EXPANDED PROGRAMME ON IMMUNIZATION Hospital-based study on neonatal tetanus I raq. - T he epidem iological pattern of neonatal tetanus (NT) in Baghdad area was studied by an analysis o f N T cases adm itted -to the Ibn-El-Khateeb Hospital in Baghdad (IKHB) over a 5-year period (1980-1984). T o evaluate th e im pact of the EPI, the admission rate of N T cases was analysed over 2 years, from 1 Septem ber 1985 to 30 August 1987. T he total num ber of cases of all infectious diseases adm itted to the IKHB over the 5-year period was 24 738. Tetanus was the cause in 1 226 admissions <5% of all admissions), out of which 1 043 cases (85.1%) were due to neonatal tetanus (Table l) . T he average num ber of N T casesadm ined yearly was 209; no signifi cant differences were found among yearly admissions in this period. T he num ber of N T cases adm ined to th e hospital in 1985 and 1986'was 130 and 80, respectively, showing a downward trend (Fig. 1).
PROGRAMME ÉLARGI DE VACCINATION Etude hospitalière sur le tétanos neonatal I raq. - Le tableau épidém iologique du tétanos néonatal dans la région de Bagdad a été étudié dans le cadre d’une analyse des cas admis à l’Hôpital lbn-El-K hateeb,Bagdad, pendant une période de 5 ans (19801984). Pour évaluer l’im pact du PEV, on a analysé lé taux d’admission pour tétanos néonatal pendant 2 ans, du 1er septem bre 1985 au 30 août 1987. Toutes les maladies infectieuses confondues, le nombre total de cas admis à l’H ôpital lbn-El-K hateeb pendant la période de 5 ans s’est élevé à 24 738. Pour 1 226 de ces admissions (soit 5% du total)-le tétanos était en cause, avec 1043 cas (85,1%) de tétanos néonatal (Tableau 1). On a enregistré une moyenne actuelle d e 209 cas d e tétanos néonatal, sans différence notable d’une année à l’autre au cours de cette période. Toutefois, à partir de 1985, le nombre d’hospitalisations pour tétanos néonatal (130 en ! 985 et 80 en 1986) est en régression (Fig. 1).

The male:female ratio was higher for N T cases (2.8:1) than for non-NT cases (1.5:1) or for the total admission of all infectious diseases (1.7:1). Most of N T cases (734 = 70.4%) were admitted to the hospital between the fourth and ninth days of life (Fig. 2). T he peak of admissions was on the seventh day and the mean age on admission was 8.7 days.
Le taux de masculinité était plus élevé pour le tétanos néonatal (2,8:1) que pour les autres téjanos (1,5:1) ou que pour l’ensemble des maladies infectieuses (1,7:1). „ La plupart des hospitalisations pour tétanos néonatal (734, soit 70,4%) concernaient des sujets âgés de 4 à 9 jours (Fig. 2), avec un pic à 7 jours et une moyenne de 8,7 jours.

Out of 1 043 cases, 660 newborns died, 199 were cured and 184 were discharged at the request of their family. The casefatality rate (CFR) among 859 cases who stayed in the hospital was 77%. CFR was related to the age of newborns at admission: 89% in newborns admitted in their second to ninth day of life and 41% in newborns in the 10-30-day group. Most of the newborns died between 6 and 10 days of age. The mean age at death was 9.9 days and the peak of deaths was on the eighth day of life (Fig. 2). N T incidence shows seasonal variations. The number of cases was lowest in the honest pan of the year, from May to September
Sur 1 043 cas, il y a eu 660 décès, 199 guérisons et 184 sorties de l’hôpital à la demande de la famille. Le taux de létalité pour les 859 nouveau-nés restés à l’hôpital a été de 77%. On a observé qu’il était lié à l’âge au moment de l’hospitalisation: de 89% en cas d’hospitalisa tion de nourrissons âgés de 2 à 9 jours, il tombait à 41% lorsque le sujet avait de 10 à 30 jours. La plupan des nouveau-nés sont décédés entre l’âge de 6 et 10 jours. On a enregistré un âge de décès moyen de 9,9 jours, et un pic à 8 jours (Fig. 2). L’incidence du tétanos néonatal subit des variations saisonnières. C’est pendant la période la plus chaude de l’année, de mai à septembre,

and then started to increase in October before peaking in November-January. A very similar seasonal distribution of N T cases was reported from Iraq for 1954-1957 (Fig. 3). Most of N T cases (1027 = 98.5%) were delivered at home and only 16 (1.5%) in hospital. T he majority were from rural areas while the remaining 338 cases (32.4%) were from urban and suburban areas.

The Ibn-El-Khateeb Hospital is the only hospital for infectious disease in Baghdad area and most cases of tetanus from that area are hospitalized there. L ’Hôpital Ibn-El-Khateeb est le seul hôpital de la région de Bagdad pour les maladies infectieuses et la plupart des cas de tétanos qui se déciment dans cette région y sont admis. Les données de l’Hôpital Ibn-El-Khateeb confirment la tendance à la baisse observée dans l’incidence de toutes les maladies visées par le PEV, y compris le tétanos, au moyen du système de notification systématique pour 1985 et 1986.1 Les statistiques hospitalières, comme celles de la notification systématique, font apparaître que plus de 80% des cas de tétanos ayant donné lieu à une hospitalisation étaient dus au tétanos néonatal. D’après l’enquête réalisée en avril 1987, la proportion des femmes en âge de procréer ayant reçu 2 doses d’anatoxine tétanique, s’élevait à 43%. Pour faire reculer encore plus nettement le tétanos néonatal, il faut s’attacher davantage à accroître le taux de vaccination par l’anatoxine tétanique chez les femmes en âge de procréer. 1 Voir N° 18, 1988, pp 130-132
The data from K H B confirm the downward trend in the incidence of all EPI diseases, including tetanus, observed in the routine reporting system for 1985 and 1986.1 Hospital data, like routine reporting data, showed that more than 80% of hospital ized cases of tetanus were due to neonatal tetanus.
The survey conducted in April 1987 showed that the coverage with 2 doses of tetanus toxoid in women of childbearing age was 43%. In order to have a more accentuated decline of neonatal tetanus incidence, further efforts are needed to increase tetanus toxoid immunization coverage in women of childbearing age. • See No 18,1988, pp 130-132
