EXPANDED PROGRAMME ON IMMUNIZATION Graphie Monitoring of Immunization Coverage An essential component of any programme is a built-in system for continuous evaluation. Such a system should serve as an early warning of inadequate progress and lead, through investigation, to the identification of constraints and the early implementation of corrective action. The manager of a National Expanded Programme on Immuniza­ tion (EP1) having established annual goals for immunization cove­ rage is often handicapped because inadequate progress is not iden­ tified early enough to achieve the annual coverage target. A simple practical system applicable at national or county levels has been developed at the Caribbean Epidemiology Centre. This new technique employs a graphic presentation (Fig. 1) which provides the manager with an indication by month of progress towards the annual target. It is simple to update and is recommen­ ded for use as a wall chart in the manager’s office. Although this graphic presentation can be used for any target age group and type of immunization, for purposes of explanation, an example from the EPI programme in the fictitious Caribbean coun­ try of CARIBA will be used—providing each child under one year of age with three doses o f diphtheria, pertussis and tetanus (DPT) vaccine. At the beginning of each calendar year the programme manager should obtain from the statistical officer of the Health Ministry estimates of live births (2 470) minus infant deaths (70) from the previous year. This would give the estimated number of children below one year of age (2 400) who would have to be immunized if full coverage is to be achieved. This is the target which is inserted at the top of the vertical axis of th e chart aligned with the “ 100%” . The Cariba target population of 2 400 children is then divided by 12 in order to obtain the monthly target—200 to be completely immunized each month if this annual target is to be achieved. The vertical axis is then completed starting at the bottom with 200 and adding increments of 200 (200 + 200, -1- 200 + 200 + 200) to give cumulative totals e.g. of 400 and 600 for the end of February and the end of March respectively. The preliminary total for children fully immunized each month is entered in the horizontal row (Box A). These monthly totals are plotted (x----- x) on the chart and can be compared with the printed ideal curve. After all reports are received, the figure for the final adjusted monthly total is similarly entered in Box B and plotted (x----- x) on the chart. Although monthly reporting from the field immunization units is highly desirable, the same chart could be used for less frequent reporting, however, each type of immunization being administered in the national programme will require a separate chart. The estimation of the target group may be further improved by a health statistician but concern about refinement of target estimation should not preclude interim employment o f this management tool.

PROGRAMME ÉLARGI DE VACCINATION Contrôle graphique de la couverture vaccinale Un élément essentiel de tout programme est un système incorporé d ’évaluation permanente qui doit avertir rapidement de tout retard et conduire, grâce à des recherches, au dépistage des obstacles et à la mise en œuvre précoce d’une action corrective. Le responsable du Programme élargi de Vaccination national (PEV) qui a établi des objectifs annuels de couverture vaccinale est souvent handicapé parce que les retards ne sont pas détectés suffisam­ ment tôt pour lui permettre de réaliser son programme annuel dans les délais. Un système simple et pratique applicable aux niveaux national ou régional a été mis au point par le Caribbean Epidemiology

Centre ( CA REC). Cette nouvelle technique fait appel à une présentation graphique

(Fig. 1) qui fournit au responsable une indication mensuelle de l’état d ’avancement du programme par rapport à l’objectif annuel. Ce graphique est facile à tenir à jour et il est recommandé de l’affi­ cher dans le bureau du responsable. Cette présentation graphique peut être utilisée pour n ’importe quel groupe d ’âge et pour n ’importe quel type de vaccination; ici, à des fins explicatives, nous employerons un exemple concernant un pays fictif des Caraïbes, le CARIBA, et un programme PEV pré­ voyant d ’administrer à tous les enfants de moins d ’un an trois doses de vaccin contre la diphtérie, la coqueluche et le tétanos (DCT). Au début de chaque année civile, le responsable du programme devra se procurer auprès du Bureau des statistiques du Ministère de la Santé le nombre estimatif des naissances vivantes de l’année pré­ cédente (2470) dont il déduira celui des déoès infantiles (70). Il obtiendra ainsi le nombre estimatif d ’enfants de moins d ’un an (2 400) qu'il faudra vacciner pour réaliser une couverture complète. C’est l’objectif qui est inscrit au sommet de Taxe vertical du graphi­ que en regard du « 100% ». La population-cible de 2 400 enfants sera alors divisée par 12 pour donner l'objectif mensuel, soit 200 enfants qui devront être complè­ tement vaccinés chaque mois si Ton veut atteindre l’objectif annuel. L ’axe vertical sera alors complété à partir du bas; on inscrira d ’abord 200, puis on ajoutera chaque fois 200 au total précédent (200 + 200, 200 + 200 + 200) de manière à avoir 400 à la fin de février, 600 à la fin de mars, etc. On inscrira le total préliminaire d ’enfants complètement vaccinés chaque mois dans la colonne horizontale (case A). Ces totaux mensuels seront portés (x---- x) sur le graphique et Ton pourra ainsi comparer cette courbe avec la courbe idéale imprimée. Lorsque tous les rapports auront été reçus, le total mensuel final ajusté sera inscrit de la même façon dans la case B et relevé sur le graphique (x----- x). Bien qu’une notification mensuelle par les unités de vaccination sur le terrain soit extrêmement souhaitable, on peut néanmoins utiliser ce graphique à des intervalles moins fréquents, mais chaque type de vaccination administré dans le cadre du programme national demande un graphique distinct L’effectif du groupe visé peut encore être établi avec plus de précision par un statisticien sanitaire mais il ne faut pas que le souci d’améliorer cette donnée empêche l’emploi provisoire de cet outil de gestion.
