EXPANDED PROGRAMME ON IMMUNIZATION I ndonesia . — From October 1977 until February 1978, staff of the Indonesian Expanded Programme on Immunization (EPI) evaluated population coverage in the 15 provinces of the country where the programme is currently operating. Staff included workers from the national EPI office in Jakarta as well as provincial and local employees. Workers were assigned to evaluate areas other than those to which they were regularly posted. A total of 3 384 children 12-17 months old were selected using the method described in the WHO/EPI manual and examined for vaccination scars.
PROGRAMME ÉLARGI DE VACCINATION I ndonésie . — Entre octobre 1977 et février 1978, des équipes du Programme élargi de vaccination (PEV) indonésien ont évalué la couverture vaccinale dans les 15 provinces du pays où le programme est actuellement en cours. Ces équipes étaient composées d’agents du Bureau national du PEV, à Jakarta, de même que de personnels provinciaux et locaux. Les agents avaient à évaluer des régions autres que celles auxquelles ils étaient régulièrement affectés. Un total de 3 384 enfants âgés de 12 à 17 mois ont été choisis par la méthode décrite dans le manuel OMS/PEV et examinés quant à la présence de cicatrices vaccinales. Le calendrier appliqué en Indonésie prévoit l’inoculation d’une dose de BCG, d’une dose de vaccin antivariolique et de deux doses de DTC aux enfants de trois à 14 mois. Les vaccinations ont lieu tous les trois mois. Le BCG est administré avec la première dose de DTC et le vaccin antivariolique avec la seconde. On peut de ce fait présumer qu’un enfant portant les cicatrices d’une vaccination BCG et d’une vaccination contre la variole a reçu deux doses de DCT. Les résultats de l'enquête figurent au Tableau 1.
The schedule used in Indonesia calls for children to receive one BCG, one smallpox, and two DPT immunizations between the ages of three and 14 months. Immunizations are offered every three months. BCG is given with the first dose of DPT and smallpox with the second. Thus a child with both a BCG and a smallpox vaccination scar can be assumed to have received two doses of DPT. Results of the survey are shown in Table 1.

Of the 67.6% of children surveyed who received the first dose of DPT, only 14.6% did not receive a second dose. In addition to these results, a significant operational problem was identified. In principle, no child should receive a smallpox vaccina tion until he has a BCG scar, and yet over half (51.6%) of the children with only one of the two scars had smallpox rather than BCG. It is assumed that these children did not receive any DPT, and that some areas have not adopted the new policy of using smallpox vaccination as a marker for the second DPT immunization. Future training will be directed towards implementing the new policy.

Sur les 67,6% d’enfants examinés ayant reçu la première dose de DTC, 14,6% seulement n’ont pas reçu la seconde dose. Outre ces résultats, un important problème opérationnel a été mis en évidence. En principe, un enfant ne devrait pas être vacciné contre la variole tant qu’il n’a pas de cicatrice du BCG; or plus de la moitié (51,6%) des enfants ne portant qu’une des deux cicatrices présen taient celle de la vaccination antivariolique au lieu de celle du BCG. Il est à supposer que ces enfants n’ont reçu aucune dose de DTC et que certaines régions n’ont pas encore adopté la nouvelle politique qui consiste à prendre la vaccination antivariolique comme critère d ’inoculation de la deuxième dose de DTC. Les efforts futurs de formation tendront à la mise en œuvre de cette nouvelle politique.

RABIES SURVEILLANCE Colombia . — During the first eight months of 1977, Colombia notified 43 cases of rabies in rats. Even though only four of them were laboratory confirmed, rabies in rats is known to exist in the city of Bogota and its surroundings. No clear epidemiological explanation is available so far, and the matter deserves to be thoroughly investigated. Rabies in rats appears to have been negligible so far in Europe and North America.

Bien que quatre d’entre eux seulement aient été confirmés en laboratoire, on sait que la rage existe chez les rats à Bogota et dans les environs. Le fait n’a pas encore été clairement expliqué du point de vue épidémiologique et la question mérite une étude approfondie. La rage chez les rats a paru négligeable jusqu’ici en Europe et en Amérique du Nord.

SURVEILLANCE DES SHIGELLA — Une violente poussée de dysenterie bacillaire a été signalée dans la région de Mawanella. Pendant une période de 19 jours s’étendant du 9 au 28 février 1978, 109 malades ont été hospitalisés. Soixante-seize d’entre eux ont été admis à l’hôpital de base de Kegalle et 33 à l’unité périphérique de Mawanella. Presque tous les malades étaient des enfants des écoles qui ont été admis dans les services de pédiatrie. SURVEILLANCE OF SHIGELLA Sri L ank a . — An explosive outbreak of bacillary dysentery has been reported in the area of Mawanella. During a 19 day period from 9 February to 28 February 1978, 109 cases were hospitalized. Seventy-six cases were admitted to the Base Hospital, Kegalle and 33 cases to the Mawanella Peripheral Unit. Almost all the patients were admitted to the paediatric wards and the majority of them were schoolchildren. Males and females were affected equally and most cases were from the village of Ambulugala. FIÈVRE JAUNE DE BROUSSE de La Paz, qui relève du Ministère de l’Assistance sociale et de la Santé publique, a signalé récemment des cas suspects de fièvre jaune de brousse dont quatre ont eu une issue fatale. JUNGLE YELLOW FEVER — The Health Unit of La Paz, under the Ministry of Social Welfare and Public Health, recently reported ten suspected cases of jungle yellow fever, four of them fatal. These cases occurred in the Canton of La Asunta, Province of Sud Yungas, Department of La Paz, between 10 March and 28 April 1978; nine men and one woman, aged 16-50 were involved, all of them agri cultural workers.
INFLUENZA A ustralia (30 June 1978). — Scattered influenza cases have been reported since the end of June in the Melbourne metropolitan area, where an outbreak has been observed in a school (16 cases in a class of 32 children). Three strains of virus B have been isolated. ‘ See No. 11,p. 78.

CHOLERA — In early May 1978, health authorities in the State of Slo Paulo, as part of the surveillance programme on cholera, sent strains of Vibrio cholerae isolated from the sewerage system of the city of Santos to the Center for Disease Control (CDC), Atlanta, Georgia, USA, to determine the presence and the degree of toxigenicity of the organism. CDC confirmed the presence of Vibrio cholerae, biotype eltor, serotype Ogawa, but stated that this strain, as an exception to the B r a z il .

On receipt of this information, the health authorities were able to reassure the public that there was no current risk of a cholera epidemic, the fear of which had caused both the Federal and State Governments to put into force strong control measures. Since that date numerous changes have been made and the amend ments have regularly been published in the issues of the Weekly Epidemiological Record (WER). It has been decided to publish a new edition of this booklet including all the changes so far received. It is the intention to put one or two pages of the first proof of the booklet in issues of the WER and, in this way, health administrations will have a further opportun ity to send any corrections or comments prior to the final printing which will be done later this year. Information for a number of countries is shown in the back pages of this issue. Countries preceding the Netherlands in the English alphabetical order appeared in previous issues. The Quarantine Service, in the port of San Juan, is under the supervision of the United States Public Health Service in the same manner as in the ports of the continental United States. By special arrangement, Deratting or Derailing Exemption Certificates may be secured at certain intervening smaller ports. Qualified personnel are detailed from the port of San Juan to render this service. Dans le port de San Juan, le Service de la Qua rantaine est placé sous le contrôle du Service de la Santé publique des Etats-Unis tout comme dans les ports continentaux des Etats-Unis.
