Equip.: USS 99,400 oU/04/oa4tts Lagos, Nigeria COMMUNITY DIRECTED TREATMENT WITH IVERMECTIN (CDTI) OJECT IN GIM A,86 For the purpose of developing and establishing sustainable Community Directed Treatment witlr Iverrlectirr (CDTI) for the control of onchocerciasis in Nigeria, The World Health Organization's African Programrne for Onchocerciasis Control, hereinafter referred to as "WHO/APOC" on the one hand, and The National Onchocerciasis Task Force, Ministry of Health and Social Services, Nigeria, hereinafter referred to as "the Institution", on the other hand, HAVE AGREED AS FOLLOWS 1. Work to be performed Further to the Meruorandum for the African Programme for Onchocerciasis Control (APOC), PHASE Il (2002-2007) and the Phasing out Period (2008-2010) (the "Memorandum"), rvhiclr was endorsed by tlte Government of Nigeria, this Agreement is concluded for the purpose of developing and establishing strstainable cot-t.trnunitl,directed ivermectin distribution Project in Imo and Abia States of Nigeria. The Project natned "NGIM ,486" shall be carried out in accordance rvith tlre sustainability rvork plans, btrdgets and justifications notes subrnitted by the NOTF of Nigeria in September 2003and January 2004, reviewed and approved by the Managernent of APOC. The Project shall be undertaken in I6 Local Governrnent Areas in Imo State (Aboh Mbaise, Ehinre Mbano. Ezinihitte, Ihitte Uborna, Isiala Mbano, Ikeduru, Ideato Nor1h, Ideato South, Mbaitoli, Nwangele, Ngor Okpala. Oborvo, Okigrve, Onuir.uo, Orlu and Owerri North) arrd 8 Local Governnrent Areas rn Abia State( Isuikrvuato, Uttruttneochi, Berrde, Arochukrvu, Ikrvuano, Ohafia, Ukrva East and lku,a West) of Nigeria PROPOSAL OF TECHNICAL SERVICES - LETTER OF AGREEMENT 2OO4 a2 The plans of work and the time line as defined in the sustainability plan may be modified by mutual agreement of the parties taking into account the operating experience and needs of the programme by the useof "lmplementation letters" issued by "WHO/APOC,'. 2- Obligations of WHO/ApOC 2.1. Total Financial Obligation The total financial obligation of WHO/APOC, underthis Agreement is US$l0g,540 (one hundred andeight thousand, five hundred and forty dollars us). This total financial obligation of wHo/Apoc will be shared as follows: - Imo State CDTI project: US$ 59,000 - Abia State CDTI project: US$ 49,540 The maximum obligation of APOC for any budget line item in this Agreement is the amount indicated approved budget line, and the total obligation miy not exceed the sum of these budget lines. The amended and approved budgets for 2004, are attached herewith as Annexes I a and I b. 2.2. Capital Equipment rn I I the wHo/APoC will retain funds proposed for the purchase of capital Equipment and make the purchasesthrough its central purchasing system. The Institution will be responsible for swit clearance of these goods onarrival. The equipment and supplies furnished by WHo under this agreement will be handed over to theInstitution upon arrival at the port of entry in the country, against approfriate receipts. 2.3. Audit of accounts wHo will retain funds for financial audit of the Project and shall have the right to inspect the accounts at any time by way of its internal auditors, its external auditors or other auditors contlacted for the purpose. Theauditors shall have the right to inspect all transactions pertaining to the projects between the Institution,s bankaccount and actual payments for goods and services and to examine supporting ao"r*"niuii"r rrJr"l4r"r,accounting entries in the Institution's books. In order to facilitate such reiortinglnd audit, the Institution shallenSurethataccurateandsystematicaccountsarekeptinrespectofthefundedactivities.',!..v^rrv!rl9 l 3. For its part, the Institution, in agreement with its own Government, shall provide all the inputs requiredfrom the Government, the NGDos andother partners, including all the staff necessary for the execution of theProject in accordance with this Agreement and as set forth in ihe Memorandum uni in the approved projectproposal. Financial arrangements 5' 1 ' The total amount of expenditure estimated by this Agreement is us$10g,540 (one hundred andeight thousand, five hundred and forty dollars us). of t-he above, us$99,400 ( ninety nine thousand and fourhundred dollars US) will be retained and used by wHo/APoc for the purchase or u vehicle and otherequipment needed for the performance of the ero.lect activities.The breakdown of the total amount retainedis as follows: - Imo State CDTI project: US$ 51,500 r - Abia State CDTI project: US$ 47,900 5'2' The remaining amounts of uS$9,140 (nine thousand, one hundred and forty dollars us;,is tJ ueshared as follows: From this remaining amount as detailed above, a first instalment of us$5,000 (five trrousand doilars US) wilrbe paid to the Institution as soon as possible after the signature of the Agreement by both parties, provided thatall the other conditions precedent to disbursement have been met.The first installment will be shared as follows: - Imo State CDTI project: - Abia State CDTI project: - Imo State CDTI project: - Abia State CDTI project: us$ 7,500 us$ 1,640 us$ 4,000 us$ 1,000 5'3' The subsequent instalment(s) will be paid on request from the Institution accompanied by astatement reconciling expenditure already made to the various uuaget lines. The payment of these replenishments will be subject to: (a) The reception of the statement mentioned above; (b) The regular transmission at the end of each month of a copy ofthe bankwith a list of all cheques drawn on the account.
