SMALLPOX SURVEILLANCE United K ingdom.— The patient with smallpox previously reported now has some renal involvement and her condition is giving cause for concern. She is still in the Catherine de Barnes Smallpox Iso­ lation Hospital in Birmingham where she has been since 24 August 1978. Health authorities have identified 290 individuals having had possible contact with the patient during her illness. Of these, 48 are recorded as close contacts. Contact occurred at home, at the East Birmingham Hospital (where the patient was previously hospitalized on 24 August), or at the University of Birmingham Medical School. These persons have been vaccinated and placed under daily medical surveillance at home. Active surveillance will continue until 16 days have elapsed since last contact with the patient or with a possibly infected area. No secondary cases have occurred. Four persons under surveillance were hospitalized with different illnesses. In none of them has the diagnosis of smallpox been made. Three have been discharged from hospital; the fourth person under surveillance was the 77-year-old father of the patient who developed nausea and a temperature of 37.6 °C on 1 September. No rash developed and the patient’s condition improved. On 5 September he died suddenly but it is thought to have been due to a coincident condition and not to smallpox. Vaccination has been restricted to ail persons who may have come in contact with the patient, to individuals in touch with these contacts and to selected members of the community. A check of absenteeism records at the University of Birmingham Medical School, for the period dating back to 1 June 1978, has not revealed other possible cases. Investigations indicate that only the one case of smallpox has occurred and that this originated in a laboratory known to retain variola virus.

Aucune éruption n ’est apparue et l’état du patient s’est amélioré. Le 5 sep­ tembre, il est décédé brusquement mais on croit qu’il s’agit d ’une maladie concomitante et non de la variole. On n ’a vacciné que toutes les personnes pouvant avoir été en contact avec la malade, les gens en relation avec ces personnes et des membres choisis de la collectivité. Une vérification des absences à l’Ecole de Médecine de l’Uni­ versité de Birmingham depuis le 1er juin 1978 n ’a pas révélé d ’autres cas possibles. L ’enquête donne à penser que ce cas de variole est unique et que la malade a été infectée dans un laboratoire que l’on sait conserver du virus variolique.
