(continued from page 257) through genetic engineering and the development of immunoassays for both antigen and antibody associated with the “delta” antigen. The diagnosis of dengue has been facilitated by the development of monoclonal antibodies to all four dengue types. The detection of IgM antibodies during the acute phase of both dengue and Japanese ence­ phalitis is of value in rapid diagnosis. The development of micro­ scopic slides containing stable inactivated, formalin-fixed antigens for Lassa and Ebola viruses has facilitated the detection o f antibody by immunofluorescence.

Reagents of Quality Control and Distribution The preparation of reagents can be commercial or WHO sponsored but irrespective of the source, it is essential that working reagents supplied by WHO should consist of large lots suitable for quality control and wide distribution. Quality control must be carried out by at least two reference laboratories separate from the producer and must include not only serological reagents but also the solid phase supports and any other materials used in each assay system. Advance m biotechnology might change the availability and quality o f reagents and the feasibility of production in the near future but this eventuality should not delay the implementation of current plans. General Recommendations The Group recommended that a coordinated programme should be developed to ensure the availability of reagents within the network of WHO Collaborating Centres and National Laboratories for the diag­ nosis of the following diseases: — viral hepatitis — respiratory viral diseases, including measles — viral gastroenteritis — arthropod- and rodent-borne viral diseases — rubella — herpes virus group diseases. In order to implement a programme in each of these areas it was agreed that coordinators within the WHO Collaborating Centres, together with their associates, be appointed and assume responsibility for the following: (1) identify specific tests to be recommended for rapid diagnosis, taking into account considerations of cost, simplicity and accura­ cy; (2) define the reagents and test material required and identify specific suppliers of the reagents; (3) define minimum standards of quality and performance o f reagents and other material; (4) develop a strategy for distribution o f reagents within the network of WHO Collaborating Centres for Reference and Research and coo­ perating national laboratories; (5) assist WHO in the development and implementation o f training courses and the selection of suitable candidates for training; (6) evaluate the rapid viral diagnostic tests performed in field situa­ tions; (7) solicit and review information on new tests and new reagents, including monoclonal antibodies, which might have application in rapid viral diagnosis.

Dans le domaine de l’hépatite, on peut notamment citer la propagation du virus de l’hépatite A dans des systèmes de culture tissulaire et l’utilisation d’antigènes pour le titrage immunologique des IgM, la production récente de l’antigène central de l’hépatite B à partir de bactéries, par manipulation génétique, et la mise au point de titrages immunologiques à la fois pour l’antigène et l’anticorps associés à l’antigène «delta». Le diagnostic de la dengue a été facilité par l’obtention d’anticorps raonoclonaux dirigés contre les quatre types de virus de la dengue. La détection d’anticorps IgM pendant la phase aiguë de la dengue et de l'encéphalite japonaise présente un réel intérêt pour le diagnostic rapide. La réalisation de lames portant des antigènes anti-virus de Lhassa et Ebola stables, et fixés dans le formol a facilité le dépistage de l’anticorps par immu­ nofluorescence. Préparation de réactifs; contrôle de leur qualité et distribution. Que les réactifs soient préparés commercialement ou sous les aus­ pices de l’OMS, il est indispensable que ceux fournis par l’OMS soient produits en lots importants susceptibles de faire l’objet d’un contrôle de la qualité et d’une large diffusion. Ce contrôle de la qualité doit s’effectuer dans au moins deux laboratoires de référence autres que celui du fabricant et porter non seulement sur les réactifs sérologiques mais également sur la phase solide tenant lieu de support et toutes autres substances utilisées dans chaque système de titrage. 1 1se pour­ rait que les progrès de la bio-technologie modifient dans un proche avenir la disponibilité et la qualité des réactifs ainsi que la faisabilité de la production mais cette éventualité ne saurait retarder la mise en oeuvre des plans actuels. Recommandations générales. Le Groupe a recommandé que l’on élabore un programme coor­ donné en vue de garantir la disponibilité, au sein du réseau de centres collaborateurs de TOMS et de laboratoires nationaux de réactifs pour le diagnostic des maladies suivantes: — hépatite virale — maladies respiratoires virales, y compris la rougeole — gastroentérite virale — maladies virales transmises par les arthropodes et les rongeurs — rubéole — maladies dues au groupe des herpesvirus Afin de mettre en oeuvre un programme dans chacun de ces sec­ teurs, il a été convenu que les coordonateurs en poste dans les centres collaborateurs de TOMS, ainsi que leurs associés, se verraient confier les responsabilités suivantes; 1) définir des épreuves spécifiques à recommander pour le diagnostic rapide, en tenant compte de considérations de coût, de simplicité et d’exactitude; 2) définir les réactifs et le matériel d’épreuve nécessaires et désigner des fournisseurs pour chacun des réactifs; 3) définir des nonnes minimales de qualité et d’exécution pour les réactifs et autres substances; 4) élaborer une stratégie pour la diffusion des réactifs au sein du réseau de centres collaborateurs de TOMS pour la référence et la recherche et de laboratoire nationaux coopérants: 5) aider TOMS à élaborer et à exécuter des cours de formation ainsi que pour le choix de candidats valables à ces cours de formation; 6) évaluer les épreuves de diagnostic rapide des affections virales effectuées dans des conditions de terrain; 7) solliciter et examiner les informations sur de nouvelles épreuves et de nouveaux réactifs, notamment des anticorps monoclonaux, qui pourraient avoir une application en matière de diagnostic rapide des affections virales.
