La deuxième réunion technique (05/12l01) Irl SUMMARY In the framework of the implementation of the onchocerciasis vector elimination project in the Tukuyu focus, a mission was carried out from 15 November 2001 to 03 February 2002. After various courtesy visits to the WHO Representation in Tanzania and to the national structures involved in onchocerciasis control, we began the preparatory activities for the ground larviciding campaign by participating in the national awareness seminar which took place in Mbeya, the capital town of the Tukuyu focus region, from 26 to 28 November 2001. The presentation of Dr Y. Bissan, OCP/APOC expert, on the topic "ControUecological monitoring in West African OCP Countries" on27 November 2001 was a determining factor in the understanding of the activities which were to be carried out in the focus are4 mainly the impact of larviciding on the environment which seemed to be the major concem of the participants. The ground larviciding experiment made on 28 November 2001 finally convinced all those who were still worrying about the destruction of the non target aquatic fauna due to the direct spraÿng of the temephos in the rivers. This seminar at the national level was followed few days later by another one at the district level which brought together the main personalities and notables of the three (3) districts of the focus area. Finally, the main riverside villages of the targeted basins were sensitized by the NIMR technicians. After various technical meetings, 04 teams were established (03 teams for ground spraying, 01 for supervision and larval prospection) and 03 treatment rounds were established. The ground larviciding campaign in the focus effectively began on 13 December 2001 The three basins of the focus (Kiwira, Mbak4 Lufilyo) went through a weekly spraÿng of temephos at a dosing of 1.33 mÿl i.e. 0.4 I per m3/s during 06 weeks. On tlie whole, 881 liters of temephos were dropped in 53 treatment points. Conceming the immediate evaluation of treatments (larval prospection), the results are excellent. Data on adult fly catching before and during sprayings are not reliable. In our opinion, they do not reflect the reality of the dynamics of the simulium population of the focus. The analysis of the results at our disposal does not allow us to determine the in-rpact of treatments made. The isolation of the focus has some drawbacks. ln the South-East, not far froni the River Lumbira, we have some tributaries of the Nyassa lake (two small strear-ns and the River Kilondo). On the Wcst, we have the River Songwe and its tributaries. 1l Inside the focus itself, the total coverage of the breeding sites seems difficutt to achieve (access to a number of tributaries with permanent flow is practically impossibte because of the topography of the terrain). We recommend and suggest a) The search for access to all the tributaries with permanent flow; b) An update of the entomological status before the beginning of the next elimination can'rpaign; c) The treatment of the River Lumbira; d) The recruitment and training of 5 additional technicians; e) Strong actions must be taken for reliable fly catching which will reflect the reality of the dynamics of the imaginal population of the onchocercerciasis vector in the Tukuyu focus. llr Tt SUGGESTIONS and ITECOMMENDATIONS 1. Suggestions 1.1. Technical A- During the dry season, the carry of the temephos is considerably reduced making necessary sometimes to do the spraying site by site something which is practically impossible because of the topography of the area. Furtherrnore, the vector elimination requires treatment of all the vector breeding sites. Therefore, we suggest the search for access: a) At the level KI 600 above Kapeta old bridge to reduce the distance between treatment points on the confluence of Mwalisi/kisyesye-Kapeta old bridge. b) At the level of confluences of all the tributaries with a pernanent flow in the distribution area of the onchocerciasis vector. c) At the level of the right bank tributaries of River Songwe in the focus (one of them was found positive). d) The renewal of treatments before the dry season, mainly on tributaries. B- The results of the six weeks spraying of the temephos are excellent with regard to the pre- irnaginal vector population. The renewal of treatments will be done after some months of suspension. We suggest a general evaluation of the density of the aquatic stages of the vector for the entire hydrographic network of the focus mainly at the upstream of the first treatmept points, with a conservation of the specimen collected in carnoy's and 70 % alcohol. 1.2. Adrninistrative Considering the likely increase of the number of the treatment cycles, the poor condition of two vehicles of the current fleet, we suggest the acquisition of 03 additional vehicles (to be negotiated with the other national onchocerciasis projects and the national coordinator). 2. Recommendations 1- Considering that: - The treatment of the river Lumbira is indispensable for the elimination of the vector in the Tukuyu focus, - Access to its unique treatment point is virtually irnpossible by foot (ntore than 06 hours of walking). If the rehabilitation of the road which goes near the treatment point is not done by the beginning of the control operations, u,c recommend: a) The purchase oI a small dinghy equipped with an outboard engine; lr) The recruitment of an experienced boatmau ou a tenrporary basis, c) Tlie installation of a water gauge. ?- Considering: -Thcuecessityoflarval ;trospcctioustocvaluutc-thc'cf-licic-nc1,ol-s1tra1,i11gl11 .q team indepcndent from the larvicidiug tcanrs; - The likely increase of the number of rivers to be treated ( R Lurnbira), - That this job is physically very tiring because ol the particular geography of the focus, rve rccommend: lv a) 05 teams of two entomological technicians for the ground larviciding campaign of the Tukuyu focus (04 teams for the treatment cycles and one for larval prospections and supervision); b) The recruitment and training of 5 additional technicians. 3- The river discharge is one of the three factors which determine the quantity of larvicide to apply to each reach. In the absence of a water gauge, discharges are rneasured by using floats. Considering that this exercise requires several hours, (2 hours for rnain streams, 30 mn to t hour for tributaries) we recommend the continuation of installation of water gauges on: - The lower kiwira ( Ipyana), - The upper Lufilyo (Lufilyo primary school), - The lower Lufilyo (Tapio bridge or Mabunga primary school), - The lower Lumbira (catholic mission). 4 - The evaluation of the vector elimination efforts in the Tukuyu focus will be made by using the data of the S. damnosum s.1. female catchings. Consequently, we recommend that strong actions be taken to make the results more credible, reliable so that they can reflect the reality of the simulium dynamics of the focus. I I - RESUME Dans le cadre de la mise en oeuvre du projet d'élimination du vecteur de l'onchocercose dans le foyer de Tukuyu, une mission a été entreprise du I5lLIl200l at 03/02/2002. Après les differentes visites de courtoisies à la représentation de I'OMS en Tanzanie et aux structures nationales impliquées dans la lutte contre l'onchocercose ; les activités préparatoires de la campagne des traitements au sol ont débuté par notre participation au sérninaire national de sensibilisation tenue du 26 at 28llll20}l à Mbeya chef lieu de la région du foyer de Tukuyu. La présentation du Dr Y. Bissan, Expert OCP/APOC sur les thèmes: Control / ecological monitoring in West African OCP Countries le 27/lI/2OOl fut déterminante. Elle a aidé à la compréhension des activités qui seront menées dans l'aire du foyer notamment l'impact des traitements sur l'environnement qui semblait être le souci majeur des participants. La démonstration des traitements au sol faite le 28lll/2001à fini par soulager tous ceux qui étaient encore préoccupés par la destruction de la faune aquatique non cible qu'entraînerait f injection directe du téméphos dans les eaux des rivières. Ce séminaire de niveau national fut suivi quelques jours après par un autre de niveau District qui a regroupé les principales personnalités et notables des trois districts de I'aire du foyer. Enfin les principaux villages riverains des bassins visés furent sensibilisés à leur tour par les techniciens de la NIMR. A l'issue de differentes réunions techniques 04 équipes ont été constituées( 03 équipes pour les épandages, 01 pour la supervision et le contrôle larvaire ) et 03 circuits de traitement furent établis. La campagne des traitements au sol du foyer a effectivement débuté le I3/lZ/Z0Ol. Les trois bassins du foyer ( Kiwira, Mbaka,Lufilyo ) ont été soumis aux épandages hebdomadaires de téméphos à la dose de 1.33 mg/l soit 0.4 I par m3/s pendant 06 semainès. Au total 881 litres de téméphos ont été injectés sur 53 points. Sur le plan de l'évaluation directe des traitements (contrôle larvaire) les résultats sont excellents. Les données des captures d'adultes avant et pendant les épandages ne nous inspirent pas confiance. A notre avis elles ne reflètent pas la réalité de la dynamique de la population simulidienne du foyer. L'analyse des résultats en notre possession ne nous permet pas de déceler l'impact des traitements effectués. L'isolement du foyer a quelques failles. Au Sud Est, non loin de la R Lumbira se trouvent des affluents du lac Nyassa (deux petits cours d'eau et la R Kilondo ). A l'Ouest, la R Songwe et ses affluents. A l'intérieur même du foyer, la couverture totale des gîtes semble difficile à réaliser(l'accès à nombre d'affluents à écoulement permanent est pratiquement impossible du fait de la topographie du terrain). Nous recommandons et suggérons: a) la recherche d'accès à tous les affluents à écoulement permanent b) une mise à jour de la situation entomologique avant le début de la prbchaine campagne d'élimination c) le traitement de la R Lumbira d) le recrutement et la formation de 05 techniciens supplémentaires c) que des actions vigoureuses soient prises pour des captures fiables qui refléteront la réalité de la dynamique de la population imaginale du vecteur de l'oncho"..ào." du foyer de Tukuyu. RAPPORT de MISSION 15 I lll 2001 au 04 I 02 12002 Campagne d'élimination du vecteur de l'onchocercose dans le foyer de Tukuyu Sud Est delaTanzanie en Afrique de l'Est 2. Introduction Après les missions d'évaluation de la sensibilité des larves de simulium damnosurn sl dans le foyer de Tukuyu et celle relative à la planification de la campagne d'épandage au sol du térnéphos comme larvicide anti simulidien et à la formation du personnel technique, une nrission du 15/11/01 au 0410212002 a été entreprise pour la mise en æuvre effective des activités visant à l'éradication du vecteur de l'onchocercose du foyer de Tukuyu. 3. Objectifs : - Participer aux differentes activités de sensibilisation, - Coordonner les épandages au sol du larvicide. 4. Première réunion technique (voir rapport de réunion en annexe). Le 22l11l01 s'est tenu une réunion à laquelle a pris part M. Kalinga Akili M Mwaikouyole Adam M.Abillu Peter M Barro Tele OCP Odiemre Elle avaiL pour objectif d'échanger sur 1'organisation pratique de la campagne cl'épanclage du témé1thos ct de la dénronstration du traitetneut au so[ à l'attention des participants au sénriuairc dc sensibilisation clcs autorités natiou:rles daus le cadre dc l'élintiuatron tltr vectcur rlc l'ortchocercosc cltr lbycr ilc'l-ukttytt ctt sttisc dc lanccnrent oftlcicl tlcs i'Plrrttlltr.lcs. t -2- Les sujets ci-après ont été discutés : l) Réajustement des points d'application 2) Les échelles et les mesures des débits des cours d'eau 3) La logistique 4) Le matériel 5) Le personnel 6) Le circuit de traitement 7) Divers 5.1. Réajustement des points d'application Compte tenu du niveau d'eau extrèmement bas cette année, la portée du larvicide pourrait être considérablement réduit, aussi un réajustement de points de dosage s'irnpose.(voir cafte appendice I) 5.2. Les écltelles de crue et mesure de débit : Sur les quatre échelles existantes, seule celle de la prison civile sur la Kwi 100 est fonctionnelle. Il serait souhaitable que les trois autres soient réparées. De nouvelles échelles sont nécessaires notamment sur le moyen Kiwira sur la haute Mbaka le moyen Lufilyo. Ces retarages et les nouvelles installations allégeraient le travail des équipes mais surtout rendraient les débits plus précis et les doses d'insecticide à épandre plus adéquates. Selon un des participants, des contacts intéressants avaient été pris avec le responsable régional de l'hydrologie qui s'était proposé de satisfaire nos requêtes moyennant un perdiem pour son personnel. Un rapport dans ce sens a été adressé au responsable du projet. Nous souhaitons que ces travaux soient réalisés dans les plus bref délais. Nous avons déterminé au niveau de chaque bief et sur l'ensemble du réseau hydrographique les points sur lesquels des mesures de débit devront être effectuées avant chaque épandage. (voir carte appendice II) 5.3. La logistique a) Moyen de transport Le projet dispose 03 véhicules tout terrain (2 Toyota hilux I voirure Suzuki Vitaria ) 05 Motos Honda XL 125S (sur chaque moto fixer un pofte bagage) -3- b) Dépôts insecticides Dépôt central Tukuyu base, 05 dépôts secondaires en plus du dépôt central ont été choisis à savoir : * Isoko Hospital * Lema Dispensary * Ikuti Health Center * Kambasegela Dispensary + Itete Hospital 5.4.La gestion du stock insecticide Les ravitaillements des dépôts secondaires seront faits à partir du dépôt central et les équipes s' y approvisionneront. Pour un suivi des stocks du téméphos nous recommandons L'établissement de fiches de gestion des stocks au niveau central et également à celui des dépôts secondaires. La confection de jauges graduées pour évaluer après chaque prélèvement le niveau de f insecticide dans le fût. Le dépôt central sera sous la responsabilité directe du coordonnateur du projet. Quant aux stocks secondaires, la garde sera assuré par les responsables des structures sanitaires où ils se trouvent. 5.5. Le matériel Une liste de matériel avait déjà été soumise à l'approbation du responsable du projet; à cette liste nous avons ajouté un certain nombre de matériel indispensable. Nous avons également demandé la conception puis la fixation d'un porte bagage sur chacune des motos. Ceci permettra le transport d'une certaine quantité d'insecticide. On pourrait ainsi traiter plusieurs points sans avoir à revenir au dépôt secondaire et réduire la fréquence des ravitaillements. 5.6. Le personnel Au niveau de la national Institute for Medical Research (NIMR) station de Tukuyu, le projet d'élimination du vecteur dispose de 3 techniciens et d'un biologiste formé aux tâclies de lutte contre les vecteurs de l'onchocercose. En plus de ce personnel qualifié. 3 urembres dc l'écluipe de recherche OCP et un technicien de l'équipe nationale Oncho cle Bobo Dioulasso. sc-ioittdroltt à eLrx pour coustitucr les équipes qui auront en charge les éltanclagcs lan,icirlcs. Les circuits Nous avons convenu de la constitution de 3 équipes pour assurer les épandages du réseau hydrographique du foyer. Un circuit provisoire a été établi à l'attention de chaque participant pour réflexion et amendement éventuel.( voir annexe) 5.8. Divers En attendant l'arrivée des techniciens OCP, l'accent sera mis sur les préparatifs de la démonstration du traitement au sol, servant de lancement officiel de la campagne d'élimination du vecteur de l'onchocercose dans le foyer de Tukuyu, dans le cadre du séminaire national de sensibilisation et I'acquisition du matériel complémentaire indispensable. 6.Le séminaire national de sensibilisation <<26 atu28llll200l » La participation de I'OCP a porté sur les sujets ci après Control / ecological monitoring in West African OCP Countries (présenté par Dr Bissan) puis la démonstration du traitement au sol (M BARRO TELE et l'équipe de la NIMR). La préoccupation majeure des participants à ce séminaire était le volet environnemental. Ils furent apaisés par la communication du Dr Bissan. Les travaux en commission adoptèrent l'utilisation du téméphos comme larvicide du foyer. La démonstration de l'épandage au sol du téméphos a été faite le 28/1112002 en présence de tous les participants au séminaire et le District Commissionner de Tukuyu. Une introduction a été faite par le coordonnateur du projet le Dr Maegga, Ies explications techniques par l'expert OCP en la matière puis sous la direction de l'expert, la pratique de l'épandage par les techniciens nationaux. Nous référant aux commentaires de quelques spécialistes en pesticide participant au séminaire, cette démonstration les a convaincus de la faisabilité du projet rnais aussi de I'impact mineure du téméphos sur la faune aquatique non cible eu égard à la quantité du produit par rapport au débit, disent-ils. 7. Accueil de l'équipe des tecltniciens OCP à Dar Es Saluam (03/12/2001) A uotre arrivée à Dar Es Salaarn, (le Dr Bissan et uroi-mêntc) en route pour Tukuyu n'avons pas été accueillis à ['aéroport, fort de cette expérience, llous avorls jtrgé prudent de trous rendre dans la capitale Tanzanierule pour recevoir l'équipe de techniciens OCP. Lcs trois technicicns sont finalement arrir,és lc 03/12l01 Iurent longues et coûteuscs (200 dollars $ par personne). Lcs lonnalités t\ l'aéroport -5- 8. Les visites de courtoisie à Dar Es Salaam (03t12t2001) 8.1. A M. Franzen ONG Après l'aéroport, nous avons été reçus par M Charles Franzen de I'ONG IMA (interchurch medical assistance) auquel la présentation des membres de l'équipe a été faite et l'objectif de notre mission exposé ; à savoir les traitements au sol dans le cadre du projet d'élimination du vecteur de l'onchocercose dans le foyer de Tukuyu. 8.2. Au Représentant de I'OMS (0311212001) En compagnie de M Franzen, toute la délégation a été reçue par le Représentant intérimaire de I'OMS, le Dr Ameri, qui après les présentations d'usage et le traditionnel mot de bienvenu, s'est réjoui particulièrement de la mise en Guvre effective du projet d'élimination du vecteur de I'onchocercose dans le foyer de Tukuyu auquel dit il a participé à l'élaboration des premières propositions du projet. Il a ensuite souhaité un excellent séjour et surtout une bonne mission. J'apprécierai un débriefing à votre retour a-t-il conclu. 9. taprise de contact avec le personnel de l'Institut for Medical Research station de Tukuyu (0511212001) Après la visite de courtoisie rendue au Coordonnateur du projet, chef de l'institut, le Dr B Maegga à leur arrivée, M Bileou Soulemana de la recherche zone Est Kara, M Bakayoko Siaka de la recherche Zone Ouest Odienné et M Alassane Siaka Coulibaly de la base opérationnelle de Bobo ont pris contact avec leur homologues techniciens le 05 décembre: respectivement avec M Kalinga Akili entomologiste du projet, et M Mwaikonyiole Adam, M Peter Abilou et M Addow Kibweja tous techniciens de laboratoire. Ils ont ensuite été présentés aux membres du personnel administratif de la station. Le seul problème à ce niveau fut celui de la communication ; M Bileou s'exprimant quelque peu en anglais, les deux autres techniciens ayant des niveaux dérisoires ou inexistants dans cette langue. Aucun de nous ayant aucune notion de la langue usitée qui est le kisiswahili. 10. La deuxième réunion technique (05/12101) CeLte deuxième rencontre avait pour but : l) Faire le point sur le matéricI existant 2) D'échanger sur le plan de travail établi lors dc la première réunion 3) Dén-rarragc des activités d'épandage -6- 10.1. Le point sur le matériel La liste du matériel établie pendant notre précédente réunion n'a pas été honoréejusqu'à ce jour; il semblerait que la liste et les factures pro format sont en possession du Dr Maegga. Le groupe a prié M Kalinga de recontacter le coordonnateur pour une rapide actiol. 10.2. Les échanges sur le plan de travail établi lors de la première réunion (06/12t200t) Compte tenu des moyens logistiques mis à la disposition des équipes en charge des traitements, du désir exprimé par le coordonnateur du projet relatif à l'utilisation maximum des motos, le circuit précédemment adopté et approuvé par le Dr Maegga a été profondément revu. Les trois équipes établies lors de la première concertation sont maintenues. Il a été convenu que toutes les équipes partent de Tukuyu chaque semaine le dimanche soir pour un circuit de quatre jours par semaine.(circuit en annexe). Le principe retenu : une équipe est composée de deux personnes « un technicien national et un technicien APOC/OCP »>. les équipes gardent la même dénomination mais un roulement des techniciens au niveau de chaque équipe de manière à ce qu'à I'issue du séjour tous les techniciens aient une parfaite connaissance de tous les points d'application. L'équipe A qui assurera les épandages au niveau des affluents rive droite, du cours aval et supérieur de la rivière Kiwira, et compte tenu de la nature du terrain utilisera la plus neuve des deux Toyota Hilux. L'équipe B se servira de la voiture Vitara pour couvrir leur circuit (moyen kiwira et le bassin de la R. Mbaka). Les techniciens en charge du circuit C seront basés à ltete. Ils y seront transportés avec deux motos qui leur serviront de moyen de déplacement pour couvrir tout le circuit. Nous avons également institué un débriefing sur les activités de la semaine précédente tous les lundis ou mardis. Ce plan de travail qui implique quatre nuits et quatre journées entières de traitement nécessite de longues marches à travers un terrain particulièrement accidenté, a fait Ie consensLls du groupe après de longues discussions. Il a été décidé de le porler à la connaissance du Dr B. Maegga, coordonnateur du llrojet alr coul's d'une nouvelle rencontre prévue le lendemain. -7 - ll. Réunion élargie au coordortnateur du projet le Dr B. Maegga (07/12/01) Comme pour la première concertation, un rapport de notre deuxième rencontre technique a été fait au Dr Maegga pour lui permettre de prendre connaissance de l'essence de nos débats dans l'optique que des actions immédiates puissent suivre. Cependant comme nous I'avons fait pour la première fois, nous avons invité le Dr Maegga à échanger sur le contenu du rapport. Au cours de ce meeting, nous avons longuement expliqué au Dr Maegga que, sans le matériel sollicité, les activités ne sauraient démarrer et qu'iI y avait là urgence. Outre cette question, les explications du groupe ont porté singulièrement sur les moyens de transport (chauffeurs, véhicules, et motos). Le nouveau circuit a fait I'objet d'une attention toute particulière. Tous les détails relatifs à chaque circuit ont été exposés. Le coordonnateur du projet nous a promis que dès le lendemain matin, tout sera mis en ceuvre pour l'achat du matériel demandé et qu'elle n'avait aucune objection notamment en ce qui concerne le nouveau circuit proposé. 1 1.1. Le per diem des techniciens nationaux Les indemnités de mission à payer aux techniciens lors des sorties sur le terrain a été le point d'achaupement entre le personnel national de l'équipe et le coordonnateur du projet. Les derniers circuits élaborés ont été présentés au Dr Maegga qui les a approuvés et acceptés. Ils nécessitent chacun 04 jours de mission et nuits à l'extérieur de la station; Madarne Maegga dans un premier temps semble-t-il n'a autorisé que le paiement de 02 nuits sur 4. Ce n'est qu'après de longues discussions que [e Dr Maegga accepta le paiement des quatre nuits. Toutefois, elle opta pour une diminution des taux journaliers. De 15 000 a 11 000 Tsh soit une coupe de 4 000 Tsh parjour. Cette mesure eu pour conséquence une déprime, une démoralisation, voire un début de démotivation des nationaux ; C'est grâce aux instructions reçues du Directeur APOC et transmises au Dr Maegga que ce problème a été solutionné. 12. ACTIVI T ES TE C H NIQU ES 12.1. Le Démarrage des épandages La campagne de traitement au sol visant l'élin-rinatiou de sirnulium damnosum sl du I-oyer dc I'onchocercose de Tukuyu par épandage du ténréphos, a effectivement déltrarré le l2ll2l20)1 par lc clépart cles trois équipes sur le terrain pour couvr-ir les trois circuits. Les épandages Méthodologie Les débits Ils ont été obtenus par la lecture de l'unique échelle de crue pour le haut kiwira et par les mesures débit au flotteur pour le reste des cours d'eau ou simplement estimé sur les plus petits affluents. Le larvicide : Le larvicide utilisé est le téméphos, un organophosphoré à toxicité insignifiante pour la faune non cible. Lurrrbira. -9- I2.3. Les résultats 12.3.L. Les contrôles larvaires Méthodologie Nous avons opté tant que faire se peut, pour une inspection des gîtes avant et après épandage du larvicide au niveau de chaque point d'application. De plus, une équipe indépendante spécialement affectée était chargée de l'évaluation générale des traitements sur une base hebdomadaire. a) Sur les rivières principales Résultats excellents aucun gîte visité n'a été trouvé positif après la première semaine de traitement. b) Sur les affluents Résultats également satisfaisants, nous avons noté une absence totale de nymphe de S damnosum sl au niveau de tous les gîtes inspectés avant et après chaque traitement Cependant quelques points ont été trouvés positifs sur le Lu 300 (kasyabona affluent du Lufilyo) à hedikota à la semaine 04 et sur Mb 300 (affluent du Mbaka) à kiloba de la semaine 2 àla semaine 4. L'absence totale de nymphe indique une excellente efficacité du téméphos. Les larves de simulie (notamment des autres espèces) occasionnellement trouvées pourraient provenir des gîtes en amont des premiers points d'application ou des affluents inaccessibles décrochent arrivent par dérive sur les biefs traités. Ces dérives sont certainement provoquées par de forts courant suite aux pluies par moment importantes en cette saison. I2.3.2. Les captures d'adultes Méthodologie Les captures de S. damnosum pour évaluer la densité de la population simulidienne du foyer sont effectuées depuis plusieurs années. Un certain nombre de points de capture ont été choisis sur les rivières principales. Quatre d'entre eux sont régulièrement suivis. I[ s'agit de : Lwangwa-Masoko sur la R. Kiwira Kambasegela sur la R. Mbaka Lufilyo crater et Tapio bridge sur la R.Lufilyo. La fréquence des captures est bimensuelle (une sortie de deux jours successilb de capture toutes les deux semaines). Le nourbre de jour de capture au niveau de ces points varic e ntre 04 à 06 jours/rnois- t -10- L'équipe de capture est composée de deux personnes travaillant alternativement toutes les demies heures. Les résultats de ces captures sont consignés sur des frches dites de capture. Y f,rgurent le total horaire et le nombre total de femelles capturées dans la joumée. Les captures débutent à 06 heures du matin pour s'achever à l8 heures. L'examen d'un grand nombre de ces fiches de capture nous a conduit au constat que Ia plupart de femelles prises l'ont été que pendant la première heure de la journée ou éventuellement à la deuxièrne. Nonobstant quelques exceptions, aucune mouche n'est prise entre la troisième heure et l8 heures. Ce constat semble indiquer que les captures ne sont faites que sur une infime partie de la journée, (la première ou la deuxième heure). Compte tenu du rÿhme journalier de piqûre de S damnosum connu et des observations faites lors de nos visites aux captureurs sur le terrain, les résultats obtenus nous semblent peu probants, peu fiables et ne permettent pas une bonne étude de la dynamique de la population imaginale du vecteur. (voir en annexe tableaux et fiches de capture). Cependant, l'analyse de ces résultats révèle un pic d'agressivité observé de juillet à septembre (plus de 717 lemelles capturées en deux jours à Tapio bridge sur le bas Lufilyo e1 septembre 2001). Les données en notre possession ne nous permettent pas d'apprécier I'impact des six semaines de traitement efficace du foyer commencé en mi-décembre 2001. Toutefois notons une tendance à la baisse. Les demiers résultats attendus la confirmeront ou l'infinneront. 13. Commentaires 13.1. Les points d'applications L'établissement des points d'application a été fait en tenant compte de deux principaux facteurs qui sont : 1 - la portée du larvicide 2 - l'accès Compte tenu du niveau des cours d'eau exceptionnellement bas cette année, des résultats des prospectious laites après notre mission de 2000, llous avons procédé à un réajustement des poir-rts de dosage qr-ri ont vu leur nombre croître de 36 a 53 (voir carle). Sur la rivière Mbaka lcs prospectiorrs de routine de la semaine 05 ont révélé la présence de stades aquatiques de S damnosum sl en amont du point d'application. A la suite de prospections imnrédiaterlent entreprises dans l'optique de déLerntiner l'inrportance, et l'étcnduc dc ccttc colonisation du vcctcur; un r)or.rvL'au point d'épandasc a été établi en anlorlt cle cette trottvelle liurite cle l'aire de distribution de l'espèce vccl.ricc dc l'onchocercose sur cctte rivièrc. - 11- Un réexamen des limites de répartition de S. damnosum sl devrait se faire en tenant compte, des écoulements voire des conditions climatiques, chaque fois que cela s,avère nécessaire notamment avant la reprise prochaine de la campagne au niveau de tous les bassins du foyer. 13.2. Les débits cours d'eau Il n'existe qu'une seule échelle de crue fonctionnelle sur le haut Kiwira. Tous les débits pour les différentes quantifications de l'insecticide ont été obtenus par la méthode de mesure de débit au flotteur. 13.3. Les faiblesses de I 'isolement du foyer Au Sud-Est du foyer A quelques kilomètres de la R Lumbira se trouve la rivière kilondo. Elle est aussi importante et pérenne. Elle pourrait également abriter S damnosum sl. De plus, entre ces deux cours d'eau se trouvent deux petites rivières pérennes. A l'Ouest du foyer La rivière Songwe côtoie le cours inférieur de la R Kiwira. Les affluents rive gauche de la R Songwe dont ['un d'eux a été trouvé positif la R Chrja qui se trouve à 2.5 km de la KI 600 (Mwalisi affluent de la kiwira ). 13.4. Quelques facteurs pouvant limiter l'impact des traitements La rivière lumbira située au sud ouest du foyer sur la côte est du lac Nyassa n'a pas été traitée en raison de l'inaccessibilité de son point d'épandage( à plus de 06 heures de marche du village lumbira ). - Les mouvements simulidiens ( les migrations et les moussons) - Les confluences inaccessibles des affluents à écoulement permanent se trouvant dans les zones de distribution du vecteur au niveau du foyer. Généralement situées dans les montagnes, ces cours d'eau n'abritent pas S damnosum sl sur leur cours principales mais ils débouchent souvent sur un gîte de la rivière principale qui s'étend à l'embouchure. La masse de ['insecticide lors de son passage à ce niveau de leur embouchure ne parvient pas aux larves qui s'y trouveraient. I 3.5. Quelques difficultés - Les moyens de transport Nous avons utilisés 04 véliicules dor-rl. 03 Toyota tout terrain et une petite voiture Suziki Vitara 03 portes. Une seul Toyota esl en bon état. La Suziki vitara est inadaptée à ce t1,1tc d'activitel. Ccftc insuffisance de moyen de lrausport a linrité les activités de l'équipe dc strpcrvision ct dc llrospcction. ...t... -t2- 14. SUGGESTTONS ef RECOMANDATTONS 14.1 . Suggesfions 14.1 .1 . Technique A- A l'étiage, Ia portée du tèmephos est considérablement réduite nécessitant parfois l'épandage gîte par gîte, ce qui est pratiquement impossible eu égard à la topographie du tenain. De plus l'élimination du vecteur exige le traitement de tous les lieux de reproduction du vecteur aussi nous suggérons la recherche d'accès : a) au niveau KI 600 en amont de Kapeta old bridge pour minimiser la distance entre le point d'application confluence Mwalisi / kisyesye - Kapeta otd bridge. b) Au niveau des confluences de tous les affluents à écoulement permanent dans la zone de distribution du vecteur de l'onchocercose c) Au niveau des affluents rive droite de la R Songwe dans le foyer (un d'eux a été trouvé positif) d) La reprise des traitements avant l'étiage ; singulièrement sur les affluents B- Les résultats des six semaines d'épandage du téméphos sont excellents du point de vue de la population pré imaginale. La reprise des traitements se fera après quelques mois de suspension. Nous suggérons une évaluation générale de la densité des stades aquatiques du vecteur de tout le réseau hydrographique du foyer avec préservation des spécimen récoltés en carnoy's et alcool a70o notamment en amont des premiers points d'applications. 14.I.2. Administrative A - Compte tenu de l'augmentation probable du nombre de circuits, du mauvais état de deux véhicules du présent parc, nous suggérons l'acquisition de 03 véhicutes supplémentaires (à négocier avec les autres projets Oncho national et le coordonnateur national). 14.2. Recommandations 1- Cornpte tenu que : - le traitement de la rivière Lumbira est indispensable à l'élirnination du vecteur dans le foyer de Tukuyu - l'accès à son unique point d'application est pratiquernent impossible à pied (plus de 06 heures de nrarche). Si la réhabilitation de la route qui passe non loin du point n'est pas effective d'ici le début des opérations de lutte - Nous recommandons a) L'achat d'un petit zodtac équipe d'un moteur hors bord b) Le recrutement à titre temporaire d'un piroguier professionnel c) L'installation d'une échelle de crue - 13 -2- Compte tenu : - de la nécessite des contrôles larvaire pour évaluer l'efficacité des épandages ; par une équipe indépendante des équipes assurant les épandages ; - de I'augmentation probable du nontbre des cours d'eau à traiter, ( R Lumbira ) que ce boulot est physiquement très éprouvant du fait de la géographie particulière du foyer, nous recommandons : a) 05 équipes de deux techniciens entomologiques pour la campagne de traitement au sol du foyer de Tukuyu ( 04 pour les circuits d'épandage et une pour les prospections et la supervision )b) Le recrutement et la formation de 05 techniciens supplémentaires. 3- Le débit de la rivière est l'un des trois facteurs qui déterminent la quantité de larvicide à appliquer au niveau de chaque bief. En absence d'échelle de crue, les débits solt obtenus par la méthode de mesure de débit au flotteur. Compte tenu du fait que cet exercice nécessite plusieurs heures, (02 heures pour les cours d'eau importants 30mn à 01 heure pour les affluents) nous recommandons la poursuite des installations sur : Ia basse kiwira ( Ipyana ) la haute Lufilyo ( Lufilyo prirnary school ) la basse Lufilyo ( Tapio bridge ou Mabunga primary school ) Ia basse Lurnbira ( mission catholique ). 4 - L'appréciation de tous les efforts concourant à l'élimination du vecteur dans le foyer de Tukuyu se fera en utilisant les données des captures des fernelles de S damnosum. Sl. Par conséquent, nous recommandons que de vigoureuses actions soient prises pour rendre les résultats plus crédibles, fiables afin qu'ils reflètent la réalité de la dynamique sirnulidienne du foyer. t4- 16. They said the seminars were conducted in 2 days while the next 2 days (Saturday and Sunday). Were utilized for larviciding in Mwalisi (KI 600) and its tributaries. On Sunday, while on trip going to Kyela for treeting Lower Kiwira, they meet Mr. Barro who recommended to postponed larviciding the section. He said it was too late or rather closer to the start of next circuit (Wednesday). Thus it was agreed to the consensus, by the team members, OCP Consultant and VE Proiect Coordinator to recommence the circuit at the same day oiher teams. 3.r.2: RENIARKS AND PROPOSITION: The following were noticed:- That the Ileje circuit is difficult because of geographical location of dosing points. They are spaced and costly to access by car. One point at Chikumbulu Primary School was reported diffrcult to be reached bv car due to rough road and damaged bridge. The team cautioned about more the diffrculties to be faced during rain season to comnrence soon For that, they insisted the use of new pick up for all trips if necessary. For smooth and cost effective operation in this circuit, there is a neeci for treatment to start early morning at the Prison then Lrrmbe. Lubangalala. (point in Lufilyo river) was very diff,rcult to apply insecticide as the flow is high with turbulence. REMARKS AND PROPOSITION: T'he follorving were noticed:- = Due to poor efticacy of Temephos in upper Kisyesye and Mwalisi they proposed to shorten the distanr.e by.- *Shifting the existing uppe' rnost dosing points downstream to a distance possible to acÆess. *Adding a mid dosing point just upstreâm to their confluence. To be exactly,for Mwalisi the distance of 2km can be accessed while for Kisyesye aboutlkm The two newly dosing points will clear breeding sites at srjchés before their confluence which indeed are positive. -) Because in Kiwira river there wasn't nice place accessed for Cischarge rrl€âsuremênt and that the team rnanaged to locate suitable place at Kandete, they suggested that:- (i) Two tealns treating Kiwira river should share discharge measurement at the site so that eac.h team make use of it at their respective river section. (ii) If (i) above is acceptable then the team treating upper KI 600 river should start the circuit on lower Kiwira soon after àÈtaining the river discharge. = In this seasonal rainfall where by the rivei discharge goes up and down there is a need to dispense some drums of Temephos to chosen depôts. They rised their concerned following a report of one member of the teani got injured by Motor bike accident. The VEP Coordinator responded that changes will be a function of availability of all cars and especially when it is raining. Dr. Maegga informed the scarcity of cars especially when Vector Collectors are to be sent for fly catctr, a second means of Monitoring effectiveness of larviciding.
