DIARRHOEAL DISEASES CONTROL PROGRAMME Technical Advisory Group T he ninth meeting of the Technical Advisory Group (TAG) of the Diarrhoeai Diseases Control (CDD) Programme was held in Geneva on 14-18 March 1988.1 T he Group reviewed the activ­ ities of the Programme during 1987, its revised budget for the 1988-1989 biennium and a preliminary budget for 1990-1991. In addition, it examined in detail the status of research on diarrhoea case management and the CDD Medium-Term Programme for the period 1990-1995. The major conclusions and recommenda­ tions of the TAG are summarized below. Services component The TAG commended the progress made m the past biennium in efforts to reduce mortality associated with acute watery diar­ rhoea, and noted with satisfaction the Programme’s intention to increase activities related to the reduction of diarrhoea incidence. It endorsed the overall policy of directing support primarily to strengthening the capabilities of national CDD programmes and expressed the view that coordination of donor inputs by a strong national programme is the best means of assuring a consistent approach to diarrhoeai disease control by all parues concerned. The Group urged the Programme to explore ways of strength­ ening support to national CDD activities by regional, subregional and particularly country WHO offices, and to carry out a general review of staff support for country programmes. It also endorsed the steps taken towards assuring closer collaboration between the CDD and Acute Respiratory Infections (ARI) Control Pro­ grammes and expressed its belief that this would benefit both programmes. • Programme implementation: T he TAG urged the Pro­ gramme to continue to collaborate with countries in sound pro­ gramme planning, including the establishment of national poli­ cies on case management. Noting that while problem identifica­ tion in country programmes has been achieved, the analysis of problems and action to overcome them have been insufficient, it urged that high priority be given to this activity. Inadequate supervision is frequently identified as a problem in programme implementation and the Programme should find ways to ensure that supervisory skills acquired through CDD training are ap­ plied in regular, systematic activities. PROGRAMME DE LUTTE CONTRE LES MALADIES DIARRHÉIQUES Groupe consultatif technique La neuvième réunion du Groupe consultatif technique (TAG) du programme de lune contre les maladies diarrhéiques (LMD) s’est tenue à Genève du 14 au 18 mars 1988.1Le Groupe a examiné les acuvités du programme en 1987, son budget révisé pour l’exercice biennal 19881989 et un budget préliminaire pour 1990-1991. En outre, il a examiné en détail l’état de la recherche sur la prise en charge des cas de diarrhée et le programme LMD à moyen terme pour la période 1990-1995. Le Groupe a insisté pour que le programme se préoccupe de la façon dont pourrait être renforcé le soutien apporté aux activités LMD nationales par les bureaux de l’OMS aux niveaux régional et sous-régional et surtout à celui des pays, et pour qu’il entreprenne une révision générale de l’appui en personnel fourni aux programmes nationaux. Il a également approuvé les mesures prises afin d’assurer une collaboration plus étroite entre le programme LMD et celui des infections respiratoires aigues étant convaincu qu’elle serait profitable aux 2 programmes. • Mise en œuvre des programmes : Le TAG a instamment demandé au programme de continuer à collaborer avec les pays à la planification rationnelle des programmes, et notamment la mise sur pied de politiques nationales concernant la prise en charge des cas. Notant que si les problèmes dans les programmes de pays ont été identifiés, les efforts entrepris pour les analyser et les résoudre ont été insuffisants et ie TAG a insisté pour qu’une priorité élevée soit accordée à cette activité. L ’en­ cadrement insuffisant est une cause fréquemment incriminée de mau­ vaise exécution d’un programme et le programme LMD doit trouver les moyens d’assurer que les techniques de supervision qu’il enseigne soient mises en application dans des activités régulières et systématiques. belief that the development of a national CDD programme and of the health infrastructure should be mutually reinforcing, whereas focussing on short-term gains through campaigns and similar approaches can be counter-productive and detrimental to the development of both, • Training : The TAG .expressed ns satisfaction at the com­ pletion of the revised Programme Managers’ and Supervisor Skills Training Courses and urged their wide use as à major programme activity. Concerned, however, at the low coverage so far achieved in both supervisory and case-management training, it urged the Programme to collaborate with countries to define target populations for training, to set targets and to plan for their achievement, and also to seek alternatives to supplement fixed training courses that would meet individual country needs. The Programme should more clearly define what it considers to be adequate training and undertake an assessment of national train­ ing activities being implemented with locally developed materials and methods. T he quality and extent of activities of Diarrhoea Training Units should be monitored in order to further strengthen their capabilities, and methods should be sought that would help programmes to evaluate their training activities in general. Stressing the importance of obtaining the support of influential, university-based clinicians for national programmes, the TAG suggested that this could be stimulated by involving them in the preparation of teaching materials and in actual train­ ing activities in Diarrhoea Training Units. T he Programme should also encourage the review and, where necessary, the modification of widely used, influential medical textbooks in order to further promote the institutionalization of appropriate diarrhoea case management.

TAG a estimé que le développement d’un programme LMD national et celui de l’infrastructure sanitaire devraient se renforcer mutuellement, tandis que chercher à obtenir des résultats rapides, par le truchement de campagnes et de méthodes similaires risque d’aller à l’encontre du but recherché et de nuire au développement de l’un et de l’autre. • Formation: Le TAG s’est félicité que soit achevée la révision des cours de formation pour directeurs de programmes et des cours de formation aux techniques de supervision et a insisté pour que ces cours soient largement utilisés et deviennent une des grandes activités du programme. Mais, préoccupé par la faible couverture de la formation aux techniques de supervision et à la prise en charge des cas, le TAG a insisté pour que le programme collabore avec les pays afin de déterminer des populations cibles pour les activités de formation, de fixer des objectifs et d’établir des plans pour leur réalisation, et afin de rechercher des solutions de rechange adaptées aux besoins particuliers des pays et destinées à compléter les cours de formation réguliers. Le programme doit définir plus clairement ce qu’il considère comme une formation appropriée et entreprendre une évaluation des activités nationales de formation mises en œuvre avec des méthodes et des matériels élaborés sur place. Il faudra surveiller la qualité et l’ampleur des activités des unités de formation au traitement de la diarrhée afin de renforcer leurs capacités et mettre au point des méthodes pour aider les programmes à évaluer leurs activités de formation en général. Considérant qu’il est important pour les programmes nationaux d’être soutenus par des per­ sonnalités du monde médical enseignant à l’université, le TAG a émis l’avis qu’on pourrait stimuler l’intérêt de ces personnalités en les asso­ ciant à la préparation des matériels didactiques et aux activités de for­ mation qui se déroulent dans les unités de formation au traitement de la diarrhée. Le programme doit également encourager l’examen, et le cas échéant, la révision des manuels médicaux les plus connus et les plus utilisés afin de contribuer à officialiser les méthodes appropnées de prise en charge des cas de diarrhée. • Frise en charge des cas: Le TAG a instamment invité le pro­ gramme à poursuivre ses efforts pour accroître l’utilisation tant des SRO que des aliments liquides de fabrication domestique recommandés et s’est félicité des efforts pour assurer l’efficacité de la prise en charge des cas dans les services de santé et à domicile. Il a instamment invité le programme à diffuser largement les informations nécessaires pour aider les autorités nationales à restreindre la fabrication, la publicité et la distribution de médicaments inappropriés et à continuer d’enseigner au personnel sanitaire quelles sont les indications, peu nombreuses, où les antibiotiques peuvent être utilisés à bon escient pour le traitement de la maladie. • Communication: Le TAG s’est déclaré préoccupé par les consé­ quences malencontreuses des campagnes menées uniquement dans les médias pour mieux informer les familles et les amener à améliorer leur comportement; il a insisté pour que le programme s’efforce davantage d’obtenir la coopération d’autres organismes afin d’établir un système optimal et intégré de communication. Les activités de communication doivent être soigneusement planifiées, complètes et durables ; elles doi­ vent être incorporées à l’ensemble du programme LMD et étroitement coordonnées avec les autres composantes du programme, par exemple la formation et l’approvisionnement en SRO. Les écoles peuvent eue un important canal de communication et les programmes nationaux LMD doivent veiller à ce que tes matériels utilisés pour l’éducation sanitaire à l’école soient vraiment appropriés et collaborer à leur préparation.

• Case management: T he TAG urged the Programme to con­ tinue its efforts to increase the use of both ORS and recom­ mended home fluids, and commended its efforts to assure effec­ tive case management in both health facilities and the home. It urged the Programme to make widely available the information necessary to help national authorities limit the manufacture, advertising and distribution of inappropriate drugs, and to con­ tinue to train health staff in the correct, limited indications for antibiotic use in the treatment of diarrhoea.

• Communications: The TAG expressed concern about the adverse effects that have resulted from the use of mass media campaigns alone as a means of improving the understanding and practices of families, and urged the Programme to increase its efforts to obtain the cooperation of other agencies in achieving an optimal, integrated approach to communication. Communkauons activities should be carefully planned, comprehensive and sustainable; they should be incorporated into the overall CDD programme and closely coordinated with other programme com­ ponents such as training and ORS supply. Schools are considered to be a potentially important channel of communication and national CDD programmes should try to assure the appropriate­ ness of, and help prepare materials for, health education in schools. • Monitoring and evaluation: The TAG again noted the dif­ ficulties faced by the Programme in obtaining good quality data for programme planning and evaluation, and recognized that it would take time to achieve improvements in this area. It con­ sidered that country programme profiles, if better used, could stimulate an awareness of management information needs and facilitate information exchange at all levels. Data collection should be emphasized during different programme activities. The TAG noted and endorsed the efforts made to develop sen­ tinel surveillance systems, an alternative approach to the meas­ urement of childhood mortality, guidelines for assessing the qual­ ity of diarrhoea case management in health facilities and the home, guidelines for cost analysis in CDD programmes, and a method for measuring the prevalence of dehydration through a community-based survey. The Group expressed the view that the

purpose and expected outcome of comprehensive reviews of CDD programmes should be clearly defined in advance and the reviews planned accordingly. In most cases they should focus on a limited number o f issues and devote a maximum effort to the analysis of, and plans to solve, specific problems of importance. The TAG also noted that better follow-up of programme reviews is still required.

Composante «recherche» Le TAG a examiné la version récemment révisée du document «Priorités de recherche biomédicale et épidémiologique des groupes de travail scientifiques mondiaux» et a demandé que les recommandations concernant les priorités futures soient prises en considération lors de la prochaine révision du document. • Immunologie, microbiologie et élaboration de vaccins: Le TAG a réaffirmé que le programme devait poursuivre la mise au point de vac­ cins contre la diarrhée à rotavirus, le choléra, la fièvre typhoïde, la shigellose et la diarrhée à Escherichia coli entérotoxigène ; il a souligné le rôle important que pouvait jouer le programme dans ce domaine en encourageant des recherches sur les vaccins intéressant les maladies particulièrement importantes pour les pays en développement, en orga­ nisant des essais dans ces pays et en veillant à ce que ces essais soient bien conçus et orientés vers les besoins prioritaires des services de santé. Le Groupe a recommandé que les essais des vaccins soient conçus, chaque fois que possible, de façon à évaluer dans quelle mesure le vaccin est capable de prévenir l’infection et la maladie, mais aussi, d’en atténuer la gravité — ce qui peut être particulièrement important pour réduire la mortalité. . . . Le T AG a noté avec satisfaction qu’un vaccin buccal vivant qui induit des réponses en anticorps contre chacun des 4 sérotypes majeurs pourrait bientôt être éprouvé sur le terrain; dans l'optique d’une éventuelle intégration d’un vaccin antirotavirus dans les activités du PEV, il est très important d’étudier la possibilité d’une interférence avec le vaccin anti­ poliomyélitique buccal. Le Groupe a demandé qu’un rapport lui soit soumis à sa prochaine réunion sur les derniers résultats obtenus après 3 ans de suivi des essais de terrain menés au Bangladesh sur les vaccins anticholériques oraux contenant soit la sous-unité B et des germes entiers tués, soit uniquement des germes entiers. Il s’est déclaré déçu devant les résultats d’un essai en Indonésie montrant que m la formule liquide du vaccin antityphoidique buccal vivant Ty21a ni le même vaccin enrobé dans les gélules gastro-résistantes n’ont donné leniveau de protection précédemment observé lors des essais exécutés au Chili. Ayant examiné les résultats plus prometteurs de 2 essais de vaccin anti­ typhoidique polyosidique Vi injectable, le TAG a reconnu l’importance potentielle de ce vaccin; il a suggéré qu’on continue les essais en y incluant des enfants de moins de 5 ans dans une région à forte incidence typhoïdique et que l’on entreprenne des recherches pour accroître l’im­ munogénicité du vaccin dans ce groupe d’âge. Le TAG a instamment demandé que soient poursuivis les efforts pour mettre au point des vaccins oraux contre Shigella (en particulier S. flexneri et S. dysenteriae), notamment en entreprenant sur des volontaires des essais destinés à déterminer l’innocuité, l'immunogénicité et l’efficacité des vaccins; si les résultats de ces études se montrent encourageants, des essais prati­ ques devraient également être entrepris. Le TAG a aussi instamment demandé que des efforts soient faits pour intensifier la recherche sur la mise au point de vaccins contre £ coli entérotoxigène. - • Epidémiologie et prévention de la maladie: LeTA G a approuvé les efforts faitsparle groupe de travail scientifique (GTS) qui se préoccupe principalement de déterminer l’impact des interventions, surtout celles qui concernent les pratiques d’alimentation infantile et d’hygiène per­ sonnelle et domestique susceptibles de réduire la morbidité diarrhéique. Il a reconnu qu’un effort intensif de la pan du Secrétariat serait néces­ saire pour que ces recherches puissent se développer comme il convient et aboutir à des résultats positifs et il a recommandé instamment que le GTS continue à collaborer étroitement dans ce domaine avec la com­ posante «services». Le TAG a demandé qu’un plan de soutien des recherches sur la mise en oeuvre (c’est-à-dire des recherches visant à préciser la façon dont une intervention peut être utilisée à une échelle praticable et avec un bon rapport coût-efficacité) lui soit soumis à sa prochaine réunion. • Prise en charge des cas: Le TAG a aprouvé les efforts qui se poursuivaient en vue de mettre au point une formule de SRO de prix abordable et facilement conditionnable, capable de réduire notablement

Research component T he TAG reviewed the recently revised document “Biomedi­ cal and epidemiological research priorities of global Scientific Working Groups” and requested that its recommendations on future priorities be taken into consideration when the document is next revised. • Immunology, microbiology and vaccine development: The T A G reaffirmed that the Programme should pursue the devel­ opment of vaccines against rotavirus diarrhoea; cholera, typhoid fever, shigellosis and enterotoxigenic Escherichia coli diarrhoea, and emphasized the role it could play in encouraging research on vaccines of particular importance to developing countries, in organizing trials in such countries, and in ensuring that trials are adequately designed and address priority health service needs. T he Group recommended that vaccine trials be designed, wher­ ever possible, to assess the vaccine’s capacity both to prevent infection and illness, and to reduce severity ; the latter effect may be parucularly important from the point of view of impact on mortality. T he TAG noted with satisfaction that a live oral rotavirus vaccine that induces antibody responses to each of the 4 major serotypes will soon be ready for field testing; with a view to the eventual incorporation of rotavirus vaccine in the EPI schedule, it would be particularly important to examine the possibility of interference with oral polio vaccine. The Group requested an updated report at its next meeting on the results obtained after 3 years of follow up in the field trial in Bangladesh of the oral cholera vaccines containing B-subunit and killed whole cells or whole cells alone. It expressed its disappointment with the results of a trial in Indonesia indicating that neither a liquid formulation of the Ty21a live oral typhoid vaccine nor the same vaccine in enteric-coated capsules afforded the level of protection previously observed in field trials in Chile. Having reviewed more promising results from 2 field trials of parenteral Vi polysaccharide typhoid vaccine, the TAG recognized the potential importance of this vaccine, suggested that further trials include children under 5 years of age in an area of high typhoid incidence, and en­ couraged research to increase the immunogenicity of the vaccine in this age group. It urged continued support for efforts to develop oral Shigella vaccines, especially for disease caused by 5. flexneri and 5. dysenteriae, including trials in volunteers to determine vaccine safety, immunogenicity and efficacy; if the results of such studies are encouraging, field trials should be undertaken. The TAG also urged thaï efforts be made to expand research on the development of vaccines for enterotoxigenic E. coli.

• Epidemiology and disease prevention: T he TAG endorsed the effort made by the Scientific Working Group (SWG) to focus primarily on research designed to test the impact of interventions, especially those concerned with infant-feeding practices and per­ sonal and domestic hygiene, aimed at reducing diarrhoea mor­ bidity. It recognized that an intensive effort by the Secretariat will be required for the successful development and performance of such research and urged continued close collaboration between the SWG and the services component in this area. It requested that a plan for the support of implementation research (i.e., research that seeks to define how an intervention can be used on a practical scale in a cost-effective manner) be submitted for review at its next meeting. • Case management; The TAG endorsed the continued ef­ forts to develop an affordable, packageable ORS formulauon that can substantially reduce the duration and volume of diarrhoea. It

was pleased to learn that in its initial trial an ORS solution con­ taining L-alanine reduced considerably the stool output in cholera patients, and expressed the view that an effect of this magnitude is likely to be required to justify a change in ORS formulation. The TAG noted with satisfaction that the beneficial effects of feeding in limiting the nutritional loss during acute diarrhoea have now been firmly established, but urged that research on optimal dietary regimens based on locally available staple foods continue. Research to identify effective and practical anti-diarrhoeal substances has failed to yield agents of practical value and should receive lower priority than heretofore. How­ ever, studies should continue on cholestyramine and on other drugs for which there is evidence of safety and efficacy from studies in animal models or in vitra Greater emphasis should be placed on research to define an improved approach to the treat­ ment of dysentery, especially that caused by Shigella, and guide­ lines for early treatment of dysentery by peripheral health work­ ers need to be developed, possibly after community-based studies. The TAG looked forward to an increase in research on persistent diarrhoea, particularly on such topics as the microbial etiology of the syndrome, the role of antibiotics in its treatment, and optimal dietary management including the role of various micronutnents.

Les recherches menées pour identifier des substances antidiarrhéiques effi­ caces et pratiques n ’ont pas réussi à mettre en évidence des agents vraiment utilisables et n’ont plus à être traités de façon aussi prioritaire. On devrait toutefois poursuivre les travaux sur la cholestyramine et sur d’autres médicaments dont des études réalisées sur des modèles animaux ou in vitro ont démontré l’innocuité et l’efficacité. Il convient de mettre davantage l’accent sur les recherches visant à améliorer le traitement de la dysenterie, notamment celle qui est provoquée par Shigella, et des directives doivent être élaborées concernant le traitement précoce de cette maladie par les agents de santé périphériques, ce qui pourrait nécessiter des études à base communautaire. Le TAG a exprimé l’espoir que les recherches concernant la diarrhée persistante s’intensifient sur­ tout sur des sujets tels que l’étiologie microbienne du syndrome, le rôle des antibiotiques dans son traitement et la prise en charge diététique optimale, notamment en ce qui concerne le rôle des divers micronutri­ ments. • Recherche sur la corrélation entre les maladies diarrhéiques et l’infection par le viras de rinununodéfidence humaine (VIH) : Le TAG a noté qu’une étude prospective a été entreprise dans une région où l’infection à VIH est endémique pour déterminer si cette infection risque d’accroître la gravité ou la durée des atteintes diarrhéiques chez les jeunes enfants. Reconnaissant que l’infection à VIH peut influer sur la réponse de l’hôte à la vaccination et que des réactions secondaires sérieuses peuvent se manifester chez les sujets infectés par le VIH à la suite de l’administration de certains vaccins vivants, par exemple des vacdns antirotavirus vivants, le TAG a déconseillé d’entreprendre l’es­ sai de tels vaccins dans les régions où l’infection à VIH est endémique, tant que l’on ne disposera pas d’un vaccin expérimental de sécurité et d’efficadté bien établies. • Renforcement de la recherche: Le TAG a noté que certaines contraintes, qui gênent la conduite d’une recherche de qualité dans les pays en développement, ont un caractère récurrent, en particulier le manque de personnel possédant les qualifications nécessaires en matière d’épidémiologie, de recherche comportementale, de communication et de traitement et d’analyse des données. Afin de déterminer comment les ressources que le programme entend consacrer au renforcement de la recherche pourraient être utilisées au mieux pour aborder ces questions, le TAG a prié le Secrétariat de préparer, pour qu’il l’examine à sa prochaine réunion, un résumé des activités de renforcement de la recherche entreprises au cours des 3 années écoulées, avec u n exposé des résultats qui semblent avoir été obtenus, des dépenses encourues et des propositions concernant le meilleur usage qui pourrait être fait des ressources du programme dans ce domaine. • Gestion du programme et questions financières: Le TAG a fait siennes les orientations du programme telles qu’elles ont été esquissées dans le programme à moyen terme pour 1990-1995 et a recommandé que les cibles fixées pour 1995 soient réexaminées lors des réunions futures. Il a approuvé u n budget révisé de US$ 23,8 millions pour 1988-1989, ce qui représente une augmentation de 4% par rapport au budget initialement approuvé en 1987, et a pris note avec satisfaction de la situation financière actuelle du programme.

• Research on diarrhoeal diseases and human immunodefi­ ciency virus (HIV) infection: T he TAG noted that a prospective study of HIV infection as a risk factor for increased severity or duration of diarrhoea in young children is being developed in an HIV-endemic area. Recognizing that HIV infecuon may affect the host response to vaccination and that serious side effects might follow the immunization of HIV-infected persons with certain live vaccines, e.g., live rotavirus vaccine, it considers it inadvisable to undertake trials of such vaccines in HIV-endemic areas until a candidate vaccine of proven safety and efficacy is available.

• Research strengthening: The TAG noted that certain con­ straints in conducting quality research in developing countries are recurrent, including a lack of personnel with the necessary skills in epidemiology, behavioural research, communications, and data management and analysis. To enable it to consider how Programme resources for research strengthening might best be used to address these issues, it requested the Secretariat to provide for review at its next meeting a summary of research strength­ ening acnvities undertaken during the past 3 years, describing their cost and apparent achievements, and offering proposals for a better use of programme resources in this area.

• Programme management and finance: The TAG endorsed the directions outlined for the Programme in the Medium-Term Programme for 1990-1995 and recommended that the targets for 1995 be reassessed at future meetings. It approved a revised 19881989 budget of US$ 23.8 million, which represents a 4% increase over the original budget approved in 1987, and noted with satis­ faction the current financial situation of the Programme.
