographic location, the genetic and antigenic characteristics of these viruses differ. Human infections with swine influenza A(H3N2) viruses have been documented in Asia, Europe and North America.5

Grippe A(H3N2)v Les virus grippaux A(H3N2) sont enzootiques dans les populations porcines de la plupart des régions du monde. Influenza A(H3N2)v activity from 27 September 2016 to 27 February 2017 One case of A(H3N2)v infection was identified in Canada. The individual developed symptoms of respiratory infection in October 2016, was hospitalized with pneumonia and recovered. The individual had confirmed exposure to ill swine. The HA gene of the virus isolated from the case, A/Ontario/RV3236/2016, was similar to cluster IV-B A(H3N2) viruses currently circulating in swine populations in the USA and Canada. Antigenic testing of A/Ontario/RV3236/2016 revealed that ferret antisera raised against existing cluster IV-A wild type viruses and CVVs (A/Minnesota/11/2010 and A/Indiana/10/2011) reacted well with this virus. Influenza A(H3N2)v candidate vaccine viruses Based on the available antigenic, genetic and epidemiologic data, no new A(H3N2)v CVVs are proposed. The available A(H3N2)v CVVs are listed in Table 11. As the viruses continue to evolve and as new data are generated, new A(H3N2)v CVVs may be developed.

Activité de la grippe A(H3N2)v du 27 septembre 2016 au 27 février 2017 Un cas d’infection par le virus A(H3N2)v a été identifié au Canada. Le patient concerné a présenté des symptômes d’infection respiratoire en octobre 2016, a été hospitalisé pour une pneumonie et s’est rétabli. Il a été confirmé que ce patient avait été exposé à des porcs malades. Le gène de l’hémagglutinine du virus isolé chez ce cas, A/Ontario/RV3236/2016, était semblable à celui des virus A(H3N2) du groupe IV-B qui circulent actuellement dans les populations porcines des États Unis d’Amérique et du Canada.
