W LE POINT SUR LES ËPIDËMIES Syndrome respiratoire aigu sévère (SRAS) L’Organisation mondiale de la Santé s’occupe actuellement de coordonner la riposte interna tionale face aux flambées de syndrome respira toire aigu sévère (SRAS), une pneumonie atypi que d’étiologie encore inconnue. Des cas ont été notifiés par 8 pays depuis février 2003. Au 19 mars 2003,264 cas suspects ou probables de ce syndro me (dont 9 mortels) avaient été notifiés à TOMS. La transmission locale du syndrome a été confir mée au Canada, à Hong Kong, Région adminis trative spéciale de la Chine (RAS), à Singapour, à Taiwan (Chine) et au Viet Nam. Les informations épidémiologiques les plus récentes peuvent être consultées sur: http://www.who.int/csr/sars/en/. Afin de répondre aux demandes d’assistance, des équipes de terrain du Réseau mondial OMS d’alerte et d’action en cas d’épidémie (GOARN) ont été déployées au Viet Nam et à Hong Kong RAS pour aider les autorités sanitaires locales à mener les investigations nécessaires et à lutter contre les flambées. L’OMS enquête aussi avec les autorités chinoises sur une flambée de pneumo nie atypique survenue en novembre 2002 dans la province de Guandong, laquelle a atteint un pic en février de cette année. L’Organisation étudie actuellement avec les autorités sanitaires de Sin gapour la nécessité éventuelle d’une aide inter nationale. L’OMS a installé des bases logistiques et mis à la disposition des pays affectés des stocks de maté riel de protection et d’équipement clinique. Le 15 mars dernier, TOMS a publié un avertissement aux voyageurs conseillant à toute personne se rendant dans les régions touchées d’être attenti ve à l’apparition de symptômes dans les 10 jours suivant son retour.
Severe acute respiratory syndrome (SARS) The World Health Organization is currently coordinating a global response to reported outbreaks of severe acute respiratory syn drome (SARS), an atypical pneumonia of un known etiology. Cases have been reported from eight countries since February 2003. As of 19 March 2003, 264 suspect and probable cases of SARS (including 9 deaths) have been reported to WHO. Local transmission of SARS has been confirmed in Canada, Hong Kong Special Administrative Region of China (Hong Kong SAR), Singapore, Taiwan (China) and Viet Nam. In response to requests for assistance, WHO Global Outbreak Alert and Response Net work (GOARN) field teams have been de ployed in Viet Nam and Hong Kong SAR where they are helping local health autho rities to investigate and control the out breaks. WHO is working with Chinese health authorities to investigate an outbreak of aty pical pneunomia that occurred in Guandong province in November 2002 and peaked in February of this year. WHO is in discussion with health authorities in Singa pore to explore whether international assis tance is required. WHO logistic bases have been established and supplies of protective and clinical equipment are being made available to affected countries. In addition, WHO published a travel advisory notice on the 15 March 2003, advising all individuals travelling to affected areas to be watchful for the development of symptoms for a period of 10 days after returning.

case, a middle-aged man business man who has travelled extensively in South-East Asia before becoming unwell, was admitted to hospital in Hanoi on 26 February 2003 with a high fever, dry cough, myalgia and mild sore throat. Over the following 4 days he developed symptoms of adult respiratory distress syndrome, requiring ventilatory support, and severe thrombocytopenia. Despite intensive therapy he died on 13 March after being transferred to an isolation facility in Hong Kong SAR. On the basis of data from the SARS foci in Hanoi and Hong Kong SAR, the incubation period has been estimated to be 27 days, but usually 35 days. Attack rates of >56% among health care workers caring for patients with SARS is consistent in both the Hong Kong and Hanoi foci.

Les observations faites pour les foyers de Hong Kong et de Hanoi correspondent à un taux datteinte de plus de 56 % pour le personnel de santé soccupant des malades souffrant de ce syndrome.

Clinical features The most common early symptoms in patients progressing to SARS have included fever (100%), malaise (100%), chills (97%), headache (84%), myalgia (81%), dizziness (61%), rigors (55%), cough (39%), sore throat (23%) and runny nose (23%). In many cases, patients often first presented with severe headache, dizziness and myalgia. Temperature persisted above baseline during disease progression. In some cases, there was rapid deterioration with low oxygen saturation and acute respiratory distress requiring ventilatory support. Tableau clinique Les symptômes précoces les plus courants chez les patients dont létat évolue vers le SRAS sont les suivants: fièvre (100%), sensation de malaise (100%), frissons (97%), céphalées (84%), myalgies (81%), vertiges (61%), raideurs (55%), toux (39%), angine (23%), écoulement nasal (23%). confluent infiltrates. Patchy CXR changes are sometimes noted in the absence of chest symptoms. Adult respiratory distress syndrome has been observed in a number of patients in the end stages. Patients may present with a normal blood picture. However, by day 3 or 4 of the illness, lymphopenia is commonly observed ($50%) and thrombocytopenia less commonly. Elevated liver enzymes and abnormal partial prothrombin are sometimes seen. Creatinine phosphokinase is raised in some cases as is C-reactive protein.

Treatment Hospitalized patients have received multiple antibiotic therapy. Antibiotics used alone or in combination have included azithromycin, aminoglycosides, ceftriaxone, doxycycline and ciprofloxacin. No clinical improvement has been attributable to the use of antibiotics. The antiviral agent ribavirin, given intravenously in combination with high-dose corticosteroids, may have been responsible for some clinical improvement observed in critically ill patients in Hong Kong SAR. Intensive and good supportive care, with and without antiviral agents, have also improved prognosis. Although the number of patients who have been discharged from hospital is very low, a significant proportion of previously critically ill patients have now stabilized and no longer require intensive care. Respiratory protection and barrier nursing are advised for all health care workers and visitors in close contact with reported cases. Traitement Les patients hospitalisés ont été placés sous antibiothérapie multiple. Les antibiotiques utilisés seuls ou en association ont été lazithromycine, les aminoglycosides, la ceftriaxone, la doxycycline et la ciprofloxacine. On na encore observé aucune amélioration de létat clinique avec lantibiothérapie. Ladministration par voie intraveineuse de ribavirine, un antiviral, en association avec des corticoïdes à doses élevées peut, en revanche, être à lorigine dune certaine amélioration clinique observée chez des patients dans un état critique à Hong Kong. Les soins intensifs et un bon traitement symptomatique, avec ou sans agents antiviraux, ont également permis daméliorer le pronostic. Bien que le nombre de patients ayant pu quitter lhôpital soit très faible, une proportion significative des malades précédemment dans un état critique sont à présent dans un état stationnaire et ne nécessitent plus de soins intensifs. La protection des voies respiratoires et les techniques de soins en isolement sont recommandées pour le personnel soignant et les visiteurs en contact rapproché avec les cas signalés. Laboratory findings Preliminary investigations in two patients with SARS in Germany and Hong Kong SAR have demonstrated paramyxoviruslike particles under electron microscopy. Tests using the polymerase chain reaction technique on the same samples have also been positive. However, more samples need to be examined and further investigation undertaken before a definitive cause can be identified. Laboratories around the world are continuing to collaborate in the global effort to identify this causative agent. Résultats de laboratoire Les premières investigations menées sur deux patients souffrant du syndrome, lun en Allemagne et lautre à Hong Kong RAS, ont mis en évidence des particules ressemblant à un paramyxovirus à la microscopie électronique. Des tests damplification génique effectués sur ces mêmes échantillons se sont également avérés positifs. Il faudra néanmoins examiner dautres échantillons et entreprendre des investigations plus approfondies pour pouvoir identifier définitivement lagent infectieux.
