DENGUE HAEMORRHAGIC FEVER IN THE SOUTH-EAST ASIA AND WESTERN PACIFIC REGIONS LA FIÈVRE HÉMORRAGIQUE DENGUE DANS LES RÉGIONS DE L’ASIE DU SUD-EST ET DU PACIFIQUE OCCIDENTAL During the last two decades epidemics of dengue haemorrhagic fever (DHF) have been occurring and spreading in the South-East Asia and Western Pacific Regions o f the World Health Organiza­ tion. All four known types o f dengue virus have been isolated in almost all the countries involved and Aedes aegypti has been iden­ tified as the main vector species o f dengue viruses in areas where haemorrhagic fever has been reported. The epidemiological situation was reviewed at an Inter-Regional Seminar on Mosquito-Borne Haemorrhagic Fevers in the SouthEast Asia and Western Pacific Regions held under the auspices of WHO in Bangkok in October 19641 and then in October 1969 at the Second Regional Seminar on Virus Diseases: Mosquito-Borne Virus Diseases (Arboviruses) sponsored by the WHO Regional Office for the Western Pacific.2 In recent years the incidence of D H F has been increasing in several countries of the two regions under study. Thus in 19721973 Thailand saw its largest D HF epidemic and Indonesia moved from an endemic to an epidemic D H F status. Also during the same period important outbreaks o f the disease affected Burma, Malaysia, the Philippines, Viet-Nam (Republic of) and Singapore. Des épidémies de fièvre hémorragique dengue (FHD) se pro­ duisent depuis une vingtaine d ’années avec une extension crois­ sante dans les Régions de l’Asie du Sud-Est et du Pacifique occi­ dental de l’Organisation mondiale de la Santé. Les quatre types connus de virus ont été isolés dans la quasi-totalité des pays frap­ pés, et Aedes aegypti s’est révélé être la principale espèce vectrice dans les régions où des cas de fièvre hémorragique ont été déclarés. La situation épidémiologique a été examinée par un séminaire interrégional sur les fièvres hémorragiques transmises par les mous­ tiques dans l’Asie du Sud-Est et le Pacifique occidental, patronné par l’OMS à Bangkok en octobre 1964,1 puis en octobre 1969 par un deuxième séminaire sur les maladies à virus transmises par les moustiques (arbovirus) qui s’est tenu sous les auspices du Bureau régional de l’OMS pour le Pacifique occidental.2 L ’incidence de la FH D s’accroît depuis quelques années dans plusieurs pays des deux Régions considérées. C’est ainsi qu’en 1972-1973 la Thaïlande a connu sa plus vaste épidémie de FH D et qu’en Indonésie le syndrome est passé de l’état endémique à l’état épidémique. Dans la même période, d ’importantes épidémies se sont produites en Birmanie, en Malaisie, aux Philippines, au Viet-Nam (République du) et à Singapour.

Burma The occurrence of the haemorrhagic fever syndrome in epidemic form was detected in Rangoon for the first time in 1970; 1 654 cases of haemorrhagic fever were reported (81 deaths), and the rate per 10Q 000 population was 92.0. The outbreak started in May and lasted to the end of December. In 1971, 685 patients with the haemorrhagic fever syndrome were treated at hospitals in Rangoon, of whom 34 died. Epidemiological features of the outbreak were similar to those of the 1970 epidemic, except that whereas in 1970 the disease was confined to Rangoon, in 1971 it spread to other parts of the country including Bassein and Moulmein. Birmanie La fièvre hémorragique dengue s’est manifestée pour la pre­ mière fois sous la forme épidémique à Rangoon en 1970:1 654 cas (dont 81 mortels), soit 92,0 pour 100 000 habitants. L ’épidémie a duré de mai à fin décembre. En 1971, les hôpitaux de Rangoon ont accueilli 685 patients, dont 34 ont succombé au syndrome. Le tableau épidémiologique a été analogue à celui de 1970, à cette différence près qu’en 1971 la maladie n ’a pas été circonscrite à Rangoon mais s’est étendue à d'autres parties du pays, à Bassein et à Moulmein notamment. The percentage of dengue cases among hospitalized patients with haemorrhagic fever has been increasing over the years, from 6% in 1970 and 19% in 1971, to 26% in 1972 and to 48% in 1973.

Indonesia Until a few years ago, the D H F syndrome was thought not to be present in Indonesia. However, Indonesia was a likely site for the occurrence of the disease, since the vector A. aegypti is found there and antibodies against all four serotypes of dengue virus were shown to be present in a high percentage of schoolchildren sur­ veyed early in 1969. No confirmed cases of D HF were reported from Indonesia until 1969. In this year, 198 cases with 40 deaths were notified while in 1970 the number of cases and deaths reported increased to 461 and 91, respectively. In 1971 there was a decrease both in the numbers of cases (268) and of deaths (40). Haemorrhagic fever has been repeatedly endemic in the larger cities on the island of Java, such as Jakarta, Yogyakarta, Semarang and Surabaya. Beginning in November 1972, Indonesia experienced its first large D H F outbreak, the centre of which was in cities and towns in Central Java. Previous centres of endemicity were again involved and for the first time the disease was reported in Bali, Sulawesi and Sumatra. Up to July 1973 the number o f cases reported was highest in Semarang (2 516), followed by Surabaya (1 702) and then Surakarta (1 457). The mortality rates, which during the early years had been rather high,—frequently 25% or more — were somewhat lower varying between 2% and 10%. In 1972 a total of 1400 D H F cases with 134 deaths were reported while in 1973, 10274 cases with 451 deaths were notified.

Indonésie L ’Indonésie se croyait il y a quelques années encore exempte du syndrome, et ce bien que le vecteur A. aegypti y fût présent et que des anticorps contre les quatre sérotypes de virus de la dengue y eussent été décelés chez un pourcentage élevé d ’écoliers prospectés au début de 1969. Or, si aucun cas confirmé de FH D n ’a effectivement été signalé jusqu’en 1969, il s’est produit cette année-là 198 cas dont 40 mortels, chiffres qui sont passés en 1970 à 461 et à 91 respectivement. En 1971, toutefois, on a assisté à un recul tant du nombre de cas (268) que du nombre de décès (40). La fièvre hémorragique s’est révélée durablement endémique dans les grandes villes de l’ile de Java telles que Jakarta, Yogyakarta, Semarang et Surabaya. A partir de novembre 1972, l’Indonésie a subi sa première épidémie extensive de FH D , centrée sur diverses agglomérations urbaines du centre de Java. Les centres d ’endémie précédents ont de nouveau été impliqués et la maladie s’est étendue à Bali, Sulawesi et à Sumatra. Les statistiques établies à la date de juillet 1973 ont montré que les points les plus touchés étaient Semarang (2 516 cas), Surabaya (1702 cas) et Surakarta (1457 cas). L a mortalité, assez élevée les années précédentes puisqu’elle attei­ gnait ou dépassait souvent 25%, a notablement baissé, oscillant entre 2% et 10%. Il y a eu au total 1400 cas de FH D dont 134 mortels en 1972 et 10 274 cas et 451 décès en 1973.

Malaysia The first case of laboratory-confirmed DHF occurred in November 1962 in Penang (Georgetown). Dengue haemorrhagic fever has been a notifiable disease in Malaysia since April 1971.1 In 1973, 969 D H F cases with 56 deaths were reported. The largest number of cases occurred during the months of July to September with a peak in July (358). The most seriously affected areas were in the States of Selangor (446 cases with 20 deaths), Johore (216 cases with 33 deaths) and Negri Sembilan (131 cases with one death).

Philippines Since 1954 cases of DHF have almost continuously been occur­ ring in many areas of the Philippines. In several years the endemic level of occurrence has been interrupted by epidemics. The most extensive outbreak occurred in 1966 (9 384 cases). In 1972, 1 570 D HF cases with 83 deaths were reported from the city of Manila. In 1973 incidence decreased: up to November only 413 haemorrhagic fever cases with ten deaths, and eight cases of dengue fever, were reported in the Philippines. Through the years most of the cases have been notified from Manila. However, the disease has also been recognized in epide­ mic form in many cities and towns on several islands in the Philip­ pines.

Philippines Depuis 1954, des cas de FHD n ’ont pour ainsi dire pas cessé de se produire dans de nombreuses régions des Philippines. Au cours de plusieurs années, l’état endémique a fait place à une situation épidémique. L ’épidémie la plus extensive a eu lieu en 1966 (9384 cas). En 1972,1 570 cas de FH D dont 83 mortels ont été déclarés dans la ville de Manille. L ’incidence a régressé en 1973; en effet, B n ’avait été signalé en novembre que 413 cas dont dût mortels de fièvre hémorragique et que huit cas de dengue dans le pays. Si au long des années la maladie a principalement touché Manille, elle a néanmoins donné lieu également à des épidémies dans de nombreuses villes des Philippines.

Viet-Nam ( Republic of) Dengue haemorrhagic fever has probably occurred in Saigon since 1960. However, proper attention was drawn to its presence in the country only in 1963, when numerous cases were notified in two western provinces An-Giang and Kien-Phang. The epidemic affected 331 children, causing 116 deaths. Clinical haemorrhagic fever was also reported from Saigon. It was in 1963 that confirma­ tion of the disease by virus isolation was first obtained. Haemorrhagic fever has continued to occur in Viet-Nam every year during 1963-1968. There was a great increase in incidence in 1969, when 2 813 cases with 87 deaths were reported. In 1970, 242 cases with 38 deaths were recorded, o f which 196 cases with 36 deaths were notified from the Province of Kien-Giang. In 1971, 93 cases with 22 deaths were reported, of which 63 cases with 15 deaths were notified from the same province. In 1972 cases and deaths numbered 1 670 and 219, respectively. In 1973 incidence of the disease increased markedly: 14 320 haemorrhagic fever cases Sec No. 4, p. 33.

Viet-Nam ( République du) La fièvre hémorragique dengue sévit sans doute à Saigon depuis 1960, mais elle n ’a commencé à retenir sérieusement l'attention dans le pays qu’en 1963, année où de nombreux cas ont été déclarés dans deux provinces occidentales, celles d ’An-Giang et de Kien-Pbong. L ’épidémie a frappé 331 enifants, dont 116 ont suc­ combé. L a fièvre hémorragique clinique a également été signalée à Saigon. C’est à 1963 également que remonte la confirmation de la maladie par isolement du virus. La fièvre hémorragique s’est manifestée au Viet-Nam au cours de chacune des années de la période 1963-1968. L ’année 1969 a été marquée par un fort accroissement de l ’incidence: 2 813 cas dont 87 mortels. En 1970, la morbidité était tombée à 242 cas dont 38 mortels, dont 196 cas et 36 décès pour la seule province de Kien-Giang. La régression s’est poursuivie en 1971 avec93 cas dont 22 mortels, dont 63 cas et 15 décès pour la même province. La situa­ tion s’est en revanche aggravée en 1972:1 670 cas dont 219 mortels. Singapore Haemorrhagic fever caused by dengue virus was first recognized in Singapore in 1960, when over 200 cases were reported. The disease was mild and most of the cases occurred in older children and young adults. There were no episodes of shock and no deaths. Since 1962 a severe disease more closely resembling the Thai and Philippine syndromes has been observed with increasing frequency. A high incidence o f haemorrhagic fever was recorded in 1967 and 1968 (826 and 848 cases, respectively) and a marked decrease in 1970 and 1972 (71 and 64 cases, respectively). There was a great increase in 1973 when 1479 cases were observed.

Singapour La fièvre hémorragique provoquée par le virus de la dengue a été décelée pour la première fois à Singapour en 1960 (200 cas). La mala­ die avait un caractère bénin et frappait électivement les enfants âgés et les jeunes adultes. Il n ’y a pas eu à déplorer d ’épisodes de choc ni de décès. A partir de 1962, des formes plus sévères fort voisines des syndromes thaïlandais et philippin ont été observées avec une fréquence croissante. La fièvre hémorragique a accusé une forte incidence en 1967 et en 1968 (826 et 848 cas respectivement) mais a notablement régressé en 1970 et en 1972 (71 et 64 cas respectivement). Il s’est ensuite produit une sérieuse recrudescence en 1973 (1479 cas).

Thailand A major epidemic of DHF occurred in Bangkok in 1958 ; relatively smaller epidemics had been reported in previous years. Since 1958 outbreaks have occurred every year.1 Dengue haemorrhagic fever was first made notifiable in July 1964 for the Bangkok-Thon Buri area, and a weekly notification system has since been extended to cover the entire country. The total number of D H F cases in 1973 (8 288) represents an increase of 8.3% over the average number of cases reported during the three-year period 1969-1971 and a decrease of 65.2% from the 23 831 cases recorded in 1972.2* The total number o f deaths from DHF in 1973 (310) represents a decrease o f 54.3% from the 678 deaths recorded in 1972. The case-fatality rate increased from 19% in 1972 to 3.7% in 1973. While in the previous year all provinces reported cases of DHF, in 1973 two provinces—Phetchaburi and Phuket—did not. The highest number of cases (1 046) were reported from the Province of Bangkok in the central region o f Thailand. Morbidity was also high in Chanthaburi (668 cases) and Khon Kaen (567 cases). The other provinces that were seriously affected in 1973 ( > 400 cases) were Thon Buri, Ubon Ratchathani, Chiang Mai and Nakhon Ratchasima. Udon Thani had the next highest number reported with 280 cases. The largest number of cases (5 692) occurred during JuneSeptember, with a peak in July (1 547 cases). Thaïlande Bangkok a connu en 1958 une violente épidémie de FHD qui avait été précédée d ’épidémies plus restreintes au cours des années antérieures. Des épidémies se sont produites chaque année depuis 1958.1 La fièvre hémorragique dengue a été soumise à déclara­ tion dans la région de Bangkok-Thon Buri en juillet 1964, date depuis laquelle un système de notification hebdomadaire a été étendu à l ’ensemble du pays. Les 8 288 cas de FHD enregistrés en 1973 traduisent une augmen­ tation de 8,3 % par rapport au nombre moyen de cas signalés pen­ dant les trois années comprises entre 1969 et 1971 et une diminu­ tion de 65,2% par rapport aux 23 831 cas consignés en 1972.“ Pour ce qui est de la mortalité par FHD, les 310 décès enregistrés en 1973 représentent une diminution de 54,3% par rapport aux 678 décès relevés en 1972. MENINGOCOCCAL MENINGITIS U nited K ingdom . — The number o f reports of laboratoryconfirmed cases of meningococcal meningitis have been increasing recently in common with notifications of this disease to the Registrar General. In 1967, 388 cases o f meningococcal meningitis were reported by laboratories in the United Kingdom. This increased subsequently until 1970 when 544 cases were reported. Though this number then did not change very much in 1971 and in 1972, it increased substantially in 1973 to 843 cases. So far in 1974 (up to the end of May) 657 cases have been reported compared to 457 at the same time last year. These figures suggest, as do the Registrar General’s, that the national incidence of meningococcal meningitis is rising. Increases in the incidence of meningococcal meningitis have also currently been observed in some countries in Europe, and a recent report from Sâo Paulo in Brazil also suggests that a marked rise in the incidence of meningococcal meningitis has occurred in 1971-1973. An epidemic wave of meningococcal meningitis with an infection rate similar to the present one previously occurred in that city during 1946-1949; and it is possible that infection with the meningococcus is cyclic. The records of laboratory reports for the 12 years before 1967 show that an increase in laboratory reports of meningococcal meningitis occurred in 1955-1956 and again in 1962-1964.

Ce phénomène s’observe actuellement dans quelques autres pays d ’Europe et un rapport récent de Sâo Paulo (Brésil) témoigne d’une progression marquée de la maladie en 1971-1973. Une vague épi­ démique de méningite à méningocoques avec un taux d ’infection semblable à celui qu’on observe actuellement s’était déjà produite dans cette ville en 1946-1949: il est donc possible que l’infection à méningocoques soit cyclique.

Les rapports adressés par les laboratoires pendant les 12 années précédant 1967 montrent que la méningite à méningocoques a été signalée plus fréquemment en 1955-1956 puis, de nouveau, en 1962-1964.

Chile (information dated 31 July 1974). — 1* The epidemic which started at the end of May in the general population of San­ tiago ended in mid-July, after reaching its peak during the week 17-23 June. The extent of the epidemic appears to have been moderate and all age groups were affected. Strains of virus A have been isolated and evidence of infection with virus A was obtained from serological tests of several paired sera. S ingapore (information dated 31 July 1974). — From mid-May to mid-July, an influenza epidemic was observed throughout the Republic of Singapore and, at its height, about 1500 new cases occurred daily. A few fatalities were reported among the aged.
