GRIPPE

Canada (22 January 1978). — 1 In several provinces, an increase in the incidence of cases of mfiuenza-like illness has been reported. The highest influenza activity has been observed in the Toronto area and, to a lesser extent, in other areas of Ontario. During the past week, strains of virus A have been isolated in Ontario (41 strains), British Colombia, Alberta, Saskatchewan, Manitoba and Quebec. The strains identified were similar to A/Texas/1/77 (H3N2) in Ontario (40 strains identified), British Colombia and Alberta, whereas in Quebec they were similar to A/Victoria/3/75 (H3N2). E g y p t (29 January 1978). — Since 9 January, an increase in the incidence o f influenza-like illness has been observed in Cairo and several regions of the country. Three strains of an influenza virus A similar to A/Victoria/3/75 (H3N2) have been isolated.

Berlin (West). — An influenza outbreak with a high attack rate has been observed in a group o f apprentices. The outbreak began on 27 January and reached a peak from 30 January to 6 February. Three strains of influenza virus A similar to A/USSR/90/77 (H1N1) have been isolated. Germany, F ederal Republic of (13 February 1978). — 2 There has been a continued slight increase m the instance of influenza-like respiratory diseases, particularly among children and adolescents. Isolates of both influenza virus A (H1N1) and A (H3N2) have been identified. H ungary (3 February 1978). — 3 Influenza has now reached an epidemic level except in the western region of the country. During the past week, the morbidity rate from mfluenza-like illness for the whole of Hungary was 2.1 %. The age groups between 1 and 24 years were particularly affected, but the disease also occurred in other age groups. During the week, 25 strains of virus A have been isolated; 20 are related to A/USSR/90/77 (H1N1) and five to A/Victoria/3/75 (H3N2). N e t h e r l a n d s . — 1 During the week ended 28 January, a consider­ able increase in the incidence of influenza-like illness was observed all over the country. Twelve strains of influenza viruses similar to A/Texas/1/77 (H3N2) or to A/Victoria/3/75 (H3N2) were isolated.

H ongrie (3 février 1978). — 3 La grippe a maintenant atteint un niveau épidémique excepté dans la région occidentale du pays. Pakistan (8 February 1978). — sFive strains of influenza virus A close to A/Texas/1/77 (H3N2) have been isolated at the National Health Laboratory of Islamabad. S w e d e n (5 February 1978). — 4 The incidence of influenza-like illness shows a moderate but significant increase in Stockholm, Gothenburg and Malmo. Absenteeism figures for the Stockholm region attained the epidemic threshhold during the fifth week of 1978. Two further strains of influenza sub-type A (H3N2) have been isolated. U n it e d K in g d o m (10 February 1978). — 6 Although there is so far little evidence of influenza in the general population in England, outbreaks among children and young adults in schools and institu­ tions have become widespread throughout the country since the beginning of January and continue to be reported in considerable numbers. There is no evidence yet of a decline in the incidence. but the disease everywhere is described as mild with few complica­ tions and no deaths. A few outbreaks caused by A/Texas/1/77 (H3N2) have occurred in geriatric units in which deaths have resulted. The viruses isolated have been A/USSR/90/77 and A/Texas/1/77 in about equal numbers. A/Victoria/1/75 is also circulating but to a lesser extent. In Scotland, returns from spotter general practitioners in the west, north-west and north-east up to 28 January 1978, showed only a small rise in the rate of influenza-like illness. Two strains of influenza virus A were isolated from sporadic cases in Glasgow; one was similar to A/Texas/1/77 (H3N2) and the other to A/Victoria/ 3/75 (H3N2). PRELIMINARY TESTS OF INFLUENZA VACCINES A (H1NI) N orway. — Twenty-two individuals, from 20 to 65 years of age, were given one dose o f inactivated whole virus vaccine containing A/USSR/92/77 (H1N1) antigen, 21ug HA per dose.* Pre-vaccina­ tion blood was drawn immediately before administering the vaccine and post-vaccination blood was taken 14 days later. From the results presented in Table 1, it can be seen that there was a satisfactory response in those aged 23-31 years, presumably persons primed during the 1947-1957 prevalence of H1N1 virus. In contrast, and in agreement with the data from the United King­ dom, 1 the antibody response in those under 23 years of age was poor and these unprimed individuals may require a higher initial dose or repeated injections in order to give a satisfactory response. In addition, for those over 52 years of age, assumed to be previously primed with H1N1 antigens, the post-vaccination titres were o f low order of magnitude.

ESSAIS PRÉLIMINAIRES DE VACCINS ANTIGRIPPAUX A (H1N1) N o r v è g e . — On a administré à 22 personnes, âgées de 20 à 65 ans, une dose d ’un vaccin viral entier inactivé contenant l’antigène A/USSR/92/77 (H1N1) à raison de 21 «g HA par dose.* Un prélè­ vement de sang a été pratiqué immédiatement avant la vaccination (Pre Vacc.), un autre 14 jours plus tard (Post Vacc.). II ressort des résultats résumés au Tableau 1 que la réponse en anticorps a été satisfaisante chez les personnes âgées de 23 à 31 ans, lesquelles avaient probablement été en contact avec des antigènes H1N1 entre 1947 et 1957, période de prévalence de ces antigènes. Par contre, — et cela concorde avec les données venant du RoyaumeUni 1 — la réponse a été médiocre chez les sujets de moins de 23 ans qui, n ’ayant pas eu de contact antérieur avec ces antigènes, ont peut-être besoin d ’une dose initiale plus forte ou d ’injections répétées pour présenter une réponse satisfaisante. En outre, chez les personnes de plus de 52 ans, censées avoir été précédemment en contact avec des antigènes H1N1, les titres post-vaccinaux ont été peu élevés. 1Voir N» 5, p. 35.
